Health Related Quality of Life among Organ Transplant Recipients (장기이식환자의 건강관련 삶의 질)

  • Purpose: This study was aimed to investigate the health related quality of life and related factors of organ transplant recipients. Method: The participants were 188 people who had liver(86), kidney(81), or heart(24) transplanted. Data on the demographic characteristics, transplantation-related characteristics, symptom frequency or discomfort measured by Transplant Symptom Frequency and Symptom Distress Scale by Lough et al(l987), and health related quality of life measured by SF-36(version 2) were collected. Result: Overall health related quality of life score was 492.1 for 100scoring and, 344.9 for norm based. Physical functioning showed the highest quality of life score (77.5) and vitality showed the lowest(51.l). The kidney transplanted showed the highest quality of life (504.4) and the heart transplanted showed the lowest(426.7) Quality of life was related with occupation(p=.016) and symtom discomfort(p < .0001). Conclusion: The health related quality of life of transplated patients was lower than the norm of American. Further studies need to be done to identify the norm of Korean and to investigate the effect of releving symptom discomfort on the increasing the health related quality of life.

The Effect of Maternal State Anxiety on the Children's Somatic Symptom in Attention-Deficit/Hyperactivity Disorder (주의력 결핍 과잉행동장애 아동에서 어머니의 상태불안이 아동의 신체증상에 미치는 영향)

  • Objectives : The aim of the present study was to examine whether anxiety and depression of children and mothers were associated with the children's medically unexplained somatic symptoms in attention-deficit/hyperactivity disorder(ADHD). Methods : 83 clinic-referred boys with ADHD and 52 boys without ADHD were included in this study. The frequency of the medically unexplained somatic symptoms, such as general-ache, headache, nausea, eye problems, skin problems, abdominal pain, vomiting was evaluated using the somatic symptom domain of the Child Behavior Checklist(CBCL). Children's anxiety and depression were evaluated using Kovacs Children's Depression Inventory(CDI) and Spielberger's State-Trait Anxiety Inventory(STAI) for children. Maternal anxiety and depression were measured by Spielberger's State-Trait Anxiety Inventory(STAI) and Beck's Depression Inventory(BDI). Stepwise linear regression analysis was used to examine the hypothesis. Results : Score of maternal state anxiety affects the T score of the somatic symptom domain in CBCL significantly(adjusted $R^2$=0.057 ; p=0.026). Conclusion : The frequency of medically unexplained somatic symptom of children perceived by mothers was associated with the high level of maternal state anxiety in ADHD. Frequent maternal report of the children's somatic symptom may be a warrant for the evaluation and management of the maternal state anxiety in ADHD.

  • Objectives : Select and analyze the case record of dyspnea and wheezing asthma recorded in Xu Ming Yi Lei An that is the most abundant and wide in contents in existing case records that are systematic, comprehending relatively modern Traditional Chinese Medicine to secure more deep and objective basis of Traditional Chinese Medicine approach for dyspnea and wheezing asthma to analyze and review possibility for clinical application in this study. Methods: The study was conducted with the case records of dyspnea and wheezing asthma in whole Xu Ming Yi Lei An. Pattern identify and classify selected case records and again classified with deficiency syndrome and excess syndrome. Also analyzed prescriptions and herbs used in the case records. Nature of herbs and properties and flavors that were used in the case records were classified and frequency of each nature of herbs were analyzed. Applicable case records were interpreted and suggested prescriptions, pulse feelings, pattern classification were analyzed and described. Results : Among the 5254 case records stated on the complete collection, it is researched that there are 63 case records for the symptom complex of dyspnea as 1.2% of the whole case records, and the case records on the symptom complex of wheezing asthma are 14 as the 0.27% of the total examples. 63 case record examples related with symptom complex of dyspnea were pattern identified and classified. As a result, deficiency syndrome of the Kidney(33 %), deficiency syndrome of the Spleen(26.0%), Wind-Cold(12.3%), phlegm turbidity(12.3%), Heat in the Lung(8.2%), asthenia of the Lung(8.2%) were investigated as above order. 14 case record examples related with wheezing asthma were pattern identified and classified. As a result, phlegm-Heat(26.3%), upper excess and lower deficiency(26.3%), external affections Wind-Cold(15.8%), Dampness-phlegm(10.5%), Lung asthenia(10.5%), Cold phlegm(5.3%), mutual deficiency and detriment of Heart and Kidneys(5.3%) were investigated as above order. Symptom complex of dyspnea has 67.1% of deficiency syndrome, 32.9% of excess syndrome resulting more deficiency syndrome than excess syndrome. Symptom complex of wheezing asthma has 42.1 % of deficiency syndrome and 57.9% of excess syndrome resulting more excess syndrome than deficiency syndrome. In case of symptom complex of dyspnea prescription used in the case record, the order of frequency is as following. Palmijihwang-tang, Bojung-ikgitang, Yungmijihwang-tang, Ijintang, Sojaganggitang, Igongsan. In case of symptom complex of wheezing asthma prescription in the case record, Yungmijihwang-tang, Ohotang, Dodamtang were mostly used. Herbs used in case records of symptom complex of dyspnea are Ginseng Radix, Poria, Glycyrrhizae Radix, Aconiti Iateralis Preparata Radix, Atractylodis Macrocephalae Rhizoma, Dioscoreae Rhizoma, Angelicae Gigantis Radix, Rehmanniae Radix Preparat, Pinelliae Rhizoma, Zingiberis Rhizoma Recens are mostly used. Nature of herb properties used for symptom complex of dyspnea and symptom complex of wheezing asthma are herbs that are warm properties. When the symptom complex of dyspnea and the symptom complex of wheezing asthma were treated. if the patient felt tenderness at Pyesu, doctors conducted pricking blood around the opposite Pyesu or Sipseon acupoint. when the patient didn't have tenderness at Pyesu by soft press, pricking blood was performed both sidees, right and left Pyesu. In case of the treatment of symptom complex of dyspnea and symptom complex of wheezing asthma, when they got treatment, when the symptom complex of disease is severe, a doctor cauterized the opposite Pyesu while the other Pyesu felt tenderness, and decided how the above treatment is performed whether the degree of the symptom compolex of disease is severe or not. In case of the treatment of symptom complex of dyspnea and symptom complex of wheezing asthma, if the person felt tenderness at Pyesu and is caught by the Wind-Cold pathogen, slight acupuncture is treated at relevant Pyesu with Fire needling. When patient with symptom complex of dyspnea and symptom complex of wheezing asthma cannot hawk sputum up from the oral and laryngopharynx, suction method is treated. Conclusion : With this study, actual traditional and clinical pattern identification form and characteristics of symptom complex of dyspnea and symptom complex of wheezing asthma were recognized. Modern case report utilizing in clinical application need to be secured and an incurable disease asthma need to be diagnosed and improvement for treatments have to be searched through other case records.

A clinical study for Chuna Treatment on Neck pain (추나요법의 경항통 치료에 대한 임상적 고찰)

  • Objective : The study aims to find out the effects of Chuna treatment on neck pain. Methods: Out of 40 neck pain people, we divided 20 people in group A was treated Acupuncture and China, 20 people in group B was treated Acupuncture. We measured Visual Analog Scale(VAS) and symptom grade before and after treatment in each group's. Statistical analysis was performed by using SPSS program. Results : 1. Symptom grade III was the highest frequency before treatment in Group A and Group B. After treatment, grade 0 was The highest frequency in Group A But grade 0, II were the highest frequency in Group B. 2. Group A was Significantly decreased in VAS compared with Group B. Conclusion: These results suggest that Chuna treatment is useful on the patients of neck pain.

A Retrospective and Cross-sectional Study on Symptom Improvements in Benign Prostatic Hyperplasia Patients Treated with Electroacupuncture and Bee Venom Acupuncture (전침 및 봉약침치료를 시술한 양성전립선비대증 환자의 임상증상개선에 대한 후향적 단면연구)

  • Obejectives : To investigate the clinical effect of electroacupuncture and bee venom acupuncture for benign prostatic hyperplasia(BPH) symptoms. Methods : Among 39 outpatients who were treated at a BPH clinic, 8 were finally selected for analysis by charts-review, retrospectively. They were treated by electroacupuncture and bee venom acupuncture more than 3 times. We estimated the effectiveness of the treatments by using the International Prostate Symptom Score (IPSS) questionnaire and checking their nocturia frequency. On the basis of these variables, we analyzed the statistical significations of symptom improvements. Results : Among 8 patients, the IPSS and nocturia frequency showed significant improvement after the final treatment compared with baseline (P = .0078, P = .0078). The IPSS significantly decreased even after the 1st treatment and also after the 2nd treatment compared with the baseline (P = .0078, P = .0078). The IPSS at post final-treatment, post 1st- and post 2nd treatment also showed significant improvement compared with IPSS at baseline in 6 severe symptomatic patients(P = .031, P = .031, P = .031). Nocturia frequency decreased after the 1st treatment compared with baseline and showed significant improvement between baseline and post 2nd treatment (P = .0078). Conclusions : In a retrospective and cross-sectional study investigating the clinical effects of electroacupuncture and bee venom acupuncture on BPH, these treatments proved to have significant therapeutic effects, particularly for various symptoms involving frequent nocturia.

An investigation of the relationship between cutaneous allodynia and kinesiophobia, gastrointestinal system symptom severity, physical activity and disability in individuals with migraine

  • Background: To investigate the relationship between cutaneous allodynia (CA) and kinesiophobia, gastrointestinal system (GIS) symptom severity, physical activity, and disability, and to determine whether CA, pain, and disability were influencing factors for kinesiophobia, GIS symptoms, and physical activity in individuals with migraine. Methods: The study included 144 individuals with migraine. CA, kinesiophobia, GIS symptoms, physical activity level, and migraine-related disability were evaluated with the Allodynia Symptom Checklist, the Tampa Kinesiophobia Scale (TKS), the Gastrointestinal Symptom Rating Scale (GSRS), the International Physical Activity Questionnaire-7, and the Migraine Disability Assessment Scale (MIDAS), respectively. Results: The CA severity was only associated with TKS (r = 0.515; P < 0.001), GSRS-total (r = 0.336; P < 0.001), GSRS-abdominal pain (r = 0.323; P < 0.001), GSRS-indigestion (r = 0.257; P = 0.002), GSRS-constipation (r = 0.371; P < 0.001), and MIDAS scores (r = 0.178; P = 0.033). Attack frequency (P = 0.015), attack duration (P = 0.035) and presence of CA (P < 0.001) were risk factors for kinesiophobia. Attack frequency (P = 0.027) and presence of CA (P = 0.004) were risk factors for GIS symptoms. Conclusions: There was a relationship between the CA and kinesiophobia, GIS symptoms, and disability. CA and attack frequency were found to be risk factors for kinesiophobia and GIS symptoms. Migraine patients with CA should be assessed in terms of kinesiophobia, GIS, and disability. Lifestyle changes such as exercise and dietary changes and/or pharmacological treatment options for CA may increase success in migraine management.

A Comparative Study on Climacteric Symptom and Health Behavior in Climacteric Women according to Hormone Replacement Therapy(HRT) (호르몬 대체요법실시 갱년기 여성과 비실시 여성의 갱년기 증상과 건강행위 비교)

  • Purpose : This study was designed to describe the relationship on climacteric symptom and health behavior according to hormone replacement therapy (HRT) in climacteric women. Method : Data were collected through self-reported questionaries which were constructed to include a climacteric symptom and health behaviors of climacteric women. The subjects for this study were 135 climacteric women between 45-65 years of age living in Kuri city. Among them. 65 were in the experimental group and the other 70 were in the control group. The data were analyzed by descriptive statistics, t-test, $x^2-test$, ANOVA, and Pearson Correlation Coefficient. Results : The results were as follows 1) There was no significant difference on the climacteric symptom between the group of women receiving HRT and those who did not (t=1.973, p=0.162). 2) The mean health behavior score for the group of women receiving HRT was significantly higher than those who did not(t=2.557, p=0.042). 3) The climacteric symptom and health behavior were positively correlated (r=.289, p=0.015) in women receiving HRT, while these were negatively correlated (r=-.242, p=0.043) in women without HRT. 4) Women in the HRT group showed higher climacteric symptom than the those In the control group in most items. 5) The mean score for health behavior was 2.76 in the hormone replacement therapy group and 2.35 for the control group. 6) For women in the experimental group, climacteric symptom was significantly different by the level of education (F=7.371, p=.001). 7) For women in the control group, climacteric symptom was significantly different by the number of children (F=2.873, p=.044), the level of education (F=5.616, p=.006), the number of abortion (F=3.231, p=.015), and the status of menstruation (F=4.129, p=.020). 8) For women in the experimental group, health behavior was significantly different by the level of education (F=7.351, p=.001), frequency of exercise (F=8.106, p=.000), and the status of menstruation (F=3.446, p=.032), 9) For women in the control group, health behavior was significantly different by frequency of the exercise (F=7.381, p=.001). Conclusion : This study results indicated that there was no association between the HRT and climacteric symptoms, while there was positive correlations between the HRT and health behavior. Therefore. it is assumed that HRT increases the positive health behavior of women.

  • This study is a descriptive survey to explore the health lifestyle of adults. The study subjects are teachers of elementary. middle and high school. and staffs of research institutes located in Chungchung Province and Daejon city. The data was collected from Jan. to march 1997 through self reporte for structured questionnaire. Fantastic check list of Wilson and Ciliska for Health Lifestyle Assessment and DSM-III-R for somatic symptom were used as tools. Data was analysed by frequency, $X^2_test$, t-test and Anova using SAS program. The results are as follows: 1. There were statistically significant differences In drinking(t=7.75, P=.000), exercise(t=-2.99, P=.003)and interpersonal relationship(t=2.22, P=.027) among 10 health lifestyle between smoking group and non-smoking group, in drinking(t=17.98, P=.000), exercise(-4.71. P=.000), and job satisfaction(t=2.22, P=.027) between drinking group and non-drinking group, and in eating habit(t=-2.00, P=.045), drinking (t=4.47, P=000), exercise (t= -16.49, P=000), keeping traffic law(t= -2.68, P=.007), personality (t= -2.05, P=.040) and anxiety/depression(t=-3.47, P=.000) between exercise group and non-exercise group. 2. There was statistically significant difference in cardiovascular symptom(F=4.22, P=.0l) among somatic symptoms of subjects according to exercise level. 3. There was statistically significance difference in lifestyle according to smoking level(F=, 3.33, P=.011), drinking level(F=9.17, P=.0001) and exercise level(F=11.93, P=.000l), and in somatic symptom according to sex(t=-3.93, P=.0001), weight(F=3.83, P=.022), exercise level (F=3.29, P=.03) among general characteristics. 4. There was statistically significant difference between sex in general (t= -3.64, P=.0001), gastrointestinal(t=-2.21, P=.02), musculoskeletal(t=-3.92, P=.001), and total symptom (t= -3.92, P=.0001). 5. There was statistically very highly signigicant difference In weight according to smoking($x^2=25.18,\; P=.001)$ and exercise$(x^2=16.46,\; P=001)$. 6. There was statistically significant difference in frequency between smoking group, drinking group and exercise group$(x^2=24.52,\;P=.001)$. Among a number of habit, smoking, drinking and exercise are important factors of human health to prevent related disease morbidity and death. It is essential for industrial health nurse to committ in this subject considering the influence of those factors and lifestyle on health. There is also a relationship of weight with smoking and exercise, the frequency of overweight/obesiy in smoking/ no-exercise group were high. It is quite necessary for the people having cardiovascular symptom to exercise to lower morbidity and mortality. The industrial health nurse has to keep In mind on this point and consider of time and facilities of fitness of employee. It needs to explore the cause by further research on somatic symptom of women. This research shows that concerning the relationship between smoking, drinking, and exercise, health care provider must take not only management of disease, but health behaviors and lifestyle into consideration.

A Study on the Relationship between the Present Physical Symptom Distress and Experience of Sanhujori, the Traditional Postpartal Care in Korea - Centered on Women of arthritis - (관절염 여성의 신체적 불편과 산후조리 경험정도와의 관계 연구)

  • This correlational descriptive study sought to define the relationship between the experience of Sanhujori, Korean tradition non-professional postpartal care after delivery and abortion and present physical symptom distress of arthritis female who visited to outpatient clinic of rheumatic internal medicine at three hospital located in Seoul, Pusan, Chongju, Korea. Data from a convenience sample of 98 women who orally agreed to be respondent were collected from September 1, 1998 to October 31, 1998 for two months by way of interview with semi-structured questionnaire. Data analysis consisted of frequency, percentage, mean, S. D., Pearson Correlation Coefficient, t-test, ANOVA and Scheffe test as a post hoc by SPSS. The results of the study were as follows ; Mean age of participants as 52.8 years and mean number of children 3.3. Mean frequency of child birth was 3.1 times per woman, 67.4% of respondents had menopause, 57.0% did not have Sunhujori after abortion. The health status implies the subjective health status women perceived, which came from the three points of view of the present, comparative with other of same age and changed after delivery. The respondents of 76.1% perceived them as unhealthy and the main sites of physical symptom distress were upper & lower extremities including knee and hand 34.8%, shoulder 26.5%, waist 22.4%. Women perceived the etiology of the arthritis as 'did Sanhujori wrongly' 36.7%, 'aging process' 24.5%, 'stress' 16.3%, 'overwork' 15.3%, 'Immunocompromize' & 'physical constitution' 7.1% respectively, 'character' 3.1%, 'genetic' 2.0%, 'malnutrition' 1.0%. The mean period of Sanhujori after delivery was 20.4 at the first child and 18.1 at the second child. The higher frequency of child birth, the shorter period of Sanhujori. For the subjective evaluation of whether the women did Sanhujori well or not, the rate of 'did Sanhujori well' was the highest rank in the first child and the rate of 'did not particularly Sanhujori well' was the highest rank in the last child. There was a significant positive correlation between physical function disability and rheumatoid arthritis symptom at the level of 1% of significance statistically(r=.406). And a positive correlation between physical function disability and Sanhubyung symptom at the level of 5% of significance statistically(r=.224). There was a significant positive correlation among rheumatoid arthritis symptom, Sanhubyung symptom and menopause symptom at the level of 1%-5% of significance statistically. Most of all, the correlation between Sanhubyung symptom and menopause symptom was the higher than others. There was a negative correlation between the present physical symptom distress and experience of Sanhujori(r=.-130), however it was not significant statistically. However, there was a positive correlation between subjective health status and experience of Sanhujori at the level of 1% of significance statistically(r=.328). In conclusion, this finding reconfirmed the positive relationship between the perceived health status and experience of Sanhujori after delivery among women of arthritis. It provides a challenge to the professional care givers to study further on the effects of Sanhuiori after abortion or delivery on the physical symptom distress from the variouis aspects through the cross-sectional and longitudinal research. The strategy for the development of the appropriate intervention for primary prevention of sequele after childbirth and quality of care for desirable health outcomes for postpartal women with considering deeply on the relationship between women's health and postapartal care.

Retrospective Study on Overactive Bladder (과민성 방광에 대한 임상 결과 분석 : 후향적 연구)

  • Purpose: The aim of this study was to estimate the effect of Korean medicine treatment on overactive bladder. Methods: 64 patients of overactive bladder received Korean medicine treatment for at least 30days. We studied retrospectively symptom disappearance rate, the relationship between the improvement rate and age, scale of symptoms, BMI(Body Mass Index), WHR(Waist Hip Ratio). Results: Symptoms of 27 patients were disappeared, and those of 28 patients were improved. But 9 patients didn't show the improvement of their urination problems. The symptom disappearance rate was 43.08%. The improvement rate was 85.15%. The mean${\pm}$standard deviation of daily frequency was reduced from 10.05128 to 7.44872 after treatment(p<0.0001) The symptom disappearance rate was reverse-related with the OABSS score, KHQ and WHR. The improvement rate of married patients was higher than unmarried patients. And especially that was co-related with the frequency of acupuncture and moxibustion treatment. Conclusion: The results of this study suggested that Korean medicine treatments, such as acupuncture, moxibustion and herbal prescriptions have positive efficacy on overactive bladder patients in ordinary clinical practice.
