A study on the evaluation of elderly care facilities for elderly people with dementia using the therapeutic environment elements (치유환경요소를 이용한 치매노인요양시설 평가 연구)

  • This study is to realize the importance of the therapeutic environment of nursing facilities for elderly people; to comparatively analyze the therapeutic environment elements of the facilities in Gwangju, Jeonnam area, Seoul, and Japan by classifying the therapeutic environment elements according to two aspects on the basis of preceding studies; and to find out the differences among the three regional groupsnaFirst, all therapeutic elements were put together on the basis of two preceding studies based on the two aspects. Second, an expert group was organized primarily with architects and care-givers to find out the therapeutic environment elements of nursing facilities for elderly people with dementia, and to draw up an evaluation standard. Third, the regional differences were analyzed, by comparing the distribution of therapeutic elements of the facilities in Gwangju-Jeonnam area, Seoul, and Japan, through the found therapeutic environment elements. Consequently, eighteen therapeutic environment elements were found out of total 32 elements, and the elements were classified into nine factors based on the meaning of each element. The therapeutic environment elements were showed highest in Japan, and lowest in Gwangju-Chonnam area according to the comparative case analysis based on the nine factors. Especially, there were great regional differences in terms of awareness, safety, orientation, privacy, and freedom of choice.

A Study on post-hospital Home health Care Needs in a General Hospital (일 종합병원 퇴원환자의 가정간호요구조사)

  • The purpose of this study is to provide a basic data of home health care nursing centered in the hospital by analyzing home health care needs. Data were collected from June 15. 2000 to June 23. 2000 through questionnaires taken by 208 patients to be discharge in a general hospital. The Home Health Care Need instruments used for collecting data was developed by the researcher. The data obtained were analyzed using frequency. percentage. mean. standard deviation. Chi-square test. t-test. one-way ANOVA. Pearson's correlation coefficients. Cronbach's alpha coefficients and Factor Analysis. The results of the study were as follows: 1. The perception of home health care nursing were 65.4% of subjects had never heard about home health care service and 2.4% of subjects knew about the methods & contents in detail. About the demands of home health care, 58.2% of the subjects were willing to use home health care and 41.8% weren't. 2. Regarding the areas of home health care needs. basic nursing care area was the highest. Education/ counselling, exercise/ hygiene. therapeutic nursing care was orderly. Injection and medication management of basic nursing care area were the highest. 3. In relation to characteristic-related diseases and home health care needs. as for the discharge type, the existence of sores, paralysis showed significant differences with home health care needs in the area of exercise/ hygiene(p<,05). Home health care needs in the area of therapeutic nursing care and exercise/ hygiene showed significant differences with the existence of pain. Home health care needs in the area of therapeutic nursing and basic nursing areas were significantly different in the use of catheter/ assistant instrument(p<.05). Home health care needs in the area of therapeutic nursing. exercise/ hygiene and basic nursing care showed significant difference with diagnosis(p<,05). In conclusion, awareness about home health care nursing were very low, home health care needs in the area of basic nursing was the highest. Home health care needs showed significant difference with discharge type, sores, paralysis, existence of pain, use of catheter/ assistant instrument and diagnosis.

Importance-Performance Analysis of Clinical Nutrition Management in Convalescent Hospitals in the Gyeongnam Area (경남지역 요양병원 임상영양관리에 대한 중요도와 실행도 분석)

  • The purpose of this study was to analyze the importance-performance of clinical nutrition management in convalescent hospitals. The research was carried out based on questionnaires administered from March to April, 2015 to 73 dietitians at 40 convalescent hospitals in the Gyeongnam area. There was a statistically significant difference between the mean scores for importance (4.01/5.00) and performance (2.95/5.00) of clinical nutrition management. The importance and performance grid analysis showed that participation in a nutritional management committee, administration of patients using a cooperation program among hospital departments, cooperation with a medical team on patient's nutrition status, nutrition initial assessment, nutrition care process for patients showing malnutrition, nutrition care process for tube feeding patients, management of a therapeutic diet, meal management using dietary slip instructions including a therapeutic diet, and explication of a therapeutic diet for patients scored high regarding importance and performance (doing great area). Medical records on patient's nutrition management, and nutrition counseling for requested patient scored low regarding the importance and high regarding performance (overdone area). Participation in medical rounds, personal nutrition education for patients, group nutrition education for patients, nutrition education for medical teams, development of a menu for therapeutic diet and standardized recipes, and provision of information on diet therapy for patients after discharge scored low regarding importance and performance (low priority area). Accreditation of convalescent hospitals and interest of medical professionals in clinical nutrition management were effective variables for the importance-performance gap of clinical nutrition management. In conclusion, the accreditation process and positive awareness of medical professionals with regard to clinical nutrition management had positive effects on reduction of the importance-performance gap in clinical nutrition management at convalescent hospitals. The strength of clinical nutrition management in the accreditation and development of an education program for increasing medical team or administrator interest in clinical nutrition management could lead to improvement of clinical nutrition management for elderly patients in convalescent hospitals.

Landscape Characteristics of Environmentally Friendly Apartments in the Dimension of Therapeutic Garden (치료정원 관점에서 본 친환경 아파트의 조경실태)

  • Apartments recently built have been strategically designed in many ways, and one of the most striking aspects is to utilize the idea of environmental friendliness and the application to the living environment. Especially, the number of environmentally friendly apartments that concern the health of residents have been increasing and their landscape is notably differentiated. The purpose of this research study was to examine the landscape of the environmentally friendly apartments that have been built in the past five years, and to assess its features in the aspects of therapeutic garden. Based upon the review of previous studies, this study constructed a checklist that was used to evaluate the planning components of the landscape during site visits. This study found that overall landscape selected in the study was well-planned. However, it was suggested that more features of therapeutic garden such as vegetation area which both encourages interaction with residents and produces therapeutic benefits be adopted in planning the advanced therapeutic garden of environmentally friendly apartments.

A Study of the Relationship between Compliance with Therapeutic Regimens and Physiological Parameters of Hemodialysis Patients (혈액투석 환자의 치료지시 이행정도와 생리적 지표와의 관계)

  • Purpose: This study was done to investigate correlations between compliance and physiological parameters of hemodialysis patients. Method: The subjects were 102 patients on hemodialysis at 3 hospitals in B city. Data was collected using Shon(l986)'s questionnaire and measuring physiological parameters (serum urea nitrogen, creatinine, hemoglobin, albumin, potassium, phosphorus, interdialytic weight gain). Result: Mean scores of compliance with the therapeutic regimen was $4.00\pm$ 0.55 on a 5 point scale. The area of visiting hospitals and taking medicines . were shown to have high compliance with therapeutic regimens; on the other hand, the areas concerning diet and symptoms were shown to be low. Interdialytic weight gain and phosphorus were significantly related to the compliance with therapeutic regimens. Conclusion: Hemodialysis patients' therapeutic compliance was related to the physiological parameters(potassium, phosphorus, interdialytic weight gain). Therefore, these findings give hemodialysis patients useful information for raising their therapeutic compliance.

  • The possibility of using polypyrrole as a drug delivery system(DDS) has been studied using indicate (Phenol red) and substance with therapeutic activity(Sodium salicylate). In aqueous solution, negative potential is applied to polypyrrole then anion(with therapeutic activity) of sodium salicylate is released by redox processes of polypyrrole. The release amount of anionic drugs from polypyrrole is measured by UV-visible spectrometer which can measure UV-absorbance of materials. Electrode area that use for release amount measurement is$50mm^{2}(5{\times}10mm)$,and thickness of polypyrrole membrane is $15{\mu}m$. DC 1V applied in saline solution, the release amount according to time increased gradually. In various electrode area, release amount of anionic drug is directly to electrode area.

Conformal Radiotherapy in a Patient with Cancer at the base of the Tongue in a Previously Irradiated Area (방사선치료 조사영역 내에 발생한 설암 환자에서 입체조형방사선치료 경험 : 증례보고)

  • Objectives: We report an interim result of conformal radiotherapy in a patient with early stage cancer at the base of the tongue, which developed in a previously irradiated area. Materials and Methods: A 64-year-old male patient was diagnosed with T4N0M0 supraglottic cancer. He received 72Gy of radiation therapy from 21 November 1988 to 24 February 1989. He had local failure and underwent a salvage total laryngectomy on 28 August 1989. Subsequently, he did well. In early 1999, he suffered from throat pain. He had a 2.5cm ulcerative mass at the base of his tongue, in the area that had been irradiated previously. Biopsy showed squamous cell carcinoma. After workup, he was diagnosed with base of tongue cancer with T2N0M0. Surgery was not feasible because the morbidity was not acceptable. Since it was difficult to re-irradiate the area with a curable dose using conventional 2D radiation therapy with an acceptable morbidity, we decided to try conformal radiotherapy. We used 7 static beam ports with field sizes from $7x6.4\;to\;8x8cm^2$, using 6 and 10MV photons. The fractionation regimen was 1.8Gy, 5 times per week. He received 64.8Gy in 36 fractions from 9 April 1999 to 1 June 1999. Results: In the 21 months since radiotherapy, the patient has not experienced any acute or chronic complications, such as xerostomia. He experienced relief of pain shortly after the start of radiotherapy, showed a complete response, and is still doing well. Conclusion: Conformal radiotherapy can be used to treat cancer that develops within a previously irradiated field, with curative intent.

  • It has been demonstrated that the injection of naked DNA expressing vascular endothelial growth factor 165(VEGFl165) to the affected area can provide significant therapeutic effects on peripherol artery occlusive diseases. Success with this type of gene therapy highly depends on the quality of the vector delivering the therapeutic gene, especially in terms of the level and duration of gene expression in the localized area. We have recently developed a vector expressing VEGF165(pCK-VEGF) for the treatment of peripheral artery occulsive diseases and demonstrated high level expression of VEGF165 in mouse skeletal muscle. This study was designed to assess the acute toxicity of intramuscularly injected pCK-VEGF in BALB/c mice and Sprague-Dawely rats. There was no evidence of any changes in clinical signs, body weights, or gross pathological signs. We estimate LD50 values of pCK-VEGF higher than 50mg/kg in mice and 20 mg/kg in rats by intramuscular injection.

  • In the case of designing a high dose rate remote controlled afterloading treatment room with existing hospital facilities. We must construct the effective protective barriers so as to reduce the primary and scattered radiation up to the maximum permissible dose level. It is difficult to reinforce the barrier thickness of the shielding requirements because of the limited space and the problem of the existing building structure at the surrounding area. Therefore we can reduce the intensity of primary radiation to the required degree at the location of interest with installing the appropriate I shaped Pb barriers between the radiation source and the shielding wall of the concrete. As a result, it was possible to reduce the intensity of the primary radiation below the M.P.D level by using additional Pb barriers instead of increasing thickness of concrete wall.

  • The objective of this study was to find out the present status of the physio - therapeutic facilities in oriental hospitals The material were used in this study The literatures or annual reports of 15 oriental hospitals which present the status of input resources of physio-therapy departments and performances, Major findings of this study were as follows The average scale of input resource of physio-therapy departments are in difference among hospitals a) number of rooms : 4.5 in average(5.1 in university hospitals, 3.5 in other hospitals) b) size of therapeutic facility' area : $20.0m^{2}$ in average($19.3m^{2}$ in university hospitals, $20.3m^{2}$ in other hospitals) c) number of therapists : 8.6 in average(9.3 in university hospitals, 7.5 in other hospitals)
