Number of existing permanent teeth is associated with chronic kidney disease in the elderly Korean population

  • Background/Aims: The aim of this study was to assess the association between the number of existing permanent teeth and chronic kidney disease (CKD) in a representative sample of the elderly Korean population. Methods: A total of 2,519 subjects who participated in the Korean National Health and Nutrition Examination Survey were cross-sectionally examined. The number of existing permanent teeth was evaluated by clinical oral examination. CKD was defined based on definition and classification by Kidney Disease: Improving Global Outcomes (KDIGO) 2012 guidelines. Multivariable logistic regression analyses were performed controlling for age, gender, income, education, tooth-brushing frequency, periodontitis, state of dentition, smoking, alcohol consumption, hypertension, obesity, diabetes mellitus, and hypercholesterolemia. Subgroup analyses by age and gender were also performed. Results: The number of teeth was significantly associated with CKD after controlling for all potential confounders (adjusted odds ratio [AOR], 1.67; 95% confidence interval [CI], 1.04 to 2.70 for lower number of teeth; AOR, 1.59; 95% CI, 1.14 to 2.23 for moderate number of teeth). In the subgroup analyses, the association was highlighted in females aged 75 years over (AOR, 2.55; 95% CI, 1.05 to 6.20 for lower number of teeth; AOR, 1.95; 95% CI, 1.01 to 3.80 for moderate number of teeth). Conclusions: Our findings suggest that the number of existing permanent teeth may be associated with CKD among Korean elderly.

  • Objectives: This study aimed to investigate the effects of the amount and duration of smoking on gingival symptoms in adolescents. Methods: We used the web-based survey data of youth health behaviors from 2016 conducted by the Korean Centers for Disease Control and Prevention. The subjects of the study were 67,983 middle and high school students. Data analysis was performed using the SPSS Ver 20.0 program, and complex sample multivariate logistic regression analysis was performed to analyze gingival symptoms according to short- or long-term smoking habits. Results: In subjects with short-term smoking habits, there was no significant difference in gingival symptoms according to the smoking amount. However, after adjusting for socioeconomic characteristics and oral health behaviors in subjects with long-term smoking habits, a heavy smoking amount (${\geq}0.52PY$) had a 1.25 times higher effect than a light smoking amount (<0.52 PY) on gingival symptoms. Long-term heavy smoking (${\geq}0.52PY$) and low daily tooth-brushing frequency in high school students were factors affecting gingival symptoms. Conclusions: When conducting a project on the oral health of adolescents, it is argued that specific investigations and measures should be set up regarding health behavior factors, such as the amount and duration of smoking among adolescents.

전문가 치주 예방관리 전·후 치주질환자의 P. gingivalis 세균 수 변화 (Changes in the number of P. gingivalis bacteria before and after professional periodontal prevention care in periodontal disease patients)

  • Background: This study aimed to investigate the changes of the amount of P. gingivalis in saliva by professional preventive treatment of periodontal disease by dental hygienists. Methods: A total of 109 subjects participated in this study with informed consent. The control group (38 subjects) performed oral hygiene management individually. The periodontal treatment group (35 subjects) underwent root planning once every 3 weeks for a total of 4 times. The professional periodontal prevention group (36 subjects) underwent interdental cleaning and professional tooth brushing once every 3 weeks for a total of 4 times. Paired T test and analysis of variance were performed to compare the difference among the groups in the amount of P. gingivalis. Results: The copies of P. gingivalis in the professional periodontal prevention group decreased from 773.62±1,198.09 to 241.40±430.40 after treatment significantly. The control group decreased from 525.22±582.54 to 244.29±385.88 after treatment. The periodontal treatment group showed insignificant change of P. gingivalis. Conclusions: This study showed the professional periodontal prevention was more effective than periodontal treatment in decrease of P. gingivalis.

우리나라 청소년의 치간관리용품 미사용에 관한 요인 (Factors related to non-use of interdental care products by Korean adolescents)

  • Objectives: The objective of the study was to investigate the factors related to non-use of interdental care products (ICP) in adolescents. Methods: The 2019 Korean Youth Risk Behavior Web-based Survey was used, examining 57,303 subjects. Complex-sample chi-square test and logistic regression analyses were performed to identify differences in ICP according to sociodemographic characteristics and oral health behavior and to determine the factors related to non-use. Results: The ICP non-use rate was 70.9%. Factors related to the non-use of ICP were middle or low economic status (OR=1.367 and 1.677). The other significant variables were less than one tooth brushing per day (OR=1.542), no scaling within 12 months (OR=1.631), and receiving dental treatment either once or two times or more (OR=1.757 and 2.195). Conclusions: To increase the ICP usage rate, continuous attention is needed from schools and families in addition to interventions at the national level.

한국 성인의 구강건강 특성과 우울의 관련성 : 2021년 지역사회건강조사 자료분석 (Effect of oral health on depression : an analysis of the 2021 Korea Community Health Survey )

  • Objectives: This study aimed to identify factors related to oral health and depression in Korean adults and contribute to the development of a mental health program to improve depression through oral health management. Methods: Data was obtained from the 2021 Korean Community Health Survey. The chi-square test was used to determine the differences in depression experience relative to general participant characteristics and their oral health. To determine the odds ratio and 95% confidence interval, multiple logistic regression analysis was used. All statistical analyses were performed using the SAS software (version 9.4). Results: The results suggest that depression may be influenced by gender, age, household generation, drinking habits, subjective health level, moderate to high exercise, breakfast, high blood pressure, diabetes, subjective oral health level, chewing discomfort, and tooth brushing. An increasing level of depression was associated with decreasing subjective oral health level (1.34 times), uncomfortable chewing (3.08 times), and frequency of toothbrushing after lunch or before going to bed (1.23 times and 1.58 times, respectively). Conclusions: Our study confirmed a close relationship between oral health and mental health. In developing health programs for improving depression, appropriate oral health care should be considered.

한국 청소년의 스마트폰 사용과 구강질환 증상과의 관련성 (Relationship between hours of smartphone use and oral disease symptoms in Korean adolescents)

  • Objectives: This study aimed to determine the relationship between hours of smartphone use and oral disease symptoms in Korean adolescents. Methods: In this study, data from the 2022 Youth Health Behavior Online Survey were used. The final analysis was conducted on students who responded that they have used a smartphone (N=50,375). Chi-squared test and multivariate logistic regression analysis were used to analyze the relationship between oral disease symptoms and smartphone use time (p<0.05). Results: Consequent to examining the effect of smartphone use on oral disease symptoms, it was found that oral symptoms, such as pain during chewing and tingling throbbing, increased as smartphones were used for >4 h (p<0.05). Additionally, oral symptoms, such as gingivalgia bleeding and bad breath, appeared to increase as smartphones were used for >6 h (p<0.05). However, using smartphones for <0-2 h or 2-4 h had no effect on oral disease symptoms. Conclusions: In adolescents, oral disease symptoms increased significantly with increasing hours of smartphone use. Therefore, adolescents should be guided so that they use smartphones for <2 h or >4 h. Moreover, guidance on forming the habit of tooth brushing after eating food should be provided to prevent oral disease symptoms.

Digital Shade Analysis System을 이용한 전치부의 색조 특성에 관한 연구 (SHADE ANALYSIS OF ANTERIOR TEETH USING DIGITAL SHADE ANALYSIS SYSTEM)

  • Statement of problem : A scientific examination and understanding of overall aspects of the natural dentition is the first step involved in making a satisfactory selection in the shade of an aesthetic prosthesis. Proper natural shade selection of the prosthetic restoration that is in harmony with the remaining dentition is as important aesthetically, as harmony of form and function in the anterior dentition. Clinically, the most commonly applied method of shade selection has been visual, but because of the subjective nature inherent to this method, shade selection results are variable and can be influenced by such factors as the technician, the type of shade guide used, and the type and intensity of the lighting. Purpose : The purpose of this study was to develop a more objective and scientific approach to examining and understanding the shade of teeth, which has in turn lead to the development of a number of shade analysis devices that present a more objective method of shade analysis. Material and Method : In this study, the shades of healthy anterior teeth were examined and analyzed using the recently developed digital shade analysis of the $ShadeScan^{TM}$ System. The study examined 80 individuals in their twenties, 40 males and 40 females, presenting 6 healthy, unrestored maxillary anterior teeth. Tooth brushing and oral prophylaxis were performed prior to evaluation. The ShadeScan handpiece was used to acquire images of the 6 maxillary anterior teeth. These images were analyzed using the Vita/Classical mode of the $ShadeScan^{TM}$ Software, and shade maps of each tooth were acquired and divided into cervical, middle, and incisal thirds. The shade distribution of each third, left and right symmetry, and gender differences were investigated and analyzed. Results : The results of the study are as follows : 1. An overwhelming majority of the examined teeth were found to possess shades belong to Group A, with the greatest variations occurring at the middle and cervical thirds of the maxillary central and lateral incisors, in both male and female subjects. 2. Canines of both male and female subjects showed left and right symmetry with uniform shade distribution of A4 and C4, while the lateral and central incisors showed left-right symmetry of the incisal 1/3 with a uniform shade distribution of A2 and A3 shades 3. No significant differences in shade distribution were seen between genders in maxillary canines, whereas maxillary central and lateral incisors showed differences at the middle and cervical thirds between male and female subjects The results of this study show that with the exception of maxillary canines, maxillary anterior teeth display a diverse shade distribution as well as gender differences. Conclusion : Clinically, when making a shade selection using the existing shade guide, one must consider the fact that even a single tooth consists of a variety of shades. The results of this study show that when selecting a shade from a number of groups is difficult, shades from A group are the most consistent with the natural shade or maxillary anterior teeth.

18개월 유아의 섭식, 구강위생 관리실태 및 구강상태에 관한 기초조사 (A PILOT SURVEY ON THE STATE OF FEEDING, ORAL HYGIENE CARE TOOTH ERUPTION AND CARIES IN 18-MONTH OLD INFANTS)

  • 본 조사는 소아치과에서 중요한 시기인 생후 18개월 유아에 대한 체계적인 연구를 위한 기초 조사의 일환으로 부산대학교 병원과 일신기독병원에서 출생한 생후 18개월 유아 154명을 대상으로 구강검진을 하고 그 보호자들을 대상으로 직접 면담을 하여 수유 및 섭식실태, 구강위생관리 실태, 치아맹출과 우식 실태를 조사하여 다음과 같은 결과를 얻었다. 1. 73%의 유아에서는 여전히 수유가 시행되고 있었으며 그 중 82%는 수면중 수유하거나 수유후 적절한 구강위생관리 없이 잠드는 상태였다. 2. 고형간식은 과일 비스킷류, 치즈, 빵, 사탕류의 순으로, 음료는 물, 유산균 발효유, 우유, 가당쥬스, 생과일 쥬스의 순으로 많았으며, 전체의 61%가 불규칙한 섭취를 하고 있었다. 3. 구강위생관리에 있어서는 92%가 어떤 방식으로건 관리를 하고 있었고, 유아용 칫솔을 이용하는 경우가 가장 많았으며, 그 시작시기는 평균 13.8개월, 1일 1.6회를 실시하고 있었고 잇솔질의 주체는 유아가 흉내낸 후 보호자가 마무리하는 경우가 77%로 대부분을 차지하였다. 4. 모든 유절치는 맹출된 상태였으며 제1유구치는 86%, 유견치는 66%의 유아에서 관찰되었고 개인별 치아의 수는 평균 14.1개였다. 5. 우식 이환율은 27.3%이었고 전체우식의 73%는 상악 유절치에 나타났으며 dmft index는 0.97이었다. 본 조사를 통하여 이 연령대 유아 보호자들을 대상으로 적절한 이유시기, 올바른 간식습관과 구강위생방법에 대한 교육이 절실함을 알게 되었다.

Dextranase 함유 구강 세정액의 치태 억제 및 치은염 예방 효과에 관한 임상적 연구 (A Clinical Trial of Dextranase-Containing Mouthwash on the Inhibition of Plaque Formation and Gingivitis)

  • A novel glucanhydrolase(DXAMase) from a mutant of Lipomyces starkeyi(KSM 22) has been shown effective in hydrolysis of mutan, reduction of mutan formation by Streptococcus mutans and removal pre-formed sucrose-dependentadherent microbial film and DXAMase has been strongly bound to hydroxyapatitie. These in vitro properties of Lipomyces starkeyi DXAMase are desirable for its application as a dental plaque control agent. This study was performed to determine the adjunctive oral hygiene benefits and safety of dextranase(Lipomyces starkeyi KSM 22 DXAMase)-containing mouthwash when used alongside normal tooth-brushing. This 6-month clinical trial was placebo-controlled double-blind design evaluating 1U/ml dextranase mouthwash and 0.12% chlorhexidine mouthwash. A total 39 systemically healthy subjects, who had moderate levels of plaque and gingivitis were included. At baseline, 1, 3 and 6 months, subjects were scored for plaque accumulation(Turesky modification of Quingley-Hein's plaque index), gingivitis status($L\ddot{o}e$ and Silness gingival index), and tooth stain(Area and severity index system by Lang et al). Additionally, oral mucosal examinations were performed and subjects questioned for adverse symptoms. Two weeks after pre-experiment examinations and a professional prophylaxis, the subjects provided with allocated mousewash and instructed to use 20-ml volumes for 30s twice daily after toothbrushing. All the groups showed significant increase in plaque accumulation since 1 month of experiment. During 6 months' period, the Dextranase mouthwash group showed the least increase in plaque accumulation, compared to the Chlorhexidine mouthwash and placebo groups. As for gingival inflammation, all the groups showed significant increase during 6 months of experiment. The Experimental group(Dextranase mouthwash) also showed the least increase in gingival index score, compared to the Positive control(Chlorhexidine mouthwash)as well as the Negative control(placebo)groups. Whereas the tooth stain was increased significantly in the Positive control group, compared to the baseline score and the Negative controlgroup since 3 months of mouthrinsing. It was significantly increased after 6 months in the Experimental group, still less severe than the Positive control group. As for the oral side effect, the Experimental group showed less tongue accumulation, bad taste, compared to the Positive control group. From these results, mouthrinsing with Lipomyces starkeyi KSM 22 dextranase provided adjunctive benefits to toothbrushing, comparable to 0.12% chlorhexidine mouthwash in inhibition of plaque accumulation and gingival inflammation and local side effects were if anything less frequent and less intense than chlorhexidine, with long-term use of the mouthwash. All data had provided positive evidence for Lipomyces starkeyi KSM 22 dextranase as an antiplaque agent and suggested that further development of dextranase formulations for plaque control are warranted.

일부 농촌지역 주민들의 OHIP-14와 EQ-5D에 관한 연구 (A study on OHIP-14 and EQ-5D of residents in some rural areas)

  • Objectives : OHIP-14 and EQ-5D were used, targeting the residents of farming communities to identify the elements that influence oral cavity's health and quality of life due to health and to identify the importance of oral cavity's health in order to increase health of adults' oral cavity and quality of life via improved health. Methods : This research was conducted from July 17th, 2010 to August 16th, 2010 targeting 600 residents in Goryeong-gun, Gyeongsangbuk-do, aging over 40. The data has been analyzed using Mann-Whitney U test, Kruskal-Wallis test and hierarchical multiple regression through SPSS Win Program 18.0 version. Results : 1. OHIP-14 and EQ-5D based on general characteristics showed lower oral health-related quality of life and health-related quality of life on the following cases: women (p=0.004, p<0.001), older (p<0.001, p<0.001), lower scholastic ability (p<0.001, p<0.001), lower average of average spending money (p<0.001, p<0.001), higher number of chronic disease (p<0.001, p<0.001), less drinking (p=0.012, p=0.008), lower perceived oral health and health status (p<0.001, p<0.001) and non smoking showed only EQ-5D (p<0.001). 2. OHIP-14 and EQ-5D based on oral health behavior showed lower oral health-related quality of life and health-related quality of life on the following cases: no periodic oral check-up (p<0.001, p<0.001), less experience of oral health education (p<0.001, p<0.001), horizontal tooth-brushing method(p<0.001, p<0.001) and lower frequency of tooth-brushing showed only OHIP-14 (p=0.042). OHIP-14 and EQ-5D based on oral health status and subjective oral symptom showed lower oral health-related quality of life and health-related quality of life on following cases: number of existing tooth less than 20 (p<0.001, p<0.001), the number of missing teeth more than 9 (p<0.001, p=0.044), DMFT (Decay, Missing, Filling Teeth) index more than 18 (p<0.001, p<0.001), wears denture (p<0.001, p<0.001), edentulous (p<0.001, p=0.002), have xerostomia (p<0.001, p<0.001) and have chewing discomfort (p<0.001, p<0.001). 3. Factors affecting OHIP-14 were gender, age, perceived oral health status, perceived health status, number of existing teeth, dental status, xerostomia and chewing discomfort, and the of reliability (how well it explains) the final model was 48.7%. EQ-5D showed relevance on gender, age, presence of chronic disease, perceived health status, xerostomia, chewing discomfort and oral health-related quality of life, and the reliability of the final model was 42.9%. Conclusions : In order to improve the quality of life of ruralists, oral health needs to be improved or remained by increasing the rate of possession of the existing teeth and preventing the loss of teeth. In order to do so, improvement of accessibility of dental clinic, change of direction from treatment-centered to prevention-centered health care system, development of oral health education program and various oral health care policies which would vitalize continuous oral health care system are considered to be necessary.
