The socioeconomic impact of Korean dental health insurance policy on the elderly: a nationwide cohort study in South Korea

  • Purpose: The purpose of this retrospective study was to investigate the relationships of types of dental insurance coverage in Korea with sociodemographic characteristics and the prevalence of systemic and oral diseases, as well as to evaluate the socioeconomic impact of Korean dental insurance policies. Methods: Sample cohort data from 2006 to 2015 were obtained from the National Health Insurance Service. Patients were divided into 2 groups. The exposed group comprised patients who received insurance benefits for complete dentures, removable partial dentures, and implant care, while the control group comprised patients who did not receive these benefits. The type of insurance coverage and the prevalence of systemic and oral diseases were compared between the 2 groups. Results: Patients who received benefits in the form of complete dentures, removable partial dentures, and implants had similar sociodemographic characteristics in terms of sex, age, income quintile, and type of insurance coverage to the control group. The prevalence of hypertension, anemia, renal disease, rheumatoid arthritis, osteoporosis, asthma, and cerebral infarction was higher in the exposed group than in the control group (P<0.05). The prevalence of periodontal diseases and dental caries was also higher in the exposed group. Conclusions: Korean dental health insurance policy has been beneficial for the medical expenses of low-income and elderly people suffering from a cost burden due to systemic diseases. However, since there is a tendency to avoid invasive interventions in older patients due to the high risk of systemic diseases, insurance coverage of dentures may be more helpful from a socioeconomic perspective than coverage of dental implant treatments.

구강건조증에 대한 필로카핀 구강양치액의 효과 (Effect of Pilocarpine Mouthwash on Xerostomia)

  • 구강건조증이란 주관적인 구강 내 건조감으로 정의되며, 이는 약물, 타액선 질환, 방사선 치료, 쉐그렌 증후군, 심리적 요인 등과 같은 다양한 원인에서 비롯된다. 구강 건조증 환자 중 현저한 타액선 기능 감소가 존재하는 경우 구강캔디다증, 치아우식증, 치주질환, 미각변화, 구취 등의 병발증이 나타날 수 있다. 이러한 구강건조증의 치료로는 우선적으로 구강건조를 유발하는 원인요소를 제거하거나, 환자의 불편감을 감소시키기 위한 대증요법이 주가 되며 실제로 타액 분비 기능이 감소된 경우 이로 인한 합병증을 예방하기 위한 치료와 타액분비를 자극할 수 있는 약물치료를 시행할 수 있다. 이 중 타액분비를 촉진시키는 약물인 필로카핀은 구강건조증 치료제로 널리 사용되어 왔다. 하지만 발한작용, 비뇨기 및 위장관계의 비정상적인 기능유도, 심혈관계 및 호흡기계에 대한 위험 등의 부작용이 있어 천식, 만성폐질환, 심혈관계 질환자에게는 주의 깊은 사용이 요구되며, 특히 조절되지 않는 천식환자의 경우 필로카핀의 절대적 금기증으로 사용이 금지된다. 이처럼 구강건조증 치료에 있어 필로카핀은 부작용으로 인해 전신적인 투여에 많은 제한이 있다. 따라서, 필로카핀의 부작용을 최소화하기 위해 국소적으로 사용되는 방법 중의 하나인 필로카핀 양치액을 이용하여 치료한 증례를 통해 그 효과를 확인해보고자 하였다.

클로로프렌의 유해성과 작업환경 관리 (Hazards of Chloroprene and the Workplace Management)

  • 본 연구는 신발, 타이어, 접착제 제조에 많이 사용되는 클로로프렌(chloroprene)에 대해 취급근로자의 건강장해 예방을 위해 유해성을 조사하고, 취급사업장에 대해 작업환경을 측정 노출농도 및 용량반응을 통해 위험성을 평가하였다. 클로로프렌은 국제암연구소(IARC)로부터 발암추정물질(2B), 표적장기 전신독성 및 생식독성물질로 국내 취급 사업장은 27개소, 연간 약 1,300톤이 사용되었으며, 노출 위험성은 노출환경에 따라 Monte-carlo simulation을 이용하여 산출하였다. 그 결과 국소배기장치가 없으며 1일 4시간 초과 작업시 발암을 일으킬 수 있는 위험성의 평균값은 26,404로, 국소배기장치가 있으며 15분 미만의 단시간 작업 경우 2,199에 비교해 매우 높은 위험성을 보였다. 그리고 표적장기 전신독성의 경우도 169.06으로 단시간 작업 경우 4.10에 비교해 표적장기 전신독성 발생 위험이 높은 것으로 평가되어 물질의 노출시간 및 환기방식에 깊은 상관관계가 있는 것으로 나타났다. 따라서 클로로프렌 노출에 의한 근로자의 건강장해 예방하기 위해 국소배기장치의 가동과 방독마스크의 착용 등 노출저감을 위한 작업환경 관리의 필요성이 있었다. 특히 클로로프렌은 발암성, 생식독성 물질로 미국 산업안전보건청(OSHA)에서는 TWA를 5 ppm($18mg/m^3$) 이하로 관리하고 있으며, 본 유해성조사 결과에서도 현행 노출기준 10 ppm이하에서 표적장기독성, 생식기 장해 등 생체에 미치는 영향이 있는 것으로 나타나 있어, 위험성이 강하게 예측되므로 현재 사용하고 있는 우리나라 작업환경 노출기준(TWA 10 ppm)도 5 ppm 수준으로 강화하여 근로자 건강장해 예방에 기여할 필요성이 있는 것으로 판단되었다.

Assessment of the correlation between various risk factors and orofacial cleft disorder spectrum: a retrospective case-control study

  • Background: Orofacial clefts (OFCs) comprise a wide range of malformations, including cleft lip, cleft palate, and cleft lip with cleft palate, which can vary in terms of etiology, severity, and disease burden. Objective(s): This study aimed to evaluate the correlation between various risk factors and orofacial cleft disorder spectrum in newborns. Study design: A total of 323 cases and 400 controls were enrolled in this study and evaluated in terms of the maternal history of abortion or miscarriage, child's sex, maternal and paternal age, maternal history of systemic disease, history of medication therapy during pregnancy, birth order, consanguineous marriage, and complications during pregnancy. Results: Analysis of the results suggested that consanguineous marriage, a maternal history of abortion/miscarriage, and complications during pregnancy could potentially increase the risk of OFCs in children (P < 0.05). However, the analyses revealed that the other variables could not potentially increase the risk of OFCs (P > 0.05). Conclusion(s): Multiple cofactors may simultaneously contribute to the formation of such abnormalities; therefore, a comprehensive, multidisciplinary care program is necessary to ensure a successful pregnancy period and the birth of a healthy newborn.

Rebuilding Operational Risk Management Capabilities: Lessons Learned from COVID-19

  • Globally, COVID-19 has significantly impacted many different organizations and people. From the banks' perspective, this pandemic has affected banks' corporate and retail customers. Also, banks had to adjust to distributed workforce model. This paper analyses the lessons learned from the COVID-19 pandemic, which can be effectively used to rebuild banks' Operational Risk Management capabilities. The present study used the survey research methodology, which includes structured questionnaires completed by senior banking professionals to analyze the learnings from COVID-19 and understand the distributed workforce model and remote working effectiveness. Findings: The Pandemic accelerated the pace of digital transformation. The lockdown imposed due to the pandemic led to employees working remotely, which has been effective because of enhanced digital capabilities. However, enhanced monitoring is required to prevent data-related issues, and action needs to be taken to address challenges faced in having a remote distributed workforce model, like negative impact on on-the-job learning, data-related risks, and employee wellbeing. COVID-19 is an unprecedented event that could not have been predicted in any scenario analysis. This crisis has highlighted various systemic drawbacks that need to be addressed. Banks can apply the lesson learned from this Pandemic to become more robust in the future.

식료품제조업 전기설비의 전기화재방지를 위한 위험우선순위 분석 (Risk Priority Analysis for Preventing Electrical Fires in Food Manufacturing Electrical Facilities)

  • This study aims to introduce a method for risk assessment analysis aimed at preventing electrical fires within the manufacturing industry. Initially, we provided a functional block diagram illustrating the electrical equipment employed in various processes within a food manufacturing company in Chungcheongbuk-do. Subsequently, we categorized the components of each machine outlined in the functional block diagram and determined the priority of failure for each classified component. Upon implementing the model in the frying process of the food manufacturing company, specific components, including MC, motor, mixer heater, electric wiring, and terminal block, exhibit a risk priority number (RPN) of 40 or higher. This identified an electrical issue within the manufacturing process and environment. Consequently, we propose a methodology to identify latent failure modes that could escalate into fires or systemic issues if not promptly addressed. It is essential to note that while the FMEA presented in this study may not immediately impact food manufacturing, its applicability extends to various workplaces.

  • Jaundice is one of the most common gastrointestinal conditions found in neonatal period, and most jaundice is benign. But because of the possibility of bilirubin toxicity, every newborn infants must be examined to identify the development of severe hyperbilirubinemia. To prevent the development of severe hyperbilirubinemia, promote and support successful breast-feeding, perform a systemic assessment before discharge for the risk of severe hyperbilirubinemia, provide early and close follow-up program, and treat with phototherapy or exchange transfusion or other therapeutic modalities, if indicated, are recommended.

Has the overall debt burden reached a threshold where it poses a systemic risk, thereby making the economy vulnerable to a "Lehman Moment" - with disorderly unwinding of the private sector and sovereign debt? What are the short and longer term implications of China's growing debt problems on domestic economic growth and the broader global political economy? What has Beijing done to ameliorate the problem, how effective were its efforts, and what must it do to deal with this problem?

Predictors and management of intravenous immunoglobulin-resistant Kawasaki disease

  • Kawasaki disease (KD) is a systemic vasculitis that mainly affects younger children. Intravenous immunoglobulin (IVIG) resistant cases are at increasing risk for coronary artery complications. The strategy on prediction of potential nonresponders and treatment of IVIG-resistant patients is now controversial. In this review the definition and predictors of IVIG-resistant KD and current evidence to guide management are discussed.

A Review of HLA Genes in Pharmacogenetics: Risk Assessment of Adverse Drug Reactions

  • Adverse drug reactions (ADRs) is a hypersensitivity reactions to specific medications, and remain a common and major problem in healthcare. ADRs suchc as drug-induced liver injury and life-threatening severe cutaneous adverse drug reactions including Stevens-Johnson syndrome, toxic epidermal necrolysis, and drug rash with eosinophilia and systemic symptoms can be occurred by uncontrolled expansion of oligoclonal T cells according to genetically predisposing HLA. In this review, I summarized the alleles of HLA genes which have been proposed to have association with ADRs caused by different drugs.
