Congenital heart disease in the newborn requiring early intervention

  • Although antenatal diagnostic technique has considerably improved, precise detection and proper management of the neonate with congenital heart disease (CHD) is always a great concern to pediatricians. Congenital cardiac malformations vary from benign to serious conditions such as complete transposition of the great arteries (TGA), critical pulmonary and aortic valvular stenosis/atresia, hypoplastic left heart syndrome (HLHS), obstructed total anomalous pulmonary venous return (TAPVR), which the baby needs immediate diagnosis and management for survival. Unfortunately, these life threatening heart diseases may not have obvious evidence early after birth, most of the clinical and physical findings are nonspecific and vague, which makes the diagnosis difficult. High index of suspicion and astute acumen are essential to decision making. When patent ductus arteriosus (PDA) is opened Widely, many serious malformations may not be noticed easily in the early life, but would progress as severe acidosis/shock/cyanosis or even death as PDA constricts after few hours to days. Ductus dependent congenital cardiac lesions can be divided into the ductus dependent systemic or pulmonary disease, but physiologically quite different from each other and treatment strategy has to be tailored to the clinical status and cardiac malformations. Inevitably early presentation is often regarded as a medical emergency. Differential diagnosis with inborn error metabolic disorders, neonatal sepsis, persistent pulmonary hypertension of the newborn (PPHN) and other pulmonary conditions are necessary. Urgent identification of the newborn at such high risk requires timely referral to a pediatric cardiologist, and timely intervention is the key in reducing mortality and morbidity. This following review deals with the clinical presentations, investigative modalities and approach to management of congenital cardiac malformations presenting in the early life.

Acute Hepatotoxicity and Toxicokinetics of Acetaminophen in Mice (마우스에서 아세트아미노펜의 급성간독성과 독물동태학)

  • As the development of a pharmaceutical product is a dynamic process which involves continuousfeed-back between non-clinical and clinical studies, the integration of pharmacokinetics into toxicity testing became increasingly important in recent years. Toxicokinetic measurements in the toxicity studies is considered to be an important scientific approach in the interpretation of the toxicology findings and the promotion of rational study design development. Primarily this research project was conducted to determine the systemic exposure achieved in acute toxicity test and its relationship to dose level and the time course of the toxicity study. Acute hepatotoxicity study and its relevant toxicokinetic study in mice were performed using acetarninophen (AA) as a model compound. The correlation between acute hepatotoxicity indices and toxicokinetic parameters following intraperitoneally administration of various dosages of AA in mice was evaluated and discussed minutely in the text. Based on these studies, single-dose toxicity testing of AA including kinetic studies was evaluated in ICR mice for 7 days and interpreted in the text. Our results from the integration of toxicokinetic monitoring into single-dose toxicity study enable to elucidate the relation of the exposure achieved in toxicity study to toxicological findings and assist in the selection of appropriate dose levels for use in repeated-dose toxicity or later studies.

Endoscopic Endonasal Transsphenoidal Resection of Solitary Extramedullary Plasmacytoma in the Sphenoid Sinus with Destruction of Skull Base

  • Solitary extramedullary plasmacytomas are isolated plasma cell tumors of soft tissue that typically do not metastasize. They are rare and account for 4% of all plasma cell tumors. To our knowledge, only 14 cases of solitary extramedullary plasmacytomas in the sphenoid sinus have been reported. A 32-year-old man presented to our department with complaint of ocular pain in the right eyeball and diplopia. Physical and neurological examinations revealed intact and prompt direct and indirect light reflexes in both pupils and limitation of extraocular muscle movement seen with the lateral gaze of the right eyeball. Magnetic resonance imaging suggested the presence of mucocele or mycetoma, therefore surgical resection was performed with endoscopic endonasal transsphenoidal approach. Histopathology was consistent with plasmacytoma. Systemic work-up did not show any evidence of metastasis and the sphenoid sinus was the sole tumor site, and therefore the diagnosis of solitary extramedullary plasmacytoma was confirmed. We report a rare case of solitary extramedullary plasmacytoma in the sphenoid sinus with successful treatment using the endoscopic endonasal transsphenoidal resection and adjuvant radiotherapy.

  • Implanting a pacemaker is the most often used intervention for treating bradycardia. The most commonly used pacemaker is the intracardiac pacemaker, yet it can have many complications. An infected pacemaker can spread to systemic infection and the condition of the patient can quickly get worse, so if an infected pacemaker is suspected, then the pacemaker must be removed. Apart from the use of interventional methods such as a loop or a weight, we can take a more aggressive approach by using extracorporeal circulation for removal of the pacemaker. We report here on two cases in which extracorporeal circulation was used to remove the infected pacemakers.

  • This study is aimed to find a control factor for the marine casualty of fishing vessel by using the risk quantitative method of marine casualty, and sequentially timed event analysis for the reason finding. The result is that collision, sinking, and capsizing took high risk need to be strongly controlled. And the accident reason and control factors distinguished are as follows. The 82.2% of collided fishing vessels have negligence of look out as a immediate cause, while it was judged that underlying causes were the characteristics of vessels and fishing method, lack of suitable complement, fatigue, the prejudice or ignorance on steering and sailing rules. So the control factors are; firstly needs educational control measures and in terms of systemic control approach expand the range of the certificate of competency more smaller tonnage.

  • Recently, the korean-style houses are shedded new light. So, this study analysed what it is required and improved for preservation and vitalization of traditional korean village. At tool of analysis and systemic analytical approach, the problem of traditional korean village figured out. and then, the method and Importance of improvement are deducted. As a result of this study, it is represented to need that the preservation of architectural factors in the self korean-style house, but the more important things are deducted cooperation of citizen, political support and consideration of government at level of urban planning. There must be considered for planned conservation and development, increasing value by recreation of historicity and culture in city unit of community.

Establishment of a Simple and Rapid Gene Delivery System for Cucurbits by Using Engineered Zucchini Yellow Mosaic Virus

  • The infectious full-length cDNA clone of zucchini yellow mosaic virus (ZYMV) isolate PA (pZYMV-PA), which was isolated from pumpkin, was constructed by utilizing viral transcription and processing signals to produce infectious in vivo transcripts. Simple rub-inoculation of plasmid DNAs of pZYMV-PA was successful to cause infection of zucchini plants (Cucurbita pepo L.). We further engineered this infectious cDNA clone of ZYMV as a viral vector for systemic expression of heterologous proteins in cucurbits. We successfully expressed two reporter genes including gfp and bar in zucchini plants by simple rub-inoculation of plasmid DNAs of the ZYMV-based expression constructs. Our method of the ZYMV-based viral vector in association with the simple rub-inoculation provides an easy and rapid approach for introduction and evaluation of heterologous genes in cucurbits.

  • Steven-Johnson syndrome (SJS) and toxic epidermal necrolysis(TEN) are severe mucocutaneous reaction which are most frequently caused by drugs. Although the incidence of SJS and TEN is known to be relatively low, outcomes may be fatal. A systematic approach is required because morbidity rate is currently increasing and oral lesion is frequent. We investigated the clinical features and outcomes of 6 patients diagnosed as SJS and TEN and referred from the department of dermatology, Chonnam National University Hospital for oral care. Ketoconazol, Ofloxacin, Chlorphenesin, Amoxicillin, Pontal, Harnal, and Ciprofloxacin were suspected as the causative drugs. Average treatment period was 3.2 weeks, and two patients were referred to 'burn-patients' hospital. Most of oral lesion were cured be normal tissue, but scares with discoloration were observed. For intraoral management, antibiotic disinfection and steroid application were performed according to systemic treatment principles. Additionally, ingestion of zinc, antioxidants, and vitamin was recommended. The establishment of oral treatment principles is demanded because it has not been yet. Also, through investigation of drug side effect and careful prescription are required.

  • One of the major reasons for recent blackout, like August 14, 2003 blackout in the US and Canada has been insufficient voltage/reactive power support. For the stable reactive power management, a new approach for the voltage monitoring and control structure is required in the market environment. This paper proposes the effective localized-voltage control scheme using the information from pilot buses at each zone. In this paper, the steady state voltage monitoring and control (SSVMC) is adopted and illustrated for the voltage control scheme during steady state because it is thought as the systemic algorithm to explain voltage profile phenomenon before and after contingencies. And the concept of electrical distance is applied to simultaneously achieve both clustering the voltage control zone, and selecting the pilot bus as the representative node at each control zone. Applying SSVMC based on the structure with clustering and pilot bus enables system operators to monitor and understand the system condition much more easily, to monitor and control the voltage in real-time more manageably, and to respond quickly to a disturbance. The proposed voltage control scheme has been tested on the IEEE 14-bus system with the numerical analysis to examine the system reliability and structure efficiency.

  • Based upon Shennong's Ancient Chinese Medical Textbook and Tsorngji Mingyi Byelu. Ginseng has been widely used for over 2,000 years in oriental countries. Scientific basic medical study or clinical study on ginseng was seal·toed 1910's in Eastern countries and from the 1950's in Western countries To obtain kotvledge of clinical studies on Korean ginseng. Although wide and profound research has been carried on the effect of ginseng (diabetes cardiovascular diseases, hypertension, liver diseases. gastrointestinal disorders soress, bram function. aging, antiradiation effect. anemia. hemopoiesis. immuomodulating effect. and tonic effect). Systemic clinical study to determine the therapeutic effects of speciblc disease have hardly been done even in other countries Clinical study or researches with human as the target. on ginseng has been performed in the field of body tenperazure. Pulse, clinical symptoms and hematological findings . fatigue, porformances. anemia. essential hypertension. blood sugar. serum cholesterol. lipid and prolactin. adrenocortical function. impotence. hypospermia. male sterility, climacteric disorder. anticancer effects. cancer preventive effects. and viral hepatitis. adverse effects. and prefered type of ginseng. At the same time as trying preventives or therapeutics from dietary oi natural products scientific research to support that ginseng is not a mystery. should be porformad to prove the effectiveness of Korean ginseng in the treatment of certain diseases using scientific methods or epidemiological approach.
