자기공명영상을 이용한 24마리의 개에서의 Chiari 1형 유사 기형 진단 (Magnetic Resonance Imaging in 24 Dogs with Chiari type 1-like Malformations)

  • 사람에서, Chiari 1형 기형은 소뇌 탈출과 척수공동증이 특정인 발달성 장애이다. 이러한 사람의 Chiari 1형 기형과 유사한 질환이 cavalier King Charles spaniels에서 흔히 나타났다. 그러나, 이러한 Chiari 1형 유사 기형이 다른 종의 개에서 진단 보고된 증례가 거의 없는 실정이다. 이에 본 연구에서는 24 마리의 개에서 자기공명영상 장치를 이용하여 Chiari 1형 유사 기형으로 진단된 증례를 보고하고자 한다. 이 개들은 다양한 신경 증상과 다양한 정도의 소뇌 탈출, 척수공동증, 두개관내 거미막 낭종 또는 뇌수종의 병발 질환들을 나타내었다.

  • Arachnoid cysts are intra-arachnoid collections of cerebrospinal fluid. They may cause neurological deficit through expansion that can compress normal neural tissue and obstruct cerebrospinal fluid flow. Intracranial arachnoid cysts were found in 4 patients aged 10 months to 20 months(mean age 15 months). The presenting symptoms included circling(dog 2,3), seizure(dog 1), progressive weakness(dog 1,2,3), increasing scoliosis(dog 3), worsening spasticity(dog 3), and salivation(dog 3). One patient showed no symptoms from the intracranial arachnoid cyst. There were no significant findings in complete blood count and serum chemistry. Computed tomography(CT) with/without magnetic resonance imaging(MRI) scans were performed in these cases. Hypodense intracranial arachnoid cyst was observed on CT images in dog 4. Sagittal T1-weighted(T1W) and T2-weighted(T2W) images and transverse T1W and T2W images revealed a intracranial arachnoid cysts with syringohydromyelia(dog 3) and hydrocephalus(dog 3,4) in the brain. Clinical signs of three cases were improved by medication.

Foramen Magnum Decompression with Adipose Tissue Grafting for Caudal Occipital Malformation Syndrome in a Maltese Dog

  • A 7-year-old intact female Maltese dog was referred to the hospital with a history of paresis in the hind limbs, left head turn, and a loss of balance that persisted for 2 weeks. Her condition was initially managed with steroids, prescribed by the referring veterinarian, but her neurological symptoms were not alleviated. Physical and neurological examinations, radiography, computed tomography, and magnetic resonance imaging were performed. Based on the findings on these examinations, caudal occipital malformation syndrome (COMS) with syringohydromyelia was diagnosed. Medical treatment was not effective in the previous trial; therefore, foramen magnum decompression, durotomy, and free autogenous adipose tissue grafting were performed. After 3 days, an improvement was observed in the clinical symptoms and was maintained for 8 months postoperatively. Based on the results, it is suggested that the decompression method with a fat graft may be considered an effective surgical treatment for the management of COMS that did not respond well to previous medical treatment.
