  • Background: Though a large proportion of cancer survivors are assumed to be commonly affected by sleep disturbance, few studies have focused on short sleep problems and its correlates among Korean cancer survivors. The purpose of this study was to evaluate the prevalence of short sleep in adult cancer survivors from a nationwide population-based sample and to identify risk factors for short sleep duration. Materials and Methods: Based on the fourth and fifth Korea National Health and Nutrition Examination Surveys (2007-2012), 1,045 cancer survivors and 33,929 non-cancer controls were analyzed. The prevalence of short sleep was compared between these two groups. Associations between short sleep and its correlates were evaluated using multiple logistic regression among cancer survivors: odds ratios (ORs) and 95% confidence intervals (95%CIs) were estimated after adjusting for sociodemographic factors, lifestyle factors, psychological conditions, and cancer-related factors. Results: About 8.1% of cancer survivors slept for less than 5 hours per day (6.2% men and 9.3% women), whereas this was the case for only 3.7% of non-cancer controls. Cancer survivors who had the lowest household income level showed a significantly higher likelihood for short sleep (adjusted OR 2.82, 95%CI 1.06-7.54). Self-reported poor health and depressive symptoms were found to be associated with significantly increased likelihood for short sleep in cancer survivors (adjusted OR 3.60, 95%CI 1.40-9.26 and adjusted OR 2.00, 95%CI 1.17-3.42). Gastric cancer survivors had a 3.97-fold increased risk for short sleep (95%CI 1.60-9.90). Conclusions: The prevalence of short sleep occurs at a high rate among the Korean cancer survivors, which may indicate a poorer quality of life and a higher risk of future complications in survivorship. Targeted interventions that can assist cancer survivors to cope with sleep disturbances as well as ensuring psychological stability are warranted to reduce the latent disease burden.

  • There are approximately 25,724 shelters to which people can be quickly evacuated for safety in case of emergency across the nation, and Seoul has about 3,870 shelters. Those nationwide shelters are located at a point within a five-minute radius for quick evacuation. Seoul's shelter capacity can hold 285% of its population. The problem is, however, that there is no knowing how many shelters are reachable within five minutes when considering walking speed according to individual differences in age, height, health state, and physical condition. In addition, available service areas become different according to the spatial allocation and distribution of shelters with possible vulnerable points. This study thus defined the pedestrian walking speed at 1m/s, 1.3m/s, and 2m/s by reviewing previous studies and conducted network analysis of the Location Allocation Model with the designated shelters and road networks in Seoul. The results identified the shelter service and vulnerable areas in each administrative district of Seoul according to walking speeds. It was analyzed that the vulnerable areas in which the elderly could not reach a shelter were more than twice as big as those of adult men and women with a fast walking speed.

  • Elevated serum concentration of inflammation markers is known as an independent risk factor of metabolic syndrome (MS) and dietary intake is an important factor to control MS. The purpose of this study was to investigated the hypothesis that inflammatory indices are associated with dietary intake and diet quality index-international (DQI-I) in subjects with MS. A cross-sectional study was conducted on 156 men and 73 postmenopausal women with MS, defined by three or more risk factors of the modified Adult Treatment Panel III criteria. Serum levels of high sensitive C-reactive protein (hs-CRP), adiponectin were examined and nutrients intake and DQI-I were assessed using a semiquantitative food frequency questionnaire. The total DQI-I score was significantly higher in female subjects ($65.87{\pm}9.86$) than in male subjects ($62.60{\pm}8.95$). There was a positive association between hs-CRP and polyunsaturated fatty acid intake (p < 0.05) and a negative association between adiponectin and lipid (p < 0.05), total sugar (p < 0.01), and total fatty acids (p < 0.05). When the subjects were divided into 5 groups by quintile according to serum adiponectin and hs-CRP level, there was no association between DQI-I score and hs-CRP levels. Moderation score of DQI-I was significantly higher in highest quintile group than the lower quintile groups. Therefore, our results provide some evidence that dietary intake and diet quality are associated with inflammation markers and dietary modification might be a predictor to decrease risk for metabolic syndrome complications. However further research is needed to develop the dietary quality index reflecting the inflammatory change by considering the dietary habit and pattern of Koreans.

  • The prevalence of liver and intestinal helminth infections, including Opisthorchis, Haplorchis, Phaneropsolus, hookworms, Enterobius, and Taenia, was surveyed in Khammouane province, Lao PDR. Fecal specimens were collected from 1,242 people (590 men and 652 women) in 3 Mekong riverside villages and were examined by the Kate-Katz thick smear technique. The overall helminth egg positive rate was 81.1%. The positive rate for small trematode eggs, including Opisthorchis viverrini, heterophyids, and lecithodendriids, was 81.1% and the positive rate for hookworms was 6.7%. To obtain adult worms, 35 people who were positive for small trematode eggs were treated with 20-30 mg/kg praziquantel and 10-15 mg/kg pyrantel pamoate, and then purged. Diarrheic stools were collected from 33 of these people and searched for helminth parasites using a stereomicroscope. Mixed infections with various helminths (Haplorchis taichui, Haplorchis yokogawai, Prosthodendrium molenkampi, Phaneropsolus bonnei, echinostomes, hookworms, Trichostrongylus spp., Trichuris trichiura, Enterobius vermicularis, and/or Taenia saginata) were found. The total number of helminth specimens collected was 20,907 (approximately 634 per person). The most common species was H. taichui, followed by P. molenkampi, 0. viverrini, P. bonnei, E. vermicularis, hookworms, and Trichostrongylus spp. These results show that diverse species of intestinal nematodes, trematodes, and cestodes are infecting humans in Khammouane province, Lao PDR.

  • Craniofacial growth pattern is an important diagnostic data in the course of orthodontic diagnosis and treatment planning ; it also has great influence in the establishment of occlusion as well as shaping and development of face. There have been many studies to classify different craniofacial growth patterns and attempts to predict growth patterns. This study aimed to correlate craniofacial growth pattern and symphysis morphology. 120 adult patients with age from 19 to 39 (mean age : 23.1) were chosen as subjects , using lateral cephalometric films. their anterior to posterior facial height ratios were calculated. They were divided into 3 groups - clockwise growth pattern with $56\%-62\%$(36subjects), counter-clockwise growth pattern group with $65\%$-80\%$(43subjects) and normal growth pattern group with $62\%-65\%$(41subjects). Symphysis morphology and Prominence evaluation in each subject were studied and the following conclusions were drawn : 1. In comparison of symphysis morphology between the sex groups, men showed large symphysis height and prominence. 2. Concerning the symphysis morphology, the clockwise growth pattern group showed larger height, H/D ratio and actual length but smaller depth, angle, effective length and E/A ratio compared to the counter -clockwise growth pattern group. 3. Those with smaller prominance of symphysis showed clockwise growth tendency and those with larger prominance showed counter-clockwise growth tendency.

  • Objectives : The object of this study was to evaluate repeatability of portable spirometry and to determine an appropriate equation for predicted value. Materials and Methods : The subjects were 76 men and 45 women recruited from among Kyunghee University oriental medical students. Portable spirometry was performed via the procedure recommended in ATS guidelines. Repeatability of spirometry was checked against ATS repeatability standards and NHLEP QC grade. Percentage of predicted value of non-smokers was calculated through a Caucasian population-based regression equation(by Morris et al.) and Korean population based regression equation(by Kim et al.). These were compared. Result : 91.7% of subjects matched repeatability standards of ATS and 95% of subjects got interpretable NHLEP QC grades of A, B, and C. Mean percentage of predicted value by Morris et al. were as follows: Mean of FVC% was 90.86, FEV1% was 96.66, FEV1/FVC% was 107.48. Mean percentage of predicted value by Kim et al. were as follows. Mean FVC% was 92.38, FEV1% was 89.77, FEV1/FVC% was 96.95. Conclusions : Results of this study show that more than 90% of subjects meet repeatability criteria. This supports a role for the portable spirometer as a primary oriental medical office tool. Further, results are a verify that they Korean population-based predicted value equation is a more appropriate standard than the Caucasian population-based one, but a still more accurate standard is needed for the Korean adult population.

  • The Lombard effect is an involuntary response to speakers' experiences in the presence of noise during voice communication. This study aimed to investigate the Lombard effect by comparing the acoustic features of children and adults under different listening conditions. Twelve male children (5-9 years old) and 12 young adult men (24-35 years old) were recruited to produce speech under three different listening conditions (quiet, noise-55 dB, noise-70 dB). Acoustic analyses were then carried out to characterize their acoustic features, such as F0, intensity, duration, and vowel space area, under the three listening conditions. A Lombard effect was observed in the intensity and duration for children and adults who participated in this study under adverse listening conditions. However, we did not observe a Lombard effect in the F0 and vowel space areas of either group. These findings suggest that children can adjust their speech production in challenging listening conditions as much as adults.

  • In this study, we found that Haplorchis taichui, a heterophyid intestinal fluke, is highly prevalent, with heavy worm loads, among riparian people in Saravane and Champasak province, Lao PDR. Fecal specimens were collected from 1,460 people (717 men and 743 women) in 12 riparian (Mekong river) districts and were examined by the Kato-Katz fecal smear technique. The overall helminth egg positive rate was 78.8% and 66.4% in Saravane and Champasak province, respectively. The positive rate for small trematode eggs (STE), which included H. taichui and other heterophyids, Opisthorchis viverrini, and lecithodendriids, was 69.9% and 46.3% in Saravane and Champasak province, respectively. To obtain adult flukes, 30 STE-positive people were treated with 40 mg/kg praziquantel and then purged. Whole diarrheic stools were collected 4-5 times for each person and searched for fluke specimens using a stereomicroscope. Mixed infections with various species of trematodes (H. taichui, Haplorchis pumilio, O. viverrini, Prosthodendrium molenkampi, Centrocestus formosanus, and Echinochasmus japonicus) and a species of cestode (Taenia saginata) were found. However, the worm load was exceptionally high for H. taichui compared with other trematode species, with an average of 21,565 and 12,079 specimens per infected person in Saravane and Champasak province, respectively, followed by H. pumilio (41.9 and 22.5, respectively) and O. viverrini (9.4 and 1.5, respectively). These results show that diverse species of intestinal and liver flukes are prevalent among riparian people in Saravane and Champasak province, Lao PDR, with H. taichui being the exceptionally dominant species.
