The Interpretation of Joint Line Tenderness in Meniscal Injury (반원상 연골판 손상에서의 관절선 압통의 해석)

  • Purpose: Meniscal injuries are very common sports problems and indications for knee surgery. We analyzed the effectiveness of joint line tenderness retrospectively. Materials and Methods: From May 2005 to June 2006, 76 knees which were diagnosed meniscal injury and performed arthroscopic surgery by same surgeon at military hospital were included. We analyzed sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of joint line tenderness in meniscal diagnosis. Results: The joint line tenderness gave such results (78.8%, 32.6%, 52.6%, 47.3%, 66.7% for sensitivity, specificity, accuracy, positive predictive value, negative predictive value respectively). We got similar results in analyses with medial meniscal lesion, lateral meniscal lesion, and combined instability patients. Conclusion: The joint line tenderness is a easy and comfortable maneuver but, it's effectiveness is low when it is used lonely The composite examinations including MRI and diagnostic arthroscopy for meniscal injuries of the knee perform much better than joint line tenderness alone.

  • This is a case report and review of literature of the facial asymmetry corrected by various surgical methods. In case 1, facial asymmetry resulted from osteochondroma of condyle and corrected via condylectomy only. In case 2, unilateral condylar hyperplasia with compensatory maxillary growth resulted in severe facial asymmetry. Procedures used in case 2 are vertical ramus osteotomy, condylectomy, genioplasty, mandibular inferior border ostectomy and Le Fort I osteotomy.

  • South Korea suggest that the United States government should delay the date to hand over wartime operational control (OPCON). It is fixed on December 1, 2015. Typically the president's position as head of the executive authority is coming from the Commander-in-Chief of the Armed Forces. Now we should know the commander-in-chief of the nation's forces and the meaning of OPCON. The relationship between sovereignty and the commander-in-chief of the nation's forces is a little different. This is one of the efforts to enlarge the efficiency of Armed Forces. Of course, operational control is little control of the sovereign personnel, logistics, administration, discipline, organizing, training, etc. It does not control all matters. The current ROK Combined Forces Command itself is made with reference to the North Atlantic Treaty Organization(NATO). If we insist our own sovereignty is infringed because of OPCON, that claim is coated in irreparable damage to the national interest.

Vitamin D Status in South Korean Military Personnel with Acute Eosinophilic Pneumonia: A Pilot Study

  • Background: A relationship between low vitamin D levels and the development or outcomes of respiratory diseases has been identified. However, there is no data on the vitamin D status in patients with acute eosinophilic pneumonia (AEP). We evaluated the vitamin D status in patients with AEP among South Korean military personnel. Methods: We prospectively compared the serum levels of total 25-hydroxyvitamin D [25(OH)D], 25(OH)D3, and 25(OH)D2 among patients with AEP, pulmonary tuberculosis (PTB), and community-acquired pneumonia (CAP). Results: In total, 65 patients with respiratory diseases, including AEP (n=24), PTB (n=19), and CAP (n=22), were identified. Of the 24 patients with AEP, 2 (8%) had deficient total 25(OH)D levels (<10 ng/mL), 17 (71%) had insufficient total 25(OH)D levels (${\geq}10$ to <30 ng/mL), and only 5 (21%) had sufficient total 25(OH)D levels (${\geq}30$ to <100 ng/mL). The difference in the total 25(OH)D levels among patients with AEP, PTB, and CAP was not statistically significant (p=0.230). The median levels of total 25(OH)D, 25(OH)D3, and 25(OH)D2 were 22.84, 22.84, and 0.00 ng/mL, respectively, and no differences in the 25(OH)D level were present among patients with AEP, PTB, and CAP with the exception of the total 25(OH)D level between patients with AEP and PTB (p=0.042). Conclusion: We have shown that low vitamin D levels are frequently found in patients with AEP and are comparable with those in patients with PTB and CAP.

The novel gene LRP15 is regulated by DNA methylation and confers increased efficiency of DNA repair of ultraviolet-induced DNA damage

  • LRP15 is a novel gene cloned from lymphocytic cells, and its function is still unknown. Bioinformatic data showed that LRP15 might be regulated by DNA methylation and had an important role in DNA repair. In this study, we investigate whether the expression of LRP15 is regulated by DNA methylation, and whether overexpression of LRP15 increases efficiency of DNA repair of UV-induced DNA damage in HeLa cells. The results showed (1) the promoter of LRP15 was hypermethylated in HeLa cells, resulting a silence of its expression. Gene expression was restored by a demethylating agent, 5-aza-2'-deoxycytidine, but not by a histone deacetylase inhibitor, trichostatin A; (2) overexpression of LRP15 inhibited HeLa cell proliferation, and the numbers of cells in the G2/M phase of the cell cycle in cells transfected with LRP15 increased about 10% compared with controls; (3) cyclin B1 level was much lower in cells overexpressing LRP15 than in control cells; and (4) after exposure to UV radiation, the LRP15-positive cells showed shorter comet tails compared with the LRP15-negative cells. From these results we conclude that the expression of LRP15 is controlled by methylation in its promoter in HeLa cells, and LRP15 confers resistance to UV damage and accelerates the DNA repair rate.

Trivial Trauma and Non Pathological Delayed Splenic Rupture: A Case Report (경미한 외상에 의한 지연성 비장 손상)

  • Although a majority of patients with splenic rupture present acutely, up to 15% present with a delayed rupture days to weeks following a substantial abdominal injury. The mortality for patients presenting with acute splenic rupture is approximately 1% whereas that associated with delayed rupture approaches 15%. Although many cases of delayed splenic rupture have been reported, the majority of those reports present delayed splenic rupture associated with an underlying systemic disorder such as liver or kidney disease, or another hematologic disorder. We found a delayed splenic rupture case that documented the normal spleens of young healthy soldiers after trivial abdominal trauma, and we have had successful treatment experience with delayed rupture of a normal spleen after trivial trauma. Therefore, we want to review the literature and discuss the phenomenon of delayed rupture of the spleen following trivial trauma.

Recent U.S. Efforts on RMA/MTR and Suggestions for ROK Military Progress (미국의 군사혁명(RMA/MTR)추세와 우리군의 미래발전방향)

  • Sun Tzu once clarified that "One who knows the enemy and knows himself will not be endangered in a hundred engagements, " while Clausewitz explained that "A victory will be guaranteed for one who can overcome 'fog and friction' at a battlefield." Now the U.S. is striving hard to develop an American version of RMA (Revolution in Military Affairs)/MTR (Military Technical Revolution) by utilizing its rapidly emerging information technology in the information age. The U.S. firmly believes that its RMA/MTR with less defense expenditure can provide all combat players in the battlespace with "dominant battle awareness, " by which no nation will be able to challenge U.S. military superiority. In this paper, the recent efforts of the U.S. Armed Forces on RMA/MTR will be briefly introduced, some lessons and expertises from the U.S. RMA/MTR are extracted, and, in this regard, several suggestions are provided for the progress of the ROK Armed Forces.gress of the ROK Armed Forces.

  • We experienced a case of an athlete with a painful mass on the distal peroneal musculature after sports activity, and diagnosed as the entrapment syndrome of superficial peroneal nerve. We treated the case with the mini-open and subcutaneous fasciotomy to release the entrapped peroneal nerve. We report the case with a review of the literature.

  • Ophthalmic complications after tooth extraction are rare but discomforting events. This paper reports the rare complications of a 20-year-old male patient who presented with transient blurring of vision after surgical extraction of several teeth under general anesthesia. Additional diagnostic tests were performed to discern the reason for the pseudomyopia. A literature review was carried out by searching for articles published from 1936 to 2019 using the keywords "dental," "ophthalmic," "complication," "blurring of vision," and "accommodation disturbance" in PubMed. Only six patients with detailed ophthalmic symptoms similar to those of our patient have been reported. If blurred vision or a myopic shift in refraction is present, pseudomyopia should be suspected, and cycloplegic refraction is essential for diagnosis. The condition improves spontaneously

  • The so-called “Quick Game”-models resemble a central element within the Compound Gaming approach of military systems analysis proposed by Huber [1, 2]. This paper introduces such an analytical model designed to investigate the battle-effectiveness of alternative army structures. The nature of its results is discussed from numerical experiments covering the (German) Army Structure 4, Loser's Area Coverage Defence Concept, and Afheldt's Area Defence Concept. It turns out that Loser's concept promises to have a comparatively high potential to stabilize the early conventional forward defence, hence being an interesting alternative for further analysis in depth.
