The Use of Group Drumming With Korean Middle School Students in School Violence Prevention (중학생 대상 집단 타악기 연주 활용 학교폭력 예방 프로그램)

  • The purpose of this study was to examine how a therapeutic drumming intervention would impact middle school students with regard to school violence prevention. Participants were all in the third-year class of a middle school in Korea. A school music teacher and a music therapist designed and implemented the program collaboratively, and mainly used dyadic, synchronized, and improvisational drumming based on the Social Emotional Learning core competencies. A total of 65 students participated in a weekly 45-minute program for 10 weeks. Ten participants out of 65 were selected for interviews and the rest of the 55 participants were asked to fill out an open-ended survey. Content analysis of the survey and interviews produced 492 meaningful statements, which were categorized into seven themes: somatic responses to drumming, emotional processing, group cohesion, empathy, relationship with peers, self-esteem, and self-regulation. The findings indicated that dyadic, synchronized, and improvisational drumming may promote prosocial behaviors in students of this age. The author discussed that drumming produces physical input directly from the instruments, which prompts students to identify and empathize with their own or others' emotions. This study therefore suggests that collaborative work between school music teachers and music therapists may positively impact middle school students' prosocial behaviors, as they pertain to school violence in Korea.

Postnatal cytomegalovirus infection in an extremely premature infant transmitted via breast milk: A case report (산모의 모유를 통하여 감염된 극소 저체중 출생아에서의 거대세포바이러스 감염)

  • Cytomegalovirus (CMV) is one of the most commonly encountered viral pathogens in newborn infants and is found in 0.3-2.4% of all live births. It has been demonstrated that 40-96% of seropositive mothers shed the virus via their breast milk. Breast milk containing CMV can cause almost one-third of CMV infections occurring in infants. A case of postnatal CMV infection in an extremely premature infant (gestational age $24^{+5}$ weeks, birth weight 750 g) transmitted via breast milk is presented. For neonatal intensive care unit (NICU) management of severe thrombocytopenia, anemia, and sepsis syndrome, the infant received repeated transfusions of platelets; intravenous (IV) immunoglobulins; and gamma- irradiated, filtrated packed red cells and was fed her mother's breast milk since the second week of life. CMV infection was diagnosed with positive CMV immunoglobulin M (IgM) and positive urine CMV culture at the second month of life. Considering the negative CMV IgM and urine CMV culture at birth, postnatally-acquired CMV infection was suspected and confirmed with completely identical nucleotide sequence alignments of the infantile blood isolate and the maternal breast milk isolate. To our knowledge, this is the first case of proven postnatal CMV infection transmitted via breast milk in an extremely premature infant in Korea.

  • The purpose of this study was analyze life condition and needs for welfare service of women with disabilities in adult in Gyeongbuk. And to suggest ways to support effective. Method was conducted a questionnaire survey of 465 subjects. The results of this study was as follows: First, The women with disabilities in young adults was two generations, one children, high school graduate, spouseless, mental disabilities. Also, The needs for welfare services about economic activities and employment was high. Second, The women with disabilities in middle-age people was two generations, two children, high school graduate, spouse, physical or cerebral palsy disabilities. Also, The needs for welfare services about daily activity support, economic activities, employment and pregnancy, childbirth, childcare was high. Third, The women with disabilities in older adults was single-person household, three children, elementary school graduate, spouseless, physical or cerebral palsy disabilities. Also, The needs for welfare services about health and medical care was high. Finally, They needs support in economic activities, employment and daily activity support and pregnancy, childbirth, childcare, and health, medical care. Based on this, Suggestions for effective support were suggested.

Thalidomide Combined with Chemotherapy in Treating Patients with Advanced Colorectal Cancer

  • Objective: To assess the safety and effectiveness of thalidomide (produced by CHANGZHOU PHARMACEUTICAL FACTORY CO.LTD) combined with chemotherapy in treating patients with advanced colorectal cancer. Method: A consecutive cohort of pretreated patients with advanced colorectal cancer were treated with thalidomide combined with chemotherapy. And chemotherapy for patients with advanced colorectal cancer were administered according to the condition of patients. Thalidomide was orally administered at a dosage of 50mg/day to 150mg/day before sleeping for at least 14 days. After at least 14 days of treatment, safety and side effects were evaluated. Results: There were 12 female and 3 male patients with advanced cancer recruited into this study, including 9 patients with colon, 6 patients with rectal cancer. The median age of patients was 57(41-82) years. Partial response was observed in 2 patients (2/15), and stable disease in 3 patients(3/15). Incidences of Grade 1 to 2 myelosuppression was observed in 1/15 patients, and Grade 1 to 2 elevation of hepatic enzyme was recorded in 1/15 patients. Adverse effects on the gastrointestinal tract were documented in 1/15 patients, and were Grade 1. No Grade 3-4 toxicities were diagnosed. No treatment related death was found. Conclusions: Thalidomide combined with chemotherapy was safe and mildly effective in treating patients with advanced colorectal cancer. However, further study should be conducted to clarify the effectiveness of this combination.

Inflammatory Breast Cancer: a Single Centre Analysis

  • Background: Inflammatory breast cancer (IBC) is an aggressive form of locally advanced breast cancer characterized by rapidly progressive breast erythema, pain and tenderness, oedema and paeu d'orange appearance. It accounts for 1-3% of all newly diagnosed cases of breast cancer in the west. Data on IBC from India are lacking. The aim of our study was to assess the clinical-pathological parameters and outcome of IBC at, All India Institute of Medical Sciences, a large tertiary care centre. Materials and Methods: We screened 3,650 breast cancer cases registered from January 2004 to December 2012 and found 41 cases of IBC. Data included demographics as well as clinical, radiological and histopathological characteristics, and were collected from clinical case records using the International Classification of Diseases code (C-50). Patients who presented with IBC as a recurrence, or who had a neglected and advanced breast cancer that simulated an IBC were excluded from this study. Results: The median age was 45 years (range 23-66). The median duration of symptoms was 5 months. The American Joint Committee on Cancer stage (AJCC) distribution was Stage III - 26 and IV - 15 patients. Estrogen receptor (ER), progesterone receptor (PR) positivity and human epidermal growth factor receptor 2 (HER2/neu) positivity were 50%, 46% and 60%, respectively. Triple negativity was found in 15% of the cases. All the non metastatic IBC patients received anthracycline and/ or taxane based chemotherapy followed by modified radical mastectomy, radiotherapy and hormonal therapy as indicated. Pathological complete remission rate was 15%. At a median follow-up of 30 months, the 3 year relapse free survival and overall survival were 30% and 40%respectively. Conclusion: IBC constituted 1.1% of all breast cancer patients at our centre. One third of these had metastatic disease at presentation. Hormone positivity and Her2 neu positivity were found in 50% and 60% of the cases, respectively.

Social Determinants of Health and 5-year Survival of Colorectal Cancer

  • Background: Early in the 21st century, cancers are the second cause of death worldwide. Colon cancer is third most common cancer and one of the few amenable to early diagnosis and treatment. Evaluation of factors affecting this cancer is important to increase survival time. Some of these factors affecting all diseases including cancer are social determinants of health. According to the importance of this disease and relation with these factors, this study was conducted to assess the relationship between social determinants of health and colon cancer survival. Materials and Methods: This was a cross-sectional, descriptive study for patients with colon cancer registered in the Cancer Research Center of Shahid Beheshti University of Medical Science, from April 2005 to November 2006, performed using questionnaires filled by telephone interview with patients (if patients had died, with family members). Data was analyzed with SPSS software (version 19) for descriptive analysis and STATA software for survival analysis including log rank test and three step Cox Proportional Hazard regression. Results: Five hundred fifty nine patients with ages ranging from 23 to 88 years with mean${\pm}$standard deviation of $63{\pm}11.8$ years were included in the study. The five year survival was 68.3%( 387 patients were alive and 172 patients were dead by the end of the study). The Cox proportional hazard regression showed 5-year survival was related to age (HR=0.53, p=0.042 for>50 years versus<50 years old) in first step, gender (HR=0.60, p=0.006 for female versus male) in second step, job (HR=1.7, p=0.001 for manual versus non manual jobs), region of residency (HR=3.49, p=0.018 for west versus south regions), parents in childhood (HR=2.87, p=0.012 for having both parents versus not having), anatomical cancer location (HR=2.16, p<0.033 for colon versus rectal cancer) and complete treatment (HR=5.96, p<0.001 for incomplete versus complete treatment). Conclusions: Social determinants of health such as job, city region residency and having parents during childhood have significant effects in 5-year survival of colon cancer and it may be better to consider these factors in addition to developing cancer treatment and to focus on these determinants of health in long-time planning.

  • Purpose: To identify injuries and profile the characteristics of injured skiers and to use this information in designing injury-prevention programs and protective equipment. Materials and Methods: Injured skiers treated at a clinic of a ski resort located in Kangwon province in Korea were reviewed from 1998 to 2001. An injury was defined as any accident occurred during alpine skiing that required medical treatment. All injured skiers were asked to fill in a questionnaire which documented the mechanism of injury, their experiences , and the slope where they were injured Results: 1197 patients experienced ski injuries over a three-year-period. The overall incidence of ski injuries was 6.4 injuries per 1000 skiers. There was a slightly higher proportion of men than women and the mean age was in the third decade. The injury rate was higher for beginners and for less difficult slopes . The sprain and ligament injuries were most common and knee injuries accounted for 30$\%$ of all ski injuries . Conclusion: In our study , ski injuries occurred more often compared with foreign reports. it was concluded that an insufficient injury prevention system, an increasing number of skiers and a growing proportion of beginners was responsible for such a result.

Clinical Manifestations and Treatment Outcomes of Eosinophilic Gastroenteritis in Children

  • Purpose: The aim of the present study was to investigate the clinical features and outcome of eosinophilic gastroenteritis (EGE) in children. Methods: Our study enrolled 24 children who were diagnosed with EGE from 1993 to 2014 at the Department of Pediatrics, Seoul National University Children's Hospital. The patients' clinical manifestations, treatments, and outcomes were reviewed from the medical records. Results: The mean age at diagnosis was 5.3 years. Most patients had gastrointestinal symptoms including diarrhea (54.2%) and abdominal pain (45.8%). Peripheral eosinophilia was present in 91.7% of the patients. Thirteen patients (54.2%) showed anemia, and 15 patients (62.5%) had hypoalbuminemia. EGE was classified as mucosal, subserosal, or muscular in 75.0%, 20.8%, and 4.2% of cases, respectively. Three patients showed gastroduodenal ulcers upon endoscopic analysis. A history of allergy was reported in 13 patients, including atopic dermatitis, allergic rhinitis, and asthma. Five patients (20.8%) improved with food restrictions. Among the 19 patients treated with steroids, 11 (57.9%) discontinued steroid treatment without subsequent relapse, 4 (21.1%) relapsed after ceasing steroid treatment, and 4 (21.1%) showed no response to steroids. Two patients who were resistant to steroids underwent therapeutic surgery. The presence of gastroduodenal ulcers was significantly associated with relapse and steroid resistance. Conclusion: A high suspicion of EGE is warranted when children have nonspecific gastrointestinal symptoms and peripheral eosinophilia. Most patients improved with food restrictions or steroid treatment, although one-third of patients showed a relapse or steroid resistance.

Relationship Between Organizational Effectiveness and Job Stress of Radiotechnologist (방사선사의 조직 효과성과 직무 스트레스의 관계)

  • This study was conducted in order to analyze job stress factors of radiotechnologists in order to inprove the work efficiency and to organize system management plans. For data collection, total 261 radiotechnologists, who work in Busan, took part in structured questionnaire survey. The collected data were analyzed by using SPSS/PC+ Win 13 version. For verification of the differences among groups, one-way ANOVA was executed. For analyzing efficiency of system, which is affected by job stress, multiple regression analysis was taken. First, work stress classified by age group was found as the biggest stress factor among radiotechnologists in their 30s. Among radiotechnologists in their 20s, 40s and 50s, the biggest stress factor was communication problems. In terms of, job stress classified by division, no statistical difference was observed among groups. Second, as the result of system efficiency analysis, high values were found in organizational immersion and job satisfaction among radiotechnologists in their 20s and oncology division has the highest values in concentrating the system and job satisfaction. For this result, statistically significant differences were observed among groups. Third, in terms of the sub-factors of work stress, communication problem was found as the variable that exerted statistically significant impact to organizational effectiveness. However, no statistically significant differences were observed inadministrative and personal problems. Job stress decreases as communication within the organizations or communication with other divisions becomes more successful. In other words, system efficiency can be enhanced by decreasing conflicts within organization through successful communication.

Promoting Policy for Creative Economy and Regional Development in Korea (창조경제정책논의와 지역발전)

  • This paper criticized the recent Korean 'creative economy policies' focused on regional developmental implications. Even though the policies targeted to promote ICT new startups and build virtuous circle of ICT industrial ecosystem in Korea as a whole, the outside regions of the Seoul-Busan industrial axis where the bases of ICT industries are very weak would suffer from systematic exclusion in ICT investments and deepening regional disparities. Second, ICT-centered policies would selectively affect or operate commensurate with the size of regions in this low-growth, after-financial crisis age. Third, the possibilities of regional insularity and lock-in in these low levels of 'related variety' regions would worsen the industrial competitiveness. Lastly, the policies should be reoriented to fortify region-based creative economic ecosystem based upon triple helix learning region.
