Purpose: This study was conducted to assess the effects of robot-assisted gait training with visual feedback on gait, balance, and balance confidence in patients with chronic stroke. Methods: Thirty subjects with chronic stroke were randomly assigned to two groups: the experimental group (n=15) and the control group (n=15). The experimental group performed robot-assisted gait training for 30 minutes and the control group performed gait training with assisted devices training for 30 minutes after both groups performed conventional physical therapy for 30 minutes. Both groups performed the therapeutic interventions for 5 days per week, for a period of 4 weeks. For assessment of the 10 m walking test (10 MWT), Figure of 8 on the walk test (F8WT), Timed-Up and Go test (TUG), and Berg Balance Scale (BBS) were used to test the gait and balance, and the Korean version of the Activities-specific Balance Confidence Scale was used to test the balance confidence. Results: The experimental group showed significant improvement in the 10 MWT and the K-ABC (p<0.05), and the control group showed significant improvement in the BBS and the TUG (p<0.05). In four measurements, there were significant differences between the two groups (p<0.05), and the control group showed significant improvement in the F8WT at pre and post intervention (p<0.05). Conclusion: This study demonstrated that Robot-assisted gait training with visual feedback is an effective intervention for improving straight gait abilities and balance confidence, while the control group showed some improvement in curve gait and balance. Thus, we suggest both Robot-assisted gait training with visual feedback and gait training with assisted devices training exercise as a therapeutic intervention in chronic stroke rehabilitation.
This paper describes the mechanical and control design of a robotic device for providing therapeutic assistance to arm movement following stroke. This is a new robot for arm therapy applicable to the training of activities of daily living in homes and clinics. This instrument has one degrees of freedom, and is equipped with position and force sensors. Repetitive movement can improve movement performance in patients with neurological or orthopaedic lesions. The application of robotics can serve to assist, enhance, evaluate, and document neurological and orthopaedic rehabilitation of movements. The new robot, the mechanical structure, the control circuit, the sensors and actuators and some safety aspects.
자폐 범주성 장애를 갖는 아이들이 늘어나고 있으며 지능형 로봇의 연구가 활발하게 진행됨에 따라 사회적 상호작용의 결여, 의사소통의 부적절함 등 특징적 증상을 보이는 자폐성 장애 아동들의 치료 및 교육을 위한 치료용 로봇이 필요하다. 본 논문에서는 고양이 형태의 치료용 로봇에 대한 설계 및 동작을 다루고 있다. 현장 실험의 결과를 토대로 동작을 구상하여 감성 치료를 유도하였으며 동작에 필요한 센서, 자폐아 특성을 고려한 로봇 구조를 설계하였다. 아울러 현장 실험을 진행하여 동작의 효율성을 보였다.
이전 연구에서 자폐성 장애의 여러 특징적 증상을 갖는 아이들의 치료를 위해 사용될 수 있는 로봇을 설계 및 제작하여 현장실험이 진행 되었으며 기존 로봇은 터치에 의한 아이들과 상호작용을 통해 감정 표현 동작을 한다. 이러한 터치 상호작용에 감정 교육 및 치료를 위해 인공신경망을 이용한 동작 설계를 하였다. 하지만 이러한 물리적 접촉은 치료 활동의 초기에 사용되기 어려워 초기 치료 효과를 기대하기 어려웠다. 이에 본 논문에서는 동작 방식을 보완하여 음성 정보를 이용한 빠른 상호작용을 통해 치료 활동이 초기에 가능하고 유연한 대처와 다양한 상황에서 로봇이 사용될 수 있는 동작 설계를 기술한다. 이에 필요한 요소로서 음성 데이터 수집 방법 및 인공신경망을 이용한 음성 인식 구조가 설계되었으며 실험을 통하여 분류 결과를 분석하였다. 이렇게 설계된 인공신경망은 향후 다양한 음성 데이터를 수집하여 정확도를 향상시키고 현장실험을 통하여 동작의 효용성을 살펴볼 것이다.
In this paper, we propose and examine the feasibility of the robot-assisted behavioral intervention system so as to strengthen positive response of the children with autism spectrum disorder (ASD) for learning social skills. Based on well-known behavioral treatment protocols, the robot offers therapeutic training elements of eye contact and emotion reading respectively in child-robot interaction, and it subsequently accomplishes pre-allocated meaningful acts by estimating the level of children's reactivity from reliable recognition modules, as a coping strategy. Furthermore, for the purpose of labor saving and attracting children's interest, we implemented the robotic stimulation configuration with semi-autonomous actions capable of inducing intimacy and tension to children in instructional trials. From these configurations, by evaluating the ability of recognizing human activity as well as by showing improved reactivity for social training, we verified that the proposed system has some positive effects on social development, targeted for preschoolers who have a high functioning level.
본 연구의 목적은 국내에서 이루어진 자폐스펙트럼장애아동을 대상으로 한 로봇 활용 융합연구동향을 살펴보는 것이다. 이를 위하여 최근에 장애 아동의 교육적, 치료적 중재에 연구되고 있는 로봇연구 동향을 살펴보고자 하였다. 연구방법으로, 자폐를 키워드로 2009년 이후에 발표된 논문을 검색하여 총 17편의 연구 논문을 최종 선정하였고, 활용로봇, 독립변인, 종속변인 유형, 연구방법을 바탕으로 하여 주제별 범주로 묶어 내용 분석하였다. 연구결과, 그 중 자폐아동을 위한 로봇개발은 발전이 빠른 분야 중 하나이며, 자폐스펙트럼장애아동의 중재에 유용한 것으로 나타났다. 연구를 통해 향후 로봇활용의 일반화를 촉진할 수 있는 적용 절차 및 단계 등에 대한 세분화된 접근이 필요함을 알 수 있었다.
목적 : 본 연구의 목적은 뇌졸중 환자에서 로봇보조치료의 효과를 알아보기 위해 체계적 고찰을 통해 연구결과를 종합하고, 로봇보조치료에 관한 연구들을 비교하고 분석하고자 하는 것이다. 연구방법 : 2012년 6월부터 2012년 9월까지 외국학회지에 게재된 논문을 EMBASE, MEDLINE등을 통하여 검색하였다. 최종적으로 12개의 Randomized Controlled Trials(RCT) 논문을 대상으로 분석하였다. 결과 : 선정된 12편의 연구는 Pedro score 2~7점까지 다양했으며, 다양한 중재방법을 통하여 로봇보조치료의 효과를 극대화시키기 위한 연구들이 많았다. 주된 결과 측정도구로는 상지기능, 일상생활활동 수행능력을 평가하는 도구가 사용되었다. 로봇보조치료의 결과로는 상지기능과 일상생활활동 수행능력에서 향상을 보이나 로봇보조치료군과 고식적인 재활치료군 사이에 유의한 차이는 보이지 않았다. 결론 : 로봇보조치료는 뇌졸중 환자에게 그 효과를 극대화시키기 위해 다양한 중재방법으로 적용되고 있으며, 중재방법에 따라 효과의 차이가 나는 것으로 나타났다. 본 연구는 로봇보조치료의 임상적 적용에 대한 근거를 제시하며, 향후 연구에서는 다양한 중재방법을 적용하여 로봇보조치료의 효과를 극대화할 수 있는 방법에 대한 연구가 필요하다.
목적 : 본 연구는 국내에서 로봇치료가 중재도구로 사용된 연구 사례들을 PICO (Patient, Intervention, Comparison, Outcome) 형식에 따라 체계적으로 정리하여 국내 로봇치료의 연구 현황을 알아보고자 하였다. 연구방법 : 한국학술연구정보서비스(RISS)와 국가전자정보도서관(NDSL) 데이터베이스에서 최근 9년간의 연구를 주요 검색용어 '로봇치료'로 하여 국내 학술지와 학위논문 총 710개의 연구를 검색하였다. 로봇을 치료적 중재도구로 사용한 국내연구 중 원문을 구할 수 있는 연구를 기준으로 최종 15개를 선정하였다. 선정된 연구는 PICO 형식을 통해 체계적으로 정리하여 제시하였다. 결과 : 연구도구의 질적 수준은 근거기반 연구수준 5단계 분류 방법을 사용하였다. 그 중 질적 수준이 3단계 이상인 연구는 13개(86.6%)였다. 로봇치료를 사용한 연구를 중재 분야별로 나눈 결과 언어, 보행, 인지, 발달 그리고 상지의 다섯 가지 영역에 대한 연구가 진행되었음을 알 수 있었다. 결론 : 국내에서 로봇치료는 상지와 하지의 중재를 포함한 언어, 인지, 발달 등의 다양한 영역의 재활을 위해 사용되고 있었다. 본 연구가 국내 로봇치료와 관련된 다양한 영역의 적용에 필요한 기초자료로 활용되기를 바란다.
목적 : 본 연구에서는 상지 로봇 치료가 아급성기 뇌졸중 환자의 상지기능에 단기적으로 미치는 영향을 알아보는 것이었다. 연구방법 : 본 연구는 뇌졸중 편마비 진단을 받고 1회 1시간씩 상지 로봇 치료와 과제 지향적 훈련(task-oriented training)를 받았던 환자 20명의 의무기록을 이용한 후향적 연구로서, 중재 전/후의 3차원 동작분석검사 결과 값을 토대로 두 중재간의 변화량을 비교 하였다. 결과 분석은 기술 통계와 대응표본 t검정을 사용하여 결과 값을 파악 하였다. 결과 : 연구 결과 상지 로봇 치료를 한 경우 팔 뻗기를 하는 동안 팔굽관절 움직임의 순발력, 효율성, 부드러움에서 향상을 보였으며, 과제 지향적 훈련(task-oriented training)과 비교 하였을 때 팔굽관절의 부드러움에서 유의한 차이를 보였다(p<.05). 결론 : 단기적 상지 로봇 치료는 아급성기 뇌졸중 환자의 팔뻗기 시 팔굽관절 움직임에 효과를 보였으며, 추후 장기적인 연구를 통해 상지 기능의 움직임 개선에 대한 효과 입증이 필요하다.
Purpose: This systematic review aims to determine whether robot-assisted training is more effective in gait training for persons with subacute hemiparetic stroke. Methods: This study adopted a systematic review study design focused on subacute hemiparetic stroke, and four core academic databases were searched until June 11, 2021, for relevant studies, including PubMed, Embase, the Cochrane Library, and ProQuest Central. The review included randomized controlled trials (RCTs) evaluating the effects of robotic-assisted training on gait performance in persons with a diagnosis of subacute hemiparetic stroke. The selected RCT studies were qualitatively synthesized based on the population, intervention, comparison, outcome, settings, and study design (PICOS-SD). Results: The study selected five RCTs involving 253 subacute hemiparetic stroke patients and performing robotic-assisted gait training using the following devices: the Lokomat, Morning Walk, Walkbot, ProStep Plus, or Gait Trainer II. Five RCTs were eligible for the meta-analysis after quantitative synthesis, and the results showed that the robot-assisted gait training group had a greater gait performance than the control group based on the 10-meter walk test, Berg balance scale, Rivermed mobility index, functional ambulation category, and modified Barthel index. Conclusion: The results of this study showed that the gait performance of subacute hemiparetic stroke patients changes throughout robot-assisted gait training, but there were no indications that any of the clinically relevant effects of robot-assisted training are greater than those of conventional gait training. Further, the small sample size and different therapeutic intensities indicate that definitive conclusions could not be made.
