Marked recovery 이상 호전된 돌발성 난청 환자 치험 3례 (Three Cases of Sudden Sensorineural Hearing Loss with Marked or Complete Recovery)

  • Objective : This study describes the cases of sensorineural hearing loss (SSNHL) patients with marked or complete recovery. Methods : Three patients received oriental medical treatments continuously for more than 1 month and measured the hearing recovery. For diagnosis on these cases, pure tone audiometry (GSI 38, Welch allyn company) was used. For treatment, acupuncture, herbal-medicine and moxibustion treatment were applied. Results : All patients of this report had the recovery of hearing. In 2 cases, the pure-tone 5 frequency (0.25, 0.5, 1, 2, 4 KHz) average improved by more than 35 dB (Marked recovery). In 1 case, it was within the normal limits after oriental medical therapy (Complete recovery). In addition, tinnitus and vertigo decreased in all patients. Conclusions : We may expect that the oriental medical therapy improvers the hearing recovery in the patients of SSNHL.

  • Objectives : The purpose of this study is to present 3 cases reports of psoriasis treated by Damwi-hwan - focused on lipid metabolic abnomarilty. Methods : We administered Damwi-hwan to three patients who showed psoriasis with lipid metabolic abnormality and evaluated the results by Psoriasis Area and Severity Index(PASI). Results : After the treatment three patients showed improvements in PASI. PASI of patient 1 changed from 15.4 to 0.4; patient 2 from 32.7 to 1.2 ; patient 3 from 7.6 to 0.6. Conclusions : The results suggest that Damwi-hwan can be an effective treatment for psoriasis who has lipid metabolic abnormality.

  • Objectives : Pityriasis rosea is a rash that occurs most commonly in people between the ages of 10 and 43, but may occur at any age. It is common disease but I was haven't meet any case by oriental medical treatment. Methods : We experienced one case of Pityriasis rosea treated with oriental medical treatment, acupuncture and herbal medication. Results : The patient suffer from a large, scaly, pink patch on the chest and back and arm and leg. He have very terrible itching sign. After treatment his itching was subsided and large, scaly, pink patch was disappear. Conclusions: Oriental medical treatment could be safe and effective method in pityriasis rasea

  • Background and Objective : Allergic purpura is a generalized vasculitis characterized by cutaneous purpura, art hritis, abdominal pain, gastrointestinal bleeding and nephritis. However, the pathophysiology of the Allergic purpura remains obscure. Treatment at present is symptomatic and supportive. There is few reports on Allergic purpura treated with oriental medical treatment. Methods : We conducted two patients with Allergic purpura seen at Won-Kwang University Ik-San Oriental Medical Hospital. We treated Allergic purpura by a herbal medicine, acupunture, etc. Conclusions : We suggest that the oriental medical treatment and management have an effect with Allergic purpura which means Podoyug in Oriental medicine.

비내시경 활용 비염 변증 지표의 평가자 간 신뢰도 연구 (Inter-rater Reliability Study on Pattern Identification Using Nasal Endoscopy for Rhinitis)

  • Objectives : To identify whether pattern identification using nasal endoscopy for rhinitis can be applied as a tool for evaluating rhinitis in routine care setting, we performed a inter-rater reliability study on this pattern identification. Methods : Two Korean medicine doctors assessed 290 left/right nasal endoscopy photograph cases of rhinitis patients with pattern identification using nasal endoscopy. This pattern identification consist of four assessment items, nasal membrane color(pale/hyperemia), nasal membrane humidity(dryness/dampness), rhinorrhea(watery/yellow), and turbinate membrane edema(atrophic/edematous). Cohen's kappa statistic and Percentage agreement were used to evaluate the inter-rater reliability. Results : Inter-rater percentage agreement and Kappa coefficient for left nasal endoscopy photograph cases was from 'slight' to 'moderate'(% agreement: 40.00-67.59%/Kappa: 0.06-0.407). Only the agreement of 'rhinorrhea (watery/yellow)' item was moderate(% agreement: 67.59%/Kappa: 0.407). Inter-rater percentage agreement and Kappa coefficient for right nasal endoscopy photograph cases was also from 'slight' to 'moderate'(% agreement: 42.41-68.97%/Kappa: 0.109-0.465). Only the agreement of 'rhinorrhea(watery/yellow)' item was moderate(% agreement: 68.97%/Kappa: 0.465). Conclusions : It is necessary to resolve problems such as cut-off value setting, bipolar evaluation values(pale/hyperemia, dryness/dampness, watery/yellow, atrophic/edematous) and weighting items. Further rigorous studies that overcome the limitations of the current research are warranted.

안이비인후피부과 외래환자의 임상적 분석 (A Clinical Analysis of Ophthalmology, Otorhinolaryngology & Dermatology)

  • We analyzed statistics study in 1394 patients, who had visited to the Dept. of dermatology, ophthalmology & otorhinolaryngology Hospital of Oriental Medicine Sangji University from July, 2003 to December, 2004. The results were as follows. 1. Distribution of ophthalmology, otorhinolaryngology and dermatology classification was 14.23% in ophthalmology, 13.92% in rhinology, 4.79% in laryngology, 23.05% in dermatology, 8.25% in otology, 2.14% in stomatology and 33.63% in other. 2. Ophthalmology group were 12.39% males and 14.16% females in 61years old over. In treatment term, opthalmology patients who had treated for 2 weeks within were 85.84%. Blepharism and asthenopia were the most common ophthalmologic disease. 3. Rhinology group were 18.10% males in 11-20 years old and 14.48% females in 10 years old within. In treatment term, rhinology patients who had treated for 2 weeks within were 70.59%. Rhinitis and sinusitis were the most common rhiniologic disease. 4. Otology group were 19.08% males and 16.80% females in 61years old over. In treatment term, otologic patients who had treated for 2 weeks within were 64.89%. Tinnitus and vertigo were the most common otologic disease. 5. Laryngology group were 10.53% males and 17.11% females 41-50 years old. In treatment term, laryngology patients who had treated for 2 weeks within were 85.53%. pharyngitis and abnormal pharyngolarylgeal region sensation were the most common laryngologic disease. 6. Stomatology group were 11.76% males in 31-40, 41-50 years old and 17.65% females in 61 years old over. In treatment term, stomatology patients who had treated for 2 weeks within were 82.35%. Stomatitis and labium spasm were the most common stomatologic disease. 7. Dermatology group were 11.75% males and 15.30% females in 21-30 years old. In treatment term, dermatology patients who had treated for 2 weeks within were 72.95%. Acne and urticaria were the most common dermatologic disease. 8. Other group were 8.99% males in 41-50 years old and 14.04% female in 61 years old over. In treatment term, other group patients who had treated for 2 weeks within were 69.29%. Bell's palsy and common cold were the most common other disease.

한방화장품이 안면부의 탄력 개선에 미치는 영향 (The Efficacy of Korean Herbal Cosmetics for Facial Skin Elasticity)

  • Objective : The purpose of this study was to evaluate the effect of korean herbal cosmetics for facial skin elasticity. Methods : We recruited patients and divided them to cosmetics group and control group randomly. We provided korean herbal cosmetics to cosmetics group and subjects in cosmetics group applied the provided cosmetics for 4 weeks. Control group waited and applied cosmetics that had previously used for 4 weeks. We assessed all subjects in 0 week, 2 weeks and 4 weeks. Primary assessment instrument was moire topography photo scale. Second assessment instrument was skin moisture, oiliness and elasticity which assessed with skintouch system v1.0, detox/regeneration questionnaire score and self-evaluating facial wrinkle/looseness score. Result : Moire topography photo scale improved in cosmetics group than control group significantly. Skin moisture and elasticity assessed with skintouch system improved in cosmetics group than control group significantly while skin oiliness assessed with skintouch system have not improved significantly. Detox/regeneration questionnaire score and self-evaluating facial wrinkle/looseness score improved in cosmetics group than control group significantly. Conclusion : Korean herbal cosmetics application might be effective method for facial skin elasticity.

아토피 피부염의 中醫 外治法 考察 (A Study of external therapy of Atopic dermatitis)

  • Atopic dermatitis is frequently encountered in the Dermatology clinic and the treatments are very various but the effects are not good enough. Especially in case of infants, taking herb med or acupuncture treatments are difficult so we would like to treat the atopic dermatitis using external therapy. We chose the oriental medicine textbooks and the oriental medicine journals that external therapy was used and divided into Yugobub(油膏法), Buchubbub(敷貼法), Sechukbub(洗滌法). The results were as follows: 1. In the frequency of medicine was Coptidis Rhizoma(黃連), Phellodendri Cortex(黃栢), Sophorae Radix(苦蔘), Cnidii Fructus(蛇床子), Allumen(枯礬) etc. 2. The effects of medicine were Chungyul(淸熱), Haedok(解毒), Zosub(燥濕), salchung(殺蟲), Gupung(祛風), Jiyang(止痒) etc.

일당귀 에탄올 추출물의 Hyaluronic Acid 합성 효과에 대한 실험적 연구 (An Experimental Study on the Effect of Angelica acutiloba Ethanol Extract on Hyaluronic Acid Synthesis)

  • Objectives : Hyaluronic acid(HA) is a mucopolysaccharide, occuring naturally in living organisms. It is one of the most hydrophilic molecules, so it has been known as being related to skin hydration and anti-aging. The purpose of this study is to examine the effects of Angelica acutiloba ethanol extract on hyaluronic acid synthesis. Methods : To determine cytotoxicity and hyaluronic acid synthase 2 gene expression, hyaluronic acid production in HaCaT cells, MTT assay and RT-PCR ELISA was used. Results : There was no cytotoxicity in $50{\mu}g/ml$ concentration Angelica acutiloba extract in MTT assay. Hyaluronic acid synthase 2(HAS2) gene expression was increased by all treated concentration Angelica acutiloba extract. Hyaluronic acid production was higher in $50{\mu}g/ml$ & $100{\mu}g/ml$ concentration Angelica acutiloba extract than control group. Conclusions : Hyaluronic acid production was increased by Angelica Acutiloba extracts. Therefore, We suggest that Angelica acutiloba can make a contribution to the moisturing effect on human skin. 혈관염 및 자반증의 한의학적 치료에 대한 국내 임상 논문 분석 (Review of Korean Medicine Treatments for Purpura and Vasculitis in Korean Journals)

  • Objectives : The purpose of this study is to analyze how purpura and vasculitis have been treated with Korean medicine and potentially to present with future direction of research and treatment. Methods : We searched clinical studies from the Korean databases including Oriental medicine Advanced Searching Integrated System(OASIS), Korean Traditional Knowledge Portal(KTKP), National Discovery for Science Leader(NDSL), Research Information Sharing Service(RISS), using keywords related to "Purpura" and "Vasculitis" from January 2000 to May 2019. Results : A total of 20 studies were selected for analysis. More than half of the patients provided with detailed information were under 19 year-olds, and upper respiratory infection, stress and fatigue, seasonal factor were among the most frequently stated as predisposing factors. Among many treatment modalities, herbal medicine was the most frequently used, followed by acupuncture and herbal acupuncture. 23 basic herbal medicine formulas were retrieved from 20 articles, most frequently used being Guibi-tang(歸脾湯加味), Samul-tang(四物湯加味) and Yukmijihwang-tang(六味地黃湯加味). In total, 122 Korean medicine herbs were used, most frequently used herbs being Glycyrrhizae Radix et Rhizoma(甘草), Angelicae Gigantis Radix(當歸), Poria Sclerotium(茯?) and Paeoniae Radix(芍藥). By its category, herbs were mostly classified into Tonyfying and replenishing medicinal(補益藥), Heat-clearing medicinal(淸熱藥) and Exterior-releasing medicinal(解表藥). Patients with total treatment period of 3 months or under were 2/3 of all cases provided with detailed information. Follow up periods were short in general with only 6 cases of over 6 months. The primary motive of patients to receive Korean medicine treatments was unresponsiveness or adverse effects of western medication. Conclusion : Through this literature review, we could find out tendencies of Korean medicine treatments of purpura and vasculitis up to date and some points that may have clinical significance.
