INFLUENCE OF A SODIUM HYPOCHLORITE GEL ON MICROLEAKAGE OF COMPOSITE RESIN RESTORATIONS (차아염소산 나트륨의 사용이 복합레진 수복물의 미세누출에 미치는 영향)

  • This study evaluated the influence of chemomechanical caries removal agent $Carisolv^{TM}$(MediTeam, Sweden) for composite resin adhesion to sound human permanent and primary dentin. The buccal/labial surfaces of 80 permanent molars and 80 primary incisors were used. Four types of adhesives and one composite resin were used; AQ Bond(Sun Medical, Japan), Clearfil SE Bond(Kuraray, Japan), Single Bond(3M, USA), Scotchbond Multi-Purpose(3M, USA) and Z100(3M, USA). One drop of $Carisolv^{TM}$(MediTeam, Sweden) was pretreated on the dentin for 0 second(control) and 60 seconds. The specimens were thermocycled for 1,000 times in baths kept 5 degrees C and 55 degrees C with a 30 seconds dwell time. Shear bond strengths were tested and the data was statistically analyzed using one-way ANOVA with subsequent post hoc Scheffe test at p<0.05. $Carisolv^{TM}$ treatment significantly decreased the shear bond strength. Shear bond strength of permanent dentin was significantly higher than that of primary dentin. Clearfil SE Bond treatment groups showed the highest shear bond strength and AQ Bond treatment groups showed the lowest shear bond strength.

A Comparison of Conventional Cytology and ThinPrep Cytology of Bronchial Washing Fluid in the Diagnosis of Lung Cancer (폐암의 진단 검사 중 기관지 세척액에서 ThinPrep검사법과 기존의 세포검사법의 유용성에 대한 비교)

  • Background: A ThinPrep$^{(R)}$ Processor was developed to overcome the limitations of conventional cytology and is widely used to diagnose various cancers. This study compared the diagnostic efficacy of conventional cytology for lung cancer with that of the ThinPrep$^{(R)}$ cytology using the bronchial washing fluid. Methods: The bronchial washing fluid of 790 patients from Jan. 2002 to Dec. 2006, who were suspected of gaving a lung malignancy, was evaluated. Both ThinPrep$^{(R)}$ and conventional cytology were performed for all specimens. Result: Four hundred forty-six men and 344 women were enrolled in this study, and 197 of them were diagnosed with cancer from either a bronchoscopic biopsy or a percutaneous needle aspiration biopsy. ThinPrep$^{(R)}$ cytology showed a sensitivity, specificity, positive predictive value, negative predictive value and false negative error rate of 71.1%, 98.0%, 92.1%, 91.1%, 8.9%, respectively. The conventional cytology showed sensitivity, specificity, positive predictive value, nagative predictive value and false negative error rate of 57.9%, 98.0%, 90.5%, 87.5%, 12.5%, respectively. For central lesions, the sensitivity of conventional cytology and ThinPrep$^{(R)}$ were 70.1% and 82.8%, respectively. Conclusion: ThinPrep$^{(R)}$ cytology showed a higher sensitivity and lower false negative error rate than conventional cytology. This result was unaffected by the histological classification of lung cancer. Therefore, ThinPrep$^{(R)}$ cytology appears to be a useful method for increasing the detection rate of lung cancer in bronchial washing cytology test.

Surface characteristics and stability of implants treated with alkali and heat (알칼리와 열처리에 의한 임플란트의 표면 특성 및 골유착 안정성에 관한 연구)

  • Statement of problem: Bioactive materials must have the ability to spontaneously form a bone like apatite layer on their surface and induce direct biochemical bonding to bone. A simple chemical treatment via alkali and heat has been revealed to induce bioactivity in titanium. Purpose: The purpose of this study was to evaluate the surface characteristics and stability of alkali and heat treated implants. Material and methods: Specimens were divided into three groups; group 1 was the control group with machined surface implants, groups 2 and 3 were treated with alkali solutions and heat treated in the atmosphere and vacuum conditions respectively. The surface characteristics were observed with FESEM, XPS, TF-XRD and AFM. Stability was evaluated with the resonance frequency analysis, periotest and removal torque values. One-way ANOVA and Duncan test were used for statistical analysis. Results: 1. Groups treated with alkali and heat showed similar characteristics. Groups 2 and 3 showed high compositions of Na ions on the surface with sub-micron sized pores compared to group 1. Group 2 showed mixed compositions of anatase and rutile with superior contents of rutile. 2. Resonance frequency analysis : The ISQ of group 2 showed significantly higher values than that of groups 1 and 3 at 12 weeks. The ISQ of groups 1 and 2 showed significant increase after 4 weeks, and the ISQ of group 3 increased significantly after 2 and 4 weeks respectively (P < .05). 3. Periotest: The PTV of groups 1 and 2 showed significant decrease after 4 weeks, and the PTV of group 3 showed significant decrease after 2 and 4 weeks respectively (P < .05). 4. Removal torque analysis: The removal torque value of group 2 was significantly higher than those of groups 1 and 3 at 2, 4 and 8 weeks. The removal torque values of groups 1 and 3 showed increase at 4 and 12 weeks, but the removal torque value of group 2 showed increase after 4 weeks (P < .05). Conclusion: An oxide layer with appropriate crystal structure and amorphous sodium titanate layer can be obtained on titanium implants through alkali and heat treatment in the atmosphere, and even alkali and heat treatment in vacuum conditions, provided a bioactive surface containing sodium. These surface layers can be considered to be effective for enhancement of osseointegration and reduction of healing period for implant treatment.

Effects of activators contained in adhesives on dentin bond strengths (상아질 접착제에 포함된 활성제의 사용 유무가 자가중합 복합레진의 상아질에 대한 전단결합강도에 미치는 영향)

  • Statement of problem: An incompatibility between the initiator systems of self-curing composite resins and light-curing adhesives was supposed recently. Purpose: The purpose of the study was to evaluate the influence of activators for self/dual bonding on dentin shear bonding strengths. Material and methods: Fifty human molars were divided into 5 groups. A flat dentin surface was created for each tooth. A self-curing composite resin (Luxacore) was bonded with the following adhesives (n = 10); One-Step, Prime&Bond NT, AdheSE, Prime&Bond NT and AdheSE were also used in combination with activators. Shear bond strengths were measured after 24 hours of water storage. The specimens were loaded in shear in the Instron until failure at a 1 mm/min crosshead speed. Data were compared using one-way ANOVA and Tukey HSD test (${\alpha}$= 0.05). Results: The dentin adhesive systems in order of decreasing median bond strength were One-Step > Prime&Bond NT with activator, AdheSE with activator > Prime&Bond NT, AdheSE. Among adhesives, One-Step had the highest bond strength. Prime&Bond NT with activator had higher bond strengths than Prime&Bond NT that was used alone, and so was AdheSE. Conclusion: Shear bond strengths were increased in Prime&Bond NT and AdheSE when these were used with activators comparing used without activators. But using activators was not effective clinically comparing One-Step.

Study of Anti-oxidant Analysis to Vegetable Juice Containing Barley Sprouts (보리새싹 함유 녹즙의 항산화력 분석에 관한 연구)

  • This study was conducted to analyze the anti-oxidant activity of green juice containing barley sprouts. The specimens used in the experiments were supplied byCSJ company located in Jincheon-gun, Chungbuk province. Polyphenol and flavonoid was detected in Huttuynia cordata Thunb fermentation sprout juiceat $1.075{\pm}0.174mg/mL$ and $2.736{\pm}0.187\;quercetin\;mg/mL$, in organic dandelion at $1.048{\pm}0.173mg/mL$ and $2.685{\pm}0.245\;quercetin\;mg/mL$, kan andRakat $1.013{\pm}0.083mg/mL$ and $3.624{\pm}0.237\;quercetin\;mg/mL$, broccoli blending juice at $1.494{\pm}0.255mg/mL$ and $3.826{\pm}0.286\;quercetin\;mg/mL$, and barley sprouts at $1.043{\pm}0.220mg/mL$ and $1.990{\pm}0.223\;quercetin\;mg/mL$, respectively.Antioxidant activity testing revealed the electron donating ability of Huttuynia cordata Thunb fermentation sprout juice for DPPH was 81.97%. Moreover, the free radical scavenging ability measured by ABTS assay showed that the activity was increased in the order of broccoli blending juice, Huttuynia cordata Thunb fermentation sprout juice, organic dandelion, 'kan andRak', barley sprout, and thatthe effects were similar to those of ascorbic acid at $100{\mu}g/mL$. Statistical processing is using SPSS 24, analysis of distributed layout and Duncan's multiple range (p<0.05). Therefore, the use of functional assistance material of products in the future indicates that it is worth a healthy functional drink.

Adenoviral Lower Respiratory Tract Infections in Children; Serotypes and Clinical Characteristics (소아의 아데노바이러스 하기도 감염증: 혈청형 및 임상상)

  • Purpose : This study was performed to characterize the epidemiologic and clinical features of acute adenoviral lower respiratory tract infections(LRTIs). Methods : Virological analysis was done from respiratory specimens obtained from patients with LRTIs hospitalized to other hospitals and referred to the Department of Pediatrics, Seoul National University Children's Hospital(SNUCH) from June 1998 to July 2000. Viral diagnosis was made by isolation of viruses employing HEp-2 cell culture and indirect immunofluorescent staining with monoclonal antibodies. Serotypes of adenoviruses were determined by neutralization test using antiserum for types 1, 2, 3, 4, 5, 6, 7 and 11. Medical records of children admitted to the SNUCH were reviewed retrospectively. Results : Adenovirus was isolated from 118(9.0%) of 1,305 children with LRTIs. Serotypes were 3(39.0%), 7(16.9%), 1(11.0%), 2(7.6%), 4(7.6%), 6(5.9%), 11(2.5%), and 5(0.8%) and 10 strains(8.5%) were not neutralized by antisera included in the study. Infections by type 3 and type 7 occurred in outbreaks. Male to female ratio was 1.0:0.9 and mean age was 1.95 years. The clinical diagnoses were pneumonia(83%), acute tracheobronchitis(12%) and bronchiolitis(5%). Associated symptoms, signs and abnormal laboratory findings included cough(100%), sputum(73.5%), fever(54.2%), rale(59.3%), wheezing(34%), anemia(35%) and leukopenia(15.8%). Mortality was in 13.5%. Residual radiologic sequelae was identified in 32.6% of the patients followed. Conclusion : These data confirms that adenovirus may cause severe lower respiratory tract diseases, and infections by type 3 and 7 may occured in outbreaks.

Morphometric Study on the Arterial Palmar Arch of the Hand (손바닥 동맥활에 관한 형태계측 연구)

  • Purpose: Deviations of arterial palmar arches in the hand can be explained on the embryological basis. The purpose of this study was to provide new information about palmar arches through cadaver's dissection. The values of the location and diameter in these vessels were analyzed in order to support anatomical research and clinical correlation in the hand. Methods: The present report is based on an analysis of dissections of fifty - three hands carried out in the laboratory of gross anatomy. A reference line was established on the distal wrist crease to serve as the X coordinate and a perpendicular line drawn through the midpoint between middle and ring fingers, which served as the Y coordinate. The coordinates of the x and y values were measured by a digimatic caliper, and statistically analyzed with Student's t - test. Results: Complete superficial palmar archs were seen in 96.2 % of specimens. In the most common type of males, the superficial arch was formed only by the ulnar artery. In the most common type of females, the superficial arch was formed anastomosis between the radial artery and the ulnar artery. The average length of the superficial and deep palmar arch is $110.3{\pm}33.0mm$ and $67.9{\pm}14.0mm$ respectively. Regarding the superficial palmar arch, ulnar artery starts $-16.1{\pm}5.1mm$ on X - line, and $2.5{\pm}24.5mm$ on Y - line. Radial artery appears on palmar side $7.7{\pm}3.2mm$ on X - line, and $20.9{\pm}10.9mm$ on Y - line. But radial artery starts on $6.3{\pm}3.6mm$ on X - line, and $3.4{\pm}5.1mm$ on Y - line. Digital arteries of superficial palmar arch starts on $6.1{\pm}3.7mm$, $33.9{\pm}8.8mm$ on index finger, $1.8{\pm}3.4mm$, $40.1{\pm}7.3mm$ on middle finger, $-3.2{\pm}4.9mm$, $42.6{\pm}7.0mm$ on ring finger, and $-8.9{\pm}5.1mm$, $42.5{\pm}80mm$ on little finger in respective X and Y coordinates. Radial artery of deep palmar arches measured at the palmar side perforating from the dorsum of hand. It's coordinates were $9.7{\pm}4.8mm$ on X - line, $21.7{\pm}10.2mm$ on Y - line. Ulnar artery was measured at hypothenar area, and it's coordinates were $-20.4{\pm}6.3mm$ on X - line, and $30.6{\pm}7.4mm$ on Y - line. Conclusions: Anatomically superficial palmar arch can be divided into a complete and an incomplete type. Each of them can be subdivided into 4 types. The deep palmar arch is less variable than the superficial palmar arch. We believe these values of the study will be used for the vascular surgery of the hand using the endoscope and robot in the future.

Studies on the Effect of Water Content, Curing Temperature and Grain Size Distribution of Soils on Unconfined Compressive Strength of Soil-Cement Mixtures. (함수비, 양생온도 및 흙의 입도가 Soil-Cement의 압축강도에 미치는 영향에 관한 연구(I))

  • In order to investigate the effect of the water content and the accelerated curing on the strength of the soil-cement mixtures, laboratory test of soil cement mixtures was performed at five levels of water content, four levels of accelerated curing temperatures, three levels of normal curing periods, and six levels of accelerated curing time. Also this study was carried out to investigate the effect of grain size distribution of 21 types of soils on the strength of soil-cement mixtures at four levels of cement content and three levels of curing time. The results are summarized as follows: 1. Optimum moisture content increased with increase of the cement content, but maximum dry density was changed ununiformly with cement content. Water content corresponding to the maximum strength was a little higher than the optimum moisture content along the increase of cement content. 2. In molding the specimens with the optimum moisture content, the maximum strength appeared at the wet side of the optimum moisture content. 3. According to increase of curing temperature as 30, 40, 50, and 60$^{\circ}C$, unconiiend compressive strength of soil-cement mixtures increased, the rate of increase at the early curing period was large, and approximately 120 hours was suifficient to harden soil-cement mixtures completely. 4. The strength of soil-cement mixtures at the curing temperature of 10$^{\circ}C$ decreased at the rate of 30 to 50 percent than at the curing temperature of 20$^{\circ}C$, and the strength of soil-cement mixtures at the curing temperature of 0$^{\circ}C$ increased a little with increase of curing time. 5. Although the strength of soil-cement mixtures seemed to be a little affected by the temperature difference between day time and night, it was recommended that reasonable working period was the duration from July to August of which average maximum temperature of Korea was approximately 30$^{\circ}C$. 6. Accelerated curing time corresponding to the normal curing time of 28-day was shorten with increase of curing temperature, also it was a little affected by the cement. Accelerated curing time that the strength of soil-cement mixtures for the cement of 9 percent and the curing temperature of 60was shorten with increase of curing temperature, also it was a little affected by the cement. Accelerated curing time that the strength of soil-cement mix- tures for the cement of 9 percent and the curing temperature of 60$^{\circ}C$ was 45 hours at the KY sample, 50 hours at the MH, 40 hours at the SS, and 34 hours at the JJ respectively. 7. Accelerated curing time was depended upon the grain size distribution of soil, it decreased with increase the percent passing of No. 200 sieve. 8. Relationship between the normal curing times and the accelerated curing times showed that there was a linear relationship between them, its slope decreased with increase of curing temperature. 9. The most reasonable soil of the soil-cement mixtures was the sandy loam which was a well graded soil. Assuming the base of road requiring 7-day strength of 21 kg/$\textrm{cm}^2$ being used, the soil-cement mixtures could be obtained with adding 6 percent of cement in such a sails S-7, S-8, S-9, S-10, S-11, S-12, S-13. 10. The regression equation between the 28-day and the 7-day strength was obtained as follow; q28=1.12q7,+6.5(r=0.96).

Efficiency Test for Surface Protecting Agents for the Chemical Resistance of Concrete Structures Using Sulfur Polymers (Sulfur Polymer를 사용한 콘크리트 구조물용 내화학성 표면보호재의 성능 평가)

  • Structures requiring chemical resistance are usually coated with surface protecting agents, but the cost for maintenance and re-construction is incurred due to the low durability. Therefore, in this study, sulfur was polymerized and the performance was examined so that it could be used as the concrete surface protecting agents for structures requiring chemical resistance. The evaluation results indicated that for the spray of the sulfur polymer surface coating agents, the application of the gravity type was appropriate; and for the number of coating times, about 3 cycle spray gave the best results. For the surface condition of the concrete to be coated with the surface protecting agents, outstanding quality was obtained above room temperature ($20{\sim}30^{\circ}C$), and the bond strength increased as the temperature increased. The evaluation results of the strength characteristics depending on the filler content of the surface protecting agents indicated that about 20~40% filler mixing contributed to the strength improvement as it reduced the shrinkage of the sulfur polymer. Also, the mixing of silica showed larger increase in the bond strength than the mixing of fly ash, and the most outstanding bond strength characteristics could be obtained by the mixing of both silica and fly ash. In the case of the chemical resistance, the strength reduction was minimized and outstanding chemical resistance was obtained when the fly ash and silica were substituted by 20%, respectively. The performance evaluation of the chloride ion penetration indicated that for the specimens coated with the sulfur polymer surface protecting agents, the chloride ion penetration resistance increased by 29~48% compared to the specimen without the coating of the surface protecting agent. The examination of the coating condition of the surface protecting agents, compressive strength, bond strength, chemical resistance, and salt damage resistance indicated that in the range of this study, the optimal level was when the silica and fly ash were substituted by 20%, respectively, as the filler for the sulfur polymer.

Immunohistochemical Analysis for the Expression of p27, CDK4, and Uteroglobin in Resected Non-small Cell Lung Cancer (수술로 절제된 비소세포폐암에서 세포주기와 관련된 p27, CDK4, Uteroglobin의 발현에 관한 면역조직학적 분석)

  • Background : It has been reported that the expression of protein which influences on the cell cycle is significantly involved in the development, progress, treatment response, and survival of cancer, and also that the degree of expression of p27 and CDK4 is related to the prognosis. Recent research has revealed that uteroglobin, tumor suppressor gene, is related to cell cycle. This study is focused on the relations between expression of proteins related to cell cycle and clinical index of and survival of NSCLC. Methods : We examined immunohistochemically specimens of 110 surgically resected NSCLCs for expression of p27, CDK, Uteroglobin. Tissue array slide were obtained from 110 surgically resected NSCLCs. Immunohistochemical staining was performed by immuno-peroxidase technique using avidin-biotinylated horseradish peroxidase complex. Results : In 110 patients with resected NSCLCs, the ratio of male to female was 87:13, the median age was $56.43{\pm}9.41$ yrs. The positive staining of p27 was detected in 75% of the cases. A non-statistically significant trend toward increased p27 expression was observed in smoker and squamous cell cancer. The positive staining of CDK4 was detected in 89%, which was the highest expression of protein among 3 types. The survival ratio of CDK4 negative staining group was higher than that of positive staining group, which was significant diffrernce(P<0.05). There was no association between p27 or uteroglobin expression and survival. Conclusion : The expression degree of CDK4 is related to the prognosis. This findings suggests that the measurement of CDK4 may be useful in identifying patient at high risk for disease recurrence and survival.
