설 함요가 긴장성 두통 환자의 통증 양상에 미치는 영향 (Effect on the Pain Characteristics of Tension-Type Headache by the Tongue Ridge)

  • 이갈이나 이악물기 등과 같은 비정상적인 구강기능장애의 주된 임상 소견으로 흔히 관찰되는 설 함요가 긴장성 두통의 통증양상에 어떠한 영향을 미치는가를 관찰하고자 국제두통학회의 진단기준에 부합되는 긴장성 두통이 있는 환자 중 설함요가 있는 65명을 실험군으로 하고, 설 함요가 없는 환자 65명을 대조군으로 하여 통증의 질과 강도, 양측성 유무 그리고 일상생활에서의 증감 유무 등을 관찰하고 통계 처리하여 다음과 같은 결과를 얻었다. 1. 긴장성 두통환자의 통증의 질은 설 함요의 유무에 따라서 통계학적인 차이가 있었다(p=0.049). 2. 긴장성 두통환자의 통증의 강도는 설 함요의 유무에 따라서 통계학적인 차이가 있었다(p=0.010). 3. 긴장성 두통환자의 통증의 양측성 유무는 설 함요의 유무에 따라서 통계학적인 차이가 없었다. 4. 긴장성 두통환자의 통증의 일상생활에서의 증감 유무는 설 함요의 유무에 따라서 통계학적인 차이가 없었다. 이상의 결과로 설 함요가 있는 긴장성 두통 환자는 설 함요가 없는 긴장성 두통 환자에 비해서 통증의 예통과 심도통이 더 심하기 때문에 임상적으로 진료에 임 할 때 고려해야 하며, 이에 따른 추가적이고 지속적인 연구가 필요하리라고 생각된다.

  • 이갈이나 이악물기 등과 같은 비정상적인 구강기능장애의 주된 임상 소견으로 흔히 관찰되는 구강 협점막 백선이 긴장성두통의 통증양상에 어떠한 영향을 미치는가를 관찰하고자 국제두통학회의 진단기준에 부합되는 긴장성 두통이 있는 환자중 구강 협점막 백선이 있는 79명을 실험군으로 하고, 구강 협점막 백선이 없는 환자 79명을 대조군으로 하여 통증의 질과 강도, 양측성 유무 그리고 일상생활에서의 증감 유무 등을 관찰하고 통계 처리하여 다음과 같은 결과를 얻었다. 1. 긴장성 두통환자의 통증의 질은 구강 협점막 백선의 유무에 따라서 통계학적인 차이가 없었다. 2. 긴장성 두통환자의 통증의 강도는 구강 협점막 백선의 유무에 따라서 통계학적인 차이가 있었다(p= .043). 3. 긴장성 두통환자의 통증의 양측성 유무는 구강 협점막 백선의 유무에 따라서 통계학적인 차이가 없었다. 4. 긴장성 두통환자의 통증의 일상생활에서의 증감 유무는 구강 협점막 백선의 유무에 따라서 통계학적인 차이가 없었다. 이상의 결과로 구강 협점막 백선이 있는 긴장성 두통 환자는 구강 협점막 백선이 없는 긴장성 두통 환자에 비해서 통증의 강도가 더 심하기 때문에 임상적으로 진료에 임 할 때 고려해야 하며, 이에 따른 추가적이고 지속적인 연구가 필요하리라고 생각된다.

  • Objective: Tension Type Headache is the most common primary headache, and the prevalence is highest among people in their thirties. The 2009 domestic headache epidemiological survey indicated a prevalence rate of 30.8% per year. The patients often complain of feeling "as if they are tightening their head or wearing a band." No other symptoms, such as nausea and vomiting, accompany the headache, and the physical and neurological findings are normal. Korean medicine treatment of tension type headache is mainly with acupuncture, not herbal medicine. In the present study, a patient with tension type headache was admitted to the Korean medicine hospital for treatment. We report on the patient's progress and the effects of the herbal treatment. Methods: The patient was treated with Gaegyeolseogyeong-tang, acupuncture, herbal acupuncture therapy, and physical therapy. We used the Korean Headache Impact Test-6 and Numeric Rating Scale to assess changes in tension type headache symptoms. Results: The patient was diagnosed with Qi-stagnation syndrome (氣滯證) and Ganyangsanghang-headache (肝陽上亢頭痛). The patient was hospitalized for 42 days and showed recovery from the tension type headache without any adverse events. Conclusion: Patients with tension type headache could be treated with Korean medicine.

The relationship between primary headache and constipation in children and adolescents

  • Purpose: Many patients presenting with headache also complain of constipation; the relationship between these two symptoms has not been explored in detail. The aim of this study was to investigate the association between primary headache and constipation. Methods: This retrospective study included all children who attended the Inje University Sanggye Paik Hospital complaining of headache, and who had been followed up for at least 100 days. Patients were divided into 2 groups: group A, in whom the headache improved after treatment for constipation, and group B, in whom headache was not associated with constipation. Results: Of the 96 patients with primary headache, 24 (25.0%) also had constipation (group A). All 24 received treatment for constipation. Follow-up revealed an improvement in both headache and constipation in all patients. Group B contained the remaining 72 children. Comparison of groups A and B indicated a significant difference in sex ratio (P=0.009, chi-square test). Patients with probable tension-type headache were more likely to be in Group A (P=0.006, chi-square test). Conclusion: Resolution of constipation improves headache in many patients diagnosed with primary headache, especially those with probable tension-type headache. We suggest that either constipation plays a key role in triggering headache, or that both constipation and headache share a common pathophysiology.

슬링 운동 시 착용한 목보조기가 긴장성 두통을 가진 전방 머리 자세가 있는 성인의 근긴장도와 두통에 미치는 영향: 무작위 대조 예비연구 (Effect of Sling Exercise Wearing a Neck Orthosis on Muscle Tension and Headache in Adults with Forward Head Posture and Tension Headache: A Randomized, Controlled, Preliminary Study)

  • PURPOSE: This study was conducted to investigate the effect of the sling exercise wearing a neck orthosis on the craniovertebral angle, muscle tension, and headaches in adults with a forward head posture and tension headache. METHODS: In this single-blinded, randomized, controlled, comparative study, a total of 22 adults with forward head postures and tension headaches were randomly assigned to the experimental group (sling exercise wearing a neck orthosis, n = 11) or the control group (sling exercise without a neck orthosis, n = 11). All participants undertook the sling exercise program (3×/week for 4 weeks). The craniovertebral angle, muscle tension, and headache were measured before and after the 4-week training. RESULTS: Significant improvements were observed in the craniovertebral angle, muscle tension, and headache in the experimental group (p < .05). This group also showed a larger decrease in the muscle tension and headache (upper trapezius, -4.97 Hz vs -1.70 Hz, p < .05; splenius capitis, -5.44 Hz vs -2.54 Hz, p < .05; headache, -19.73 score vs -14.64 score, p < .05, respectively). CONCLUSION: The sling exercise wearing a neck orthosis could be an effective way to relieve the symptoms caused by a forward head posture. It could also be a more effective way of decreasing muscle tension and headaches than the sling exercise without wearing a neck orthosis.

Mulligan 기법이 긴장성 두통에 미치는 영향 (The Effect of Mulligan Technique Affecting for Tension Headache)

  • Objective : To report the positive effect of Mulligan technique affecting for Tension headache without using the medicinal substances. Methods : Among the students with Tension headache those under and over twenty years of age, and selected twenty students with more longer and severe pain from 170 students in March 2, 2007. Experiment was enforced for twenty minute in a day and two times in a week from second week of April to after four weeks. Results : The Results of this study were summarized by six subjects ; 1. Visual analog scale was positively decreased from 3.05 to 0.75. 2. Brief Pain Inventory on task was positively decreased from 2.90 to 1.10. 3. Brief Pain Inventory of sentiment was positively decreased from 3.90 to 0.75. 4. Brief Pain Inventory on leisure hours was positively decreased from 2.60 to 0.7. 5. Brief Pain Inventory in personal relationship was positively decreased from 2.45 to 0.5. 6. Brief Pain Inventory while sleeping was positively decreased from 2.15 to 0.4. Conclusion : Mulligan technique can affecting the positive effects for those who has Tension headache. It is especially reduces pain from Tension headache.

만성두통환자 치료에 통증유발점 치료 및 제 2 경추신경절 차단술의 효과 (The Effect of Trigger Point Injection and $C_2$-ganglion Block for the Patients with Chronic Headache)

  • Headache is a common disease of the general population. But the main problem in any study of headache has been that of defining the disease entities. In 1988, the Headache Classification committee of the International Headache Society introduced operational diagnostic criteria for all headache disorders into 13 major group; migraine, tension-type headache, cluster headache and chronic paroxysmal hemicrania etc. Sjaastad was the first to describe "cervicogenic headache", one of various head pain syndromes that probably originate in the cervical spine. Between March 1995 and June 1995, we studied 78 out-patients of the Department of Neuro pain clinic, Sanggye Paik Hospital, Inje university. We divided the patients into three study group: Fifty-three patients with tension-type headache, 13 with cervicogenic headache, and 12 with migraine headache. The reponse of trigger point injection and $C_2$-ganglion block in patients was investigated. We paid particular attention to the response of trigger point injection in patients of the three group. The effect of trigger point injection was more marked in tension-type headache group than in the other categories. The pain reduction after $C_2$-ganglion block was more marked in cervicogenic headache group than in the others.

Effects of relaxation approach with self-exercise on head posture, static postural stability, and headache in persons with tension-type headache

  • Objective: Tension-type headache is caused by hormones, foods, irritants, stress, obesity, fatigue, and neck and head trigger points-prolonged abnormal posture. The purpose of this study was to evaluate the effects of relaxation approach on head posture, static postural stability, and headache in persons with tension-type headache. Design: Randomized controlled trial. Methods: Thirty-five persons with tension-type headache participated in this study. This study was a pretest-posttest with a control group design for a duration of 4 weeks (60 min/3 times/1 wk). The participants were randomly allocated to the relaxation approach group (n=18) and the control group with conventional rehabilitation including thermotherapy and transcutaneous electrical stimulation for the same period (n=17). Outcome measures involved forward head posture (FHP), foot pressure, neck disability index (NDI), and six-item headache impact test (HIT-6). Results: Relaxation approach and control groups improved significantly in the amount of forward head posture, neck disability index, and six-item headache impact test scores after training (p<0.05). The control group was found to be significantly different in the amount of FHP, backward foot pressure, NDI, and HIT-6 after training compared to before training (p<0.05). The relaxation approach group significantly improved in forward head posture, neck disability index, and six-item headache impact test compared with control group after training (p<0.05). Neck disability index and six-item headache impact test significantly improved after training compared with before training in the control group (p<0.05). However, the foot pressure was not significantly different between relaxation approach and control groups. Conclusions: This study suggests that treatment with relaxation approach combined with self-exercise would be effective in reducing the amount of forward head posture, neck disability and headache impacts.

긴장성 두통환자에 대한 관절가동기법이 통증회복에 미치는 영향 (The Effect of Joint Mobilization Therapy on Pain Recovery for patients with Tension-type Headache)

  • Purpose: The purpose of this study was to investigate the effects of cervical mobilization on the tension type headache recovery. : The subjects were consisted of 70 patients with tension type headache. All subjects randomly assigned to cervical mobilization group and massage group. The mobilization group received cervical mobilization with modality treatment and massage group received cervical massage with modality treatment. Visual analogue scala(VAS) was used to daily headache hours and patient's pain level. Results: The results of this study were summarized as follows: 1. The Visual Analogue Scale(VAS) was mobilization group showed significant1y decreased more than massage group(p<.01). 2. The daily headache was mobilization group showed significantly decreased more than massage group(p<.01). Conclusion: mobilization is beneficial treatment for tension type headache.

긴장성 두통에 대한 추나요법 치험 3례 (3 Case Reports on Tension-type Headache Treated by Chuna manipulative Therapy)

  • Objectives: The Purpose of this study is to report three cases on tension-type headache treated by Chuna manipulative therapy. Methods: Three patients diagnosed tension-type headache were treated with Chuna manipulative therapy only. To evaluate effect of treatment, visual analogue scale(VAS) and headache impact test(HIT) were used. We checked VAS and HIT score three times(pretreatment, post-treatment, follow-up). An interval among each evaluation time is 4 weeks. Results: At post-treatment evaluation, VAS and HIT score declined significantly compared with pre-treatment. At follow-up evaluation, the pain did not increased. Conclusions: These results suggests that Chuna manipulative therapy may be effective for tension-type headache.
