High Prevalence of Opisthorchis viverrini Infection in a Riparian Population in Takeo Province, Cambodia

  • Opisthorchis viverrini infection was found to be highly prevalent in 3 riverside villages (Ang Svay Chek A, B, and C) of the Prey Kabas District, Takeo Province. This area is located in the southern part of Cambodia, where the recovery of adult O. viverrini worms was recently reported. From May 2006 until May 2010, fecal examinations were performed on a total of 1,799 villagers using the Kato-Katz thick smear technique. In the 3 villages, the overall positive rate for helminth eggs ranged from 51.7 to 59.0% (av. 57.4%), and the percentage positive for O. viverrini was 46.4-50.6% (47.5%). Other helminths detected included hookworms (13.2%), echinostomes (2.9%), Trichuris trichiura (1.3%), Ascaris lumbricoides (0.6%), and Taenia spp. (0.06%). The prevalence of O. viverrini eggs appeared to reflect a lower infection in younger individuals (<20 years) than in the adult population (>20 years). Men (50.4%) revealed a significantly higher (P=0.02) prevalence than women (44.3%). The Ang Svay Chek villages of the Prey Kabas District, Takeo Province, Cambodia have been confirmed to be a highly endemic area for human O. viverrini infection.

High prevalence of liver and intestinal fluke infections among residents of Savannakhet Province in Laos

  • The prevalence of liver and intestinal fluke infections was surveyed on residents of Savannakhet Province, Laos. Fecal specimens were collected from a total of 981 residents in 4 Mekong riverside villages and examined by the Kato-Katz thick smear technique. The results revealed that the overall helminth egg positive rate was 84.2%, and the positive rate for small trematode eggs, including Opisthorchis viverrini, heterophyids, or lecithodendriids, was 67.1 %. To obtain adult flukes, 38 small trematode egg positive cases were treated with a 20-30 mg/kg single dose of praziquantel and purged. Diarrheic stools were then collected from 29 people and searched for helminth parasites using stereomicroscopes. Mixed infections with O. viverrini and 6 kinds of intestinal flukes were found, namely, Haplorchis taichui, Haplorchis pumilio, Haplorchis yokogawai, Prosthodendrium molenkampi, Phaneropsolus bonnei, and echinostomes. The total number of flukes collected was 7,693 specimens (av. no. per treated person; 265.3). The most common species was O. viverrini, followed by H. taichui, P. molenkampi, echinostomes, H. pumilio, P. bonnei, and H. yokogawai. The results indicate that foodborne liver and intestinal fluke infections are prevalent among residents of Savannakhet Province, Laos.

Prevalence of Intestinal Helminths among Inhabitants of Cambodia (2006-2011)

  • In order to investigate the status of intestinal helminthic infections in Cambodia, epidemiological surveys were carried out on a national scale, including 19 provinces. A total of 32,201 fecal samples were collected from schoolchildren and adults between 2006 and 2011 and examined once by the Kato-Katz thick smear technique. The overall egg positive rate of intestinal helminths was 26.2%. The prevalence of hookworms was the highest (9.6%), followed by that of Opisthorchis viverrini/minute intestinal flukes (Ov/MIF) (5.7%), Ascaris lumbricoides (4.6%), and Trichuris trichiura (4.1%). Other types of parasites detected were Enterobius vermicularis (1.1%), Taenia spp. (0.4%), and Hymenolepis spp. (0.2%). The northwestern regions such as the Siem Reap, Oddar Meanchey, and Banteay Meanchey Provinces showed higher prevalences (17.4-22.3%) of hookworms than the other localities. The southwestern areas, including Koh Kong and Preah Sihanouk Provinces showed higher prevalences of A. lumbricoides (17.5-19.2%) and T. trichiura (6.1-21.0%). Meanwhile, the central and southern areas, in particular, Takeo and Kampong Cham Provinces, showed high prevalences of Ov/MIF (23.8-24.0%). The results indicate that a considerably high prevalence of intestinal helminths has been revealed in Cambodia, and thus sustained national parasite control projects are necessary to reduce morbidity due to parasitic infections in Cambodia.

농촌(農村)에 있어서의 구충감염현황(鉤虫感染現況) (On the status of hookworm infection in rural areas in korea)

  • In order to evaluate the present status of hook-worm infection in the rural areas in Korea, stool examinations were undertaken in several selected rural villages in each year of 1970 and 1977. The methods employed were cellophane thick smear technique and brine floatation technique in 3,454 specimens collected from 7 different rural villages for the prevalence rate of hookworm infection and Stoll's dilution egg counting technique for the intensity of hookworm infection. The incidences of Ancylostoma duodenale and Necator americanus infections were determind by polyethylene tube coproculture and by the evacuated adult worms after treatment with anthelmintics to the patients who were infected by hookworms. The results are as follows : 1) In 1970, 323(18.6%) out of 1,737 specimens collected from 4 different rural areas were positive for hook worm infection and the ranges of prevalence rates in those suveyed areas were shown as 6.9 to 38.2 per cent. On the other hand, 144(8.2%) out of 1,717 specimens collected from 3 different rural areas in 1977 were shown as positive for hookworm infection and 7.5 to 10.6 per cent of prvalence rates were shown in the surveyed areas. 2) In the age group under 10 years, there was no infection of hookworm in the studied areas in 1977, however in 1970 the prevalence of the hookworm infection was shown as very low in the age group of 5~9. On the whole, the hookworm prevalence steadily increased from the age of 15 in the studied areas in 1970, however in 1977 the prevalence increased from the age of 20 to the older age groups. 3) There was not observed much differences in the intensity of hookworm infection among the studied areas in 1970 and 1977. The mean EPG in each studied areas were shown under 1,000 EPG and mean numbers of evacuated worms per infected persons were counted as 9.4 to 15. 1. 4) Both A.duodenale and N.americanus have been found in all studied areas but the incidences and st-ate of distribution of two species of hookworn were shown as different in each localities.

제주도(濟州道) 및 서울 일부지역(一部地域)에 있어서의 조충(條虫) 감염율(感染率)과 유(有), 무구조충(無鉤條虫) 감염상황(感染狀況)에 관한 조사(調査) (An Epidemiological survey on the Taeniasis in Seoul city and Cheju Do, Korea)

  • The present study was undertaken to evaluate the prevalence rate of Taenia species infection and distribution of taeniasis caused by Taenia solium among Koreans in Seoul and Cheju Do In Korea during the period from August to December 1984. A total of 4,256 stool specimens from the students of Mapo Ku and 360 stool specimens from the inhabitants of Guro-Ku in Seoul was collected and examined by cellophane thick smear technique. On the other hand 1,015 stool specimens from the students of Jocheon Myun and 265 specimens from the students of Jocheon Myun and 265 specimens from the inhabitants of Aeweol Myun and Gujwa Myun in Cheju Do were also examined. The results were summarized as follows ; Four (0.1%) out of 4.256 students were positive and 2 (0.6%) out of 360 inhabitants in Seoul were positive (Table 1, 2). Positive rates of taeniasis in Cheju Do were 1.7% (17) out of 1,015 students and 12.5% (33) out of 5 villagers (Table 3, 4). In order to observe the distribution of Taenia solium infection, the scolex or a part of Taenia spp. were collected from the stool of positive cases by anthelmintic treatment. For the species identification, expelled proglottides were examined microscopically by the number of branches of the uterus, presence of vaginal sphincter or the accessory ovarian lobe etc. Three cases were infected with Taenia solium among 6 egg positive cases in Seoul. But only 1 case was infected with Taenia solium out of 7 students taking anthelmintics voluntarily by recommendation of Korean Association for Parasite Eradication (KAFPE). On the other hand, among 32 cases of egg positive cases of this study and 26 cases of KAFPE in Cheju Do, 13 cases (22.4%) were infected with Taenia solium. But 13 cases who were not examined and complained expulsion of proglottides in their stool were infected with Taenia saginata (Table 5). Among 62 persons infected with Taenia saginata, only 4 cases did not know their infection of this worm. Rut in 17 cases with Taenia solium, 7 persons did not know their infection until stool examinations were performed (Table 6).

  • Opisthorchis viverrini infection is associated with cholangiocarcinoma particularly in the cases of chronic or re-infection. This presents a serious health problem in northeastern and northern Thailand. A community base approach is required for surveillance. Therefore, in a pilot project, re-examination of O. viverrini infection was conducted in the 3 districts of Nakhon Ratchasima province, Thailand, during June and October 2015. A total of 355 participants from a 194,152 population, was selected through multi-stage sampling. O. viverrini infection was determined using modified Kato Katz thick smear technique. Participants were 229 males and 126 females, and aged ${\geq}30$ years old. Prevalence of O. viverrini infection was 2.25% (8/355 participants). O. viverrini infection was slightly higher in females (3.17%), and age group between 41-50 years (4.49%). Mueang Yang district had a highest of O. viverrini infection rate (2.82%), and followed by Bua Yai (2.48%), and Chum Phuang (1.84%), respectively. O. viverrini infection rate was increased from year 2012 to 2015 particularly in Bua Yai and Mueang Yang. These re-examinion results indicate that opisthorchiasis is still problem in community of Nakhon Ratchasima province, therefore, the provincial-wide scale is need required. Furthermore health education is need intervened in the infected group, and screening of cholangiocarcinoma is urgently concerned.

  • National surveys on the prevalence of intestinal parasitic infections have been carried out every 5-7 years since 1971 in the Republic of Korea in order to establish control measures. The present nationwide survey was conducted from June to December 2004. The 10% population sampling data of Population and Housing Census by the Korean government in 2000 was used as the survey population. One sample was selected randomly from each of the 22,858 registered subjects, and a total of 20,541 people were ultimately included in this survey. Fecal examinations were performed by the cellophane thick smear and saturated brine flotation techniques. Pinworm infection was examined by cello-tape anal swab method. This survey also included a questionnaire study for a socioeconomic analysis. The total helminth egg positive rate was 3.7%, and the estimated total positive number among nationwide people was 1,780,000. The rates in urban and rural areas were 3.1% and 6.8%, respectively. As the total egg positive rate in the 6th survey in 1997 was 2.4%, the present survey showed that there was a considerable degree of increase in the prevalence rate of intestinal parasitic infections over the 7-year period following the 6th survey. The largest increases occurred in the egg positive rates of Clonorchis sinensis and heterophyids including Metagonimus yokogawai.

Decrease of Metagonimus yokogawai Endemicity along the Tamjin River Basin

  • The Tamjin River which flows from Jangheung-gun via Gangjin-gun to the South Sea was reported to be a highly endemic area of Metagonimus yokogawai infection in 1977 and 1985. However, there were no recent studies demonstrating how much change occurred in the endemicity, in terms of prevalence and worm burden, of metagonimiasis in this river basin. Thus, a small-scale epidemiological survey was carried out on some residents along the Tamjin River basin in order to determine the current status of M. yokogawai infection. A total of 48 fecal samples were collected and examined by the Kato-Katz thick smear and formalin-ether sedimentation techniques. The egg positive rate of all helminths was 50.0%, and that of M. yokogawai was 37.5%, followed by C. sinensis 22.9% and G. seoi 4.2%. To obtain the adult flukes of M. yokogawai, 6 egg positive cases were treated with praziquantel 10 mg/kg in a single dose and purged with magnesium sulfate. A total of 5,225 adult flukes (average 871 specimens per person) of M. yokogawai were collected from their diarrheic stools. Compared with the data reported in 1977 and 1985, the individual worm burdens appeared to have decreased remarkably, although the prevalence did not decrease at all. It is suggested that the endemicity of M. yokogawai infection along the Tamjin River has been reduced. To confirm this suggestion, the status of infection in snail and fish intermediate hosts should be investigated.

유치 상아질 산부식 후 습윤 정도에 따른 조직상 (A STUDY ON THE MICROSCOPIC IMAGES OF DENTIN SURFACES IN PRIMARY TEETH ACCORDING TO SURFACE WETNESS AFTER ACID ETCHING)

  • 상아질 접착력을 높이기 위해서는 적절히 습윤한 상아질 표면을 얻는 것이 중요하다. 이런 방법에는 정확한 지침이 없으며, 주관적이고 술자에 따라 많은 차이가 있다. 본 연구에서는 적절히 습윤한 상아질을 얻는 방법에 관한 연구의 일환으로, 유치의 산부식 후. 여러 가지 방법을 이용해 상아질을 건조시킨 표본의 조직상을 획득하여 상아질 표면의 차이를 비교하고자 하여 다음과 같은 결론을 얻었다. 1. 유치 상아질을 산부식하면, 도말층이 제거되고 상아세관이 노출되며, 관간상아질과 관주상아질이 탈회되고, 교원섬유가 풍부한 이행층이 노출되는 것을 확인할 수 있었다. 2. 산부식된 상아질이 과도하게 건조되면 탈회된 상아질이 수축하여, 관간상아질이 붕괴되어 높이가 점차적으로 낮아지고 상아세관의 직경은 조금씩 감소되는 것으로 나타났다. 3. 유치 산부식 후, 10cm 거리에서 20초 동안 압축 공기로 건조시키면 상아질이 과도하게 건조, 탈수됨을 확인할 수 있었다. 4. 유치 산부식 후, 10cm 거리에서 3초 동안 압축 공기로 건조시키거나 마른 cotton, 젖은 cotton, microbrush, absorbent tissue paper로 건조시키면 상아질이 적절하게 건조됨을 확인할 수 있었다.

전북 부안군과 경남 사천군 해안지역에서 발견한 이형흡충류 및 기타 장흡충류의 유행지 (Two endemic foci of heterophyids and other intestinal fluke infections in southern and western coastal areas in Korea)

  • 우리 나라 일부 해안 지역의 장내 기생충 감염현황을 파악하기 위해 전북 부안군 주민 153명 및 경남 사천군 주민 138명의 대변을 수집하여 셀로판 후층 도말법 및 포르말린-에테르 침전법을 실 시하였다. 전북 부안군의 기생충 감염률은 21.5% (33/153명)이었으며, 감염 기생충별 양성률은 이형흡충류 17.6%, 간흡충 2.6%, 회충 0.7%, 람블편모충 2.6%, 대장아메바 0.7% 및 작은와 포자충 0.75이었다. 경남 사천군의 기생충 감염률은 39.1% (54/138명)로서 이형흡충류 18.8%, 간흡충 12.3%, 편충 2.2%, 람블편모충 2.2%, 대장아메바 5.0%, 이질아메바 0.7%, 왜소아메바 3.6% 및 작은와포자충 4.3%의 양성률을 보였다. 부안군에서 이형흡충란 양성자 22 명으로부터 회수한 충체는 유해이형흡충이 총 3,284마리 (평균 149.3마리)였으며 표주박이형흡 충, 수세미이형홉충, 요코가와흡충, 미야타흡충, 자루이형흡충, 참굴큰입흡충, 긴이형흡충 및 Aconthopowphium kurogamo등이 중복 감염되어 있었다. 사천군에서 이형흡충란 양성자 중 6명으로부터 회수한 충체는 요코가와흡충이 총 3,007마리 (평균 501.2마리)였으며 유해이형흡충, 수 세미이형흡충, 긴이형흡충, 갈매기이형흡충 등의 이형흡충류가 중복감염되어 있었다. 이상의 결과로 우리 나라 남해 및 서해안 지역 주민들에서 담수어 또는 반염수어의 생식 습관으로 장내 흡충, 특히 이형흡충류의 유행이 지속되고 있음을 확인하였다.
