Investigation of Risk Factors of Low Serum 25-hydroxyvitamin D Levels in Korean Menopausal Women (혈청 25-hydroxyvitamin D 수준이 낮은 폐경 후 여성에서 나타난 위험 인자 분석)

  • To define the risk facots of subclinical vitamin D deficiency in postmenopausal women, circulating serum 25-hydroxyvitamin D levels, known to be the indicator of vitamin D status, were measured and risk faxtors affecting it were investigated in 27 women with low 25-hydoxyvitamin d level(ie, the study group : serum 25-hydroxyvitamin D<10ng/ml) by comparing to 55 age-matched control(the control group) were analyzed. The serum level of 25-hydorxyvitamin D was analyzed by HPLC(High Pressure Liquid Chor-matography) and analyzed biochemical parameter. The following information was obtained by interviews : sociodemographic charateristics, the intake of food containing vitamin D, proxy measure of sunlight exposure(time spent outdoors), and reprocuctive histpry of the subjects. The study group had significantly lower levels of serum calcium, increased levels of iPTH and alkaline phosphatase, Among the dietary determents, energy, protein, fat, calcium, phophorus, and vitamin D intakes were lower in the study group than the normal group. The time spent outdoors in a day was not significantly different between the two groups. However, during the day, a specific time of time spent out doors between 12:00 and 14:00 was significantly lower in the study group. Logistic analysis revealed that vitamin D and calcium intake were more important affecting factors than the time spent outdoors in post menopausal women.

A Study on Vitamin D Status and Factors Affecting It in Young Adults (정상 성인의 혈청 Vitamin D 수준과 이에 영향을 주는 변인에 관한 연구)

  • In this study, the serum level of 25-hydroxyvitamin D(25-(OH)D) was measured by high pressure liquid chromatography(HPLC), and factors affecting it were investigated in 72 young adults age ranging from 21 years to 39 years with normal bone density. The mean level of serum 25-(OH)D was 20.0$\pm$6.8ng/ml in males and 26.1$\pm$12.3ng/ml in females, which was significantly higher in females (p<0.01). The serum level of parathyroid hormone(PTH) showed a negative correlation with that of 25-(OH)D(p<0.05). Time spent outdoors in a day correlated positively with the serum level of 25-(OH)D(p<0.01). During the day, a specific time between 12:00 a.m. and 2:00 p.m. showed the most significant correlation with the level of 25-(OH)D(p<0.005). Among the nutrients studied, fat and vitamin D intake were positively correlated with the serum 25-(OH)D level. Stepwise multiple regression analysis showed that the serum level of 25-(OH)D could be fit by vitamin D intake(34.7% explained), serum PTH level (27.3% explained) and the time spent outdoors during the specific time(28.4% explained).1996)

The Effect of Vitamin D Status on Bone Mineral Density of Korean (한국 성인의 Vitamin D 영양 상태가 골밀도에 미치는 영향)

  • This study was carried out to evaluate the effects of vitamin D nutritional status on bone mineral density of adults (21-49 years). To attain the aim, we measured bone mineral density (BMD) of the subjects at distal radius by single-photon absorptiometry (SPA). Serum level of 25-hydroxyvitamin D(25-(OH)D) , known to be the best indicator of indicator of vitamin D status in humans was analyzed . The factors affecting this vitamin D level were also investigated in autumn in 122 young adults. Serum level of 25-(OH)D was measured by high pressure liquid chromatography(HPLC) and biochemical variables, general health status, time spent outdoors, and dietary intakes of the subjects. BMD of the male subjects was significantly greater than that of female subjects. Weight, activity and total energy expediture (TEE) showed a positive correlation with distal BMD. The mean level of serum 25-(OH)D was 24.4$\pm$11.0 ng./ml and by sex, 26.0$\pm$6.8ng/ml for males and 23.3$\pm$12.3ng/ml for females , the level was significantly higher in male (p<0.01). there was significant correlation between BMD at distal-radius and s-25(OH)D levels (p<0.001). The serum level fo parathyroid hormone (PTH) showed a negative correlation with BMD(p<0.05), with the more obvious correlation in females. Vitamin D intake was estimated to be 3.75$\pm$2.19ug/day in average. Among the nutrients studied, protein ,fat, calcium , and vitamin D intake were positively correlated with distal BMD. When food frequencies were concerned , milk and dairy products showed a significant positive correlation with the BMD level, and driedfoods, eggs , fats and oils, and cereals also showed a positive correlation. Time spent outdoors was estimated to be about 70 minutes in average and positively correlated with the distal BMD level(p<0.01). During the day, the specific time between 12 :00pm and 2:00pm showed the most significant correlation with BMD (p<0.001). Multiple regression analysis with the variables showed that distal BMD could be fit 31.9% by the time spent outdoors a day, intake of Ca and vitamin D, and TEE. The standardized estimates were 0.344 for vitamin D intake, 0.284 for Ca intake 0.179 for the time spent outdoors a day and 0.273 for TEE. For males, s-25*OH)D level, TEE and time spent outdoors during a day showed a significant correlation. For females, intake of Ca and vitamin D could fit about 27.1% of the distal BMD.

  • The purpose of this study was to investigate women's indoor and outdoor everyday life including their housework, labor and leisure of the late Chosen period as expressed in paintings. Women's everyday life was classified into their indoor and outdoor life according to the custom of Naeoe, which had confined women's activities in the outdoors. This study analyzed 93 Paintings depicting the Due life of People. As a result, even though the Chosen period was a conservative society dominated by Confucian custom that confined women in the home, most women spent much of their time in the outdoors carrying water, washing clothes and doing farm work. Some common women participated in marketing household product, peddling and managing a drink shop. Especially, servant women such as Kineo, Muneo and Youneo labored in Public offices or Private Places. Consequently, this study suggests that the misconception should be corrected that traditional women's sex role had been confined exclusively in the home.

  • Vitamin D is an essential component of bone and mineral metabolism; its deficiency causes growth retardation and skeletal deformities in children and osteomalacia and osteoporosis in adults. Hypovitaminosis D (vitamin D insufficiency or deficiency) is observed not only in adults but also in infants, children, and adolescents. Previous studies suggest that sufficient serum vitamin D levels should be maintained in order to enhance normal calcification of the growth plate and bone mineralization. Moreover, emerging evidence supports an association between 25-hydroxyvitamin D (25[OH]D) levels and immune function, respiratory diseases, obesity, metabolic syndrome, insulin resistance, infection, allergy, cancers, and cardiovascular diseases in pediatric and adolescent populations. The risk factors for vitamin D insufficiency or deficiency in the pediatric population are season (winter), insufficient time spent outdoors, ethnicity (non-white), older age, more advanced stage of puberty, obesity, low milk consumption, low socioeconomic status, and female gender. It is recommended that all infants, children, and adolescents have a minimum daily intake of 400 IU ($10{\mu}g$) of vitamin D. Since the vitamin D status of the newborn is highly related to maternal vitamin D levels, optimal vitamin D levels in the mother during pregnancy should be maintained. In conclusion, given the important role of vitamin D in childhood health, more time spent in outdoor activity (for sunlight exposure) and vitamin D supplementation may be necessary for optimal health in infants, children, and adolescents.

Solution to Slow Down Myopia Progression

  • Purpose: To examine the effectiveness of various treatments; bifocal spectacles, orthokeratology, atropine, and time spent in outdoors; in slowing down the myopia progression for Asian adolescents (6-18age). Methods: The research focused on examining the most effective treatment in controlling myopia based on the literature sources that have been published. Through meta-analysis of various research papers that already has been done in this field, a lot of data was collected. For each treatment, the difference in axial length and spherical equivalent over time was measured and recorded. To quantitatively record the difference, both axial length and spherical equivalent was determined by value of control group value of treatment group. The paper compared the effectiveness of each treatment based on the data that was measured. Results: Adolescents who chose to spend time outdoors in order to slow down myopia progression had axial length difference of 0.03 mm and spherical equivalent difference of 0.17 D. Adolescents that used atropine had axial length difference of 0.36 mm and spherical equivalent difference of 0.92 D. Bifocal spectacle resulted in axial length difference of 0.21 mm and spherical equivalent difference of 0.59 D, and for orthokeratology 0.23 mm and 0.04 D, respectively. Axial length wise, myopia was most controlled by the atropine since there was a greatest difference between the group that got the treatment and the group that did not have the treatment. According to the spherical equivalent difference data, myopia was most controlled by atropine. Conclusion: Atropine showed the most effective result in controlling myopia among the four treatment. Again, compared to other three treatment, using atropine appeared to have greatest ability in slowing down myopia progression since adolescents who were treated with atropine had greatest difference from adolescents in the control group that had the same condition but didn't get the treatment. However, every treatment was only used for 2 or 3 years which is quite short time period to measure the long term effect of the four treatments. Also, since atropine is a pharmaceutical method to control myopia, it may harm adolescents' eyes compared to optical or environmental treatment.

Estimating Personal Exposures to Air Pollutants in University Students Using Exposure Scenario (노출 시나리오를 이용한 대학생들의 유해 공기오염물질 노출 추정)

  • Studies evaluating the health effects of hazardous air pollutants assume that people's exposure to typical pollutant level is the same as specific regional pollutant level. However, depending on social and demographic factors, time-activity pattern of people can vary widely. Since most people live in indoor environments over 88% of the day, evaluating exposure to hazardous air pollutants is hard to characterize. Objective of this study was to estimate the exposure levels of university students of $NO_2$, VOCs(BTEX) and $PM_{10}$ using the scenarios with time-activity pattern and indoor concentrations. Using data from time-use survey of National Statistical Office in 2009, we investigated time-activity pattern of university students and hourly major action. A total of 1,057 university students on weekday and 640 on weekend spent their times at indoor house 13.04 hr(54.32%), other indoors 7.70 hr(32.06%), and transportation 2.36 hr(9.83%). Indoor environments in which university students spent their times were mainly house and school. Air pollutants concentrations of other indoor environments except house and school such as bar, internet cafe and billiard hall were higher than outdoors, indicating that indoor to outdoor ratios were above 1. According to three types of exposure scenarios, exposure to air pollutants could be reduced by going home after school.

Noise Exposure according to the Time Activity Pattern and Duties of Firefighters (소방 공무원의 시간활동 양상과 직무에 따른 소음 노출 특성)

  • Objectives: The purpose of this study is to evaluate the noise exposures of firefighters according to their time-dependent activity patterns. Methods: Personal exposure levels were measured for six days and nights using noise dosimeters; three days and nights for on-duty tasks, the other days and nights for off-duty activities. Results: The total amount of time spent in the workplace was 13,677 min (67%), outside areas 4,833 min (23%), in transit 1,002 min (5%), and other indoor area 807 min (4%) during a working period. However, during off-days they spent 10,858 min (76%) at home, 1,382 min (10%) outdoors, 1,225 min (9%) other indoors, and 493 min (3%) in transit. As a result of individual exposure levels, TWA did not exceed 90 dBA of the occupational exposure limit for the majority of the firefighters, whereas the levels of Lmax were 119 dBA, which were higher than the noise levels of firefighters in USA. Sometimes during dispatching the levels of Lpeak exceeded the ACGIH exposure standard (140 dBC). The Leq levels in transit were higher than the levels in home and other indoors even though the activity time is short. Conclusions: This paper characterized the noise exposure patterns of firefighters in Korea. We suggest that special noise sources, including sirens and speaker phones, should be readjusted to reduce noise exposure.

Exposure Assessment and Estimation of Personal Exposure for Nitrogen Dioxide Using Time Weighted Average Model (시간가충치 평균모델을 이용한 이산화질소의 노출평가 및 예측)

  • Indoor and outdoor nitrogen dioxide(NO$_2$) concentrations of 122 houses were measured and compared with measurements of personal NO$_2$ exposure simultaneously . Time activity patterns were used to determine the impacts on NO$_2$ exposure assessment and time weighed average model to estimate the personal NO$_2$ exposure. Most people spent their times more than 80% of indoor and more than 50% in home, respectively. Personal NO$_2$ esposure was found to be significantly associated with both indoor NO$_2$ concentration(r=0.70) and outdoor NO$_2$ concentration (r=0.68). Using time weighted average model, personal NO$_2$ exposure was estimated with NO$_2$ measurements in indoor home, indoor workplace and outdoor home. The estimated NO$_2$ measurements were significantly correlated with measured personal exposures(r=0.69, N=122). For the difference between measured and estimated NO$_2$ exposures by multiple regression analysis showed that NO$_2$ concentrations in near workplace and other outdoors of no NO$_2$ measurements affected the personal NO$_2$ exposures(p=0.023).

Personal Carbon Monoxide Exposures and Carboxyhemoglobin Levels of the Traffic Policemen (일부 도시 교통경찰관의 일산화탄소 개인 노출과 혈중 카르복실헤모글로빈에 관한 연구)

  • The number of motor vehicles in Korea has increased to about 4 million, and the exhaust gases of these vehicles have become a more threatening factor to public health. Traffic policemen are one of the highest health risk groups since they work on roadsides where they are exposed to high levels of air pollutants. The health effects on them due to air pollution were determined by measuring personal carbon monoxide(CO) exposure and carboxyhemoglobin(COHb) level in blood. Thirty-one traffic policemen in Seoul volunteered to be subjects of the study. In October 1992, personal CO exposure was measured by a CO passive sampler. The subjects wore the CO passive sampler for 8 hours while on duty. The exposed samples were analyzed by gas chromatography. Blood samples from each subject were collected just after the exposure sampling, and were analyzed within 3 hours of blood collection by a CO-oximeter. The activities of the subjects were recorded by the subject in 30 minute intervals using an activity log sheet containing location and time spent. Personal CO exposure were ranged between 0.1 and 14.5ppm, eith an average of 5.9ppm. Carboxyhemoglobin levels ranged from 1.1% to 6.9%, with an average of 3.6%. policemen on duty outdoors had significantly higher CO exposures and COHb levels than policemen on duty indoors(p<0.01). Personal CO exposure and COHb were positively correlated, although the coefficient was not significant. The relationship between CO level and COHb level was confounded by smoking status. Among smokers, COHb level was significantly higher as CO exposure and hours worked outdoors increased.
