Validity and Reliability of Ultrasound Measurement of Knee Joint Space Width in Individuals With Knee Osteoarthritis

  • Background: Knee osteoarthritis (OA) is a single most arthritic disease. Knee joint space width (JSW) is commonly used for grading severity of knee OA. However, previous studies did not established criterion validity and test-retest reliability of ultrasound (US) image for measuring JSW. Objects: The aim of this study was to establish criterion validity and test-retest reliability of US measurement of medial and lateral knee JSW. Methods: Twenty-nine subjects with knee OA were participated. The US and X-ray were used to measure knee JSW. One sample Kolmogorov-Smirnov test was used to confirm the data normal distribution. Pearson correlation coefficient and ICC were used to calculated and establish criterion validity and test-retest reliability, respectively. Results: US measurement of medial and lateral knee JSW was highly correlated with radiographic imaging measure (r=.714 and .704, respectively). Test-retest reliabilities of medial and lateral knee JSW were excellent correlated (ICC=.959 for medial side and .988 for lateral side, respectively). Conclusion: US may be valid tool to measure knee JSW.

A Case Study of Sandplay Therapy with a Middle-Aged Woman Having Difficulty in Relationships (관계에 어려움이 있는 중년기 여성의 모래놀이치료 사례연구)

  • This study explored the sandplay therapy case of a middle-aged woman having difficulty with interpersonal relationships and wanting to increase her self-identity. The goal of the therapy was to change her relationships with others by self-examination and becoming conscious of her negative animus and femininity in a free and protected space provided by sandplay therapy. Forty-five therapy sessions were held. The client exhibited her lonesomeness and fantasies by making sandtrays of a house she would like to live in, a park she would like to relax at and dreams she would like to fulfill in the initial phase of therapy (1~8, Who am I?). In the intermediate phase of therapy (9~33, Meeting myself), she displayed scenes of her negative self and breaking away from her negative self by making sandtrays of shadows, deaths, creations, a soaring scene and a disappearing alligator. In the final phase of therapy (34~45, Discovery of the real Self), she showed scenes of her meeting a positive self by making sandtrays of hope, coexistence, start, harmony and community. This study showed the effectiveness of sandplay therapy since the client' relationships with others were improved through her self-awareness by sandplay therapy in the free and protected space.

  • This study explored the sandplay therapy case of a middle school-aged boy having severe conflicts with his mother, from the viewpoints of initiation. The goal of the therapy was to lessen the boy's conflicts with his mother within the free and protected space in sandplay therapy. Thirty-seven therapy sessions were held. The client showed the scene of having fun in the water in the most unconscious part of the tray in the initial phase of therapy (1, descending into the unconscious). In the intermediate phase of therapy (2~33, a period of suffering, confrontation with death and trials), he showed suffering, was confronted with death and underwent trials. In the final phase of therapy (34~37, seeking masculine principles and rebirth), he began to seek masculine principles instead of having conflicts with his mother and was reborn. Using sandplay therapy in a free and protected space, this study showed the effectiveness of sandplay therapy since the boy's severe conflicts with his mother were lessened. This study also confirmed the existence of an archetypal pattern in the male adolescent's psyche.

A Case Study of Sandplay Therapy for an Elementary School-aged Girl Living in a Home Suffering from Severe Conflicts between Her Mother and Older Brother : A Focus on Fairy Tales (어머니와 오빠의 심한 갈등 속에서 생활하는 초등학생 여아의 모래놀이치료 사례연구 : 민담의 관점에서)

  • This study explored the sandplay therapy case of an elementary school-aged girl living in a home in which there exist severe conflicts between her mother and older brother. This study focused heavily on fairy tales. The goal of the therapy was to encourage appropriate adjustments with the girl's situation within the free and protected space of sandplay therapy. Forty-four therapy sessions were held. The client exhibited evidence of feelings of being abandoned and of becoming lethargic in the initial phase of therapy (1~10, forsakenness/sleeping family). In the intermediate phase of therapy (11~38, regression, opposition and purification/growth), as she displayed signs of regression, princess play, the opposition of goodness and badness and purification, she grew psychologically. In the final phase of therapy (39~44, redemption/rebirth of new consciousness), she was redeemed and reborn with a new sense of consciousness. Using sandplay therapy in a free and protected space, this study showed the various motives of fairy tales in the psyche of a pre-adolescence girl and the effectiveness of sandplay therapy.

  • This study explored the sandplay therapy case of a woman with a negative mother complex who wants to be free from family life. The goal of the therapy was to make her free from family life by expressing repressed emotions for getting out of a negative mother complex under a free and protected space in sandplay therapy. Twenty-three therapy sessions were held. The client exhibited a singing woman in a gorgeous dress and things she wanted to do in the initial phase of therapy (1-4, forgotten myself). In the intermediate phase of therapy (5-15, differentiation and integration), she displayed the scenes of differentiation and integration by emotionally getting rid of a begging boy and making mandalas. In the final phase of therapy (16-23, recovered myself), she showed scenes of rebirth, upgrading and singing songs again in front of family members and many people. Through sandplay therapy in the free and protected space, this study showed the effectiveness of sandplay therapy since the client achieved individuation by showing rebirth, having a new personality.

  • Odontogenic maxillofacial space abscess in childeren was treated by the surgcal intervention combined with antiboitic therapy. Followings are the results after monitoring its progression. 1. Maxillofascial space abscess is mainly from the odontogenic infection and it may result in the severe states with the various fascial spaces and their relatives. So their early detection and treatment are needed. 2. The most common symptom in patients was the pain under palpation with painful swelling and the mouth floor elevation was observed in the sublingual space abscess. 3. In most cases, for its treatment, symptomatic therapy, antibiotic therapy, surgical incision and drainage were executed. If the infected tooth is possibly conserved, endodontic treatment is preferred, otherwise, it will be extracted as soon as possible.

  • Objectives: The purpose of this case study was to report the effect of Chuna therapy include Space Spinal Conduction Exercise and Manipulation Treatment for Functional dyspepsia patients. Methods: A 32-years-old male patients with functional dyspepsia visited the clinic ten times from March 17th 2023 to April 22th 2023. To assess the treatment outcomes, we used the Standard tool for pattern identification of Functional dyspepsia and Nepean Dyspepsia Index-Korean version (NDI-K) symptom score. Result: After treatment, Standard tool for pattern identification of Functional dyspepsia and Nepean Dyspepsia Index-Korean version (NDI-K) score decreased. Conclusion: Chuna therapy include Space Spinal Conduction Exercise and Manipulation Treatment was effective in the treatment of patient with Functional dyspepsia.

The Effects of Swimming and Low Power Laser on the Concentration of Interleukin-6 for Rheumatoid Arthritic Rats (수영과 저출력 레이저 치료가 류마티스 관절염 흰쥐의 혈청 인터루킨-6 농도에 미치는 영향)

  • This study was designed to determine the effects of swimming and low power laser on rheumatoid arthritis in Sprague-Dawley rats. Rheumatoid arthritis was induced in 36 rats among 48 Sprague-Dawley rats by the subcutaneous injection of .05 $m{\ell}$ Freund's Complete Adjuvant into the right hind paw and .05 $m{\ell}$ Freund's Complete Adjuvant into the right hind knee joint capsule. A second injection was performed by the same method using .1 $m{\ell}$ Freund's Complete Adjuvant per a rat. Arthritic rats were divided into 8 groups: each 1 week and 2 weeks of arthritic swimming, arthritic laser, arthritic case control and normal group. In this study, several experimental tests were performed to determine the concentration of Interleukin-6, the space of the knee joint and the thickness of the hind paw. The concentration of Interleukin-6 and hind paw thickness decreased in the swimming group and laser group as compared to the control group. The space of the knee joint increased significantly after the swimming exercise. Swimming and low power laser therapy positively affect rheumatoid arthritis in rats affect by decreasing the concentration of Interleukin-6 and hind paw thickness, and increasing the space of the knee joint.

Immediate Effect of the Complex Rotational Stretching Method on the Proprioceptive Sensation of the Shoulder Joint, Subacromial Space, Range of Motion, Shoulder Instability and Dynamic Function

  • PURPOSE: This study compared the effects of proprioceptive sensation, subacromial space, and dynamic function according to proprioceptive neuromuscular facilitation (PNF), static stretching (SS), and complex rotational stretching (CRS). METHODS: Thirty students without any musculoskeletal disease who volunteered to participate were included in this study. The following metrics were measured to evaluate the function and stability under the normal conditions, with the PNF, SS, and CRS: special test and flexion, extension, abduction, adduction, internal rotation, external rotation (shoulder range of motion) and reaching distance on the medial (ME), superolateral (SL), inferolateral (IL), and subacromial space and proprioceptive sensation were evaluated. All measures were analyzed using one-way ANOVA and repeated measures of ANOVA. RESULTS: A clear difference in adduction in the range of motion was observed in all groups (p < .05). Significant differences could not be identified in all values in the error test, except for Ab (p < .05). Significant differences in reach were noted in all directions of the SS and PNF in the Me, SL, and IL (p < .05). After the intervention, significant differences in the average values could be identified in all groups except for the SS group after rest (p < .05). After the intervention, there was a significant difference between the CRS and SS and PNF groups (p < .05). CONCLUSION: The application of CRS is as helpful as the existing SS and PNF for improving the joint range of motion improvement, shoulder balance, and subacromial space.

Effect of Functional Recovery for the Functional FES upon Chronic Stroke Patients with Shoulder Subluxation (어깨관절 아탈구가 있는 만성 뇌졸중환자에게 기능적 전기자극 치료가 기능회복에 미치는 효과)

  • Background: The purpose of this study was to confirm whether surface FES was effective in the function of chronic stroke patients' affected shoulder and whether FES influenced their functional abilities in ADL, as well. Method: the subjects were inpatients at a general hospital in Gyeonggi-Do, Korea and had been diagnosed as a sub-luxation in their affected shoulder. They were 26 and were separated to an experimental group and a control group, in equal number. Both groups had gotten an exercise treatment for 6 weeks. The control group didn't get any FES as opposed to the experimental group. Result: in the experimental group there was no statistical significant difference in the vertical space and the joint space, even though the space was reduced. In the control group, there was no statistical significant difference, either. Moreover, each space of their shoulders was increased. In the range of motion of each group, there was no statistical significant difference. In functional independent ability, there was a significant difference in an experimental group. In contrast, there was no difference in a control group. Conclusion: this result suggests that FES should be effective in the sub-luxation of the patients' affected shoulder and the range of motion of their shoulders. On the other hand, compared to the previous items, FES was quite effective in the functional improvement.
