Diet and Metabolic Disease Risk by Perceived Stress Level in Korean Adult Women (성인 여성에서 스트레스 인지 정도에 따른 식생활과 대사성 질환 위험)

  • Dietary components can modulate stress, inflammatory indicators, and health risk. This study examined the relationship among diet, metabolic disease risk, and perceived stress in Korean adult females using the 2017-2018 Korea National Health and Nutrition Examination Survey. A total of 4,353 adult women aged 19-64 years were classified into four groups according to perceived stress level: very high stress group (VHSG, n=225), high stress group (HSG, n=1,079), moderate stress group (MSG, n=2,532), and low stress group (LSG, n=517). Data collection included the sociodemographics, anthropometrics, blood profile, and dietary survey. After adjusting for covariates, those in the VHSG had a higher body mass index (p=0.013) and obesity rate (p=0.053) with a shorter sleep time than the LSG group. The VHSG also tended to have a higher plasma LDL-cholesterol, hsC-reactive protein and lower levels of HDL-cholesterol, vitamin A, and vitamin E than the low stress group. High stress subjects demonstrated increased breakfast skipping frequency (p<0.0001), decreased fiber intake (p=0.001), potassium (p=0.041), and vitamin A (p=0.011) than the low stress ones. Therefore the perceived stress level was associated with the inflammatory indicators, obesity, and lack of anti-inflammatory or antioxidant nutrients. The dietary components may be an important mediator of stress and metabolic disease.

Effects of Injury and/or Injured Areas on Depression in Korean Patients with Industrial Injuries (한국 산재 환자의 상병 및 상병 부위가 우울에 미치는 영향)

  • Purpose: This study aimed to determine the influence of injury and/or injured area classification on depression in patients with industrial injuries. Methods: The participants comprised438 patients who consented to participate and completed self-reported questionnaires. Data were analyzed using SPSS/WIN version 22.0 for descriptive statistics, $x^2$ test, fisher's exact test, ANOVA, and post-hoc $Scheff{\acute{e}}$ test. A stepwise multiple regression analysis was used to identify factors influencing depression. Results: The results indicated that the effect of disease classification and injured areas on depression were significantly different in patients with industrial injuries. The results further showed that severe depression was significantly higher in cardiovascular patients and patients with an injured area of the head and waist. The most powerful predictor was age (50~59 years), return to work (reemployment), disease classification (cardiovascular), and injured area (head, including vascular disease). Conclusion: This study showed that the most influential variable of depression in patients with industrial injuries were cardiovascular issues, injury areas of the head and waist, being aged 50~59 years, and reemployment. To reduce depression in these patients, it is important to develop and implement a psychiatric rehabilitation program that helps patients to formulate a concrete plan and goal for recovery, enabling patients to actively engage in their rehabilitation.

Open Lung Biopsy Procedure for Diffuse Infiltrative Lung Disease -Collective Review of 50 Cases- (미만성 폐침윤 질환에서 개흉폐생검)

  • Open lung biopsy still has important roles for the marking of diagnosis of diffuse infiltrative lung disease even though transbronchial bronchoscopic lung biopsy and percutaneous needle aspiration biopsy gain popularity nowadays. This is clinical retrospective review of the 56 patients with diffuse infiltrative lung disease undergoing open lung biopsy by minithoracotomy from 1984 to Dec. 1992 in the Department of Thoracic & Cardiovascular Surgery of Catholic University Medical College. 27 men and 29 women, aged 17 to 73 year [mean 49 year , were enrolled & divided into 2 groups;Group A consisted of patients with immunocompromised state [n=19 , Group B patients with non-immunocompromised state[n=38 . Pathologic diagnosis was made in 54 cases[96.4% of these two groups and as follows: infectious; 12 patients[21.4% , Neoplastic; 10 patients[17.9% , granulomatous; 4 patients[7.1% , interstitial pneumonia; 12 patients[21.4% , Pulmonary fibrosis; 8 patients[14.3% , others; 3 patients[5.4% , nonspecific; 5 patients[8.9% , and undetermined; 2 patients[3.6% . Therapeutic plans were changed in 39 patients[69.6% after taking of tissue diagnosis by open lung biopsy. Group B has higher incidence of infectious diseases and change of therapeutic plan than the Group A. The postoperative complications developed in 8 cases[14.3% ,and there is no difference of incidence between the 2 groups. 4 patients belongs to group A, died of respiratory distress syndrome [2 and sepsis [2 which were not related with open lung biopsy procedure. In conclusion, open lung biopsy is a reliable method to obtain a diagnosis in diffuse pulmonary infiltrates and can be performed safely, even in acutely ill, immunosuppressed patients.

Mortality Attributable to Second Hand Smoking in Morocco: 2012 Results of a National Prevalence Based Study

  • Purpose: To estimate the number of deaths attributable to second hand smoking (SHS) in Morocco in 2012. Materials and Methods: prevalence based study focusing on mortality from ischaemic heart disease (IHD) and lung cancer among non-smokers aged 35 and over. Prevalence of SHS among never smokers was gathered from a national cross sectional survey on tobacco and population attributable risk (PAR) was calculated by applying PARs to mortality. The analyses were stratified by sex, age and area of exposure. Results: Rates for exposure to SHS among men aged 35-64 years ranged from 20.0% at home to 57.4% at work. Among non-smoking Moroccans aged 35 and over, 233 (IC: 147 - 246) deaths were attributable to exposure to SHS; 156 (IC: 100 - 221) in women and 77 (IC: 44 -125) in men. A total of 173 (122 - 222) deaths were estimated to have been caused by exposure only at home, 34 (9 - 76) by exposure only at the work place and 26 (15 - 58) by exposure both at home and work places. Exposure to SHS could be responsible for 182 (128 - 237) deaths from IHD and 51 (19 - 109) from lung cancer. Conclusions: These data confirm that SHS needs urgent attention in Morocco.

Factors Affecting Regular Medical Services Utilization of Chronic Disease Patients - Focusing on the Hypertension, Diabetes Mellitus, Hyperlipidemia - (만성질환자의 정기적 의료이용에 영향을 미치는 요인 - 고혈압, 당뇨병, 고지혈증을 중심으로 -)

  • Objectives: This study aims to identify the factors associated with regular medical services utilization of chronic disease patients. Methods: The research selected 4,489 adults aged over 30, diagnosed with hypertension, diabetes, hyperlipidemia, hypercholesterolemia, from the Korea health panel. We analyzed states of regular medical service utilization using descriptive statistics. Multiple regression analysis was used to examine the main factors associated with regular medical services utilization in chronic disease patients. Results: In terms of socio-demographic factors, gender, age, marital status, education level, employment, household income and disability were significantly different between hypertension, diabetes, hyperlipidemia and hypercholesterolemia. Among health status and behavioral factors, number of chronic diseases, subjective health status, smoking, high risk drinking, regular meals, physical activity, obesity were significantly different. From the multiple logistic regression analysis, age, number of chronic diseases, obesity, type of chronic diseases were associated with regular medical services utilization. Conclusions: It is necessary to develop effective health education programs and individualized approach to improve continuous management in chronic diseases patients.

Construction of Explanatory Model for Medication Adherence in Older People with Chronic disease (만성질환을 가진 노인의 약물복용이행 설명모형 구축)

  • Purpose: The main purpose of this study was to identify factors affecting medication adherence and to develop an explanatory model for medication adherence in elders with chronic disease. Method: Empirical data were collected from 312 older adults with chronic disease and the data collect period was from August 8 to 31, 2011, and were analyzed using SPSS for Windows 19.0 program and confirmatory factor analysis with the structural equation model (SEM) procedure performed with AMOS 19.0 program. Results: Results of this study showed that perceived self-efficacy was the strongest factor influencing medication adherence, and it affected also outcome expectations positively but impediments were negatively influenced by self-efficacy. Outcome expectations and impediments subsequently acted on medication adherence with the same relationship as self-efficacy. In additional results, self-efficacy and medication adherence were further significantly affected by the factors; social support, medication knowledge, and depression. Conclusion: These results show that nursing interventions to promote medication adherence in this population should focus on self-efficacy promotion including social support, education for delivery of medication knowledge, and reduction in depression.

Comparison of Health Behaviors, Disease Prevalence between One-person women and Multiple households women in Korea (우리나라 여성 1인가구와 다인가구 여성의 건강행태 및 질병이환율 비교 : 2013년 지역사회 건강조사를 중심으로)

  • Purpose: This study compared health behaviors and disease prevalence between one-person women and multiple-household women in Korea. Methods: This study used the 2013 data from the Korean Community Health Survey (KCHS). A total of 89,807 women, aged 19-64 years were included in this study. The data was analyzed using descriptive statistics and Chi-square test. Moreover, statistical processing was performed using the SPSS 21.0. Results: Among adult women, one-person households women was 6.2%. One-person household women were older than multiple-households women, and multiple-households women were less likely to exercise regularly. And disease prevalence was higher in one-person household women than in multiple-household women. Conclusion: Residence characteristics should be considered in developing a public health program to reduce or increase modifiable health behaviors and disease prevalence. The findings from this study suggest that policies to improve the support for healthcare of one-person household women is necessary.

Ginsenoside Rgi is an Anti-apoptotic Agent

  • Primary neuronal culture was studied for observing effect of ginsenoside Rgl (Rgl) on serum-free medium induced apoptosis. Results showed that Rgl at concentration of 1 umol$.$ L-1 and 10 umol$.$L-1 could inhibit apoptosis, decrease intracellular calcium concentration in cultured cortical neurons, enhance SOD activity in both aged rat cortex and cultured cortical neurons, scavenge cytotoxic oxygen free radicals, decrease NO content and NOS activity in aged rat cortex and cultured cortical neurons, increase bel-2 gene expression in rat brain. These results provided new data for elucidating the anti-aging effect of Rgi. Rgl is considered to be a useful drug for treatment of Alzheimer's disease and brain aging.

Effect of Phosphate Binders on Vascular Calcification and Mortality in Korean Patients with Chronic Kidney Disease (만성신부전 환자의 혈관 석회화와 사망률에 미치는 인결합제의 영향)

  • Objectives: Current studies are debating on the association of vascular calcification and the benefit of treatment to lower serum phosphorus level in patients with chronic kidney disease. The aim of this study was to evaluate the association of mortality and risk of vascular calcification in patients with CKD who were taking phosphate binders. Methods: This study was conducted through retrospective medical chart review for 420 patients aged 18 years and older who were admitted for chronic kidney disease. Results: Vascular calcification was not statistically significantly associated with increased mortality in patients with CKD [16.7% vs. 19.2%; 95% CI; 0.388 to 1.818 (p=0.656)]. Intervention of calcium-based phosphate binders was not significantly associated with vascular calcification in patients with CKD [9.1% vs. 12.5%; 95% CI; 0.364 to 1.358 (p=0.292)]. Ca x P product ${\geq}55mg^2/dL^2$ was not significantly associated with increased 1 year mortality in patients with CKD [25.4% vs. 17.5%; 95% CI; 0.851 to 3.013 (p=0.142)]. Intervention of sevelamer was significantly associated with reduced 1 year mortality in patients with CKD than that of patients who did not take sevelamer [6.3% vs. 25.3%; 95% CI; 0.044 to 0.880 (p=0.020)]. Conclusion: There was not a statistically significant association between vascular calcification and phosphate binder's use. But phosphate binder use was significantly associated with decreased mortality in patients with CKD.

Pulp and periapical disease as a risk factor for osteonecrosis of the jaw: a national cohort-based study in Korea

  • Purpose: This longitudinal cohort study aimed to evaluate the relationship between osteonecrosis of the jaw and pulp and periapical disease in patients who were administered bisphosphonates. Methods: Using data from a nationwide cohort, we examined the association among dental caries, pulp and periapical disease, and osteonecrosis of the jaw in women aged >50 years who received bisphosphonates for more than 1 year between 2002 and 2015. Because of ambiguities in the diagnosis of osteonecrosis of the jaw in population-based data, we operationally defined and categorized the condition into established and potential osteonecrosis of the jaw. Results: Pulp and periapical disease significantly increased the development of both established and potential osteonecrosis of the jaw (hazard ratio, 2.21; 95% confidence interval, 1.40-3.48; and hazard ratio, 2.22; 95% confidence interval, 1.65-2.98, respectively). Root canal treatment did not have any influence on the development of osteonecrosis of the jaw. Conclusions: Pulp and periapical disease may be a major risk factor for osteonecrosis of the jaw. The study findings suggest that patients should undergo regular dental examinations to detect pulp and periapical disease before or during the administration of bisphosphonates and that root canal treatment should be considered to decrease the risk of osteonecrosis of the jaw.
