Flexor Tendon Rupture Secondary to Gout

  • Extra-articular deposition of monosodium urate crystals is a widely recognized manifestation of gout. However, gouty infiltration of flexor tendons in the hand resulting in tendon rupture is exceedingly rare. This case report highlights a patient with gouty infiltration of flexor tendons in the right middle finger resulting in rupture of both the flexor digitorum profundus and flexor digitorum superficialis. Given the extent of gouty infiltration and need for pulley reconstruction, the patient was treated with two-stage flexor tendon reconstruction. Febuxostat was prescribed preoperatively to limit further deposition of monosodium urate crystals and continued postoperatively to maximize the potential for long-lasting results. Prednisone was prescribed between the first- and second-stage operations to prevent a gout flare while the silicone rod was in place. In summary, tendon rupture secondary to gouty infiltration is the most likely diagnosis in patients with a history of gout presenting with tendon insufficiency.

  • A significant injury of ligament or tendon that might result in hypermobility and/or instability has been termed connective tissue insufficiency (CTI). Load bearing on CTI can sometimes create a malfunction of the mechanoreceptor and this in turn can aggravate the problem thus creating a chronic condition. Prolotherapy means the injection of growth factors or growth factor stimulators into the fibroosseous junction of the disabled ligament, tendon or cartilages in order to strengthen and promote proliferation of the injured Connective tissue. In the case of frequent recurrence or unresponsiveness to conservative treatment including nerve blocks in the chronic musculoskeletal disorder. We obtained good results after the prolotherapy treatment. This study will report on 5 cases of prolotherapy treatment.

Analysis of Digital Achilles Tendon of Scholarly Journals (학술지의 디지털 아킬레스건 분석)

  • Nowadays many academic libraries have considered and some have implemented the just-in-time(access) approach to collection development especially of scholarly journals. The adaption of the scholarly electronic journal based on access paradigm has brought a number of advantages and disadvantages to academic libraries. Electronic journals have promised rapid delivery of the latest research to the desktop through the ubiquitous platform of the Internet. But the promise of the e-journal is not yet a reality, and there are many misconceptions about what is actually available today. The objective of this paper is to analyse the probabilities of digital achilles tendon of scholarly journals resulting from a decline of purchase power, absence of selection(evaluation) criteria, superfluities and abuses of impact factor, access paradigm and halo effects, mismatching of demand-supply, md insufficiency in subject gateway functions.

Restylane Injection into the Vocal Cord of the Patient with Unilateral Vocal Cord Paralysis -A Case Report- (편측 성대마비에서 Restylane을 이용한 성대내 주입치료 1예 -증 례 보 고-)

  • When a person was suffered from vocal cord paralysis or glottic insufficiency, injection materials (e.g Teflon, Bovine collagen, Autologous fat & tendon, Gelfoam) into the vocal cord have been widely used. But each injection material has some disadvantage. We introduce the Restylane which is composed of a hyaluronic acid, artificially producted. It has advantage of rate foreign body reaction, proper endurance, easy to injection. The patient was 55-year-old woman who showed left vacal cord paralysis after pneumonectomy due to aspergillosis, taken the type I thyroplasty and arytenoid adduction. The middle portion of left vocal cord has some atropic mucosal change, slight chink was noted. The restylane injection into vocal cord was done with suspension laryngoscopy under general anesthesia. In the 3 month follow-up after Restylane injection, the quality of voice has been better progressively. We report a case of Restylane injection as a new method for the improvement of quality of voice.

Autograft versus Allograft for Anterior Cruciate Ligament Reconstruction - The clinical analysis of patellar tendon autografts compared with allografts - (자가 슬개건과 동종 슬개건을 이용한 전방 십자 인대 재건술의 비교)

  • Purpose : The purpose of this study was to compare the efficacy of bone-patellar tendon-bone(B-PTB) allograft with autograft B-PTB reconstruction of anterior cruciate ligament(ACL) according to the subjective and objective criteria. Materials and Methods : 42 patients were treated for acute rupture or chronic insufficiency of the ACL between March 1993 and June 1996. There were 18 autografts and 24 allografts for ACL reconstruction. At 2 years of follow-up after operation, autograft and allograft groups were compared based on subjective, objective criteria and Telos stress arthrometer. Results : The modified Feagin Scoring System revealed 16 patients$(88.9\%)$ with a satisfactory result by autografts, but 21 patients$(87.5\%)$ with a satisfactory result by allografts after 2-years follow-lip. There was no statistically significant difference between the two groups. ACL reconstruction with B-PTB allograft did not produce a significant functional deficit. But patellofemoral pain and crepitus were more frequent in the autografts$(33.3\%)$ than allografts$(8.3\%)$ (p<0.05). Conclusion : The ACL deficient knees treated with allografts for ACL reconstruction tended to be better than those reconstructed with autografts fur the reduction of patellofemoral crepitus and pain. B-PTB allograft provides an acceptable alternative to autograft tissue for reconstruction of the ACL.

The Patients' Satisfaction Degree to Arthroscopic ACL Reconstruction (관절경하 전방십자인대 재건술에 대한 환자의 만족도)

  • The arthroscopic ACL reconstruction using the bone-patellar tendon-bone has been considered as the gold standard in the treatment of the ACL insufficiency. There are many reports about the good functional outcome and the restored stability of the knee joint. But there are a few reports showing whether this surgery has good results in the patients' satisfaction. The purpose of this study is to evaluate the patients' satisfaction in this surgery. Authors analysed 32 cases who had taken the ACL reconstruction from Jannuary, 1989 to June, 1997. The average follow-up was 40 months. At the last follow-up, 32 patients reported the patients' satisfaction degrees(four degrees; very satisfied, generally satisfied, improved, no change or aggravated), KT-1000 arthrometer test, Vastus atrophy(thigh circumference), and three kinds of the knee scores(Lysholm, HSS, IKDC). Among the 32 cases, 12 cases (37.5%) were very satisfied, 17 cases(53.1%) were generally satisfied and 3 cases(9.4%) were improved. No case showed no change or aggravated. The satisfaction degree in this surgery had little relation with the age, operation time, follow-up period, combination of meniscectomy, but had significant relations with anteroposterior stability evaluated by KT-1000, thigh circumference measurements and the functional scoring of the knee(Lysholm, HSS, IKDC scores).
