Endoscopic Endonasal Transsphenoidal Resection of Solitary Extramedullary Plasmacytoma in the Sphenoid Sinus with Destruction of Skull Base

  • Solitary extramedullary plasmacytomas are isolated plasma cell tumors of soft tissue that typically do not metastasize. They are rare and account for 4% of all plasma cell tumors. To our knowledge, only 14 cases of solitary extramedullary plasmacytomas in the sphenoid sinus have been reported. A 32-year-old man presented to our department with complaint of ocular pain in the right eyeball and diplopia. Physical and neurological examinations revealed intact and prompt direct and indirect light reflexes in both pupils and limitation of extraocular muscle movement seen with the lateral gaze of the right eyeball. Magnetic resonance imaging suggested the presence of mucocele or mycetoma, therefore surgical resection was performed with endoscopic endonasal transsphenoidal approach. Histopathology was consistent with plasmacytoma. Systemic work-up did not show any evidence of metastasis and the sphenoid sinus was the sole tumor site, and therefore the diagnosis of solitary extramedullary plasmacytoma was confirmed. We report a rare case of solitary extramedullary plasmacytoma in the sphenoid sinus with successful treatment using the endoscopic endonasal transsphenoidal resection and adjuvant radiotherapy.

우슬과 초음파가 퇴행성관절염 백서의 관절 연골에 미치는 영향 (Effect of Achyrantis Radixs and Ultrasound in Osteoarthritis Rats Articular Cartilage)

  • Osteoarthritis(OA) diseases are characterized by joint pain, tenderness, limitation of movement, crepitus, occasional effusion, and variable degrees of inflammation without systemic effects. We investigated the effects of Achyrantis radixs cream treatment and low intensity ultrasound in monosodium iodoacetate(MIA) induced experimental osteoarthritis rat. Sprague-Dawley 40 rats of 7-8 weeks, weight $250\;{\pm}\;50$ g were divided into four groups including the control group and ostoarthritis group(30 rats). Histopathological examination, Mankin's score, and immunohistochemical were performed. Histological findings in control group that are similar to those observed in human osteoarthritis, such as disorganization of chondrocytes, erosion and fibrillation of cartilage surface, and subchondral bone exposure. Safranin O-fast green staining revealed that marked diffuse reduction of proteoglycans and chondrocyte treated with MIA. The Mankin's score were closely correlated to the grade of histological findings. The level of Bax and caspase-3 expression decreased experimental groups. This study shows that a Acyranthes Radix cream treatment and low intensity ultrasound exerts a beneficial influence on the severity of chondral lesion in osteoarthritis rats. This treatments could related to a reduced level of chondrocyte apoptosis through anti-apoptotoc capacities of MIA-induced apoptotic protein overexpression.

재결서 분석을 통한 어선-비어선간 충돌사고에 관한 연구 (A study on the collision between fishing vessel and non fishing vessel using the analysis of written verdict)

  • The analysis of the written verdicts in recent five years was conducted to obtain preventive measures of collision between fishing vessel and non fishing vessel. As a result, a collision much happened in offshore trap for fishing vessel and below 5,000 tons of small and medium class for non fishing vessel. A person involved in a marine accident occupied 68% in sixth class deck officer and small boat operator for fishing vessel and 29% in third class deck officer for non fishing vessel. 90% of the collision happened in a underway by operating state and 84% in sight of one another by visibility state. The systemic radar training was required since 47% of the collisions was occurred on the condition of radar operation in fishing vessel. The main cause of poor lookout was a intensive fishing and poor lookout on movement by radar for fishing vessel and one man watch system and no recognition of one another by radar for non fishing vessel. This result is expected to contribute for the decrease of collision.

  • Intrathecal drug administration system (ITDAS) can reduce the side effects while increasing the effectiveness of opioids compared to systemic opioid administration. Therefore, the use of ITDAS has increased in the management of cancer pain and chronic intractable pain. Catheter obstruction is a serious complication of ITDAS. Here, we present a case of catheter obstruction by a mass formed at the side hole and in the lumen. A 37-year-old man suffering from failed back surgery syndrome received an ITDAS implantation, and the ITDAS was refilled with morphine every 3 months. When the patient visited the hospital 18 months after ITDAS implantation for a refill, the amount of delivered morphine sulfate was much less than expected. Movement of the pump rotor was examined with fluoroscopy; however, it was normal. CSF aspiration through the catheter access port was impossible. When the intrathecal catheter was removed, we observed that the side hole and lumen of the catheter was plugged.

경근(經筋)과 인체 시상균형에 관한 소고(小考) (Meridian Sinews and Sagittal Spinal Balance)

  • Sagittal spinal balance means standing postural balance at sagittal plane. Postural imbalance with displacement of the patient's center of gravity can cause chronic back pain and ambulatory difficulty. The sagittal spinal balance is determined based on the deviation of the C7 plumb line, originating at the middle of the C7 vertebral body, from the posterior superior endplate of S1. The line is called as sagittal vertical axis (SVA). In the traditional Korean medicine, the meridian sinews, which are the most superficial pathways of the meridian system, associated with movement, muscle balance and defense. They too are separate from the main meridians, though they intersect the main meridians. Some creative and pioneer researchers in Korea thought that the anatomy trains, which suggested by Myers is a concept familiar to the meridian sinews. A reciprocal relationship between the superficial back line and the superficial front line used to be compared to the rigging of a sailboat. Therefore, We suggest that spine may be compared to a mast of the sailboat and that the sagittal spinal balance can be maintained with systemic reciprocal interacts between the front line muscles and the back.

Temporomandibular Joint Ankylosis Caused by Osteoarthritis: A Case Report Based on Cone Beam Computed Tomography Images

  • Temporomandibular joint (TMJ) ankylosis is a condition in which condylar movement is restricted because of fibrous or bony union between the mandibular condyle and temporal bone. TMJ ankylosis is most often caused by trauma, followed by systemic or local infection, and secondary to unknown causes. Diagnostic imaging plays a vital role in diagnosing TMJ ankylosis and establishing a treatment plan. Computed tomography (CT) or cone beam computed tomography (CBCT) is currently the imaging technique of choice to accurately demonstrate preoperative ankylosing masses and other surgically important findings, such as the shape of the mandibular condyle and the pathological changes in the joint. The osseous changes in the mandibular condyle are easily identified in the coronal and sagittal sections of CT or CBCT images. This report describes the case of a middle-aged woman who developed TMJ ankylosis of the left TMJ while undergoing repeated treatment for TMJ disease. We report the findings observed on radiographic and CBCT images through continuous observation.

Functional Anatomy of the Temporomandibular Joint and Pathologic Changes in Temporomandibular Disease Progression: A Narrative Review

  • The temporomandibular joint (TMJ) is one of the most unique joints in the human body that orchestrates complex movements across different orthogonal planes and multiple axes of rotation. Comprising the articular eminence of the temporal bone and the condylar process of the mandible, the TMJ integrates five major ligaments, retrodiscal tissues, nerves, and blood and lymph systems to facilitate its function. Cooperation between the contralateral TMJ and masticatory muscles is essential for coordinated serial dynamic functions. During mouth opening, the TMJ exhibits a hinge movement, followed by gliding. The health of the masticatory system, which is intricately linked to chewing, energy intake, and communication, has become increasingly crucial with advancing age, exerting an impact on oral and systemic health and overall quality of life. For individuals to lead a healthy and pain-free life, a comprehensive understanding of the basic anatomy and functional aspects of the TMJ and masticatory muscles is imperative. Temporomandibular disorders (TMDs) encompass a spectrum of diseases and disorders associated with changes in the structure, function, or physiology of the TMJ and masticatory system. Functional and pathological alterations in the TMJ and masticatory muscles can be visualized using various imaging modalities, such as cone-beam computed tomography, magnetic resonance imaging, and bone scans. An exploration of potential pathophysiological mechanisms related to the TMJ anatomy contributes to a comprehensive understanding of TMD and informs targeted treatment strategies. Hence, this narrative review presents insights into the fundamental functional anatomy of the TMJ and pathological changes that evolve with TMD progression.

Identification and Sequence Analysis of RNA3 of a Resistance-Breaking Cucumber mosaic virus Isolate on Capsicum annuum

  • Cultivated hot pepper crops showing severe mosaic symptom were found in Korea in 2004 and their causal agent was identified as Cucumber mosaic virus (CMV). These pepper crops was resistant to the virus in the filled, and they belonged to pathotype 0 (P0) resistant pepper. Resistance screening of selected pepper plants showed that a pepper isolate of CMV was the P0 resistance-breaking virus. This P0 resistance-breaking isolate of CMV, named as Ca-P1, was isolated from leaves of the virus-infected Capsicum annuum cv. Manidda that showed systemic severe mosaic symptom. Ca-P1-CMV could induce systemic mosaic symptoms on P0-susceptible (P0-S) and P0-resistant (P0-R) cultivars whereas an ordinary strain (Fny-CMV) could not infect P0-R. This result suggests that Ca-P1-CMV can overcome P0 resistant pepper cultivars. To analyze its genome sequence, the complete nucleotide sequence of RNA3 of Ca-P1-CMV was determined from the infectious full-length cDNA clone of the virus. RNA3 of Ca-P1-CMV consisted of 2,219 nucleotides. Overall sequence homology of RNA3-encoded two viral proteins (movement protein and coat protein) revealed high similarity (75.2-97.2%) with the known CMV strains. By sequence analysis with known representative strains of CMV, Ca-P1-CMV belongs to a typical member of CMV subgroup IB. The resistance and resistance-breaking mechanisms of pepper and counterpart CMV, respectively, remain to be investigated, which will enrich the genetic resources and accelerate CMV-resistant pepper breeding programs.

스텐트를 이용한 만성 궤양성 치은 병소의 국소 스테로이드 치료 (Topical Steroid Therapy using Stent on Chronic Ulcerative Gingival Lesions)

  • 만성 궤양성 치은 병소는 편평태선, 양성점막유천포창, 심상성 천포창 등과 같은 다양한 질환에 의해 유발될 수 있으며 코티코스테로이드 약물요법이 주된 치료로 이용된다. 코티코스테로이드를 복용하는 경우 위장장애, 체중증가 등의 부작뿐만 아니라 장기 복용 시 골다공증, 당뇨 및 고혈압 발생 또는 악화, 부신기능 저하, 쿠싱증후군 등의 심각한 부작용이 발생 할 수 있어 병소가 국소 부위에 제한되어 발생하거나 전신증상을 동반하지 않는 경우 스테로이드 약물의 국소 도포가 만성 궤양성 치은 병소의 주된 치료로 이용되고 있다. 하지만 국소 스테로이드 치료를 구강 내에 사용하는 경우 타액 분비, 혀, 입술, 협점막 등의 움직임에 의해 도포한 약제가 소실되어 효과가 감소되고 병소가 넓은 부위에 분포하거나 구강 내 깊숙한 부위에 발생하는 경우 환자 스스로 약물을 도포하기가 용이하지 않으며, 질병에 이환 되지 않은 정상 점막에도 약제가 도포 되는 등의 단점이 있다. 국소 스테로이드 적용 방법의 단점을 극복하고 효과를 최대화함으로써 스테로이드 복용을 최소화할 수 있는 방법으로 스텐트를 이용한 스테로이드 밀폐 요법이 보고된 바 있으나 실제 임상에서 적극적으로 활용되지는 않고 있다. 따라서 만성 궤양성 치은 병소가 발생한 환자에게 스텐트를 이용한 국소스테로이드 치료를 시행하여 양호한 치료 효과를 보인 증례를 통해 그 효용성과 임상적 활용 방안을 알아보고자 한다.

배추의 저온 스트레스 처리 시간대별 발현 유전자 네트워크 분석 (Time-based Expression Networks of Genes Related to Cold Stress in Brassica rapa ssp. pekinensis)

  • 식물은 다양한 생화학적 및 생리적 과정에 속한 유전자들의 발현 수준을 조절함으로써 저온 스트레스에 반응 및 적응을 할 수 있다. 이러한 스트레스 환경은 막 기능 손실, 세포벽의 변화, 대사 속도 변화 등과 같이 부정적인 영향을 초래한다. 따라서 본 연구는 배추(Brassica rapa ssp. pekinensis)에서의 시간 변화에 따른 저온 스트레스 반응 기작 관련 유전자 상호발현 네트워크를 구축하였다. 배추의 저온 스트레스 네트워크는 2,030개 node, 20,235개 edge, 및 34개 connected component로 구성되었으며, 구축된 네트워크는 배추에서 저온에 관여하는 유전자가 생육도 조절한다는 것을 보여 주었다. 구축한 네트워크를 이용하여 배추에서 저온 스트레스($4^{\circ}C$) 처리가 미치는 영향을 분석한 결과 WRKY 전사인자와 살리실산 신호에 의해 chitinase 부동 단백질이 활성화되고, 전신적 획득저항성을 작동하기 위해 기공 개폐 및 탄수화물 대사과정이 조절됨을 확인하였다. 또한 저온 처리 후 48시간 후에 저온 스트레스가 영양생장에서 생식 생장 및 분열 조직 단계의 변화를 초래하는 것으로 나타났다. 본 연구에서 구축한 네트워크 모델은 배추에서 저온 저항성 관련 유전자들의 발현 패턴을 정확히 유추하는 데 이용될 수 있을 것이다.
