The Effects of Driving Rehabilitation Program on Cognitive Function in Elderly (운전재활프로그램이 노인의 인지기능에 미치는 효과)

  • Purpose : The purpose of this experiment is to find out the effectiveness which exert influence on cognitive skills by using the Driver Rehabilitation program for senior citizens who are over 65 years old and live in Busan. Method : From July first, 2014 to August 28th, 2014, we researched the 60 elderly people who are over 65 and go to community relief center which is in Busan. - 30 are experimental group and the other is control group. In the experimental group, we used Nintendo wii's driving simulation program and RC Car driving program in the model road. For estimation, we used MVPT-3(Motor-Free Visual Perception Test-3), Trail Making Test - 1, Trail making Test - 2 and LOTCA(Loewenstein Occupational Therapy Cognitive Assessment). Result : Nintendo wii's driving simulation program and RC Car driving program in model road results efficient visual perception ability. This programs results effectively in visual perception ability and space perception ability. This programs results effectively in motor apraxia ability. This programs results effectively in control ability for visual perception. This programs results effectively in thinking operation. Conclusion : Nintendo Wii's driving simulation program and RC Car driving program in model road positively influence improving for visual perceptual ability and cognitive function of elderly people. Also it is considered as being more efficient for improving visual perceptual ability and cognitive function to implement basic rehabilitation training with driving rehabilitation program than basic training itself.

Scrambler Therapy for Patients with Cancer Pain - Case Series -

  • More than 80% of cancer patients experience cancer pain. Among them, more than 50% experience moderate to severe pain. To control cancer pain, a variety of methods have been used, including medications and nerve blocks. In some patients, however, it is impossible to perform nerve blocks due to caner metastasis into the epidural space, while in other patients, opioid dose escalation is impossible due to opioid side effects; thus, cancer pain management is difficult. Scrambler therapy is a novel approach for pain control that uses EKG-like pads, which are applied above and below the site of pain. Scrambler therapy synthesizes 16 different types of nerve action potentials that provide "non-pain" information via cutaneous nerves. The advantages of this treatment are that it is non-invasive and safe and has no significant side effects. In this case series, we report the treatment results of using scrambler therapy in three cancer patients with intractable pain.

  • Here we review the evolutionary history of radiation therapy technology through the festschrift of articles in celebration of the 30th anniversary of Korean Society of Medical Physics (KSMP). Radiation therapy technology used in clinical practice has evolved over a long period of time. Various areas of science, such as medical physics, mechanical engineering, and computer engineering, have contributed to the continual development of new devices and techniques. The scope of this review was restricted to two areas; i.e., output energy production and functional development, because it is not possible to include all development processes of this technology due to space limitations. The former includes the technological transition process from the initial technique applied to the first model to the latest technique currently used in a variety of machines. The latter has had a direct effect on treatment outcomes and safety, which changed the paradigm of radiation therapy, leading to new guidelines on dose prescriptions, innovation of dose verification tools, new measurement methods and calculation systems for radiation doses, changes in the criteria for errors, and medical law changes in all countries. Various complex developments are covered in this review. To the best of our knowledge, there have been few reviews on this topic and we consider it very meaningful to provide a review in the festschrift in celebration of the 30th anniversary of the KSMP.

Consideration of Imaging Studies for Degenerative Spine Disease (퇴행성 요추질환 영상의 고찰)

  • Purpose : The aim of this study is to consider degenerative spine disease theoretically and compare plain radiography which is a basic study for low back pain with MRI in cases of degenerative lumbar spine disease to find out whether the abnormalities agree with each other. Methods : In 4 cases of lumbar degenerative disease, we studied the relation of the abnormalities such as disc space narrowing, spinal space narrowing, loss of lordosis and osteophytes on plain radiography with those on MRI of HIVD, spinal stenosis and spondylolisthesis. Results : Many abnormalities such as disc space narrowing, spinal space narrowing, loss of lordosis, osteophytes and change of cortex & bone marrow on plain radiography suggest HIVD, spinal stenosis, spondylolysis or spondylolisthesis on MRI. Conclusion : For low back pain patients, plain radiography is a basic study in diagnosis of HIVD, spinal stenosis, spondylolysis or spondylolisthesis but MRI or CT scan is necessary to develop(build) a treatment plan like an operation.

Epidural Steroid Therapy as a Treatment of Post-laminectomy Low Back Pain (추궁 절제술 후의 요하지통에 대한 경막외 Steroid 주입요법)

  • Epidural steroid therapy has been well-established for the treatment of sciatica and low back pain. Disappointing results following surgical decompression or discectomy pain owing for to nerve root compression have led to trials of corticosteroids injected either systemically or into the intrathecal or epidural space to treat intervertebral disc. Epidural steroid is less effective in the patients with low back pain who have a history of surgical operation, so that the use of epidural morphine and methylprednisolone has been advocated for the amelioration of chronic low back pain in the post-laminectomy pain("failed back") syndrome over the past several years. We treated 47 patients with low back pain who had a history of one or two surgical procedures. We concluded that epidural steroid therapy is less effective in the patients with "failed back" syndrome than in the virgin back furthermore, there is a greater risk of complication such as inadvertent dural puncture and corresponding motor paralysis and headache.

  • In the case of designing a high dose rate remote controlled afterloading treatment room with existing hospital facilities. We must construct the effective protective barriers so as to reduce the primary and scattered radiation up to the maximum permissible dose level. It is difficult to reinforce the barrier thickness of the shielding requirements because of the limited space and the problem of the existing building structure at the surrounding area. Therefore we can reduce the intensity of primary radiation to the required degree at the location of interest with installing the appropriate I shaped Pb barriers between the radiation source and the shielding wall of the concrete. As a result, it was possible to reduce the intensity of the primary radiation below the M.P.D level by using additional Pb barriers instead of increasing thickness of concrete wall.

  • It is widely accepted that smooth muscle contraction is triggered by intracellular $Ca^{2+}$ ($[Ca^{2+}]_i$) released from intracellular $Ca^{2+}$ stores such as sarcoplasmic reticulum (SR) and from the extracellular space, The increased $[Ca^{2+}]_i$ can phosphorylate the 20-kDa myosin light chain ($MLC_{20}$) by activating MLC kinase (MLCK), and this initiates smooth muscle contraction. In addition to the $[Ca^{2+}]_i$-MLCK-tension pathway, a number of intracellular signal molecules, including mitogen-activated protein kinase (MAPK), protein kinase C (PKC), phosphatidylinositol 3-kinase (PI3K), and Rho-associated coiled coil-forming protein kinase (ROCK), play important roles in the regulation of smooth muscle contraction. However, the mechanisms regulating contraction of caldesmon (CaD), actin-binding protein, are not entirely elucidated in the presence of $Ca^{2+}$. It is known that CaD tightly interacts with actin and inhibits actomyosin ATPase activity. Therefore, the purpose of the present study was to investigate the roles of $Ca^{2+}$-dependent CaD in smooth muscle contraction. Endothelin-1 (ET-1), G-protein coupled receptor agonist and vasoconstrictor, increased both vascular smooth contraction and phosphorylation of CaD in the presence of $Ca^{2+}$. These results suggest that ET-1 induces contraction and phosphorylation of CaD in rat aortic smooth muscle, which may he mediated by the increase of $[Ca^{2+}]_i$.

A Case Study on the Effect of the Art Therapy through the Self Expression and the Interpersonal Relations by a Schizophrenic Adolescent I (정신분열증 청년의 자기표현과 상호작용의 효과에 관한 미술치료 사례연구 I)

  • Objectives : This article aims to find how effective the art therapy is on the improvement of the self expression and the interpersonal relations with others, as medium for verbal communication of schizophrenic patients. Methods : The subject of this study was one schizophrenic patient who had been treated with drugs for five years in W university hospital, as well as counseled by an art therapy oo counsel office in J city. The art therapy had been performed the total 25 sessions, fifty minute per session, one session per week, from November 28 in 2008 to April 27 in 2009. The test was by HTP. Results : The results of this study are as follows: First, in the pretest of HTP, the subject showed a little lethargy, painting very simple house-shaped figures and too simple rectangular figures corresponding to those painted by 4 or 5-year-old children. In the post-test, however, the structures of houses were described concretely and three-dimensionally, and the descriptions of people were changed into concrete figure, by which we could see the improvement effect on the self expression and the interpersonal relations. Second, in the earlier stage, there was little verbal self expression, and little self respect, staring into space and showing passive attitude without positive assertion and expression. In the later stage, however, there were positive assertion and expression, even showing sense of humor. Lastly, in the earlier stage, there were few interpersonal relations. In the later stage, however, eye contacts and the interpersonal relations were increased during conversation and in the art work process, respectively. Conclusions : These results suggest that the art therapy can be effective on improvement of the self expression and the interpersonal relations with others, as medium for verbal communication of schizophrenic patients.

Six Cases of Sudden Sensorineural Hearing Loss (돌발성난청 환자 치험 6례)

  • Sudden sensorineural hearing loss may be defined as a severe loss of hearing occurring within a short space of time without any obvious cause. Its etiology is not verified yet, treatment and prognosis are uncertain. Objectives: We reviewed 6 cases of patients who hospitalized in Kyunghee Oriental Medical Center Dept. of Otolaryngology. Using the criteria of Siegel and Research Team of the Japanese Ministry of Health and Welfare of Japan, We are to evaluate the effect of Herb medication, Acupuncture therapy, and Negative therapy. We also attempted to search effective methods of therapy of sudden sensorineural hearing loss. Methods: We treated them with Oyaksunki-san(烏藥順氣散) for softening qi-stagnation and supressing 'Wind'(順氣治風), Chungsimjihwang-tang(淸心地黃湯) for strengthening 'Heart' and 'Kidney'(補心益腎), Boikyangwi-tang(補益養胃湯) for strengthening 'Wi-qi' and 'Stomach'(補衛(胃)氣). We also used acupuncture therapy and negative therapy based on the textbook of Acupuncture and Moxibustion. Results: One patient recovered completely after 5 days therapy, two patient markedly, another two patient slightly and one patient had no improvement. Conclusions: To treat sudden hearing loss, we can use Oyaksunki-san(烏藥順氣散) in the early stage of the disease. We can treat with Chungsimjihwang-tang(淸心地黃湯) and Boikyangwi-tang(補益養胃湯) in the late period of the disease, and administer Jaeumgenby-tang(滋蔭健脾湯) for suppressing tinnitus. If there is evidence of viral infection and the patient have too severe hearing loss, We can also use steroid in combination with Herb medication in the early stage of the disease. The Western medicine demonstrated that the start time of treatment had influence on recovery. If we started Oriental medical treatment within a week from the onset of sudden deafness. we will obtain the desired results. As the Western medical treatment does, Oriental medical therapy may have no effect after one month from onset of the disease.

Trends of Life-Space Mobility in Community-dwelling Elderly (지역사회 거주 노인 대상의 생활공간 이동성(Life-space mobility) 연구 동향)

  • Objective : This study aimed to systematically examine studies on the life-space mobility in community-dwelling elderly and analyze and summarize the research trends. Methods : The Embase and PubMed databases were searched for articles on the life-space mobility of community-dwelling elderly published between January 2010 and January 2020. Based on the selection and exclusion criteria of the 335 articles, a total of 27 articles were finally selected and analyzed. Results : As a results, 11 (40.7%) cohort studies had evidence level II. This study showed that the participants in the studies were healthy elderly (81.5%), and the University of Alabama Life-Space Assessment (UAB-LSA) used the most participants (88.9%). Of the foci of the 27 finally selected studies, 8 (29.6%) were physical, 8 (29.6%) were psychosocial, 6 (22.2%) were cognitive, and 2 (7.4%) were social, and 3 (11.1%) were others. The life-space mobility of the elderly needs to be analyzed from a multidimensional point of view, and not based on a single factor such as the physical, cognitive, psychosocial, or social. Conclusion : The results of this study are expected to verify causality through the study of life-space mobility for the elderly staying in various communities and provide future directions for the study on the mobility of the elderly's and the development of community-based intervention programs.
