Purpose: This study was performed to identify the variations of nursing care cost depending on nursing care requirement and calculate nursing care cost per one day and one care requirement point. Methods: Nursing care requirement was measured by classifying 3,855 patients according to KPCS-1(Korean Patient Classification System for nurses-1). Nursing care cost was calculated from personnel expenses and nursing care requirement. Nursing cost factors were identified by multiple regression analysis. Results: Average nursing cost per patient per day was 33,588 won, Average nursing care cost per 1 patient classification score was 3,558 won. The nursing cost per 1 patient classification score was different depending on the types and levels of the hospitals. The 4th patient classification group revealed the highest nursing care cost. Nursing cost factors included the number of beds in the hospitals, seniority, number of nurses and first grade in nurse personnel accreditation ($adj-R^2$ 74.0%. p<.05). Conclusion: Nursing care requirements expressed by patient classification scores don't directly correlate with nursing care cost. Further research is needed to evaluate validity and reliability for refining KPCS-1 and to apply variable criteria to nurse personnel accreditation.

This study was conducted with the purpose of suggesting alternatives for the efficient operation of senior day care facilities in 22 cities and counties in the Jeonnam region by analyzing the efficiency of senior day care facilities. The analysis data for the study used the National Health Insurance Corporation's long-term care insurance for the elderly (2022), the Input variables were the number of facilities, the number of workers, the number of affordable, and the number of senior long-term care insurance recipients and the Output variables were the the number of users. As a result of the analysis, CCR was most efficient in Goheung-gun, Gokseong-gun, Gwangyang-si, Boseong-gun, Yeongam-gun, and Jindo-gun, BCC was most efficient in Goheung-gun, Gokseong-gun, Gwangyang-si, Gurye-gun, Damyang-gun, Boseong-gun, and Jindo-gun, and SE was most efficient in that order: Jindo-gun, Gokseong-gun, and Gwangyang-si. It turned out to be a super efficient area. In the contribution analysis, the number of affordable and workers variables were found to be variables that had a large impact on efficiency contribution. In the improvement potential analysis, the number of facilities variable was found to be a variable that had a significant impact on efficiency. Therefore, for the efficient operation of senior day care facilities, we suggest adjusting supply and demand, such as the number of facilities and affordable, and suggest that training programs to strengthen the expertise of workers who contribute greatly are required.

Purpose : This study was conducted to understand the health status and health behaviors of preschoolers to provide baseline data for developing health promotion programs. Method 'Parents of children attending day-care centers and guardians of preschoolers were recruited for the study. Data were collected from June to August2002 using a structured questionnaire. Result . Among 754 preschoolers, 17,3% were overweight, while 18.2% were underweight, while 15.9% and 23.2% were overweight and underweight respectively, among 85 children living with guardians. The most frequent infectious diseases that children have had previously were hand-foot-mouth disease, chicken pox, and measles. Current health conditions that children have frequently are respiratory disease and atopic dermatitis for bother groups. Only 54%-61% brush their teeth everyday at bedtime, 49%-54% wash their hands every time after returning home, 4.5%-8.8%wear bicycle helmets, 9,3%-11.1% use a child car seat, and 3.5%-8.1% eat fruits and vegetables five times a day, Children living with guardians were less likely toper form positive health behaviors compared with children in day-care centers. Conclusion 'Based on the study results, health professionals could plan and develop health promotion programs to change unhealthy behaviors of preschoolers targeting high-risk groups.
Objective: The study wanted to find out what changes the mother, who is the main parent of the severely disabled children, experienced in their lives through the use of day hospitals and child care services, and what difficulties they had, and discussed policy practical improvements that could satisfy both the medical and developmental needs of the severely disabled. Methods: The participants of the study were 9 mothers of a child with severe disabilities who had experience using hospitals and kindergarten. They were recruited by snowball sampling method. Focus group interview and individual interview were conducted. Results: As a result of the analysis, the mother of severely handicapped child gradually began to seek the balance of life as her child grew and became aware of the institution other than the day hospital. - (e.g) Although I was distracted and worried for the first time, I was gradually adjusting to the time away from my child thinking positively. Children with severe disabilities were also positively influenced by various experiences. They become to show som positive behavior or feeling such as brightening facial expressions or smile as they were using daycare centers or kindergartens in addition to daytime hospitals. Conclusion/Implications: Through the results of this study, comprehensive policy proposals on child care, welfare, and medical care were discussed.
Financial barrier is well known as a determinant of the perceived need for and utilization of medical care in urban areas. This study aims to; a) estimate the levels of the perceived need for and utilization of medical care, b) analyze the reasons for not receiving medical care, and c) determine factors affecting the levels of need and utilization, and major reasons for not receiving medical care. The data of household interview survey in urban areas collected for a National Survey on Reorganization of Nationwide Health Care Delivery Network is used. An interview was conducted with a total of 2, 538 households in urban areas during a 28 day period of October through November, 1981, giving an overall response rate of 99 percent. The results show a factor of third-party payer is most influential to utilization (physician visits by patient), and living standard is a determinant to explain the variance of major reasons for not receiving medical care. Therefore, financial burden is still a major determent in utilization of medical care in urban areas. Improved level of living standard and expanded coverage of the third-party payer such as health insurance and public medical assistance will increase both an overall levels of utilization of and the need for medical care in urban areas in the future. The major statistics are as follows; a) rate of the need for medical care during a 15 day period per 100 persons was 33 percent: the differential rates appeared in sex, age, living standard, and third- party payer variables, b) percent of treatment employed per 100 persons who are wanting medical care was 82: the percents were high in young ages, high levels of living standard and education, and persons covered by third-party payer, and c) economic reasons for not receiving medical care per 100 persons who are wanting medical care were occupied with 60 percent: the rates are high in old ages, low levels of living standard and persons not covered by third-party payer.
In order to provide information for the establishment and maintenance of a rehabilitation day care center for stroke patients. this study is to assess needs for the rehabilitation day care center of the stroke patients and to identify the factors influencing the needs for the center. The data were collected face-to-face interview with 223 stroke patients. using a structured questionnaire. from September 24. 2001 to November 20. 2001. Major findings are as follows. 1. Most of the participants($94.6\%$) needed rehabilitation day care center for stroke patients. $95.5\%$ of participants were willing to use the rehabilitation day care center. 2. Also the score of the needs for the center's health services was $2.84\pm60$ out of 4.00. In regards to the sub-contents. while the physical exercise therapy showed the highest mark($3.54\pm71$) in the needs. the following marks showed physical therapy($3.48\pm79$), training for the memory. thinking and judgment($3.30\pm93$). training for ADL($3.09\pm99$). health education program($3.04\pm93$). In the meantime. the expected effects from the use of the center are $2.89\pm61$ out of 4 and its sub-contents showed that the center would promote their physical and mental well-being($3.30\pm74$) and the center would be more effective than in home care($3.12\pm70$). 3. Meanwhile. the desired frequency of use in the future and distance had significant interrelation with their families living together(p<.05). In addition those who paid to use it differentiated significantly according to their ages and the types of insurance they had(p<.05). 4. The needs in degrees of speech disorder therapy and hobbies & amusements. the patients with other disease had significantly higher degrees than those patients without it (p<.05). Also in regard to the need degrees for physical therapy. healthy education programs and individual counseling including their families. the degrees of the patients with speech disorders were significantly lower than those of the patients without the disorder (p<.05). On the other hand. the patients with speech disorders were significantly higher than those patients without it in the need degree of the speech disorder therapy (p=.000). And the needs in degree concerning about speech disorder therapy. physical exercise therapy. training for ADL. medicinal substances therapy and family education were negatively correlated with the ADL (r=-.236$\sim$.305, (p<.005). 5. Finally. the expected effect of using the rehabilitation day care center showed significant differences statistically according to whether or not they had other disease (p<.05). In conclusion. the study showed the stroke patients were willing to use the center and had a high requirements for it and they especially had relatively high need degrees for the physical exercise therapy. physical therapy. training for memory. thinking and judgment. and healthy education program. And significant factors for the use of the center were their ages. types of insurance. family cohabitation. complications and speech disorders. ADL and so forth. Accordingly. the rehabilitation day care center needs to be established for the stroke patients and the center should develop rehabilitation care programs. which are individual and special programs customized for each patient's characteristics and health conditions.

The shortage of nursing personnel was become one of the most serious problems in operating pediatric oncology nursing unit which was the first pediatric oncology nursing unit in Korea. The purpose of this study was to estimate the optimal number of nursing personnel by calculating nursing care hours. The subjects were 13 staff nurses and inpatients of pediatric oncology nursing unit at Seoul National University Hospital during the period of May 20, 1996, to June 2, 1996. The number of nurses' duty was 132, the number of patients treated was 1288 for these 2 weeks. The tools used for this study were pediatric patient classification indexes and direct & indirect care indexes. Each nurse measured the time that they spent for their activities by self record under the supervision of their nurse manager. The method used to calculate the number of nursing personnel was multiplication of the average number of nursing care hours per patient per day with the number of patients. Percentage, average, t-test, F-test were used for data analysis. The results of this study were as follows : 1) The distribution of patient class : Class I & II none, Class III 86.8%. Class IV 12.9% 2) Direct nursing care hours for a patient per shift according to patient classification: Class III : 27.64 minutes, Class IV : 54.64 minutes The average direct nursing service hours for a patient per shift(3 shift) was 31.54 minutes(94.62 m/day). The average indirect nursing service hours for each patient per duty(3 shift) is 21.3 minutes (63. 91 m/day). 3) The average nursing hours for a patient per duty was 52.80 minutes(2.64h/day). 4) The group of administering medications in direct care activities showed the highest percentage (38.9%). Checking vital signs among observation took the most time am.ong each direct care activity (6.88 minutes for a patient per duty). 5) Charting took the most time of each indirect care activity(52.53 minutes/ duty/nurse). 6) The average personal time per duty is 29.40 minutes, which 'was below 30 minutes of this hospital regulations. 7) The average nursing hours that a nurse provided for a duty was 8.60 hours, which meant that a nurse worked 1.10 hours overtime. 8) Standardizing to a 33 bed to a unit, 17 nurses were needed at the present nursing level.

Purpose: The purpose of this study was to compare the effects of meatal care with 10% betadine or with normal saline on the incidence of urinary tract infection (UTI) for elderly patients with indwelling urinary catheter in the ICU. Method: A quasi experimental design with non-equivalent control group was used. The 37 patients who participated in this study were 65-year-old or older. Patients in the saline group (n=20) received meatal care with normal saline and those in the betadine group (n=17) received meatal care with 10% betadine once a day for 6 days. Urine cultures were done on the 7th day for both groups to detect UTIs. Results: No difference was observed in the incidence of urinary tract infection between the two groups, regardless of patients' gender, ability to communicate or history of operation. Conclusion: The results indicated that use of saline which is cheap and does not irritate the mucous membrane is effective in preventing UTI within the first 7 days, and can be used instead of betadine for meatal care for elderly patients with indwelling urinary catheter in the ICU.
This study purports to verify managerial effectiveness of the integrated delivery system(IDS) of Japanese health care institutions through comparing the managerial performance between hospital groups providing with both acute and nursing care and those with acute care only. Data on the managerial performance of 697 hospitals providing with nursing care together and 819 hospitals providing with acute care only were collected from Japanese Central Social Insurance Medical Councils 2001, 2003, 2005, and were analyzed using mean comparison test(t-test) between the two groups. The results revealed that there were significant differences between the two groups in such indicators as ratio of material cost, labor cost, depreciation rate, total margin, operating margin, average number of outpatient per day, average revenue of an inpatient per day, total amount of labor cost, gross revenue per employee, and labor productivity. However, we could not find out any consistent evidence which support the effect of integrated delivery system on the hospital managerial performance. Further discussion was made on the limitation of the study and future research agenda relevant to the topic.
Purpose: The purpose of this study was to determine the effect of an education program on child health and safety for teachers in child daycare centers. Method: Using a nonequivalent one group pretest-posttest design, 41 day care staff(N=41) from 12 day care centers in Seoul and D-gun in Chung-Nam were assigned to experimental group. A program that is appropriate for preventing injuries and protecting the health of young children at child daycare centers was developed. Health and safety education program was provided for the intervention group for 2 weeks two times. Results: After education session, the mean score of knowledge, self confidence, practice on safety and health in child care teachers were significantly increased. There were significant differences in the degree of knowledge according to education, and in the degree of practice according to age. Conclusions: The education program for child care teachers was effective in promoting knowledge, self confidence and practice on child health and safety. The results suggest that health and safety education program should be provided systematically and continuously for child care teachers.
