The Study on the Development of Diagnosis Algorithm of Taeeumin Symptomology (태음인(太陰人) 병증(病證) 진단 알고리즘 개발 연구)

  • Objectives : This study is aimed to develop the algorithm to diagnose Taeeumin's symptomology, by the method of literature research on Sasang Constitutional Medicine. Methods : Applying the sequential differentiations of Taeeumin's symptomology, or exterior-interior disease differentiation, favorable-unfavorable pattern differentiation, and mild-severe-dangerous-urgent pattern differentiation, "Donguisusebowon" and related literatures have been reviewed. Results and Conclusions : 1) 1st step: Taeeumin's exterior pattern and interior pattern are differentiated by the indexes of whole-body cold or heat pattern, sweating, and facial complexion. 2) 2nd step: The favorable pattern of the Taeeumin's exterior disease can be detected by indexes of the existence of fever, generalized pain while the unfavorable one by indexes of the abnormal condition of digestion and feces, and fearful throbbing. The favorable pattern of the Taeeumin's interior disease can be diagnosed based on indexes of eye pain, dry nose, dry throat, and heat symptoms that occur in various parts of the body, while the unfavorable one by indexes of thirsty, urination, feces and specific symptoms which can be induced by dryness. And in the both unfavorable patterns the dark complexion on the faces is revealed. 3) 3rd step: The mild-severe patterns of the favorably exterior disease are differentiated in terms of the condition of fever, while the mild-severe patterns of the favorably interior disease are in differentiated based on whether abnormal symptoms are revealed in the gastrointestinal tract. Both of the unfavorably dangerous-urgent patterns in exterior and interior diseases are differentiated by the symptoms such as tinnitus, dim vision, weakness of legs and back pain, and lack of strength in legs and thighs.

  • 1. Objective This study is aimed to present the effective classification of Soeumin symptomatology and the standardized signs for classification which can be applied for KCD, ICD and the insurance codification system. 2. Methods 1) Differentiate Soeumin symptomatology based on exterior-interior patterns, favorable-unfavorable patterns, and mild-severe-dangerous-urgent patterns. 2) Investigate the standard signs and symptoms to claasify Soeumin symptomatology based on exterior-interior patterns, favorable-unfavorable patterns, and mild-severe-dangerous-urgent patterns. 3. Results and Conclusions 1) The diagnosis criteria for Soeumin exterior-interior disease is based upon signs & symptoms of cold/heat, condition of stool, state of digestive system(such as digestion and appetite)among others. 2) The diagnosis criteria for Soeumin favorable-unfavorable disease is generally based upon whether the vital force of the spleen is damaged or not. More specifically, for the exterior disease, whether or not sweating is present. For the interior disease, whether or not dry mouth, body ache(a main symptom of the exterior state), and anxiousness are present. 3) For the Soeumin Wool-gwang disease, the diagnosis criteria of mild-severe disease is whether or not chills is present and the degree of body fever. 4) For Soeumin Mang-yang disease, the diagnosis criteria of dangerous-urgent disease is whether or not chills is, the degree of sweating and urine condition. 5) For the Soeumin Greater-Yin disease, Abdominal-pain bowel irritability pattern and Epigastric discomfort pattern are early state signs, Jaundice pattern is mild-state sign, edema & Greater-Yang disease Yin-toxin pattern are terminal state signs. 6) For the Soeumin interior disease, Abdominal-pain bowel irritability pattern and Epigastric discomfort pattern are of the dangerous state pattern, Jang-gual and Exuberant-Yin-repelling-Yang pattern are of the urgent state patterns.

  • The external treatment of internal diseases was first recorded in Hwangjenaegyeong(黃帝內經), and two hundred and fifty odd cases were found in the Bonchogangmok(本草綱目) and Oechisusebang(外治壽世方). Bonchogangmok(本草綱目) is a technical book devoted to explaining materia media as well as the external treatment of internal diseases. Oechisusebang(外治壽世方) is also a technical book, which make 36 references to the disease patterns of internal medicine. The medicinals employed as external treatment of internal diseases are grouped into 300 classes. These medicinals do not set limits to trees and plants but also uses birds and beasts, reptiles, grains, feces and urine, soil, stone etc., applied to various disease patterns. This study researched the medical texts in order to treat the disease patterns of internal medicine.

Comparative Study on Frequent Disease Patterns and Prescriptions by Three Societies of Korean Medicine for Gastrointestinal Disease (소화기 질환을 대상으로 한 3개 한의학파의 다빈도 병증 및 처방 비교 연구)

  • Objectives The aim of this study is to analyze the therapeutic direction and characteristics of gastrointestinal diseases by three societies of Korean medicine by comparing the frequent disease patterns and prescriptions of them. Methods The experts of the three societies of Sasang constitutional medicine, Hyungsang medicine and Sanghan-GeumGwe medicine discussed and confirmed a list of clinical data to make use for diagnosis. Then, clinical data on 200 patients suffering from gastrointestinal symptoms were collected. The each experts diagnosed appropriate disease patterns and decided prescriptions according to the method of their affiliated society. Results and Conclusions The three different societies' experts showed the similarity in the sense of categorizing disease patterns focusing on the specific physiology and pathology of the patients, while the each societies have different diagnosis systems. Treating chronic gastrointestinal patients, they treated the mental or psychological symptoms of the patients as well as their physical problems. Based on this study, it is necessary to develop a systematic and precise research model to objectively and statistically analyze the correlation of disease patterns and prescriptions by different societies.

  • The topic of this study is the translation of "Byun Jeung Rok Vol.VII(辨證錄七卷)". Byun Jeung Rok is a complete book of medicine which was written by Jin Sa tak(陣士鐸) in the early years of the Ching dynasty. Of this, the seventh volume is composed of 6 disease-patterns(病證) and 55 subtopics. Byun Jeung Rok(辨證錄) explains disease-patterns(病證) through the unique and outstanding perspective of the individual author. This topic was studied in hopes of helping later students by showing to the world, through its translation, the author's understanding of disease pattern. The organized characteristics of "Byun Jeung Rok(辨證錄) Vol.VII" is as follows. 1. Byun Jeung Rok based his theory on the traditional paradigm, Yin-Yang and Five phases. and criticized another doctor's misleading opinion by it. 2. Jin Sa tak thought it highly important to distinguish the internal injury and the external wound. and this view was influenced by Ming dynasty's doctors. 3. Jin Sa tak very often used providing nutrition to patients as a way of treatment. because he thought that a variety of diseases are caused by the weakness of organs.

Clinical Study of Menstruation Pattern according to Sasang Constitutional Diseases (사상체질 병증별 월경 양상의 임상 연구)

  • 1. Objective To investigate characteristics of menstrual patterns according to Sasang Constitutional disease, and to utilize for diagnosis and treatment of Sasang Constitutional disease. 2. Method The recruited subjects were 430 women who convinced their Sasang Constitution by taking herbal medicine and improving of chief complaint. The items of data contain age at menarche, age at menopause, menstrual cycle, menstrual duration, cyclic regularity of menstruation, severity of menstrual pain, the worst day of menstrual pain, type of menstrual pain, amount of menstruation. 3. Results 1) Severity of menstrual pain, the worst day of menstrual pain, menstrual pain accompanied with nausea were significant among Sasang Constitutional diseases. 2) In Bae-chu-pyo-byeong disease (背椎表病證), Wi-wan-han disease (胃院寒證), So-eum disease (少陰證) and Tae-eum disease (太陰證), menstrual pain was severer than other diseases. 3) In Hae-yeok disease and So-yang-sang-pung disease (少陽傷風證), the worst day of menstrual pain was a day before menstruation. In Hyung-gyeok-yeol disease (胸膈熱證), Tae-eum disease (太陰證), So-eum disease (少陰證), the frequency of a day after menstruation in the worst day of menstrual pain was higher than other diseases. In Wi-wan-han disease (胃院寒證), Mang-yang disease (亡陽證), the frequency of two day after menstruation in the worst day of menstrual pain was higher than other diseases. 4) In So-yang-sang-pung disease (少陽傷風證), Mang-eum disease (亡陰證), Eum-heo-o-yeol diasese (陰虛午熱證), Ul-gwang disease (鬱狂證), Tae-eum disease (太陰證), So-eum disease (少陰證), the frequency of menstrual pain accompanied with nausea were higher than other diseases. 4. Conclusion Menstrual patterns, especially menstrual pain is different among Sasang Constitutional diseases. Assessing menstrual pain can be used for diagnosing Sasang Constitutional diseases, deciding treatment course, determinating prognosis.

A study on the relationship between the symptom of Shanghanlun(傷寒論) and the Nutritive-Gi(營氣) (『상한론(傷寒論)』 병증(病症)과 영기(營氣)의 관계에 대한 연구(硏究))

  • Objectives : The practices of Wei-qi and Nutritive-qi are generally divided into external Mai and internal Mai. However, they are closely interrelated and practiced together. While taking these aspects into consideration, this paper attempts to make interpretations in relation with Nutritive-qi the disease pathogens that appear in Shanghanlun's disease symptoms. Methods : Using the practice and function of Nutritive-qi described in Huangdineijing, the paper shall make interpretations for the patterns of Mawhangtang, patterns of Gaejitang, and the pathologies of pain, oedema, and nosebleed as described in Shanghanlun. Results & Conclusions : The pain from the patterns of Gaejitang differ from that of the patterns of Mawhangtang. First, the pain from the the patterns of Gaejitang cannot be the main symptom. Even if there is a symptom of pain, it's severity is not serious. Second, the pain from the patterns of Gaejiang takes the form of stiffness, and not general bodily pain. The reason for this stiffness is because of the emptiness of Wei-qi that leads to the congestion of Nutritive-qi which in turn causes the lack of qi and blood flow in muscula area such as abdomins. The symptom of oedema where one's body becomes swollen comes from a number of pathogens. First, the flow of meridian becomes hindered due to external dampness, a character which tends to be adhesive when added with humidty, and this results in the blockage of water qi which then causes the coagulation of nutritive blood. Second, when toxic heat is repressed and blocked within the lesser-yang channel, lesser-yang meridian stops working, which causes nutritive blood to clog at the front and back of ears since lesser-yang channel flows through that portion of body. Third, although oedema is not specifically mentioned in the sentences, but there exists the patterns of Daechungyongtang where water lumps are formed due to the accumulation and blockage of watery dampness. The patterns of Daechungyongtang is cuased when meridian is hindered from externally discharging body fluid due to a problem with meridian that blocks the fumigated internal heat which turns into bodily fluid from being discharged externally.

An Analysis on Use Patterns of Oriental Medicine of Pond Wetland Plants for the Ecological Experience in Rural Tourism Village (농촌관광마을 생태체험을 위한 연못형습지 식물의 한방 이용형태 분석)

  • These days, the demand of farm tours is on the rise in Korea. However, most of them are related to farming experience. To overcome the situation, ecological experience is introduced. In this sense, it is necessary to obtain the contents and educational materials of ponds, the space of farming and ecological experience. Therefore, this study analyzed the use patterns of oriental medicine of pond wetland plants. Oriental medicine is used for old and traditional knowledge, and is still applied to various fields, such as medical science, pharmacy, and science. The analyzed result of this study will be helpful for the educational materials of ecological experience. This study surveyed plants of 40 pond wetlands in rural areas, and looked into the use patterns of traditional medicine by using the contents of Korea Traditional Knowledge Portal being operated by Korean Intellectual Property Office. According to the vegetation survey of the 40 pond wetlands, there were a total of 457 taxa in 108 families, 309 genera, 392 species, 59 varieties, 5 forma, and 1 subspecies. Among them, 314 taxa in 53 families, 136 genera, 265 species, 45 varieties, 3 forma, and 1 subspecies had the information on the use patterns of traditional medicine. It means that 68.8% of the plants around pond wetlands contain traditional medicine. Given other application patterns, such as food, handcraft and traditional farming, more than 70~80% are expected to include traditional knowledge. 314 taxa used for traditional medicine had 570 kinds of efficacy and 325 kinds of disease treatment. It means that one taxon has 4.0 (0~20) kinds of efficacy on average and 6.6(0~20) kinds of disease treatment. It is expected that this study result will be applied to various fields, including experience, education, and medical science. And it is considered that valuable pond wetlands are worthwhile to be conserved constantly.

Resveratrol Upregulates p21, Cell Cycle Regulator, in Gaucher Disease Cells (Gaucher병에서 resveratrol의 세포주기 조절자 p21을 통한 세포보호 효과 연구)

  • Gaucher disease (GD) is caused by glucocerebrosidase functional deficiency and the most prevalent lysosomal storage disorder (LSD), with an incidence of about 1 in 20,000 new births. Resveratrol, one kind of phytoalexin, is a produced naturally by several plants and has anti-tumor, anti-aging, anti-inflammatory and neuro-protective effects. In this paper we provide the cellular protective effect of resveratrol in both type I and type II Gaucher disease cells. Resveratrol treatment did not show any significant change in the p21 and p53 mRNA expression level, however expression level of the p21 protein, a cell cycle arrest factor, shows significant increment in both types of Gaucher disease cells. These cell cycle arrest patterns were confirmed by both MTT assay measurement and microscopy detection. In comparison, expression level of poly ADP ribose polymerase (PARP), an apoptosis indicator protein, was significantly decreased in both type I and II Gaucher disease cells after treatment with resveratrol. This result indicates that resveratrol relievescellular apoptotic stress fromtype I and II Gaucher disease cells. Therefore, we demonstrate that resveratrol inhibits cell proliferation via p21 activity and activates cellular repair systems for Gaucher disease cells. Our results provide at least one of the molecular mechanisms of Gaucher disease and may allow the verification of potential drug targets for therapeutic trials.

Of this, the eleventh volume is composed of 6 disease-patterns(病證) and 41 subtopics. The organized characteristics of "Byun Jeung Rok(辨證錄) Vol.XI" is as follows. Chapter 1 refers to the symtom, agent and prescription of Hysterorrhea(帶下病). It explains five color Hysterorrhea; white, red, black, yellow and blue. Chapter 2 refers to the symtom, agent and prescription of Oligemic Menoschesis(血枯). It explains two cases of Menoschesis. Chapter 3 refers to the symtom, agent and prescription of Metrorrhagia(血崩). It explains eight cases of Metrorrhagia. Chapter 4 refers to the symtom, agent and prescription of Dysmenorrhea(月經不調). It explains fourteen cases of Dysmenorrhea. Chapter 5 refers to the symtom, agent and prescription of Infertilitas(不姙). It explains ten cases of Infertilitas. Chapter 6 refers to the symtom, agent and prescription of Hyperemesis(惡阻). It explains two cases of Hyperemesis. Obviously the Byun Jeung Rok(辨證錄) is one of the very important texts in the oriental medicine. But there is no exact Korean translation, so I study this six chapters and translate them into Korean. Finally, I hope this thesis can be an aid of development in the knowledge informatization of Korean medicine.
