To analyze scientifically the polyprescription principle of Daekumeumja(對金飮子), which has been used for alcoholic damage, and Pyungwesan(平胃散), which has been used for indigestion, the transforming rate of hesperidin of these polyprescriptions to hesperetin was investigated. The transforming rate of the former was higher 3 times than that of the latter. The transforming rate of hesperidin of Aurantii nobilis Pericarpium was inhibited by Magnoliae Cortex, but was activated by Glycyrrhizae Radix. The activity of trypsin was inhibited by Glycyrrhizae Radix and Daekumeuja. However, Aurantii nobilis Pericarpium, Atractylodis Rhizoma, Magnoliae Cortex and Pyungwesan did not inhibited it. When human intestinal microflora were cultured with the media containing Daekumeumja, Pyungwesan and herbal medicines consisting of them, Aurantii nobilis Pericarpium and Magnoliae Cortex inhibited the activity of ${\beta}$-glucosidase. These results suggests that the therapeutic effect of Daekumeumja may be better than that of Pyungwesan on alcoholic damage and the therapeutic effect of Pyungwesan may be better than that of Daekumeumja on indigestion, although these prescriptions are consisted of the same herbal medicines.
Chronic inflammatory bowel disease (IBD) such as Crohn's disease or ulcerative colitis, affects around 2 in every 1000 individuals in western countries and its incidence, particularly amongst children, is increasing. IBD shows extreme morbidity with impact on all aspects of quality of life. If left untreated, IBD can lead to death. Conventional treatment of IBD involves powerful immunosuppressive chemotherapies and surgical intervention. Long-term anti-inflammatory medication is required and so patients are often subject to a spectrum of unpleasant side effects. Interleukin-10 (IL-10) is a cytokine that acts to suppress inflammation. When however administered by injection, the high levels of IL-10 that are distributed throughout the body also lead to side effects. Lactococcus lactis can be genetically engineered to secrete biologically active cytokines. When applied to the mucosa, these L. lactis can actively deliver such cytokines. By use of this principle we developed a new therapeutic approach for IBD. Administration of L. lactis that secretes murine IL-10 cures and prevents IBD in mice. The use of the engineered L. lactis gets around the problem of delivering IL-10, allowing dramatic reduction of the effective dose. A sincere concern exists about the possible dangers of uncontrolled, deliberate release of genetically modified microorganisms, such as could occur following application in healthcare. We engaged in the establishment of adequate means for biological growth control of engineered L. lactis by targeted gene exchange between thyA and hIL-10.
As there has recently been growing interest in Alternative Medicine, Oriental Medicals have been trying to incorporate Alternative Medicine into Oriental Medicine evolutionarily. The purpose of this study is to make a comparative study of the historical background and present condition, the view of life, the principle of therapeutics, and the various therapeutics in Alterative Medicine and Oriental Medicine from the viewpoint of Oriental Medicine. The conclusion of this study is; 1. Although Alternative Medicine has become influential mainly in the effort of finding a solution the limit of western medicine, the clinical effect of Alternative Medicine should be verified continuously. 2. Alternative Medicine includes all of the therapeutics such as Traditional Medicine, Naturopathy, Complementary Medicine and Selective Medicine whose characters are different from Western Medicine and Oriental Medicine can be regarded as a sort of Alternative Medicine. 3. The holistic, systematic view of life of Alternative Medicine, from which viewpoint human considered as a harmonious vital existence, is a sort of recurrence to the view of Hipporcrates. 4. From the viewpoint of Alternative Medicine, a disease is not partial but holistic disorder and is treated by utilizing natural products as a medicine based on the principle that pursues harmonious and balanced psychosomatic function. This principle of therapeutic is the same as Oriental Medicine is based on. 5. Alternative Medicine may be classified largely five category, (1) Eastern Therapy, (2) Manipulative Therapy, (3) Natural Therapy, (4) Active Therapy, (5) Therapy Involving External Powers. 6. Besides Alternative Medicine which is used in clinical treatment, Massage, Homeopathy, Bach Flower remedies, Meditation, and other Music Therapy and Color Therapy is relatively accordant with therapeutics in Oriental Medicine and should be more actively accepted in clinics and improved.
Purpose: This study aimed to investigate the optimal positions of safety grab bars for effective sit-to-stand (STS) movement by comparing the results of the STS movement while using a safety grab bar installed under two different conditions: the height of the grab bar installation was determined by (1) the Building Act and (2) the principle of proprioceptive neuromuscular facilitation (PNF). Methods: A total of 50 undergraduate students participated in this study, and they were required to perform an STS movement twice under each condition. A baropodometric platform for sitting and a Biorescue (RM Ingenierie, France) were used to collect and analyze changes in the center of pressure (COP) on the left and right sides before and after performing the STS movement. The average completion time for the STS movement was also measured for analysis. Moreover, the participants were asked to express their individual subjective preferences regarding the two positions of the grab bars. Results: The COP changes were significantly smaller when performing the STS movement with the grab bar installed at the height determined by the PNF principle than the Building Act (p<0.01), and the difference in the completion time of the STS movement was not statistically significant between the two conditions. Conclusion: The findings of this study suggest that the principle of PNF can be useful for planning therapeutic exercise as well as for proposing the optimal grab bar position for older adults and those with health-related issues when performing the STS movement. In addition, this may serve as a basic rehabilitation technique for maintaining remaining functions and providing functional efficiency.
Background: Vascular occlusive event is one of the etiologies of sudden sensorineural hearing loss (SNHL). Stellate ganglion block (SGB) induces dramatic and intense vasodilatation in head and neck. Based on this principle, SGB has used as one of the treatment modalities in SNHL. This study was performed to evaluate immediate response of SGB on pure tone audiogram (PTA) in SNHL. Methods: Forty patients were studied. Each patient received daily ipsilateral SGB in paratracheal approach using 0.2% bupivacaine for 2 weeks. On first, third, and fifth day of treatment, we checked their PTA twice 1 hour before (Pre-PTA) and after (Post-PTA) SGB. Pre- and Post-PTA were compared. Several factors were analyzed as a prognostic factor of therapeutic results. Results: Eleven of 40 patients revealed decreased PTA after SGB. Degree of decreased PTA were insignificant ($2.5{\pm}1.6$ dB). Initial and final PTA results was $76.2{\pm}22.5$ and $49.8{\pm}28.3$ dB, respectively. Thirty-one of 40 patients were improved their PTA over 10 dB. The recovery was mainly influenced by the severity of initial hearing loss (P<0.001) and slightly by age (P<0.05). However, the change of PTA after SGB, time interval to receive SGB, sex, site, and number of SGB were not correlated to therapeutic outcome. Conclusions: These results suggest that vasodilatation by SGB has no immediate improvement in SNHL. Therefore, we question whether SGB is beneficial to all patients with SNHL as a therapeutic modality.
레이저를 의료에 응용하는데 있어서 그 근본원리는 일반적인 레이저 응용기술과 차이가 없다고 할 수 있으나 응용대상이 인체라는 점에서 특수한 연구분야라 할 수 있다. 레이저를 의료에 응용하는 경우 종래의 의료기술과는 근본원리에 있어서 다른 경우가 보통이므로 새로운 의학기술로서의 연구개발이 수행되어야 할 것이다. 본 논문은 레이저 기초물리학을 비롯하여 레이저광과 생체조직의 상호작용, 치료 및 진단기술, 의료용 레이저와 광파이버 기술, 레이저 안전등에 관해 기술하고 있으며 레이저의 의학에의 전망에 대해 논의하고 있다.
Objectives This study aimed to review the Korean Constitution articles 14 and 20 of the "Law on suicide prevention" and investigate public perceptions of specific improvements to suicide prevention policies using results from the Korean 2018 National Survey on Suicide. Methods The questionnaire was designed to analyzing the act restricts sharing of patient information between hospitals, making it difficult to track suicide attempts. The questionnaire was also designed to suggest further medical and normative criteria for objective judgment of continuous follow-up utilizing suicide risk evaluations and proportional principle review that consider patients' and medical staff's basic rights. Results This study identified the result of the 1500 respondents, 79.1% believed that Korea should allow suicide prevention management to be implemented without requiring individual consent to protect suicide attempters. Conclusions According the results, I propose the following criteria for policy improvement: use of anonymized information and non-profit research for technical and ethical considerations, access to medical information only for therapeutic purposes, and use of surgical severity assessment criteria appropriate for Korea.
The trend of the concept for modern medicine is gradually forwarding to preventive medicine from therapeutic medical science. One of the most remarkable characteristics of oriental medicine is that it attaches greater importance to preventive medicine scientifically. The basic theory of oriental medicine principally takes roots in Huang Di Nei Jing and it is Si Qi Tian Shen Da Lun that is deep-rooted in the principle of "growing life" grounded in theory of unity of heaven and man of oriental medicine. Therefore having translated annotation of 〈Si Qi Tian Shen Da Lun·Huang Di Nei Jing Su Wen〉 which is quoted frequently and using it for appendix and comparing each views of annotators, I would like to state the results of dissertation as below. 1. Si Qi Tian Shen of title of the paper means that it keeps our health and prevent diseases in advance to control our mind and rhythm of life to the change of Yin and Yang - that is, the grower Yin, the looser Yang - following four seasons of nature environment. 2. The summary of this dissertation is that spring means things newly sprout from old ones, things become profuse, beautiful and brilliant in the summer. Autumn stands for things is harvested and allocated evenly and finally things is closed and stored. That is, in the spring and summer. If one break this principle of growth, diseases are followed after this. Therefore an excellent physician should handle diseases before they are attacked not remedy them after outbreak of diseases. It is said that a sage governs the nation before it is put into confusion. 3. These four terms standing for each season describe definitely and realistically rural life in the agricultural environment, in other words, spouting in spring, growing in summer, harvesting in autumn, storing in winter. Going with the current of the times, they have developed to theoretical concept of getting, growth, harvest, store so that implied the principle of growth for four seasons. 4. It means in a concrete way "Yang would grow in the spring and summer and Yin would grow in autumn and winter" as follow. That is, when the day is long, things act energetically and emit the warmth of life and expand the vigor of growth. On the other hand, when the night is long, things lessen their activity to protect the warmth of life and to preserve the sprit of growth. In addition, we should be concerned about the work outward and try to concretely fulfill plan of business in spring and summer. It is in autumn and winter when we should grow Yin. It does not mean that we simply grow cold yin in our body to build Yin. But according to annotators, for example, Wang Bing stated that we need to make things be cool in spring and summer, and be warm in autumn and winter for improvement. Zhang Jie Bin noted that things should not be cold in spring and summer and not be too hot to improve in autumn and winter. Those principles theorized to unify physiological status into the principle of changing of Yang and Yin in the nature and enriched the contents. 원격조정 고선량 근접치료는 새로운 각광받는 테크놀로지이다. 이에, 저자들은 3년간의 서울중앙병원의 590횟수, 116 환자를 대상으로 고선량 근접치료의 그 시술과 임상적 결과를 평가해 보고자 한다 자궁경부암 58 환자와 비인두강암 26 환자의 관내 방사선치료 471 횟수, 식도암 12 환자, 내 기관지암 11 환자, 클라스킨 종양 1 환자의 관내 방사선치료 79 횟수와 유방암 4 환자, 육종 1 환자, 요도암 1 환자의 조직내 방사선치료 40 횟수가 1989년 9월에서 1992년 8월 사이에 시행되어졌다. 추적 관찰 기간은 1 개월에서 35개월이었고, 그 중간 기간은 7개월이었다. 조직내 방사선치료를 제외한 모든 시술은 국소 마취로 시행하였고, 모든 환자에서 급성 합병증은 생기지 않았다. 또한, 6 환자를 제외하고는 모두 계획 선량대로 치료를 마칠 수가 있었다. 자궁경부암 58 환자에서, 비인두강암 26 환자 중 20 환자에서 완전 관해가 일어났으며, 증상 완화 목적으로 치료한 15 환자에서 $80{\%}$의 환자에서 소기의 목적을 이룰 수 있었다. 이 논문에서 원격조정 고선량 근접치료의 자세한 시술과 그에 따른 결과를 설명할 것이다. 이 치료의 생물학적인 효과와 적당한 선량/선량 횟수/분할치료를 평가하기 위해, 우리는 더욱 긴 추적 관찰을 시행하여야 하며, 이 새로운 근접치료 시술이 저선량 근접치로 시술보다 효과적이고 외래 환자로 통근 치료가 가능하며, 안전한 치료 방법이라고 생각하고 있다.
This report describes the studies as follows through the research into the data of Fructus Forsythiae main blended prescriptions from Dongeuybogam. Through investigation on prescriptions from Dongeuybogam with Fructus Forsythiae as an ingredient, the following conclusions were reached. 1. All kinds of infection in a boil and the malignant tumor recorded the largest number of clinical frequency of the prescriptions in therapeutic use when Fructus Forsythiae was taken as a principle medicine. 2. The prescriptions are compounded with Fructus Forsythiae as a principle medicine can apply to eye disease, nose and ear disease, throat disease, trauma, the malignant tumor, skin disease, dental disease and heat pathogenic disease. 3. Various pathogenic factors such as wind-heat, heat, fire, wind, qi, poison, cold are put to practical use. 4. The dosage of Fructus Forsythiae is 2bun(about 0.75 gram) to 3jeon(about 11.25 gram), however 1joen(about 3.75 gram) has been taken the most for clinical application. 5. With examination the formulae of presciptions containing Fructus Forsythiae, I found the basic prescriptions as well as the medicines are in conjunction with Fructus Forsythiae and the theoretic grounds of formula in combining Fructus Forsythiae as a group of medicine for prescription through this research.
