상급종합병원과 여성전문병원 간호사의 산후 간호중재 조사 (Tertiary Hospitals' and Women's Special Hospitals' Postpartum Nursing Intervention Survey)

  • Purpose: This study was done to assess development and postnatal care interventions in postnatal care intervention records for maternity ward nurses in tertiary hospitals and women's hospitals in South Korea. Methods: This mixed-method research was a Time-Motion (TM) study. Data were collected through external observation of 12 nurses in 4 wards over 24 hours. Mann-Whitney U test and independent t-test were employed for the analysis of frequency and provision time of direct/indirect care activity. $x^2$ (Fisher's exact test) was utilized to determine the difference in frequency between two groups. IBM SPSS 22.0 statistical program was employed for calculation. All statistical significance levels were at ${\alpha}=.05$. Results: According to the KPCS-1 (Korean Patient Classification System-1), women's hospitals are group 3 and tertiary hospitals, group 4. With respect to time difference in direct care, tertiary hospitals showed 791 minutes and women's hospitals, 399 a difference of 392 minutes. For time difference in indirect care, women's hospitals had 2,415 minutes while tertiary hospitals, 2,080, a difference of 335 minutes for women's hospitals. No difference was found in the average total care workload between the two institutions. Individual time also showed no difference (p>.05). Conclusion: High-risk maternal care strength in tertiary hospitals and breast-feeding strength in women's hospitals need to be benchmarked with each other.

중환자실 뇌혈관질환자에게 수행된 간호중재분석 (A study of the Nursing Interventions performed by the ICU nurses to the patients with Cerebrovascular disorders)

  • The purpose of this study was to analysis of nursing interventions performed by the ICU nurses to the patients with cerebrovascular disorder practically from one university hospital in Seoul. The data were collected from 15 nurses with 86 cerebrovascular disorder cases from one ICU with the questionaire to write frequency of nursing care done by the surveyee from May, 2, 2000 to July, 3, 2000 and the list of 66 nursing interventions selected out of 433 NIC(Nursing Interventions Classification) of Iowa University which were translated into Korean (44 items) and core nursing interventions by ICU nurses (22 items; KIm, Su-Jin, 1997). The data were analysed with SPSS program. The results are as follow : 1. The most frequently used nursing interventions were vital sign monitoring, fall prevention, cerebral edema management, dysreflexia management, neurologic monitoring, cardiac care, communication enhancement, technology management, bed rest care, respiratory monitoring in rank. 2. The most frequently used nursing intervention domains were 'Physiological : Complex', 'Physiological : basic', 'Behavior', 'Safty', 'Health system' in rank. In the domain of physiological : basic, the most frequently used nursing interventions were bed rest care, urinary elimination management, tube care : urinary, physical restraints in rank. In the domain of physiological : complex, the most frequently used nursing interventions were cerebral edema management, dysreflexia management, neurologic monitoring, cardiac care in rank. In the domain of behavior, the most frequently used nursing interventions were communication enhancement, touch, active listening in rank. In the domain of safty, the most frequently used nursing interventions were vital sign monitoring, fall prevention in rank. In the domain of health system, the most frequently used nursing interventions were technology management, specimen management in rank. 3. some difference of the frequency practicing the nursing interventions according to the shift of duties was found. For example, medication administration was common at day duty, touch was practiced at evening duty, temperature regulation was performed.

Perception of Nurse Experts on the Contribution of Nursing Interventions to NOC Nursing Outcomes in General Hospitals in Korea

  • The purpose of this study was to identify the perception of nurse experts on the contribution of nursing interventions to Nursing Outcomes Classification NOC nursing outcomes. A nursing outcome is a nursing-sensitive patient outcome primarily affected by nursing interventions. As one of the standardized language systems of nursing outcomes, the NOC must be examined for applicability before it is used in Korea. Data were collected in February and March 2003 using a 5-point Likert scale. For data collection, 230 quality improvement (QI) or quality assurance (QA) nurses from general hospitals in Korea were asked to rate the extent that nursing interventions contribute to each of the NOC nursing outcomes (2000) in their hospitals. Ninety-six nurses from 63 hospitals responded and the response rate was $41.7\%$. Mean scores for perception of contribution of nursing interventions to each of the NOC nursing outcomes ranged from 2.18 to 4.54. Vital Signs Status had the highest score (M = 4.54), and Abuse Recovery: Financial, the lowest score (M = 2.18). Of the seven NOC domains, the mean score was highest for Physiologic Health (M = 3.91) and lowest for Community Health (M = 2.92). Of the 29 NOC classes, the mean score for perceived contribution was highest for Metabolic Regulation (M = 4.32) and lowest for Community Well-Being (M = 2.92). Participants perceived that nursing interventions in general hospitals in Korea contributed, at least to a certain extent, to most of the NOC nursing outcomes. Based on these results, NOC should have relatively good applicability in Korea.

간호일지 상의 간호중재와 지각된 간호중재의 수행빈도 비교 -말기 암환자를 중심으로- (Comparison of Cancer Nursing. Interventions Recorded in Nursing Notes with Nursing Interventions Perceived by Nurses of an Oncology Unit - Patients with Terminal Cancer -)

  • Purpose: The purposes of this study were to determine the core nursing interventions in nursing notes and the practice which was perceived by nurses of an oncology unit with patients with terminal cancer. Also, comparing interventions in nursing notes with interventions in perceived practice was done. Method: Subjects were 44 nursing records of patients with terminal cancer who had died from Jan. to Dec. 2002 at C University Hospital and 83 nurses who were working on an oncology unit for more than one year. Data was collected using a Nursing Interventions Classification and analyzed by means of mean and t-test. Results: The most frequent nursing intervention was 'nausea management' in the nursing note and was 'medication administration: oral' in perceived practice. The frequency of nursing interventions in the nursing record was lower than in perceived practice. Conclusion: This study finds that nurses actually practice nursing care, but they may omit records. To correct for omitted nursing records, development of a systematic nursing record system, continuous education and feedback is recommended.

뇌혈관질환 환자의 간호진단과 연계된 간호중재의 중요도와 수행도 분석 (Comparison of Importance and Performance of Nursing Interventions linked to Nursing Diagnoses in Cerebrovascular Disorder Patients)

  • Purpose: The purpose of this study was to compare the importance and performance of nursing interventions linked to five nursing diagnoses in CVA patients. Methods: First, total 37 nursing diagnoses were identified from the analysis of 78 nursing records of CVA patients, and then top 5 diagnoses were mapped with nursing interventions. Second, each intervention was compared in terms of importance and performance by 80 nurses working at neurosurgical units from 5 general hospitals. Data were analyzed using mean, SD, and t-test using the SPSS program. Results: Selected the top five nursing diagnoses were Acute Pain, Risk for Disuse Syndrome, Decreased Intracranial Adaptive Capacity, Ineffective Cerebral Tissue Perfusion and Acute Confusion. In general, most of the interventions were scored higher in importance than performance and most of independent interventions were not performed as frequently as it perceived in importance. The interventions which scored high in performance were the interventions ordered by physician or interventions related to medication behavior. Conclusion: We identified which nursing interventions should be performed more frequently and more critically important to nursing diagnoses. We recommend further research that enhances the performance of nursing interventions to provide better quality of nursing services to the patients in practice.

급성심근경색증 환자의 증상 인지와 건강행위 이행이 내원시간에 미치는 영향 (Effects of Symptom Recognition and Health Behavior Compliance on Hospital Arrival Time in Patients with Acute Myocardial Infarction)

  • Purpose: This study was to investigate the relationship among the symptom recognition, health behavior compliance, and the hospital arrival time to identify factors influencing the hospital arrival time in patient with acute myocardial infarction (AMI). Methods: The subjects of this study were 200 patients with AMI in C hospital in D city. Data were analyzed using descriptive statistics, independent t-test, One way ANOVA, Pearson's correlation coefficients, and stepwise multiple liner regression tests. Results: Level of symptom recognition and health behavior compliance was low. The median value of hospital arrival time was 4.48 hours (ST-segment Elevation Ml was 2.43 hours and Non ST-segment Elevation MI was 7.83 hours). Among the studied factors, only symptom recognition had a statistically significant positive correlation with health behavior compliance (r=0.38, p<.001). Factors influencing the hospital arrival time were MI classification, diabetes mellitus (DM) and transport vehicle to the 1st hospital, and they accounted for 13% of the variance for hospital arrival time in AMI patients. Conclusion: To prevent the delay of hospital arrival time in MI patients, a more robust nursing strategic intervention according to MI classification and DM is necessary; further education on the importance of transportation utilization is also mandated.

대학생의 스마트폰 중독 분류군 별 중독에 영향을 미치는 요인: 자기통제력을 중심으로 (Factors Affecting on Smartphone Addiction according to the Classification of Addiction-risk Groups among College Students: A Focus on Self-control)

  • Purpose: The purpose of the study was to investigate the factors affecting on smartphone addiction according to the classification of addiction-risk groups among college students, focusing on self-control. Methods: A sample of 242 college students at K University in Seoul was recruited to participate in the study. A structured questionnaire composed of items for the self-rated smartphone addiction scale and self-control scale was used to collect data. Results: The number of students in smartphone addiction-risk group was 66 (27.3%). This study had 17.5% of the explanatory power, including perceived smartphone necessity (${\beta}=.330$, p=.007), and instant self-control (${\beta}=-.281$, p=.028) in addiction-risk group. Whereas, in non-risk group, the affecting factors included gender (${\beta}=.194$, p=.004), self-awareness of addiction (${\beta}=-.290$, p<.001), and instant self-control (${\beta}=-.281$, p<.001) with 31.3% of the explanatory power. Conclusion: There is a need to develop an intervention program to prevent the addiction of smartphones and to improve self-control among college students.

급성 백혈병 환자의 생존요인 및 사이토카인 분석 (Survival Factors and Cytokines for Acute Leukemia Patients with Chemotherapy Compared with Bone Marrow Transplantation)

  • Purpose: The purpose of this study was to predict treatment outcome of chemotherapy compared with and bone marrow transplantation in acute leukemia patients. Methods: We respectively reviewed the characteristics of subjects, cytokine, complete remission time and survival time of 111 patients with acute leukemia, admitted in St. Mary's hospital, between July 2007 and August 2008. Results: The complete remission rate with chemotherapy group was 70.8% and bone marrow transplantation group was 54.3% but without statistically significance. The prognostic factors related with survival is classification of acute leukemia and complete remission time. Conclusion: This study suggests a need for nursing research and nursing intervention for acute leukemia patients.

신경과 병동에 입원한 노졸중환자의 간호일지에 나타난 급성기와 아급성기의 간호중재 비교 (A Comparison of Interventions Recorded in Nursing Notes between Actue and Subacute Stage after a Cerebrovascular Accident)

  • Purpose: This study aimed to: 1) determine the core nursing interventions, and 2) compare acute interventions with subacute interventions recorded in the nursing notes of patients with cerebrovascular accidents (CVA). Methods: The nursing records covering the first 10 days of 30 patients with a CVA who were admitted from January to December 2004 at C University Hospital in Korea were examined. Data was collected using the nursing interventions classification (NIC) from January to April 2005. Finally, data analysis was carried out using mean, SD, and paired t-test according to domains, classes, and interventions. Results: The most frequent nursing intervention at both stage was 'Neurologic monitoring'. There were differences in interventions belonging to the 'Physiological: complex,' 'Behavioral,' 'Safety,' and 'Health system' domains between the acute and subacute stages. The frequency of interventions belonging to the 'Immobility management,' 'Neurological management,' 'Tissue perfusion management,' 'Patient education,' 'Risk management,' 'Health system mediation,' and 'Information management' classes at the acute stage was higher compared to the subacute stage. Conclusions: This study found out that nurses relatively recorded more nursing interventions during the acute stage hence the unsuccessful documentation of the subacute stage particularly in describing the specific nursing interventions at this stage.

정신과 병동 간호사의 간호중재에 대한 중요도 인지 및 수행빈도와의 관계 - 3차개정 Nursing Interventions Classification(NIC)을 적용하여 - (A Correlation Study of Perceived Importance of Nursing Interventions with Performance Frequency in Psychiatric Nursing Units Using the 3th NIC)

  • Purpose: The purpose of this study was to identify the perceived importance of nursing intserventions of psychiatric nurses according to domains, classes and interventions using the 3th NIC. Method: In this study, a 435 of 486 nursing interventions were selected from 75% consent reached by experts. Data were collected from 9 hospitals and 141 nurses(return rates: 94,0%) in Seoul, Kyonggi, Chungnam, Gwang-ju and Chonnam region from January, 2002 to February, 2002 using 4 point Likert scale. Result: Total perceived importance score was $2.905{\pm}0.463$ and total correlation score with performance frequency was r=0.295. The most important perceived domain and class was found to be ‘safety’ ($3.217{\pm}0.465$) and ‘community health promotion’ ($3.285{\pm}0.866$). The most correlated domain and class with performance frequency appeared to be ‘behavioral’ (r=0.431, p=0.000) and ‘communication enhancement’ (r=0.439, p=0.000). The most important perceived nursing intervention was found to be ‘active listening’ ($3.652{\pm}0.549$). Conclusion: In conclusion, nurses in clinical settings were found to perform less than perceive its importance. Therefore, further researches are needed to identify factors related to impede and develop strategies to improve the performance of nursing interventions.
