The Treatment of Giant Middle Cerebral Artery Aneurysm with MDS Coil - Case Report - (기계적 분리코일(MDS coil)을 이용한 소아 거대 중대뇌동맥류의 치료)

  • A 14-year old boy was admitted with stuporous mentality. CT scan, MRI and cerebral angiogram revealed SAH and a giant aneurysm of right middle cerebral artery($4{\times}5{\times}5.3cm$). To minimize surgical risk, endovascular treatment was done with MDS(mechanical detachable system)-spiral coil. Follow up MRI showed intraluminal thrombus formation of the aneurysm.

  • Intracranial hypotension syndrome typically occurs spontaneously or iatrogenically. It can be associated with headache, drowsy mentality and intracranial heamorrhage, Iatrogenic intracranial hypotension can occur due to dural pucture, trauma and spine surgery. Treatment may include conservative therapy and operation. We report a case of a 54-year-old man who was successfully treated with epidural blood patches for intracranial hypotension due to cerebrospinal fluid (CSF) leakage into the lumbosacral area after spine surgery.

  • Viral Encephalitis are a serious meningeal infection occurred by various chemical substance. Chief symptoms are headache. hyperesthesia in child and fretting. irritation in infant. Adolescence complain of a retrobulbar pain, fever, vomiturition, photophobia. Fever can cause choreic movement, mental disorder and seizure. Most of patients have complete recovery, but if there are brain lesions, it is bad prognosis. Occasionally patients complain of continuous headache, mental disorder, ataxia and marasmus. We report on a 4-year-old male patient with viral encephalitis. It has been 40 days since disease. He had disorientation mentality, dysphagia and fever.

Terson Syndrome Caused by Intraventricular Hemorrhage Associated with Moyamoya Disease

  • Terson syndrome was originally used to describe a vitreous hemorrhage arising from aneurysmal subrarachnoid hemorrhage. Terson syndrome can be caused by intracranial hemorrhage, subdural or epidural hematoma and severe brain injury but is extremely rare in intraventricular hemorrhage associated with moyamoya disease. A 41-year-old man presented with left visual disturbance. He had a history of intraventicular hemorrhage associated with moyamoya disease three months prior to admission. At that time he was in comatose mentality. Ophthalmologic examination at our hospital detected a vitreous hemorrhage in his left eye, with right eye remaining normal. Vitrectomy with epiretinal membrane removal was performed. After operation his left visual acuity was recovered. Careful ophthalmologic examination is mandatory in patients with hemorrhagic moyamoya disease.

  • Nocardia brain abscess is rare. We report on a unique case of N. farcinica brain abscess in a liver transplant recipient, following Aspergillus fumigatus pneumonia. A 43-year-old liver transplant recipient presented with altered mentality at 2 months after A. fumigates pneumonia. He was successfully treated with surgical removal and antibiotic therapy with trimethoprim-sulfamethoxazole and ceftriaxone.

  • The poems of Shin Suk-ju can be divided into two periods around the political change by King Sejo; his poems from the former period show that the poet enjoyed abundant pride and leisurely spirit in the self-satisfying world driven by his determination to maintain a pure heart and save and awaken the people during the reign of King Sejong. His ideology of awakening the people, however, was the product of his heroic consciousness to achieve immortal fame. It was his heroic consciousness and determination to sacrifice his life for fame according to the mandate from Heaven that made him join the political change by King Sejo. His poems from the latter period clearly reveal that the way of his life to pursue fame didn't bring him spiritual satisfaction and happiness. He confessed that his conscience was destroyed as he joined King Sejo in his political change and the deeds he achieved and further his life itself were all in vain. He lost the values or orders he pursued, which caused instability in his life. Facing such instability head-on, he argued that right and wrong, true and false, and good and evil mentioned in the world were all subjective and groundless. Furthermore, he realized all the things and creatures of the world were nothing but phantoms. Those perceptions he had were based on Madhyamaka of Buddhism. Going through such a thinking process, the poet wrote about his mentality of a false reputation with ideal mentality. Heroic consciousness, Buddhist thinking, and pursuit of mentality of a false reputation found in his poems make also frequent appearances in the poems by major literary men in the latter half of the 15th century such as Seo Geo-jeong. His serious searches to overcome his conscientious agony and sense of futility about life had influences on the attitudes toward life and literature of the official literary men of the times. Seong Hyeon's statement that the major literary figures of the times inherited the literary tradition of Shin Suk-ju was not a rhetoric by courtesy.

  • (1986) and (1987) represent the society of 1980s in which the professional baseball game was initiated to cover the irrational military culture. The love and marriage of sports players were the headlines of the media, and the yearly salary of the players was the hottest issue of conversation. The military culture is represented in the scenes where the coaches train the failures and inapt players in extreme drills. The films pinpoint the absurdity of military culture and win-at-all-costs mentality. The collapse of the dictatorial leadership at the end of the films is a metaphor for the collapse of the fifth Republic of Korea. The episodes where the players talk about contract money, and the trade of players and sports business were a new phenomenon of the 1980's. The fact that Oh Hyesung of chooses love instead of victory deals a big blow to the secular ambition for money, victory and dictatorial leadership. His option provides catharsis for an audience oppressed under military leadership and success driven ideology. On the other hand, Oh Hyesung of dies right at the moment of winning the world champion. He achieves neither love nor success. While Oh Hyesung of is a symbol of pure love and gives spiritual comfort to the audience, Oh Hyesung of gives a sense of hopelessness to the audience. Both of the two sports films reflect the representation of the 1980's but received opposing reviews from audiences.

  • This study criticizes the wave of liberalization, privatization, thoughtless and uncontrolled over-competition in the electronic communication networks area. This criticism is based on the 'transport economics' on one side, the 'network neutrality debate' on the other side. Through these standpoints, this study insists that it needs to deal with a social agenda related to the problems such as unplanned and unbalanced development of electronic communication networks, double/tripple-investment in the communication industry, conflicts among network companies and the decline of public values. Also does this study stress on the new ideas in the electronic communication network policy and planning to manage these problems. Electronic communication networks are converging and their borders are blurred. As a result of these flows, the field of information technology, media and contents is faced with radical changes. These changes could be so critical and complicated that nobody can diagnose their multi-level implications. However, the one thing sure is that we should start from the communication networks to think and control a communication system. Therefore, we should discuss and plan the now-and-future of communication infrastructure and its management. we need not only a market mentality but also a socio-cultural mentality. In this respect, this study does insist that it requires the vision of digital democracy and democratic communication ecology to put the steps toward social communication system.

  • Studies that can find resolutions to problems of subjective psychiatric analysis must be performed and indeed they are in the process. However there still lies many problems in researches of mentality examination, which should be the foundation of finding potential resolutions. One of the biggest problems in the conventional system is that there are many different opinions by psychiatrists depending on their educations and experiences. There is no objective standard on the subjects and there is no effective psychiatric analysis method. Also, the characteristic of such examinations is that it cannot be applied to disabilities, foreigners and infants alyce the examination is ch examinconversation. The objective of this atudy is to standardize TAT(Thematic Apperception Test)analysiBallken index method so that subjective data from the examination can be excluded and the examination thus maklysithe examination objectified. Furthermore, objective result and patients' brain wave pattern is read with BCI(Brain Computer Interface) ch exaTherenvironment to Alsare it to brain wave characteristics vectors to reate brain-wave characteristics vector DB. Therefore, such DB can be utilize durlysithe examination and diagnosis to reate objective examination method and standardized diagnosis system. Thus, mentality examination can be performed only with brain-wave scans with BCI based TAT system.

The Experiences of the Emergency Antidote Stock and Delivery Service by the Korean Poison Information Center (독극물정보센터 구축사업의 일환으로 시행한 전국단위 응급해독제 비축 및 배송경험)

  • Purpose: Antidotes for toxicological emergencies can be life-saving. However, there is no nationwide stocking and delivery system for emergency antidotes in Korea. We report on a two-year experience of a nationwide stocking and delivery trial for emergency antidotes at emergency departments in Korea. Methods: An expert panel of clinical toxicologists reviewed and made a list of 15 stocked antidote. These antidotes were purchased or imported from other countries and delivered from 14 antidote stocking hospitals nationwide 24 hours per day, seven days per week. Results: From August 1, 2011 to April 30, 2013, 177 patients with acute poisoning, with a median age of 48.5 years, were administered emergency antidotes. The causes of poisoning were intentional in 52.0% and 88.0% were intentional as a suicide attempt. Regarding clinical severity, using the poisoning severity score, 40.7% of patients had severe to fatal poisoning and 39.0% had moderate poisoning according to clinical severity. The most frequent presenting symptom was neurologic deficit, such as altered mentality (62.7%). alerted mentality (62.7%). Emergency antidotes were administered as follows: methylene blue (49 cases), flumazenil (31), N-acetylcysteine (25), glucagon (17), 100% ethanol (15), cyanide antidote kit (12), anti-venin immunoglobulin (5), pyridoxine (4), hydroxocobalamine (2), and deferoxamine (1). The median time interval from antidote request to delivery at the patient's bedside was 95 minutes (interquartile range 58.8-125.8). Conclusion: Findings of this study demonstrated the possibility of successful operation of the nationwide system of emergency antidotes stocking and delivery in Korea.
