A Review of Assessment Tools in Traditional Medicine on the High Frequent Skin Diseases (다빈도 피부질환에 대한 국내외 한의학 논문에서 평가도구 사용현황에 대한 고찰)

  • Objectives : The purpose of this study is to establish the basis of the assessment tools that can be used for the diagnosis and assessment of skin diseases by reviewing the present status of the assessment tools used in domestic and overseas traditional medicine papers for the past five years. Methods : This study is based on analysis of papers on clinical trials about atopic dermatitis, psoriasis, acne, urticaria published from 1st July 2013 to 30th June 2018. The papers were searched from Oriental Medicine Advanced Searching Integrated System(OASIS), Korean Traditional Knowledge Portal(KTKP), Pubmed, MEDLINE. Results : 40 articles on atopic dermatitis, 34 articles on psoriasis, 26 articles on acne and 10 articles on urticaria were selected. Among them, 38 articles(90%), 20 articles(85%), 22 articles(85%) and 5 articles(50%) used assessment tools in order. Conclusions : SCoring Atopic Dermatitis(SCORAD), objective SCORing of Atopic Dermatitis index(OSI), Eczema Area and Severity Index(EASI) were used in studies on atopic dermatitis. In the case of psoriasis, The Psoriasis Area and Severity Index(PASI) was used in most papers. In the study on acne, most of Korean papers used Korean Acne Grading System(KAGS), but in the overseas papers, various assessment tools were used. In the case of urticaria, the rate of use of the assessment tool was the lowest, but the assessment tool used was unified by Urticaria Activity Score(UAS). Since skin diseases can cause stress and a decrease in quality of life, the quality of life should be taken into account when evaluating treatment effects.

  • Recently many effort focused to understand the mechanical insights of melanogenesis to develop the agent for hyper-pigmentation. So this study was performed to investigate the depigmentation of many herb. This study show that many herb which were generally used for external application have some depigmentation effects. So we should use them for whitening agents and the depigmentation effects of the other natural subjects which were generally used for external application should be examined. In the future, it is nessary more study on the manufacturing method, the time of use and the dose.

A Literature study about comparison of Eastern-Western medicine on the Tinnitus (이명(耳鳴)의 동(東)·서의학적(西醫學的) 문헌(文獻) 고찰(考察))

  • Objectives : This study is performed to investigate the therapy on tinnitus through the literature of Eastern and Western medicine. Methods : This study review the definition, etiology, classification, internal and external methods of treatment of tinnitus based on 50 articles of Eastern medicine, 16 articles of Western medicine about the treatment of tinnitus. Results : The results were as follows. 1. In Eastern medicine, the cause and mechanism of disease of tinnitus arose from the state of qi-blood deficiency, internal dampness-heat and depression stagnation. In Western medicine, there are mainly caused by damage to the auditory system the lower induced changes in the upper. 2. In Eastern medicine, differentiation of syndromes classifies clinical aspects, and cause and mechanism of disease. In Western medicine, it divides into an generating region, clinical and associated symptoms aspects. 3. In Eastern medicine, internal method of treatments of tinnitus are divided into four treatments. In Western medicine, internal methods of tinnitus are vasodilators, blood-flow improvers, Metabolism improvers etc. 4. In Eastern medicine, external method of treatments of tinnitus are paste preparation method, powder preparation method, pill preparation method, acupuncture & moxibustion method. In Western medicine, external method of treatments of Tinnitus are divided into injection, other surgical therapies and adjuvant therapy. Conclusions : Until now, there is no perfect, effective single treatment. We think that Eastern medicine approach and treatment can be helpful to overcome the limitations of tinnitus cure.

Effects of Lithospermum erythrorhizon on the cytokine gene expression in human keratinocytes (자초(紫草)가 HaCaT 세포의 사이토카인 유전자 발현에 미치는 영향)

  • Objective : Propionibacterium acnes (P. acnes) is a major pathogenic bacteria for acne vulgaris. This study was performed to evaluate the effects of Lithospermum erythrorhizon extracts on the inflammatory cytokines gene expression by P. acnes in human keratinocytes, HaCaT cell line. Methods : Anti-bacterial activity and cytotoxicity of LE extracts was analyzed by agar plate culture and XTT assay. The cytokines gene expressions were assessed by real time RT-PCR for IL-8, MCP-1 and TNF-${\alpha}$. During the cell culture and treatments, amounts of secreted TNF-${\alpha}$ were measured by ELISA. Translocation of transcription factor NF-${\kappa}B$ from cytoplasm into nucleus was observed by immunocytochemistry and confocal microscopy. Results : There were no anti-bacterial effects and cytotoxicity as high as $1,000{\mu}g/ml$ of LE extracts in XTT assay. Transcription levels of inflammatory cytokines, IL-8, MCP-1 and TNF-${\alpha}$ were increased by P. acnes in HaCaT. LE extracts decreased the upregulated gene transcription levels. However, amounts of secreted TNF-${\alpha}$ were similar in HaCaT cells with P. acnes and LE extracts. Translocation of NF-${\kappa}B$ into nucleus by P. acnes was significantly inhibited by LE extracts. Conclusions : From the results of this study, LE extracts have anti-inflammatory effects on HaCaT cells by P. acnes that decreased the mRNA expressions of IL-8, MCP-1 and TNF-${\alpha}$. This anti-inflammatory effects of LE extracts could provide the potential of therapeutic substance for acne vulgaris.

A Seven Cases of Treating Atrophic Acne Scar with Subcision, Microneedle Therapy System and Herbal Peeling Therapy (절개침과 미세피부침, 약초필링을 이용한 위축성 여드름 흉터 치료 7례 임상보고)

  • Objectives : The purpose of this study is to present the effect of subcision, microneedle therapy system and herbal peeling therapy which is treatment of Korean medicine to atrophic acne scar patients. Methods : We administered subcision, microneedle therapy system and herbal peeling therapy to atrophic acne scar patients and tested the results by Qualitative Global Acne Scarring Grading System and Korean Acne Grading System as well. Results : After the treatment, all of the patients showed improvements in Qualitative Global Acne Scarring Grading System and Korean Acne Grading System as well. Qualitative Global Acne Scarring Grading System of patients changed from grade 2.71±0.71 to grade 1.14±0.86 along with the change of Korean Acne Grading System from grade 1.57±0.57 to grade 1.00±0.00. Conclusions : Subcision, microneedle therapy system and herbal peeling therapy may be effective in the treatment of moderate to severe acne scars, and could be applied in clinical practice.

The Quality of Life in Patients with Allergic Rhinitis Who Visit Daegu Oriental Hospital of Daegu Haany University (대구한의대부속대구한방병원에 내원한 알레르기비염 환자의 삶의 질 분석)

  • Objectives : Allergic rhinitis disturbs the quality of life in patients. So, it is important to analyze the quality of life in patients with allergic rhinitis. The aim of this study was to evalute the impact of allergic rhinitis on the quality of life of patients. Methods : A total of 46 patients with allergic rhinitis who visited the Dept. of Oriental Medical Opthalmology & Otolaryngology & Dermatology of Daegu Hanny Oriental Medical Center from July 10th, 2006 to September 10th, 2007 were included in this study. The patients answered the quality of life questionnaire on the scale of one to five. We calculated the frequency and importance. Results : The patients with allergic rhinitis experienced impairment of the quality of life. On the questionnaire, nasal symptoms, practical problems, generalized symptoms were frequently checked. Patients think importance in nasal symptoms, practical problems, generalized symptoms on the questionnaire. Conclusions : Considering the above results, most of patients with allergic rhinitis have problems due to generalized symptoms, practical problems, sleep disturbance, activity limitations, emotional problems, and ocular symtoms including nasal symptoms. Further studies in treatment with Korean medicine are needed.

  • Erythema nodosum is a nodular erythematous eruption predominently limited to the pretibial area but occasionally involving the arms or other areas. That is clinical entity defined easily but there are many different opinions about histopathologic findings, Recently erythema nodosum is characterized histopathologically by a septal panniculitis in which the fibrous septa of subcutaneous fat become inflamed. Erythema nodosum has been known to be frequently associated with some kinds of drugs, infections with streptococci, mycobacteria but in $60\%$, no cause is found, Treatment of erythema nodosum consists of supportive care and elimination of underlying causes. Because spontaneous resolution of the lesions can occur in 3 to 6 weeks. Although there are trial treatments are applied to erythema nodosum, ego corticosteroids, NSAIDs, potassium iodine, there isn't any definite. So We report a case of erythema nodosum which was not relieved by NSAIDS but by experimental acupuncture treatment.

A Study of the Term 'Dermatology' in Oriental Medicine (동서의 피부 질환 명칭에 대한 소고)

  • Objectives: In order to establish a base for proper treatment and management of patients with dermal problems through correct diagnosis, I considered the naming rule for dermatology in Oriental Medicine, referring to the dermatology literature compared to western medicine. In addition, this paper examines the characteristic classification of dermatology. Methods: I examined the naming rule of dermatology in Oriental Medicine and then compared the disease names in Oriental and Western medicine and the characteristic classification of dermatology referred to the records. Results: The dermal diseases have been named according to their colors and morphologies, causes, progress of symptoms, recurrent sites, the character of distribution, recurrent seasons, ages, the character of patients' jobs and locations. Sometimes some have been named by referring to their main morphologies, sites, causes, colors and seasons synthetically. However it was found some names for dermal diseases, even though the same diseases, had been named differently according to for example: historical times, condition of locations and the quality of doctors whose process of naming developed and changed over time. The relationship between Oriental and Western medicine of each name for dermal diseases is basically divided into 5 types: same names - same diseases; same names but different diseases; same diseases but different names; one disease with multiple names; and one name with multiple diseases. Considering the methods of classification, these were generally achieved according to their places of origin. It is a method unique to Oriental medicine that we classified some dermal diseases into 疥, 癬, 瘡, 風, 丹, 疱, 疹, 癰, 痘, 疽 and so on and it is very easy to diagnose which part they belong to. This was classified by putting first the causes of diseases; for instance: viruses, bacteria, fungi. Sometimes, however there was a problem, connected to the classification of morphology. Conclusions: I suggest that we need to unify and refine dermatological terms in Oriental Medicine in order to establish a base for proper treatment and management of patients with dermal problems through correct diagnoses.

A Research on Quality of Life and Cost of Oriental Medical Care in Adult Patients with Chronic Atopic Dermatitis in Korea (성인 만성 아토피 피부염 환자의 삶의 질과 한방 치료비용에 대한 고찰)

  • Objective : The aim of this study is to assess the impact of chronic atopic dermatitis (AD) on quality of life (QoL) as well as the economic burden associated with oriental medical treatment on adult patients with chronic AD in Korea. Methods : The EQ-5D health states, existence and duration of AD in the 2007 National Health and Nutrition Examination Survey (NHANES) were used to obtain the study subjects. A questionnaire including the questions on direct medical and direct non-medical costs associated with oriental medical treatment for adult patients with AD was specifically designed. Twenty-nine, members of the Korean Oriental Medical Ophthalmology, Otolaryngology and Dermatology Society (KOMOODS), completed the questionnaires. Results : The incidence of AD was around 3.09% in 2007 NHANES (sample survey, n=2981). Three months and above in duration of disease was 1.35%. The QoL data revealed as follows: 0.932 in non-chronic AD, 0.916 in chronic AD, and 0.908 in non-AD. We also stratified our analysis by age. The QoL data in 40's was statistically significant (P<0.023). But, no significant differences were reported in 20's, 30's, and 50's. The total direct medical costs of oriental medical treatment for AD was about 2,560,717 Won, and total direct non-medical costs was about 605,125 Won in a year. Conclusions : This study might be applied to find the evidence of economic evaluation in oriental medicine for AD. More rigorous studies are warranted.

Study on the possibility of Digital Infrared Thermographic Imaging as a prognosis evaluation tool for patients with facial palsy (안면신경마비 환자의 예후 평가 도구로서의 적외선 체열 촬영(DITI - Digital Infrared Thermographic Imaging) 가능성에 대한 연구)

  • Objectives : The aim of this study is to evaluate possibility of DITI as prognosis evaluation tool of facial palsy. Methods : We investigate prognosis of facial palsy through EMR(Electronic Medical Record) of inpatient from December 2016 to June 2017. We evaluated the sex, age distribution, length of hospital stay, paralysis site, number of treatment after discharge, change of H-B Grade at entrance and exit, temperature difference of both sides of DITI, and Nerve Conduction Study(NCS) with reference to EMR recorded symptom change. Results : 1. Significant correlations were not found between DITI and House-Brackmann Grade change, NCS(%), the date of admission. 2. There was a negative correlation between NCS(%) and hospitalization period in patients with facial palsy. The higher the NCS, the faster the recovery rate of facial palsy. 3. In patients with facial palsy, the temperature difference between the two sides after the DITI image shows that the affected side tends to be lower than the normal side. Conclusions : In this study, only DITI temperature difference between both sides of face is not significant in determining the prognosis of facial palsy. Further research is needed to conduct DITI at the same time and to improve accuracy through a sufficient assessment of the degree of facial palsy.
