Analysis of Personal Gait Characteristics According to Legs Imbalance Gait (하지 보행 불균형 상태에 따른 개인별 보행 특성 분석)

  • In the present study, to determine walking imbalance using the walking analysis method, where limitations in the existing walking analysis have been minimized, we propose a new walking analysis method that adopts the following: self-developed equipment to measure the angles of left-right hip joints and knee joints; a determination system using symmetry index (SI); and dynamic time warping (DTW) similarity analysis algorithm to analyze individual walking styles. Normal and imbalanced walking tests were conducted for 12 subjects without walking disorder. From the SI calculation to determine imbalanced walking, both the normal and imbalanced walking styles can be determined using the angle measurements of the left-right hip joints and knee joints. In the analysis of the individual walking styles, the similarities at the center of the lower back, left-right thighs, and dorsum of the feet of the 12 subjects in both normal and imbalanced walking cases were compared. From the similarity analysis of the measured values during the normal and imbalanced walking tests, I determined that the walking pattern does not maintain the same stance when the body parts move during walking.

Similarity of Gene Expression Profiles in Primary Brain Tumors with the Toxic Mechanism by Environmental Contaminants

  • Recently, a large number of clinical experiments have shown that exposure of organic pollutants lead to various cancers through the abnormal cell growth. Environmental pollutants, such as 2, 3, 7, 8-Tetrachloro dibenzo-p-dioxin (TCDD) and polycyclic aromatic hydrocarbons (PAHs), are carcinogen and are known to cause the cognitive disability and motor dysfunction in the developing of brain. The effects of these pollutants on neurodevelopmental disorder is well established, but the underlying mechanism(s) and similarity of gene expression profiles in human brain tumors with organic pollutants still remain unclear. In this study, we first examined the gene expression profiles in glioblastomas compared with meningioma that are kinds of primary human brain tumor by using human cDNA microarray. The results of cDNA microarray analysis revealed that 26 genes were upregulated (Z-ratio>2.0) and 14 genes were downregulated (Z-ratio<-2.0) in glioblastoma compared with meningioma. From the altered gene patterns, mitogen-activated protein kinase (MAPK) signaling related genes, such as MAP2K3, MAP3K11 and jun activated domain binding protein, and transcription factors, such as UTF2 and TF12, were upregulated in glioblastoma. Also, we tried to investigate the relation between important genes up- and down-regulated in giloblastoma and various organic pollutants. Therefore, the identification of changes in the patterns of gene expression may provide a better understanding of the molecular mechanisms involved in human primary brain tumors and of the relation between gene expression profiles and organic pollutants in brain tissue.

  • A sleep disorder is being recognized as one of the major health issues related to high levels of stress. At the same time, interests about quality of sleep are rapidly increasing. However, diagnosing sleep disorder is not a simple task because patients should undergo polysomnography test, which requires a long time and high cost. To solve this problem, an accelerometer embedded wrist-worn device is being considered as a simple and low cost solution. However, conventional methods determine a state of user to "sleep" or "wake" according to whether values of individual section's accelerometer data exceed a certain threshold or not. As a result, a high miss-classification rate is observed due to user's intermittent movements while sleeping and tiny movements while awake. In this paper, we propose a novel method that resolves the above problems by employing a dynamic classifier which evaluates a similarity between the neighboring data scores obtained from SVM classifier. A performance of the proposed method is evaluated using 50 data sets and its superiority is verified by achieving 88.9% accuracy, 88.9% sensitivity, and 88.5% specificity.

A Study on the Determination of Instantaneous Centre of Rotation and the Mandibular Movement (순간 회전 중심 결정과 하악골 운동에 관한 연구)

  • Recently the instantaneous centre concept has been used to understand the biomechanics by which a tissue derangement causes a mechanical derangement in temporomandibular joint. In this study, four male subjects without temporomandibular joint disorder and malocclusion were selected for the determination of the instantaneous centre of rotation (I.C.R) in the mandibular movement. The habitual opening and closing paths were recorded on the sagittal plate by two spring pencils attached on the lower anterior teeth which was designed for this study, and the I.C.R. was calculated by the computer program of Rouleaux's method. Also the computer graphic opening and closing movements of mandible were obtained according to the determined I.C.R. The results obtained from this study were as follows. 1. The instantaneous centres of rotation were not positioned within the condyle in the mandibular opening and closing movement. 2. There was some similarity between the anatomical curvature of the articular emience and the movement pattern of condyle. 3. The opening path and the closing path of the most superior pl)int of the condyle stowed a slight difference. 4. At the early stage of the habitual opening movement, the condyle was moved downward. 5. The opening and closing mandibular movements were simulated by the instantaneous centre of rotation which was determined by the computer program of Rouleaux's method.

A New Classification for Cervical Ossification of the Posterior Longitudinal Ligament Based on the Coexistence of Segmental Disc Degeneration

  • Objective : Classification systems for cervical ossification of the posterior longitudinal ligament (OPLL) have traditionally focused on the morphological characteristics of ossification. Although the classification describes many clinical features associated with the shape of the ossification, including the concept of spondylosis seems necessary because of the similarity in age distribution. Methods : Patients diagnosed with OPLL who presented with increase signal intensity (ISI) on magnetic resonance imaging were surgically treated in our department. The patients were divided into two groups (pure versus degenerative) according to the presence of disc degeneration. Results : Of 141 patients enrolled in this study, more than half (61%) were classified into the degenerative group. The pure group showed a profound male predominance, early presentation of myelopathy, and a different predilection for ISI compared to the degenerative group. The mean canal compromise ratio (CC) of the ISI was 47% in the degenerative group versus 61% in the pure group (p<0.0000). On the contrary, the global and segment motions were significantly larger in the degenerative group (p<0.0000 and p=0.003, respectively). The canal diameters and global angles did not differ between groups. Conclusion : Classifying cervical OPLL based on the presence of combined disc degeneration is beneficial for understanding the disorder's behavior. CC appears to be the main factor in the development of myelopathy in the pure group, whereas additional dynamic factors appear to affect its development in the degenerative group.

  • Multidimensional scaling (MDS) is an exploratory analysis of multivariate data to represent the dissimilarity among objects in the geometric low-dimensional space. However, a general MDS map only shows the information of objects without any information about variables. In this study, we used MDS based on the algorithm of Torgerson (Theory and Methods of Scaling, Wiley, 1958) to visualize some clusters of objects in categorical data. For this, we convert given data into a multiple indicator matrix. Additionally, we added the information of levels for each categorical variable on the MDS map by applying the partition method of Shin et al. (Korean Journal of Applied Statistics, 28, 1171-1180, 2015). Therefore, we can find information on the similarity among objects as well as find associations among categorical variables using the proposed MDS map.

Is Rectosigmoid Vaginoplasty Still Useful?

  • Background The ideal vaginoplasty must be successful functionally as well as have a natural appearance, and also must retain its functionality and appearance over the long term. Conventional vaginoplasty techniques have functional limitations and are associated with recurrent complications, but rectosigmoid vaginoplasty is known to have a high satisfaction rate due to its functional similarity with the vagina. We conducted the present study to assess the usability of rectosigmoid vaginoplasty over the course of long-term follow-up. Methods From March 1992 to February 2014, 84 patients were treated with rectosigmoid vaginoplasty; 44 had gender identity disorder, 29 had vaginal agenesis, 8 had female pseudohermaphroditism, and 3 had gynecologic malignancies after radical pelvic surgery. This retrospective study was based on a review of the patients' records, clinical examinations, complications, and questionnaires about appearance, function, and sexual intercourse. Results All patients who underwent rectosigmoid vaginoplasty were discharged within 2 weeks without surgical flap loss. The early complications were partial flap necrosis, difficulty in defecation, mucous hypersecretion, and postoperative ileus. The late complications were vaginal introitus contracture, vaginal prolapse, and difficulty in urination. The mean length and diameter of the neovagina 3.4 years after rectosigmoid vaginoplasty were 13.2 cm and 3.8 cm, respectively. On questionnaires about satisfaction, 70% of patients reported excellent satisfaction, 11% good, 12% fair, and 7% poor. Conclusions Rectosigmoid vaginoplasty is useful, safe, and well-accepted operative method with good functional and cosmetic results, such as natural lubrication and adequate vaginal length and width obtained without requiring the use of a dilator.

Clinical Characteristics of Peroneal Nerve Palsy by Posture

  • Objective : Posture induced common peroneal nerve (CPN) palsy is usually produced during the prolonged squatting or habitual leg crossing while seated, especially in Asian culture and is manifested by the onset of foot drop. Because of its similarity to discogenic foot drop, patients may be diagnosed with a lumbar disc disorder, and in some patients, surgeons may perform unnecessary examinations and even spine surgery. The purpose of our study is to establish the clinical characteristics and diagnostic assessment of posture induced CPN palsy. Methods : From June 2008 to June 2012, a retrospective study was performed on 26 patients diagnosed with peroneal nerve palsy in neurophysiologic study among patients experiencing foot drop after maintaining a certain posture for a long time. Results : The inducing postures were squatting (14 patients), sitting cross-legged (6 patients), lying down (4 patients), walking and driving. The mean prolonged neural injury time was 124.2 minutes. The most common clinical presentation was foot drop and the most affected sensory area was dorsum of the foot with tingling sensation (14 patients), numbness (8 patients), and burning sensation (4 patients). The clinical improvement began after a mean 6 weeks, which is not related to neural injury times. Electrophysiology evaluation was performed after 2 weeks later and showed delayed CPN nerve conduction study (NCS) in 24 patients and deep peroneal nerve in 2 patients. Conclusion : We suggest that an awareness of these clinical characteristics and diagnostic assessment methods may help clinicians make a diagnosis of posture induced CPN palsy and preclude unnecessary studies or inappropriate treatment in foot drop patients.

Safty of Alternatives for Endocirne Disrupting Substances (내분비계장애물질 대체소재의 안전성)

  • Endocirne disruptors (EDs) can cause fertility decrease, developmental disorder, and even cancer in animals. Until 90's, EDs were used in various synthetic products including paints, coatings, detergents, plastics, and plasticizers. Currently, in several countries, the production, trade and use of EDs or EDs-suspected chemicals have been regulated while activity to screen the alternatives for EDs including bisphenol-A, phthalate and nonylphenol is active. Although various toxicity test method was developed and applied for screening of alternatives, however, the safety of alternatives has been not fully demonstrated. Some alternatives have high structural similarity with existing EDs, raising the possible risk of endocrine disruption by alternatives. In an effort to develop the safe alternatives, we reviewed the effects of EDs such as bisphenol-A, phthalates, nonylphenol and their substituents. In addition, in-silico analysis for endocrine disrupting activities of some alternatives was presented.

SEX DIFFERENCES IN BEHAVIOR PROBLEM PATTERNS (문제행동 양상에서의 성차이)

  • In an attempt to study sex differences in child behavior disorders, 834 clinic-refered children(582 boys and 252 girls) between the ages of 6 to 11 were assessed using CBCL parental form and the data were factor analysed to form empirically derived syndromes for each sex. The analyses yielded eight behavior disorder syndromes for boys and ten, for girls. Six syndromes(aggressive, hyperactive, delinquent, social withdrawal, emotional lability, physical complaints) were found in both sexes while obsessive, depressive and psychotic syndromes were organized differently in boys and girls. There were also considerable differences in item composition of the six syndromes common to box sexes, suggesting that clinical features of common behavior disorders such as aggression and hyperactiveity might be different for boys and girls despite their apparent similarity. The results were discussed in terms of culturally shared attitudes and beliefs concerning sex differences in behaviors.
