Patch Augmentation for Massive Rotator Cuff Tears

  • Rotator cuff tears commonly affect the shoulder joints. Despite developments in surgical techniques and instrumentation, the treatment of massive rotator cuff tears remains challenging. The problems associated with rotator cuff repairs, such as inferior mechanical properties and high retear rates are yet to be solved. Recently, patch augmentation has been suggested as an alternative treatment because it can reinforce mechanical properties at the initial stage of healing and reduce gap formation. The purpose of this article was to comprehensively summarize the concepts and the consensus surrounding patch augmentation and evaluate the clinical and anatomical outcomes after patch augmentation for massive rotator cuff tears.

  • The goal of surgical correction for hallux valgus is to achieve a painless, shoe-wearable, and relatively straight toe with a balanced joint motion that results in aesthetically and functionally satisfactory toe. To date, there has not been a consensus on the ultimate surgical procedure for hallux valgus correction. Unfortunately, such a consensus may be difficult since it is not uncommon to encounter complications after hallux valgus correction. Postoperative soft tissue complications include difficult wound healing, infection, hypertrophy, or pain of the scar, joint stiffness, and tendon or sensory nerve damage. Postoperative bony complications include malunion, nonunion, failure of fixation, failure of angle correction, recurred deformity, osteomyelitis, and failure of balance between the metatarsal heads. Herein, we review common complications after surgical correction of hallux valgus, such as stiff joint, bony complications, recurrence of the deformity, and hallux varus.

전기자극과 vitamin AD3E가 랫드 골절치유에 미치는 영향 (Effect of electrical stimulation and vitamin AD3E on fracture healing in a rat model)

  • Electrical stimulation and vitamin $AD_3E$ administration have been shown to enhance the repair of biological tissues such as bone, ligament and tendon, The objective of this study were (a) to investgate the therapeutic effects of different levels of electrical stimulation and vitamin $AD_3E$ administration on fracture healing in a rat model and (b) to identify the most effective voltage level. Ninety Sprague-dawley rats were divided into electrical stimulation group and vitamin $AD_3E$ administration group. Electricla stimulation group was divided into four groups on the basis of the level of current delivered. The experimental groups received current (which varied by group), while the controls, with identical electrodes, received no current. In vitamin $AD_3E$ administration group, experimental group was injected 2,500 IU of vitamin A, 1,250 IU of vitamine $D_3$, 1mg of vitamine E intramuscularly. And in control group 0.1ml of saline was injected intramuscularly. After time periods (7-day, 14-day, 21-day for stimulation all rats were tested with combination of biochemical, roentgenologic and histomorphological methods. The results obtained were as follows ; In electrical stimulation groups, serum calcium and inorganic phosphorus level of experimental and control groups showed non specific change within normal physiological ranges. In vitamin $AD_3E$ administration group, serum calcium level of experimental and control groups showed non specific changes within normal physiological ranges, while experimental group showed slightly higher serum inorganic phosphorus level. According to roentgenologic and histomorphological examination, 2V 25Hz electrical stimulation group and vitamine $AD_3E$ administration group showed statistically significant improvements in bone density and ossification reaction until day 14. The terapeutic effect of stimulation on fracture healing was similiar to that of vitamin $AD_3E$ administration. In this study stimulation of 2V 25Hz was the most effective level of electrical stimulation for the healing of fracture of rats.

유리 건 피판을 이용한 수부 재건술 (Use of the Tenocutaneous Free Flap In Hand Reconstruction)

  • Purpose : This describes our experience with a tenocutaneous free flap from the dorsum of the foot or radial forearm to reconstruct the dorsal skin and extensor tendons of the hand. Material and Methods : Between february 1987 and July 1998, we treated 9 patients with composite tissue loss on the dorsal hand caused by crushing injury. Nine men had an average age of 26.4 years(range, $19{\sim}47$). We treated 5 patients with the free dorsalis pedis flap including the extensor tendons and the superficial peroneal nerve and 4 patients with reverse forearm flap including the brachioradialis tendon and/or superficial radial nerve. Flap size was average 4.4(3,2cm. Evaluation of the results was based on the survived flap rate, the recovery rates for range of motion of the metacarpophalageal joints in the operated fingers. two-point discrimination. Results : All flaps were well vascularized and survived completely. Recovery rates for range of motion of the metacarpophalageal joints in operated fingers range from $78%{\sim}99%$(average, 90%). Two-point discrimination of the transferred flaps in 5 patients average $20{\pm}3.5mm$. Conclusion : The advantages of this procedure are mass action reconstruction with tendon, one-stage operation, faster healing with less adhesion formation, and early mobilization.

관절경적 회전근 개 봉합술: 이열 봉합술 및 교량형 봉합술식 (Arthroscopic Rotator Cuff Repair: Double Rows & Suture Bridge Technique)

  • 이상적인 회전근 개 봉합술은 봉합 초기 높은 고정 강도로 봉합 부위 건-골간 간격 형성을 최소화시키며, 재활 과정 중 발생하는 반복적인 부하에도 견디는 기계적 강도를 나타내어 궁극적으로 건-골 조직의 생물학적 치유를 얻을 수 있는 방법이다. 현재 사용되는 회전근 개 봉합술 중 교량형 봉합술식은 회전근 개 부착 부위를 해부학적으로 복원할 수 있으며, 건-골간 압력 접촉 면적을 증가시키고, 방사형의 봉합 형태를 통하여 회전근 개 전체에 균등하게 압력을 분포함으로 부하를 분산시키며 생물학적 치유를 향상시킨다. 또한 건-골간 간격 형성을 최소화하며 전단 및 회전 응력에 저항력을 주어 정상과 동일한 해부학적 복원력으로 빠른 재활 운동을 가능하게 한다. 그러나 비록 교량형 봉합술식이 다른 술식에 비해 우수한 생역학적 특성을 나타내도 임상적으로 더 좋은 결과를 초래한다는 증거는 없으며, 이열 봉합술과는 비슷한 재파열율이 보고되고 있다. 회전근 개봉합술의 선택은 회전근 개 파열 크기, 파열 양상 및 건의 상태 등을 고려하여 적절하게 선택하여야 할 것으로 사료된다.

족배부 유리편을 이용한 생건 피부편이식 (Tendocutaneous Free Flap Transfer from Dorsum of the Foot)

  • Tendocutaneous free flap transfer has been usually used to treat troublesome wounds, which had extensive defect of skin and tendons, since Daniel and Taylor had reported successful free flap transfer in 1973. Among the numerous types of free flap, the dorsalis pedis flap, which could include superficial peroneal nerve, extensor tendon and second metatarsus, was widely used as composite free flap. The authors analysed 13 cases of tendocutaneous free flap transfer from dorsum of the foot which were operated at Korea University Hospital from March 1981 to August 1991. The results were as follows: 1. The average size of these flaps was $53.7cm^2$(mazimum $82cm^2$, minimum $30cm^2)$, the average number and length of tendons were 2.9(maximum, 5, minimum 1), and 9.2cm (maximum 17cm, minimum 5cm). 2. The survival rate of flaps was 100%, and functional results by Dargan's criteria were 4 in excellent, 4 good, 3 fair and 2 poor. 3. The delayed healing on donor site could prevented by the meticulous skin graft and repair of extensor retinaculum. 4. The cases of electrical burn were more worse than the traumatic cases in functional results.

상지에서의 증식치료 (Prolotherapy for the Upper Extremity)

  • 증식치료는 정상세포나 조직을 성장시키기 위해 성장인자 또는 성장인자 생성 자극제를 주사하는 것이다. 비록 수십 년 동안 논쟁이 되어 오고 있지만 갈수록 더 많은 의사들이 증식치료를 시술하고 있다. 증식치료라는 단어는 1950년 대에 Hacket에 의해 기술되었는데 인대나 근건에 있는 정상조직의 증식을 의미 하였다. 인대 염좌 등의 불완전한 치료는 만성 통증뿐만 아니라 관절불안 등을 초래하고 골관절염의 진행을 촉진할 수 있다. 증식치료는 통상적인 치료에 불응하는 이러한 근골격계 질환에 사용된다. 사용되는 증식제나 수기는 의사마다 수련과정 또는 선호도에 따라 차이가 있을 수 있지만 최근 가장 많이 사용되는 증식제는 10~25%의 포도당이다. 고해상도 초음파기기의 발전에 따라 인대나 근건 등을 실시간으로 볼 수 있게 됨에 따라 이를 증식치료에 접목하여 수기의 정밀도를 높이고 주입되는 약의 확산을 확인 할 수 있게 되었고 그 치료 결과를 기록할 수 있게 되었다.

관절경적 회전근 개 봉합술: 일열 봉합 수기 (Arthroscopic Rotator Cuff Repair: Single Row Technique)

  • 관절경적 일열 봉합법은 잘 정립된 회전근 개 봉합술로 우수한 임상성적을 보여왔다. 하지만, 수술 후 재파열의 빈도가 높은 것으로 알려져 있어 다양한 방법들이 술 후 재파열을 줄이기 위하여 시도되어왔다. 일부 연구들에서는 해부학적 회전근 개 부착부를 재건하면 봉합한 회전근 개의 치유 및 초기 역학적 강도를 증가시킬 수 있을 것이라고 보고하였고, 이열 봉합법이 해부학적 회전근 개 부착부 재건과 봉합부의 강도를 증가 시키고, 간격형성을 감소 시킬 목적으로 소개되었다. 하지만, 재부착된 회전근 개 건의 장력, 봉합된 건의 혈관 형성 등의 생물학적 치유환경이 일렬 봉합법에 비하여 우수함이 아직 입증되지 않았고 수술 후 기능적 개선도 측면에서도 두 봉합술간 차이가 없으므로, 수술 수기가 상대적으로 쉽고, 요구되는 기구 및 내고정물의 추가적 비용이 적어 경제적인 측면에 장점이 있는 일렬 봉합법은 여전히 추천되는 수술수기이다.

흡수성 고정판과 나사를 이용한 중수골 골절의 치료 (Treatment of Metacarpal Bone Fracture Using Biodegradable Plates and Screws)

  • Purpose: Metacarpal fractures are common hand injury that may require operative intervention to ensure adequate reduction and stabilization. Traditionally, titanium miniplate was used for rigid fixation of bone fractures. However, the use of permanent plate lends itself to multiple complications such as infection, exposure of the hardware, tendon adhesions, tendon rupture, prolonged pain, bony atrophy and osteoporosis (stress shielding), metal sensitization, and palpation under the skin. This study evaluated the usefulness and stability of biodegradable plates and screws for treatment of metacarpal bone fractures. Methods: There was 17 patients who had surgery for metacarpal bone fracture from April 2007 to June 2010. All patients had open reduction and internal fixation. We used absorbable plates and screws (Inion CPS$^{(R)}$) for internal fixation. Postoperative results were assessed with x-ray. Stability of plates and screws, healing process and its complications were observed by clinical and radiographic assessment. Results: All patients were successfully reduced of bone fracture, and fixations with absorbable plates and screws were stable. The mean follow up period was 7.1 months. 2 patients complained postoperative pain, but they were relieved with analgesics. All patients experienced transient stiffness, but they were relieved with active assistive range of motion after removal of splint. No patients suffered complications which could be occurred by using metallic plate. Conclusion: There was no critical complications such as re-fracture or nonunion among patients. No patients suffered side effects related with metallic implants. Biodegradable implants can offer clinically stable and attractive alternative to metallic implants to stabilize metacarpal bone fractures in the hand.
