한열허실변증(寒熱虛實辨證)을 이용한 비정격(脾正格) 적응증 고찰 (A Literature Study on the Application of Spleen Tonifying Sa-Am Acupuncture in the Diagnosis of Cold-Heat & Deficiency-Excess)

  • Objectives & Methods : We investigated the cases for which spleen tonifying Sa-Am acupuncture was applied in ${\ulcorner}$Sa-Am-chim-gu-jeong-jeon(舍巖鍼灸正傳)${\lrcorner}$ and classified them according to the concepts of cold-heat & deficiency-excess for a better understanding of the application of spleen tonifying Sa-Am acupuncture. Results : 1. Tonification of Spleen Sa-Am acupuncture can be used to treat heat syndromes such as deficient fire caused by collapse of the spleen qi, stagnation of evil qi or spleen yin deficiency, by way of tonifying spleen qi, activating the flow of qi and blood or harmonizing spleen yin and yang. 2. Tonification of Spleen Sa-Am acupuncture can be used to treat cold syndromes such as weakness of spleen yang by way of promoting fire to generate earth, strengthening spleen yang and raising the clear yang. 3. Tonification of Spleen Sa-Am acupuncture can be used to treat deficiency syndromes such as malnutrition or poor appetite caused by spleen deficiency by way of tonifying spleen qi. 4. Tonification of Spleen Sa-Am acupuncture can be used to treat excess syndromes such as stagnation of qi and blood, abdominal mass or toxication by way of tonifying spleen qi and promoting the flow of qi and blood.

"방약합편(方藥合編)" 수록(收錄) 처방(處方) 내(內)의 약물(藥物) 조합 빈도(頻度) 연구 (A study on the frequencies of medicinal herb combinations in the prescriptions of "Bangyakhappyeon(方藥合編)")

  • Through analyzing the frequencies of medicinal herb combinations in the prescriptions of "Bangyakhappyeon(方藥合編)", we could understand the characteristics of prescription composition and author's medical thoughts. So we conducted this study that analyze combination of 324 medicinal herbs in 467 prescriptions, and found meaningful combinations of 5,045. Among these, the most meaningful combination is the one of 10 herbs, Glycyrrhiza uralensis Fischer, Angelica gigas Nakai, Ziziphus jujube Mille, Atractylodes japonica Koidzumi, Poria cocas Wolf, Zingiber officinale, Rehmanniae radix preparata, Cinnamomum loureirii, Panax ginseng C.A.Meyer, Astragalus membranaceus. This combination is Sibjeondaebo-tang(Shiquandabu-tang) without Cnidium officinale and Paeonia japonica which compose Samul-tang(Siwu-tang). It means this combination is kind of modified Sibjeondaebo-tang(Shiquandabu-tang), which is more effective in tonifying qi than tonifying blood. Prescriptions of "Bangyakhappyeon(方藥合編)" involve herbs mostly used to improve weak and frail health and the herbs which are more effective in tonifying qi than tonifying blood. Through this study, we know the author' s one of medical view points on tonifying qi and blood, attaching importance to tonifying qi relatively to tonifying blood.

장자화(張子和) 보법(補法)에 대한 고찰(考察) - 이평위기(以平爲期) 사상을 중심(中心)으로 - (A Study on the Reinforcement Method of Zhang Zihe - Focusing on the Theory of Keeping Balance -)

  • Objectives : The purpose of this paper is to examine Zhang Zihe's reinforcement method based on the theory of keeping balance(以平爲期). Methods : Based on his arguments in the Rumenshiqin and formula compositions, Zhang Zihe's reinforcement method was examined. Results & Conclusions : 1. Despite preconceptions that Zhang Zihe must have been critical of reinforcement methods due to his alliance to the offensive purgative group, in reality his criticism was not aimed at reinforcement methods entirely, but rather at warming and tonifying. 2. Zhang Zihe's criticism of warming and tonifying was based on his research of the Neijing, in which he realized the pathogenic effects of fire-heat and the side effects of warming and tonifying in clinical practice. This is a common conception among the Four Great Doctors of the Jin-Yuan period. 3. Zhang Zihe focused on eliminating pathogenic qi so that the body's qi would circulate smoothly to slowly restore the source qi. The theory of keeping balance was emphasized to achieve this goal. 4. Based on the theory of keeping balance, Zhang Zihe used various medicinals with different properties and flavors to allow for the medicinals in each of the sovereign, minister, assistant and courier positions to co-operate with each other, while using food reinforcement to restore the source qi.

병후(病後) 소변빈수(小便頻數)에 대한 문헌적(文獻的) 고찰(考察) (The Literature Study of Convalescent thamuria of children)

  • Objective : This study was designed to investigate causes, symptoms and treatments for the convalescent thamuria of children with oriental medical literatures. Methods : We surveyed the oriental and western medical books from to recent published books that have articles on thamuria from one's illness. Results and Conclusions : The symptoms of convalescent thamuria of children is frequency of urination and no pain. The causes of convalescent thamuria of children are the deficiency of the spleen and pulmonary qi, the weakness of the kidney qi and inner heat caused by the deficiency of yin. The methods of treatment are tonifying the spleen and pulmonary(補益脾肺), warming the kidney and tonifying qi(溫補腎氣) and tonifying yin and removing heat(滋陰淸熱). The herbal-medications for treatment are Bojungikqitang and Onpaetang (補中益氣湯合溫肺湯), Wuquihwan(右歸丸) and Jayinganghwatang(滋陰降火湯).

류하간(劉河間)의 양신수(養腎水) 치법(治法)에 대한 고찰(考察) - 주단계(朱丹溪) 보신(補腎) 치법(治法)과의 비교(比較)를 통하여 - (A Study on the Kidney Fluid Nourishing Treatment of Liu Hejian - Through Comparison with Kidney Tonifying Treatment of Zhu Danxi -)

  • Objectives : This paper compares and analyzes Liu Hejian's kidney fluid nourishing treatment methodology with Zhu Danxi's kidney tonifying treatment methodology. Methods : The two doctors' medical theories and treatment formulas were examined to study how their medical arguments manifested in clinical application. Results : Both doctors emphasized the kidney. Liu pursued the treatment of balance based on the theory of the original qi of the Taiyitianzhen(太乙天眞), while Zhu tried to prevent the frenetic stirring of the ministerial fire by restraining one's desires. In nourishing kidney fluid, Liu sometimes used hot medicinals to treat kidney deficiency patters, where medicinals that tonify the kidney fluid were not defined clearly. Zhu, on the other hand, defined formulas and medicinals that would tonify the kidney yin clearly. Conclusions : The tradition of emphasizing the body's yin qi based on the kidney has been continued from Liu Hejian to Zhu Danxi, during which the pathology of fire and heat were examined thoroughly. In clinical application, various and specific ways of controlling the fire heat were developed.

보기약물(補氣藥物)인 인삼(人參), 황기(黃?), 백출(白朮), 감초(甘草)의 물 추출액이 생쥐 면역세포의 Cytokine분비에 미치는 영향 (Effect of Qi Tonifying herbs, Ginseng, Astragali, Atractylodis, Glycyrrhizae on Mouse Cytokine Secretion)

  • In Oriental medicine the primordial Qi and the defensive Qi are considered as important for immunity. Therefore it is anticipated that the improvement of the primordial Qi and the defensive Qi can enhance the ability of immune cells. This experiment was conducted to investigate how Ginseng Radix, Astragali Radix, Atractylodis Rhizoma Alba, Glycyrrhizae Radix, representative of Qi tonifying herbs, affect the immune system in terms of controlling and balancing immune cells. Using the MTS assay, increased proliferations were observed from herbal treated cells, among which Gins-eng showed the highest proliferation. When splenocytes were activated with anti-CD3 plus herbal extracts, levels of IFN-g and IL-4 were increased but those of IL-2 showed little change compared with the control cells. Levels of IL-2, IFN-g and IL-4 were increased in purified CD4 T cells when activated with anti-CD3/anti-CD28 but at $100\;{\mu}g/ml$ of Astragali and Atractylodis, levels of IL-2 were decreased by 11% and 42%, respectively and those of IFN-g were decreased by 55% and 12%, respectively. Under Th1/Th2 polarizing conditions, levels of IFN-g in Th1 cells treated with herbal extracts were all decreased but when it comes to IL-4, its levels were increased in Ginseng and Glycyrrhizae treated cells but decreased in Astragali and Atractylodis treated cells. Taken together, the data show that compared with other qi tonifying herbs, Ginseng and Glycyrrhizae have a tendency to favor Th2 cell differentiation in vitro.

『동의보감(東醫寶鑑)』과 『동의수세보원(東醫壽世保元)』에 나타난 우리 민족(民族)의 의학정신(醫學精神) (Medical Minds of Trational Korean Medicine in 「DongEuiBoGam」 and 「Dongyi Suse Bowon」)

  • 1. Objectives This research is purposed to present developmental direction of traditional Korean medicine, through summarizing original medical mind of traditional Korean medicine. 2. Methods It was researched as bibliographic study with chief writings of traditional Korean medicine such as "DongEuiBoGam(東醫寶鑑)" and "Dongyi Suse Bowon(東醫壽世保元)" 3. Results and Conclusions (1) Huh Jun regarded Three Precious(三要: 精, 氣, 神) as important factors in Morpho-imaginary viewpoints. So he suggested finding out the pathological states according to the states of Three Precious. (2) As Huh Jun recognized that the essence(精) is the origin of the body, he suggested the medical mind of laying emphasis on the essence. And he suggested that circulating of the essence is important to health and longevity, which is different from previous trational medicne laying emphasis on the tonifying of five organs(五臟). (3) Dong-mu suggested diagnostic method that is to find out "Natural Principle of Organ(臟腑性理)" by observation of "Knowing-Acting(知行)" and "Morphological aspect and Temper(體形氣像)". And he suggested finding out pathological states according to states of "Good Qi of each constitution(體質正氣)". (4) Dong-mu recognized that the disease should be treated by recovering "Good Qi of each constitution" with both pharmacotherapy and non-pharmacotherapy. So he uggested the medical mind of laying emphasis on "Good Qi of each constitution", which is different from previous trational medicne laying emphasis on the tonifying of five organs. (5) To be brief, Huh Jun layed emphasis on the essence and Dong-mu layed emphasis on "Good Qi of each constitution". Emphasizing internal vitality which is applyed to treatment and prevention of disease is the original medical mind of traditional Korean medicine.

동의보감(東醫寶鑑) 내상문(內傷門) 방제(方劑)에 배합(配合)된 풍약(風藥)의 활용(活用)에 대한 고찰(考察) (A Study on the Application of Wind Herbs(風藥) Blended with Prescriptions for Internal Disease Range in Donguibogam)

  • The purpose of this study was to examine how exterior-relieving herbs are blended by each prescription with emphasis on Internal disease range of Donguibogam by limiting Wind herbs in exterior-relieving herbs to apply exterior-relieving herbs to internal diseases. Prescriptions mixed with exterior-relieving herbs is a prescription adding Bojungikgitang, Yijintang, Yimyosan, Yukilsan and Gilgyeongjigaktang. The mixed exterior-relieving herbs were diversely operated according to each prescription or medicines. However, the mixed exterior relieving herbs were mixed for rising yang & tonifying qi(升陽益氣) for the most part. The exterior-relieving herbs mixed with prescriptions had the total 15 kinds, Bupleuri Radix, Cimicifugae Rhizoma among them were mixed with prescriptions for the most part. In addition, two-kind mixed exterior-relieving herbs were utilized in order of Bupleuri Radix Cimicifuga Rhizoma and Bupleuri Radix Cimicifugae Rhizoma Notopterygii Rhizoma Ledebouriellae Radix. Taken together, Wind herbs is applied several purpose, so that it can contribute to treatment and prevention of internal diseases in present-day.

응급의학 측면에서 『의학입문(醫學入門)』 관격(關格)의 분석 (An Analysis of Guange (關格) in 『Yixuerumen (醫學入門)』 from the Perspective of Emergency Medicine)

  • This study examined guange (關格) in Yixuerumen (醫學入門) from a new perspective and evaluated its value in terms of emergency medicine. We compared and analyzed the definition, symptoms, and treatment methods of guange in Yixuerumen with related clinical presentations in modern medicine. Guange includes nausea and vomiting, dysphagia, dysuria, and constipation without diarrhea. A representative disease related to dysuria is uremia, while one related to constipation, nausea, and vomiting is enterocleisis, and and a third related to dysphagia is chronic esophagitis and esophagus cancer. Li Chan, the author of Yixuerumen, recognized guange as an emergency of serious eating and elimination disorder as various symptoms of diseases (病症) gradually get worse from the perspective of external diseases (外感病) and miscellaneous diseases (雜病) and handled it in various ways including yin-tonifying medicinals (補陰藥), qi-tonifying medicinals (補氣藥), qi-regulating formulas (理氣劑), phlegm-dispelling formulas (去痰劑), and "water rising, fire falling" formulas (水升火降) in addition to emetic formulas (催吐劑), diuretic formulas (滲利劑), and interior-attacking formula (攻裏劑). Concepts like guange in Yixuerumen were somewhat useful when handling the applicable emergencies at the level of then medical treatment.

『소문(素問)·위론(痿論)』의 독취양명(獨取陽明)에 대한 고찰 (A Study on 'Specially Selecting Yangming' in 『Suwen·Weilun』)

  • Objectives : The purpose of this paper is to study how medical experts understood 'Specially Selecting Yangming' for clinical application. Methods : After collecting medical texts related to 'Specially Selecting Yangming', its meaning and clinical application were examined thoroughly. Results : 1. The meaning of 'Specially Selecting Yangming' was identified in different ways according to the meaning of 'Yangming.' It was perceived as Stomach, Yangming meridian, Stomach meridian of Foot Yangming, Spleen and Stomach, Intestine and Stomach by medical experts. 2. Chen Wu-ze and Liu Wan-su understood Wei disease as a form of consumption, and regarded 'Specially Selecting Yangming' as tonifying Yangming while Zhang Zi-he put emphasis on Communicating Stomach Qi(胃氣) through vomiting or purgation. Sun Yi-kui insisted on applying it as a precaution. 3. Li Dong-yuan emphasized the cause of Wei disease to be Damp-Heat in the Summer and suggested Qingshuyiqitang and Qingzaotang as remedy. 4. Zhu Zhen-heng's Purging the South and Reinforcing the North is a more fundamental way of treating Wei disease by stimulating the Water-Fire Axis. Conclusions : Through a diachronic study of 'Specially Selecting Yangming' and its remedy, most medical experts regarded 'Specially Selecting Yangming' as tonifying Yangming, but tonifying Liver and Kidney was thought to be a more fundamental treatment.
