소속 학교급과 영재교육 경험에 따른 집단적 교수효능감 및 융합교육 장애요인에 대한 교사인식 (Teacher's Perception towards Collective Teacher Efficacy and Barriers in the Integrative Education by Teaching Levels and Gifted Education Experiences)

  • 본 연구는 교사의 소속 학교급과 영재교육 경험에 따라 융합교육에 대한 집단적 교수효능감과 장애요인의 인식은 차이가 있는지 조사하였다. 241명의 초 중등교사가 집단적 교수효능감 및 융합교육의 장애요인에 대한 인식을 묻는 설문에 응답하였다. 본 연구는 소속 학교급과 영재교육 경험에 따른 집단적 교수효능감 및 융합교육 장애요인에 대한 교사인식의 차이를 조사하기 위하여 이원분산분석과 상관분석을 실시하였다. 연구결과, 소속 학교급과 영재교육 경험에 따른 집단적 교수효능감은 통계적으로 유의한 차이가 나타나지 않았다. 이에 비해 융합교육의 장애요인에 대한 교사인식은 소속 학교급 및 영재교육 경험에 따라 유의한 차이가 나타났다. 초등교사가 중등교사보다, 그리고 영재교사가 일반교사보다 융합교육을 교육현장에 적용함에 있어 그 장애요인을 인식하는 것으로 나타났다. 이에 본 연구는 융합교육의 현장정착을 위하여 소속 학교급과 영재교육 경험에 따라 고려해야 할 점과 방향성을 제안하였다.

아동간호사의 통증관리 수행에 영향을 미치는 요인 (Factors Influencing Pain Management Practice in Pediatric Nurses)

  • Purpose: The purposes of this study were to explore knowledge, barriers, and self-efficacy in relation to pain management practice, and to identify factors influencing pain management practice in pediatric nurses. Methods: A descriptive correlational study was conducted. The participants were 237 pediatric nurses from a metropolitan city. Data were analyzed using t-test or analysis of variance and Pearson correlation and multiple regression analyses. Results: The mean percentage of correct answers on the children's pain management knowledge scale was 58.8%. Child and parent related factors were the main barriers for pain management. Self-efficacy to assess children's pain across developmental stages was particularly low. Pain management practices for assessing pain and non-pharmacological interventions were relatively low. Factors significantly affecting children's pain management practice were current conditions of work department and self-efficacy in pain management, and these factors accounted for 37.5% of the variance in pain management practice. Conclusion: The results suggest that an integrative education program needs to be developed to improve self-efficacy in children's pain management practice. Moreover, good communication, building cooperative relationships with children and parents, and a more active role by pediatric nurses are required to carry out more effective pain management.

Behavioral Modification Regarding Liver Fluke and Cholangiocarcinoma with a Health Belief Model Using Integrated Learning

  • This study aimed to modify behavior regarding liver fluke and cholangiocarcinoma prevention in Chumphuang district, Nakhon Ratchasima province, Thailand through integrated learning. A total of 180 participants were included through purposive selection of high-risk scores on verbal screening. Participants attended the health education program which applied the health belief model included family based, knowledge station based, academic merit based and community based learning. Data were collected using a questionnaire composed of 4 parts: 1) personal information, 2) knowledge, 3) perceived susceptibility, severity, benefits, and barriers, 4) practice regarding liver fluke and cholangiocarcinoma prevention. The result revealed that the majority were female (79.9%), age ${\geq}60$ years old (33.2%), primary school educational level (76.1%), and agricultural occupation (70.1%). The mean scores of knowledge, perception, and practice to liver fluke and cholangiocarcinoma prevention, before participated the integrative learning were low, moderate, and low, respectively. Meanwhile, the mean score of knowledge, perceived susceptibility, severity, benefits, and barriers, and practice regarding liver fluke and cholangiocarcinoma prevention, were higher with statistical significance after participation in the integrated learning. This finding indicates that health education programs may successfully modify health behavior in the rural communities. Therefore they may useful for further work behavior modification in other epidemic areas.

보건교사의 당뇨병 아동 관리 현황 (School Nurses' Management for Children and Adolescents with Diabetes)

  • Purpose: The study purposes were to explore school nurses' experience, perceived barriers, and education needs in diabetes management at school. Methods: This study was a cross sectional study and the study participants were recruited conveniently at continuing education seminars for school nurses at Incheon Metropolitan City. Results: Data for 101 school nurses were analyzed. The nurses were all women and their mean age was $46.9{\pm}9.3$ years. About 66% of them had experience with children with diabetes at school. The school nurses reported that 74.6% of the students tested their blood glucose by themselves, the school clinic was the most common place for blood glucose tests (47.8%) and insulin injections (50.8%) and the nurses knew students' diagnosis through the student health survey (58.2%). About half of the nurses (53.7%) reported that glucagon should be available at school and 49.2% were willing to inject glucagon when necessary. The most frequently reported barrier in diabetes management was role confusion ($6.0{\pm}1.3$) and the most common educational need was emergency responses ($5.9{\pm}1.4$). Conclusions: School health policy for diabetes management and diabetes resources are necessary to minimize role confusion of school nurses, improve emergency response, and facilitate health promotion activities in diabetes management.

성인기 지적장애인과 주부양자의 특성, 사회적지지, 가족기능, 재활의 필요성 사이에 관련성 (Relationship between the Characteristics of Caregivers and Adults with Intellectual Disability and the Social Support, Family Function, and Rehabilitation Needs in Caregivers)

  • Purpose : The purpose of this study was to examine the relationship between the characteristics of caregivers and adults with intellectual disability, and social support, family function, and rehabilitation needs in caregivers. Methods : A total 98 pairs of adults with intellectual disability and their caregivers participated in this study. The researchers examined the general characteristics of the adults with intellectual disability and their caregivers. The evaluation included analysis of the level of activities of daily living, ability to communicate, and health status of the adults with intellectual disability, while the family income, health status, utility and the need for rehabilitation, social support (multidimensional scaled perceived social support, MSPSS) and family function (adaptation, partnership, growth, affection, resolve, and APGAR index) of the caregivers were measured. The data collected were analyzed to determine the relationship of the characteristics of adults with intellectual disability and the social support, family function, and rehabilitation needs of caregivers using regression and correlation analysis. Results : The rehabilitation needs were significantly correlated with the age of the adults with intellectual disability (p<.01), and the subjective health status of the caregivers (p<.05). The education level of the caregivers affected social support significantly ($R^2=.058$, p=.021). The communication ability of the adults with intellectual disability affected family function ($R^2=.071$, p=.01). The social support of caregivers had a significant effect on family function ($R^2=.488$, p<.001). Conclusion : These findings suggest that the barriers to community rehabilitation should be lowered, and the authors discussed the results of the present investigation.

소아완화의료에 대한 호스피스 완화의료 전문기관 종사자의 인식 (Palliative Care Practitioners' Perception toward Pediatric Palliative Care in the Republic of Korea)

  • 목적: 본 연구는 소아완화의료 대상 환자와 가족을 돌보는 전문기관 종사자의 환자진료 및 시스템 연계에 대한 인식을 알아보기 위해 수행된 서술적 조사 연구이다. 방법: 자료 수집은 호스피스 완화의료 전문기관(이하 전문기관)에 근로하는 전문가를 대상으로 2017년 9월부터 10월까지 진행되어 총 61건의 자료를 수거하였다. 결과: 소아완화의료를 입원형, 자문형, 가정형 중 한가지 형태로라도 제공하고 있는 기관은 11기관(18.0%)이었으며, 지역별 분포는 서울, 경기, 인천, 경상 지역에 집중되어 있어 기타 지역에서는 활용 가능한 자원이 전무한 상황이다. 개입 시 장애요인은 '훈련된 전문인력을 확보하는 것의 어려움'이며, 비암성 소아청소년 환자의 경우 '예후 및 경과 예측의 어려움 47.5% (n=28)' 으로 확인되었다. 소아완화의료 구성형태는 '성인과 다른 소아청소년의 특성, 소아청소년 전문인력 필요, 관리 및 제도보완 필요'를 이유로 응답자 중 73.8% (n=45)이 별도의 소아완화의료팀 구성이 필요하다는 의사를 밝혔다. 개입시점은 '완치가능성이 적은 소아암 진단 시'부터 진행해야 하는 것이 33.7% (n=33)로 가장 높았으나 의뢰시점은 '사망시점을 예상하기 어려우나 지속적 악화상태인 경우'가 38.2% (n=39)로 가장 높아, 전문기관으로 의뢰 전 기존 치료 병원에서 개입을 진행되는 것을 선호하였다. 응답기관 중 14.8% (n=9)만이 추후 소아완화의료 제공에 참여할 의사가 있다고 밝혔다. 결론: 2018년 두 곳으로 시작한 소아청소년 완화의료 시범사업기관에 더해 지역별로 배치되어 있는 전문기관에서 소아완화의료를 제공할 수 있도록 하여 자원의 접근성을 높일 필요가 있다. 별도 소아완화의료 구성의 필요성을 인정함에도 소아완화의료를 제공하는 것에 대한 부담감으로 실제적 서비스를 제공하는 데 어려움이 있기에 우선적으로 소아완화의료 전문가를 양성하고 교육 개발 및 의뢰 시점에 대한 논의 등을 통하여 소아완화의료 확산에 대한 현실적 논의를 진행해야 할 것이다.
