The Comparison of Muscle Contraction Time and Modulation Ratio during Reaching Tasks in Hemiparalysis, Elderly and Young Adults

  • Objective: The purpose of this study was to evaluate the Electromyography (EMG) of the upper limb during reaching tasks according to two heights in the sitting position. Design: Cross sectional design Methods: Fifteen hemiplegia, fifteen elderly, and fifteen healthy subjects have participated in this study. The targets (90% length of the subject's arm) were located at the two heights (the eye and xiphoid process). We have recorded EMG signals of seven upper limb muscles (anterior deltoid (AD), posterior deltoid (PD), pectoralis major (Pec), infraspinatus (Inf), supraspinatus (Sup), biceps brachii (Bi), triceps brachii (Tri)). The dependent variables were movement time(s), modulation ratio, working ratio, and the co-contraction ratio of the hemiplegia, elderly, and healthy at the reaching task. Two-way repeated-measures ANOVA (2-heights) was analyzed with the LSD post hoc test. Results: The study results were as follows: (1) The movement time to the target during reaching movement was significantly longer for the hemiplegia and elderly groups compared to the healthy group. (2) The modulation rate was significantly higher at eye height than the xiphoid height in AD, PD, Pec, Inf, Bi muscles, and the hemiplegia group and elderly group were significantly lower than the healthy group. Additionally, the modulation ratio showed a significant interaction between heights and groups. Conclusions: It is expected that the variables using the muscle contraction characteristics, the evaluation method of this study, can be used as an electromyography-based feedback method that can be objectively evaluated and quantified in clinical practice.

  • The aim of this study was to investigate effects of reaching distance on movement time and trunk kinematics in hemiplegic patients. Eight hemiplegic patients participated in this study. The independent variables were side (sound side vs. affected side) and target distance (70%, 90%, 110%, and 130% of upper limb). The dependent variables were movement time measured by pressure switch and trunk kinematics measured by motion analysis device. Two-way analysis of variance with repeated measures was used with Bonferroni post-hoc test. (1) There were significant main effects in side and reaching distance for movement time (p=.01, p=.02). Post-hoc test revealed that there was a significant difference between 110% and 130% of reaching distance (p=.01). (2) There was a significant main effect in side and reaching distance for trunk flexion (p=.01, p=.00). Post-hoc test revealed that there were significant differences in all pair-wise reaching distance comparison. (3) There was a significant side by target distance interaction for trunk rotation (p=.04). There was a significant main effect in target distance (p=.00). Post-hoc test revealed that there were significant differences between 70% and 110%, 70% and 130%, 90% and 110%, 90% and 130% of target distance. It was known that trunk flexion is used more than trunk rotation during reaching task in hemiplegic patients from the findings of this study. It is also recommended that reaching training is performed with limiting trunk movement within 90% of target distance whereas reaching training is performed incorporating with trunk movement beyond 90% of target distance in patients with hemiplegia.

음악요법에 관한 국내 간호학 연구 분석 (The Analysis of Nursing Research on Music Therapy in Korea)

  • Purpose: The purpose of this study was to review and analyze the trend and issues of nursing research on music therapy. This study was aimed to help direct developing better music therapy programs in the future. Method: We reviewed 57 dissertations and published articles from 1983 to Feb. 2005 that were related to music therapy in nursing. Result: Among the 57 published researches on music therapy, 47 (82.5%) were dissertations and 10 (17.5%) were published articles. Among the 57 researches, 55 (96.5%) were experiential researches and 2 (3.5%) were non-experimental researches. Surgical patients were the most popular subject for researches. The number of subject was a minimum of 7 people and maximum of 143 people. Listening to music 41 (74.5%) was the most popular program for music therapy. Among 55 researches, 33 (60.0%) had a music therapy session of 25-60 minutes and this was the most popular time duration of therapy. Among all the 108 dependent variables, 57 (52.8%) were psychological variables, 40 (37.0%) were physical variables and 11 (10.2%) were social-cognitive variables. Conclusion: There is an urgent need to develop systematic music therapy programs with different nursing interventions, as based on theories and principles while considering the physical, psychological, and social characteristic of the subjects.

약물남용의 재발 예방을 위한 인지행동접근법의 효과성 (Effectiveness of Cognitive-Behavioral Therapy to Prevent Relapse of Substance Abuse)

  • The purpose of this study was to test the effectiveness of cognitive-behavioral therapy to prevent relapse of substance abuse. A total of 8 substance abusing adult clients participated in the program. All the clients were charged with substance use at the time of implementing the program. Two master's level social workers operated the program, which was implemented in a probation office in Seoul. This study included Beliefs about substance use, abstinence self-efficacy, and coping skills as dependent variables in order to investigate the effectiveness. Cognitive-behavioral therapy emphasizes the enhancement of these cognitive and behavioral skills to prevent relapse of substance use. Research hypotheses were partially supported. That is, clients participated in the program had more negative attitude toward substance use and had more effective problem-focused coping skills. Although a research hypothesis of the enhancement of clients' self efficacy was not supported, their mean scores at the final post test were lower than scores at pretest. This study suggests that more studies be needed to in order to confirm the effectiveness of cognitive-behavioral therapy with substance abusing clients.

방사선을 조사한 혈관내피세포에 대한 Angiopoietin-1의 방사선 방어 기작 (Angiopoietin-1 Is An Radiation-induced Apoptosis Survival Factor for Human Umbilical Vein Endothelial Cells)

  • Angiopoietin-1(Ang-1) is a vasculogenic factor that signals through the endothelial cell-specific Tie2 receptor tyrosine kinase. We examined the effect of angiopoietin-1(Ang-1) on radiation-induced apoptosis in human umbilical vein endothelial cells(HUVECS) and receptor/second messenger signal transduction pathway for Ang-1's effect on HUVECs. The percent of apoptotic cells under control condition(0Gy) was $8.2\%$. Irradiation induced apoptosis was increased in a dose(1, 5, 10, and 15Gy)- and time 12, 24, 48 and 72hr)-dependent manner. The percent of apoptotic cells was approximately $34.9\%$ after 15 Gy of irradiation. Under these conditions, pretreatment with Ang-1's (50, 100, 200, and 400 ng/ml) inhibited irradiation-induced apoptosis in human umbilical vein endothelial cells in a dose-dependent manner. Two hundred ng/ml of Ang-1 inhibited approximately $55-60\%$ of the apoptotic events that occurred in the 10 Gy-irradiated cells. Pre-treatment with soluble Tie2 receptor, but not Tie1 receptor, blocked the Ang-1's antiapoptotic effects. Phosphatidylinositol 3'-kinase (P13-kinase) specific inhibitor, wortmanin and LY294002, blocked the Ang-1-induced antiapoptotic effect. Ang-1 promotes the survival of endothelial cells in irradiation-induced apoptosis through Tie2 receptor binding and P13-kinase activation. Pretreatment of Ang-1 could be beneficial in maintaining normal endothelial cell integrity during irradiation therapy.

Correlation of Curved Walking Ability with Straight Walking Ability and Motor Function in Patients with Hemiplegia

  • Purpose: In real life there are both straight-paths and curved-paths. To evaluate walking ability of both kinds, a figure-8 walking test (F8WT) was developed. The aim of this study was to validate the measure in hemiplegic patients with walking difficulties and to identify correlations of curved walking ability with straight walking ability, motor function, and walking performance ability. Methods: Twenty subjects participated in this study. Curved walking was measured by a F8WT. Straight walking ability was measured by a 10-meter walking test (10MWT). Dynamic balancea bility was measured by timed up and go (TUG) tests. Walking performance ability was measured using a modified motor assessment scale (MMAS). Motor function was measured by the Fugl-Meyer assessment (FMA) scale. Data were analyzed using Pearson correlation analysis. Linear regression analyses were performed to explore other functional tests in mobility ability by F8WT time, 10MWT (dependent variable). Results: There was a significant positive correlation of F8WT time with 10MWT and TUG. There was a significant negative correlation of F8WT time with MMAS and FMA-coordination. There was a significant positive correlation of 10MWT with TUG. There was a significant negative correlation of 10MWT with MMAS and FMA-coordination. The F8WT time for curved walking ability was attributed to 10MWT for straight walking ability as 94% level of contribution. Conclusion: The results suggest that the F8WT is a good instrument for measuring walking ability because there is a robust correlation of F8WT time with 10MWT, TUG, MMAS, and FMA-coordination in hemiplegic patients who, after stroke, have a mobility deficiency.

운동학습에 의한 왼쪽 하전두영역의 분할비등방성의 변화 (Change of Fractional Anisotropy in the Left Inferior Frontal Area after Motor Learning)

  • Purpose: This study was to delineate the structural change of neural pathway after sequential motor learning using diffusion tensor imaging (DTI). Methods: The participants were 16 healthy subjects, which were divided by training (n=8) and control (n=8) group. The task for the training was the Serial Reaction Time Task (SRTT) which was designed by Superlab program. When the 'asterisk' shows up in the 4 partition spaces on the monitor, the subject presses the correct response button as soon as possible. The training group participated in the training program of motor learning with SRTT composed of 24 digits pattern in one hour per daily through 10 days during 2 weeks. Results: In the behavioral results the training group showed significant changes in the increase of response number and the reduction of response time than those of the control group. There was significant difference in the left inferior frontal area in the fractional anisotropy (FA) map of the training group in DTI analysis. Conclusion: Motor sequential learning as like SRTT may be needed to the learning of language and visuospatial processing and may be induced for the experience-dependent structural plasticity during short period.

Methods : The authors reviewed several literatures about Younggupalbub and investigated the clinical usefulness as well found some problems with it. Results and Conclusions : Bideungpalbub is a time related-acupuncture treatment and has a possibility to make some problems for the patients and oriental medical doctors.

  • Objectives : The purpose of this study is to review the principal of Sacred tortoise method (Younggupalbub) and to investigate the usefulness of it. Methods : The authors reviewed several literatures related with the Sacred tortoise method (Younggupalbub). We investigated the clinical usefulness as well found some problems in utilizing it. Results and Conclusions : Sacred tortoise method (Younggupalbub) is an acupuncture treatment used according to the time and it may cause many problems for the patients and oriental medical doctors in the clinical use.

수술 환자에게 적용한 가온요법 연구논문 분석 (Analysis of Researches on the Warming Therapy for Surgical Patients)

  • Purpose: The main question is systematic review of the published in Korea and foreign countries on warming therapy for surgical patients. Methods: The researchers searched at Medline, CINAHL, KERIS, Adult Nursing Association, Korean Society of Nursing Science, Korean Academy of fundamentals of Nursing, and National Assembly Library web site for the published on warming therapy for surgical patients from 1980 to 2008. Words for search were operation/surgery, warming, operation/surgery and warming. Studies were included randomized controlled trial, and there were no restrictions regarding operative phase and outcome measures. Results: 36 published researches that met the criteria were mostly published in foreign countries between 2000 and 2008 and focused on surgery with general anesthesia. Sample size ranged from 21 to 60 subjects, age range between 21 and 60 years of age. Thirty different warming therapies were reported, fifty-two different dependent variables. Outcome indicators included active external warming, intra-operative, and body temperature. 'Positive effects' and 'no effects' equaled. The most frequently reported 'positive effects' were body temperature, shivering, and acid-base balance. No effects were more likely to be heart rate, blood pressure, and hemodynamics. Conclusion: Many types of warming therapy, are reported in the literature with little information about the efficacy of each, many different dependant variables were studied. There were no consistent reports as to length of time used for warming procedures. Overall, the effects of warming therapy are inconsistent. And additional research must be down before any particular method of warming can be used with confidence as to its effectiveness. Attention must be made as to the research design, better measurement of the dependent variables. This review may serve as a base.
