Preparation of a Porous Chitosan/Fibroin-Hydroxyapatite Composite Matrix for Tissue Engineering

  • Chitosan, fibroin, and hydroxyapatite are natural biopolymers and bioceramics that are biocompatible, biodegradable, and resorb able for biomedical applications. The highly porous, chitosan-based, bioceramic hybrid composite, chitosanlfibroin-hydroxyapatite composite, was prepared by a novel method using thermally induced phase separation. The composite had a porosity of more than 94% and exhibited two continuous and different morphologies: an irregularly isotropic pore structure on the surface and a regularly anisotropic multilayered structure in the interior. In addition, the composite was composed of an interconnected open pore structure with a pore size below a few hundred microns. The chemical composition, pore morphology, microstructure, fluid absorptivity, protein permeability, and mechanical strength were investigated according to the composition rate of bioceramics to biopolymers for use in tissue engineering. The incorporation of hydroxyapatite improved the fluid absorptivity, protein permeability, and tenacity of the composite while maintaining high porosity and a suitable microstructure.

Effect of Unilateral Pneumonectomy on the Secretory Function of Type II Pneumocyte and Compensatory Growing Pattern of the Residual Lung in Growing Rabbits

  • At the fifth day after right lung pneumonectomy in New-Zealand white rabbits $(0.8{\sim}1.1\;kg\;B.W.)$, phospholipid and protein concentration in the left lung lavage fluid were measured for clarification of the effect of unilateral pneumonectomy on the secretory function of the type II pneumocytes in growing rabbits. In an attempt to evaluate the effect of unilateral pneumonectomy on the compensatory growth of the residual lung, left lung weight and left lung weight-body weight ratio and DNA concentration, RNA/DNA and total DNA content in the left lung tissue were measured in pneumonectomized and in sham operated control rabbits. The lung weight of pneumonectomized rabbit was approximately two times heavier than that of the control rabbits. DNA concentration and RNA/DNA of the lung tissue were not changed but total DNA content was increased significantly. Phospholipid concentration in the lung lavage fluid of the pneumonectomized rabbits was over two times higher than that of control rabbits. from these experimental results, It is concluded that unilateral pneumonectomy in growing rabbits might cause to increase the secretion of pulmonary surfactant from type II pneumocyte of the residual lung. The cellular hyperplasia seems to be the primary response of the compensatory growing lung in unilateral pneumonectomized growing rabbits.

T1, T2강조영상, FLAIR영상의 임상 적용 (T1-, T2-weighted, and FLAIR Imaging: Clinical Application)

  • T1, T2강조영상, FLAIR (fluid attenuated inversion recovery) 영상기법은 뇌 MRI의 가장 기본적인 영상기법들이다. T1강조영상은 짧은 TR과 짧은 TE를 이용한 스핀에코 기법으로서 조직의 T1이완시간의 차이를 신호 차이로 반영하는 기법이다. 짧은 TR을 사용하면 조직 간에 종축 자기화의 회복 정도가 크게 차이나게 되며 이를 신호에 반영하는 것이다. T2강조영상은 긴 TR과 긴 TE를 이용한 스핀에코 기법으로서 조직의 T2이완시간의 차이를 신호 차이로 반영하는 기법이다. 긴 TE을 사용하면 조직 간에 횡축 자기화의 붕괴가 크게 차이나게 되며 이를 신호에 반영하는 것이다. FLAIR는 180도 반전펄스를 먼저 가하는 반전회복 (inversion recovery) 기법의 일종으로서 뇌척수액의 신호를 억제하기 위하여 2500 msec 정도의 반전시간을 적용한다.

외상환자 수액 투여에서 알루미늄 호일을 이용한 단열 효과 (The Thermal Insulation of Warm Fluid using Aluminium Foil in Trauma Care)

  • Purpose: The temperature of a warm fluid infused into a patient is lowered because the line that allows the fluid to be infused into the patient is exposed to room air. This study evaluated the effects of aluminum foil used as an insulator surrounding the fluid infusion lines when using warm crystalloid fluids to treat traumatic shock patients. Methods: The study measured the differences in fluid temperature between infusion lines with and without the aluminum-foil insulation. We used 1L of normal saline at $40^{\circ}C$ as the infusion fluid, and the fluid infusion line was 200 cm long. The differences in temperature were measured for various fluid flow rates from 12,000 mL/min to 100 mL/min. We performed three experiments at each flow rate. Results: The results showed the differences in temperature between the groups with and without the aluminum insulation were significant for flow rates above 100 mL/min. Conclusion: Hypothermia in trauma patients results in many adverse complications such as peripheral vascular constriction, tissue hypoxia, metabolic acidosis, heart dysfunction and so on. Thus, the use of warm fluids and blood components is essential to reduce the probability of hypothermia. This study showed the aluminum foil wrapped around the infusion line had an insulator effect. As a result, such a wrapping can be used to avoid the adverse effects of hypothermia.

Prevalence, infection intensity and histopathology ofPerkinsus in Venus clam, Protothaca jedoensis from Yo-su, Korea

  • Venus clam, Protothaca jedoensis was collected in lune 2003 from Yosu, Korea. The prevalence and infection intensity of Perkinsus was determined by Ray's Fluid thioglycollate medium technique and Choi's 2 M NaOH digestive assay. The prevalence of infection was 80% in Yosu; 54.2% in Taejon and 15.5% in Sachon. Consequently, infection intensity was highest in clams from Yosu (6250${\pm}$8188 Perkinsus cells/g of tissue) whereas lower levels were found in Taejon (5072${\pm}$10862 Perkinsus cells/g of tissue) and in Sachon (2452${\pm}$15300 Perhnsus cells/g of tissue). Histological observation showed that Perkinsus presented in mantle, gills, digestive tubes and gonad of clams. The hemocytic infiltrations and tissue damages were found in heavy Perkinsus-infected clams. Results after immunofluorescence staining with antibody developed from Perkinsus atlanticus suggested that there is closed relationship between Perkinsus found in Venus clam and Manila clam.

A Study on Chromosomal Mosaicism Detected through Cytogenetic Analysis

  • Mosaicism is the presence of two or more chromosomally distinct cell lines, each seen in two or more cells. Chromosomal mosaicism presents one of the most difficult problems in prenatal cytogenetic diagnosis, requiring the differentiation of true mosaicism from pseudomosaicism. To overcome associated problems we investigated 24 cases (amniotic fluid 13 cases, abortus tissue 3 cases, peripheral blood 8 cases) in which mosaicism has been found in cytogenetic analysis. 5 cases (38.5%) of 13 amniotic fluid cells in which mosaicisms showed single cell pseudomosaicism. Chromosomal true mosaicism is found in about 0.28% (8/2,826) of amniotic fluid cell cultures. The 24 cases involved 12 cases (50%) with sex chromosomal abnormalities, 7 cases (29.2%) with autosomal structural defects, 3 cases (12.5%) with autosomal abnormalities, 2 cases (8.3%) with a supernumerary marker. Mosaicism detected in amniotic fluid may represent the true mosaicism or may pseudomosaicism. If the same chromosome abnormality is seen in more than one cell and in two different cultures, it is considered a true mosaicism, whereas single-cell abnormalities from a single culture are regarded as pseudomosaicism. In this study, we describe a mosaicism in chromosome analysis, its diagnostic problems and clinical significance.

Role of Muscle Free Flap in the Salvage of Complicated Scalp Wounds and Infected Prosthetic Dura

  • Background The prosthetic dura is an essential element in the protection of the cranial parenchyma and prevention of cerebrospinal fluid leakage. Although prosthetic dura are widely used in neurosurgery, they occasionally provoke infection, which can be a major concern after neurosurgical treatment. However, removal of the prosthetic dura carries a risk of brain parenchyma injury and cerebrospinal fluid leakage. The salvage of infected prosthetic dural material has not been adequately addressed in the literature. In this study, we demonstrate the value of the combination of a meticulous surgical debridement of necrotic tissue and simultaneous muscle free flap for intractable postoperative epidural abscess without removal of the infected prosthetic dura. Methods Between 2010 and 2012, we reviewed the data of 11 patients with persistent infection on the prosthetic dura. The epidural infections each occurred after a neurosurgical procedure, and there was soft tissue necrosis with the disclosure of the underlying prosthetic dura and dead bone around the scalp wound. To salvage the infected prosthetic dura, meticulous debridement and a muscle free flap were performed. Results All 11 patients experienced complete recovery from the complicated wound problem without the need for further surgical intervention. No signs of prosthetic dural infection were observed during the mean follow-up period of 11 months. Conclusions The combination of a meticulous surgical debridement and coverage with a muscle free flap is an effective treatment for salvage of infected prosthetic dura.

Assessment of the Cerebrospinal Fluid Effect on the Chemical Exchange Saturation Transfer Map Obtained from the Full Z-Spectrum in the Elderly Human Brain

  • Purpose: With neurodegeneration, the signal intensity of the cerebrospinal fluid (CSF) in the brain increases. The objective of this study was to evaluate chemical exchange saturation transfer (CEST) signals with and without the contribution of CSF signals in elderly human brains using two different 3T magnetic resonance imaging (MRI) sequences Methods: Full CEST signals were acquired in ten subjects (Group I) with a three-dimensional (3D)-segmented gradient-echo echo-planar imaging (EPI) sequence and in ten other subjects (Group II) with a 3D gradient and spin-echo (GRASE) sequence using two different 3T MRI systems. The segmented tissue compartments of gray and white matter were used to mask the CSF signals in the full CEST images. Two sets of magnetization transfer ratio asymmetry (MTRasym) maps were obtained for each offset frequency in each subject with and without masking the CSF signals (masked and unmasked conditions, respectively) and later compared using paired t-tests. Results: The region-of-interest (ROI)-based analyses showed that the MTRasym values for both the 3D-segmented gradient-echo EPI and 3D GRASE sequences were altered under the masked condition compared with the unmasked condition at several ROIs and offset frequencies. Conclusions: Depending on the imaging sequence, the MTRasym values can be overestimated for some areas of the elderly human brain when CSF signals are unmasked. Therefore, it is necessary to develop a method to minimize this overestimation in the case of elderly patients.

유체전단응력에 의하여 3T3-L1 지방세포가 받는 영향 (Effects of Fluid Shear Stress on 3T3-L1 Preadipocytes)

  • Adipocytes affect obesity through the regulation of lipid metabolism. Physical loading is an important regulator of fat tissue. There are ongoing in vitro studies inducing mechanotransduction on 3T3-L1 preadipocytes with mechanical stimulus in order to treat obesity by inhibiting adipogenesis and provoking cell death. In this study, our goal was to suggest a new therapy for obesity by investigating whether fluid shear stress (FSS) changes transcription factors on 3T3-L1 related with adipogenesis and cell death. FSS loading was applied to 3T3-L1 preadipocytes at 1Pa and 1Hz. After loading, bright field images were taken and an immunofluorescence assay was conducted to observe actin stress fiber formation. Western blot analysis was conducted to identify the activation of the ERK pathway as well as the adipogenic factors, which including C/EBPs and $PPAR{\gamma}$. The expression of osteopontin, a protein related to inflammation in adipose tissue, and cell death related factors, Bax, Bcl-2, and Beclin, were also measured. Results showed that FSS stimulated the formation of actin stress fibers in 3T3-L1 and also that the activation of C/EBPs decreased significantly when compared with the control group. $PPAR{\gamma}$ activation in the 2 hour FSS group was lower than the 1 hour FSS group, which implied that the results were time dependent. Additionally, there were no differences in the expression of cell death factors after FSS loading. In summary, similar to other fibroblasts, the formation of actin stress fibers induced by mechanotransduction may affect the differentiation of 3T3-L1, leading to inhibition of adipogenesis and inflammation.

개 퇴행성 관절염 모델을 이용한 연골과 활액 내 단백질 분해 효소와 억제제의 작용 연구 (Proteinases and their Inhibitors in Cartilage and Synovial Fluid Acquired from a Canine Osteoarthritic Model)

  • 퇴행성관절염(Osteoarthritis,OA)은 관절 부위의 퇴행성 변화가 특징이며, 이를 진단하기 위해서는 연골세포나 활액에서 유래된 표지인자가 일반적으로 사용된다. 이번 연구에서는 개를 이용하여 실험적으로 퇴행성관절염을 유도하고, 활액과 연골세포에서 단백질 분해 효소인 matrix metalloproteinase(MMPs)와 MMPs의 활성을 억제시키는 것으로 알려진 tissue inhibitors of metalloproteinases(TIMPs)의 발현 정도를 조사하였다. 20마리의 비글견이 퇴행성관절염 모델로 사용되었으며 MMP-2 와 -9은 Western blot 분석에 의해서, TIMP-2의 농도는 ELISA(enzyme-linked immunosorbent assays)에 의해 결정하였다. 퇴행성관절염 유도 4주 후에 연골에서 분리한 연골세포에서 MMP-2의 발현은 증가되었지만 MMP-9의 발현은 감소되었다. 그러나, 퇴행성관절염을 유도한 개의 활액에서는 MMP-2와 -9의 발현이 모두 증가하는 것을 보였다. TIMP-2의 농도는 퇴행성관절염을 유도한 연골에서 분리한 연골세포에서는 높았지만, 활액에서는 낮은 농도를 보였다. 이러한 결과는 MMP-9가 퇴행성관절염 시 연골 조직의 변성에 따른 연골세포의 손상에 의해 MMP-2보다 더 활액으로 방출된다는 것을 보여주며, 활액 내 TIMP-2의 감소에 따른 MMPs의 활성이 퇴행성관절염을 더욱 악화시키는 것을 제안해준다. 결국 MMPs의 활성은, 특히 MMP-9, 개의 퇴행성관절염의 조기 진단과 치료를 위한 표지인자로서 사용할 수 있을 것으로 사료된다.
