Smoking Cessation (금연)

  • Smoking is the most important risk factor of many pulmonary diseases, including chronic obstructive pulmonary disease and lung cancer, cardiovascular disorders and other malignancies. Therefore, smoking cessation is a practical way to prevent and treat smoking-related diseases. Also, the clinicians who care the patients with smoking-related disease should pay attention to it. This article reviews briefly recent publications focused on the influence of smoking cessation in some smoking-related diseases and strategies to improve smoking cessation such as pharmacotherapy or systemic behavioral approach programs. In addition, it reviews personalized therapy based on gene typing for smoking cessation.

  • It can be characterized that contemporary architects intend to establish architectonics which can make it possible to reinterpret the attribute of movement in space and its programming differed from the traditional perception in moving in and out any space through disjoining and reorganizing human behavior and event. This intends reflect new paradigm for subjugating the compelled fuctioning of modern Architecture. This Study review those intends focused on programming as a pre-disign schema. In those intends, it connotes the effort of establishing more systemic and logical approach through deconstruction and disprogramming the existing from, space, elements and human on the basis of his own schematic conception as substitution of function in modern architecture.

Clinical Approach to Children with Proteinuria

  • Proteinuria is common in pediatric and adolescent patients. Proteinuria is defined as urinary protein excretion at levels higher than $100-150mg/m^2/day$ in children. It can be indicative of normal or benign conditions as well as numerous types of severe underlying renal or systemic disease. The school urine screening program has been conducted in Korea since 1998. Since then, numerous patients with normal or benign proteinuria as well as early stage renal diseases have been referred to the hospital. Benign proteinuria includes orthostatic proteinuria and transient proteinuria. Most causes of proteinuria can be categorized into 3 types: 1) overflow, 2) tubular, and 3) glomerular. Although treatment should be directed at the underlying cause of the proteinuria, prompt evaluation, diagnosis, and long-term monitoring of these pediatric patients can prevent potential progression of the underlying disease process. This article provides an overview of proteinuria: its causes, methods of assessment, and algorithmic suggestions to differentiate benign from pathologic renal disease.

  • In cases of brain or other organ abscess, the causative etiology or disease are not always definable. We report a case of brain, renal, and possibly lung abscesses in a middle aged woman. After close, stepwise surveillance of possible etiologic factors, we covered out a small solitary pulmonary arteriovenous fistula without any pulmonary symptoms and successfully occluded the fistula via endovascular approach. The congenital pulmonary arteriovenous fistula should be bear in mind as a cause of repeated, multiple systemic infective source spray and be pursued despite of negative initial baseline studies.

Clinical Experience of Cardiac Myxoma (심장 점액종의 임상적 고찰)

  • From 1981 to 1990, we performed operation on 6 patients with cardiac myxomas, which were located in the left atrium in 5, in the biatrium in 1. There were 3 male and 3 female patients ranging in age from 13 years to 69 years. Symptoms included exertional dyspnea, palpitation and signs of systemic illness. Diagnosis was accomplished by angiography[3 patients] and echocardiography [3 patients]. In all cases, myxomas were excised successfully through incision of interatrial septum There were no operative deaths or intraoperative embolic episode. Follow-up has been 40.0\ulcorner40.6 months[ranging from 1 month to 9 years], results of each patient were excellent We suggested that excision of intracardiac myxomas is curative and follow-up results are excellent. The transseptal approach provides adequate exposure and allows complete removal of the tumor regardless of its location.

  • This paper investigates the issue of performance testing on web browsing environments. Among the typical non-functional characteristics, index of link validity will be deeply explored. A framework to certify link correctness in web site is proposed. All possible navigation paths are first formulated to represent a usage model with the Markov chain property, which is then used to generate test script file statistically. With collecting any existing failure information followed by tracing these testing browsed paths, certification analysis may be performed by applying Markov chain theory. The certification result will yield some significant information such as: test coverage, reliability measure, confidence interval, etc. The proposed mechanism may provide not only completed but also systemic methodologies to find any linking errors and other web technologies errors. Besides, an actual practice of the proposed approach to a web-based system will be demonstrated quantitatively through a certification tool.

  • The objectives of this study were to review on the characteristics of the Agricultural Research-Extension Systems in the USA, Japan, and the Netherlands, and to draw up its implications on Korean agricultural extension system. This study was conducted by literature reviews. Based on the reviews, the following implications and recommendations should be considered at national and local level for improvement of agricultural extension system in Korea; (1) a systemic approach on the linkages of agricultural research, extension, and farmer education, (2) strengthening on-farm utilization of newly developed agricultural technology so as to promote agricultural research and development, (3) strategic partnership with agricultural administration, (4) close networking with stake-holders, (5) setting up the flexible organizational structure for carry out agricultural extension programs, (6) integration of agricultural extension service domain, (7) introduce a IPM(integrated performance management) system, (8) establish a long-term super-vision and strategic management, (9) setting up the customer-centered extension system.

A Development of Nursing Outcome Indicators for Stroke Patients (뇌졸중 환자의 간호성과지표 개발)

  • Purpose: This study was conducted to develop nursing outcome indicators based on nursing activities done for stroke patients. Method: focus group meeting and delphi technique, which consisted of clinical nurse experts, neurologist, and nursing professors. CVI(Index of Content Validity) and user validity test was performed. Result: 12 nursing diagnoses, 29 nursing interventions and nursing outcomes were identified. The former were from NIC and the latter were developed according to nursing interventions. They were verified by experts in focus group. 199 nursing activities were identified, 133 nursing outcome indicators were developed. In user validity, usefulness and usability were tested. Conclusion: This systemic approach of measuring nursing outcomes verified nurses' positive effects in changing patients health status and nursing contributions in the health care system as a profession.

  • Sjogren syndrome is a chronic systemic autoimmune disorder that chiefly involves the salivary gland and the lacrimal gland, resulting in xerostomia and xerophthalmia. Although the exact cause of the disease is not early diagnosis, treatment and observation must be emphasized because of its poor prognosis, such as the high occurrence of malignant lymphoma and other autoimmune disease that may be accompanied. In the present case, a twenty-year-old woman whose chief complaint was multiple dental hard tissue loss and xerostomia, which was misdiagnosed as iron deficiency anemia at first, but through re-evaluation and differential diagnosis it was Sjogren syndrome. the diagnosis approach was discussed in this report, suggesting that Sjogren syndrome should be considered as a differential diagnosis in a with xerostomia.

Desquamative Diseases of Gingiva (임상가를 위한 특집 3 - 잇몸이 벗겨지는 질환)

  • The gingiva consists of an epithelial layer and an underlying connective tissue layer. The oral epithelium is a keratinized, stratified, squamous epithelium. The epithelium can be divided into the following cell layer: basal layer, prickle cell layer, granular cell layer and keratinized cell layer. The desquamative disease of gingiva means exfoliative diseases of epithelial layer on the gingiva. The chronic desqumative gingivitis is usually related to the dematologic disorders that produce cutaneous and mucous membrane blisters. The cicatricial pemphigoid and lichen planus are representative diseases of the dermatologic cases. Patients may be asymptomatic or symptomatic. When symptomatic, their complaints range from a mild burning sentation to an severe pain. The clinical examination must be considered with a thorough history, and routine histologic and immunofluorescence studies. A systemic approach needs to achieve accurate diagnosis and treatment of the gingival desquamative diseases.
