입원화상환자의 특성과 항생제 사용 현황 (An Evaluation of Antibiotic Use in the Hospitalized Burn Patients)

  • Infection is one of the main causes of death in severe burn cases. Treatment of burn patient includes fluid therapy, wound care, complication care and antibiotic therapy for infection. The objective of this study was to evaluate the characteristics of burn patients, the type of isolated microorganisms and their susceptibility, and the systemic antibiotics used. This is a retrospective study of 126 burn patients treated in the Hanil General Hospital from January to December 2001. Total 126 patients were assessed with 103 males and 23 females (4.5:1). The average age was $34.8\pm17.6$ years and extent of burn $(TBSA\;\%)\;was\;24.5\pm18.5\%$. The burn was caused by electric accident $(47.6\%),\;flame\;(29.4\%),\;scalding\;(21.4\%),\;and\;chemical\;accident\;(1.6\%)$. The overall mortality rate was $7.14\%$ (9/126) and all expired patients were males. The average age (n=9) was $48.8\pm15.6$ yrs and the extent of burn was $65.0\pm19.0\%$. The causes of death were due to flame burns $(13.5\%)$ and electric burns $(6.7\%)$. The culture sites of the isolated microorganisms were wound $(85.3\%),\;sputum\;(9.3\%),\;urine\;(2.7\%),\;blood\;(1.3\%)\;and\;catheter\;tip\;(1.3\%)$. Pseudomonas aeruginosa was the most commonly isolated organism $(35\%)$, followed by Staphylococcus aureus $(30.1\%)$, Acinetobacter baumannii $(21.4\%)$, and Enterococcus spp. $(3.9\%)$. The number of systemic antibiotics administered was 4.5. The classes of the antibiotics were cephalosporines $(38.5\%)$, aminoglycosides $(31\%)$, quinolones $(13.3\%)$, penicillins $(12.4\%)$, carbapenems $(2.4\%)$, glycopeptides $(1.9\%)$ and others $(0.6\%)$. In conclusion, most of burn patients had wound infection and Pseudomonas aeruginosa was the most commonly isolated organism. Cephalosporins were administered the most frequently among antibiotics.

건유기 유방염 치료제 Nophazin리 야외응용시험 (A Field Trial as Dry-cow Therapy Using Nophazin (norfloxacin mixed cephalexin) in Mastitis Control)

  • A field test using Nophazin (drying off period therapeutics) was done from May, 1989 to October, 1989. 299 quarters of 75 cows from 28 herds were tested. The results were summarized as follows. 1. At the day 7 after parturition, CMT-was 81.60%, clots were removed perfectly, removal rate of hard, hot, pain was 99.6%, only one hard 0.35% was maintained. Recovery rate of general symptoms was 100%. 2. Recovery rate in Nophazin treatment was 100% in Micro-coccus SPP, Corynebacterium SPP, and unisolated, 97.06% in Enterobacterid SPP, 95.58% in Staphylococcus SPP, 91.67% in other Bacilli, 89.19% in Streptococcus SPP, Overall recovery rate was 95.32%, And fungi did not react. 3. The ratio of isolated microorganisms was Staphylococcus SPP 49.8%, Streptococcus SPP 16.3%, Enterobacteria SPP 15.0%, Micrococcus SPP 7.9% Corynebacterium SPP 4.4%, Pseudomonas SPP 2.6%, Bacilli 2.6% and Fungi 1.3%. 4. The range of sensitivity of each microorganism to therapeutics is as follow : Staphylococcus SPP 61.1∼94.7%, Streptococcus SPP 51.4∼91.9%, Micrococcus SPP 44.4∼94.4%, Corynebacterium SPP 70∼80%, Enterobacteria SPP 11.8∼88.2%, and other Gram negative bacilli 8.3∼83.3%. Overall sensitivity was in the range of 50.4∼92.0%, Microorganisms showed the highest sensitivity (80∼94.7%) to Nophazin. 5. New intramammary infection rate was 3.96% (9 strains) at paturition, 8.37% (19 strains) at the day 7 after paturition. Persistent organism wits 6.17% .(14 strains). 6. There was no side effect of Nophazin treatment.

근관내 주요 혐기성 병인균에 대한 수종 항생제와 근관충전용 세멘트의 항균효과에 관한 연구 (ANTIMICROBIAL EFFECT OF ANTIBIOTICS AND ROOT CANAL CEMENTS ON THE PREDOMINANT PATHOGENIC ANAEROBIC MICROFLORA IN ROOT CANALS)

  • The purpose of this study was to evaluate the susceptibility of anaerobic microorganisms to certain antibiotics and root canal cements. Prevotella intermedia(Bacteroides intermedius) ATCC 25611(serotype A), Fusobacterium nucleatum ATCC 25586, Actinomyces viscosus ATCC 15987 which are the predominant pathogenic anaerobes in dental root canals were cultured in BHI for 48 hours(Fig.1). After each $200{\mu}l$ of those broths with microorganisms was streaked on each surface of blood agar plate, 2 to 5 antibiotic discs which are impregnated with Tetrncycline, Erythromycin, Ampicillin, Clindamycin, or Vancomycin were applied on each surface of blood agar plate and cultured for 5 days anaerobically in the anaerobic chamber (Fig.2). 15 antibiotic discs for each kind of antibiotics and each species of microorganisms were tested. Also each kind of root canal cement tubes which include Zinc oxide eugenol cement, Zinc phosphate cement, Calcium hydroxide powder+DD.W., Calcium hydroxide paste(Pulpdent Tempcanal), or Vitapex(Table 1) were applied on the inoculated BAPs after $200{\mu}l$ of each experimental species of microorganisms was streaked on the surface of blood agar plates, and they were cultured for 5 days anaerobically in the anaerobic chamber(Fig.3). The sensitivity(antimicrobial effect) was determined by the diameter of the inhibition zone. The results are as follows: 1. The results of antibiotic susceptibility test(Table 2) 1) All of the tested antibiotics had antimicrobial activity with various degrees. 2) In Prevotella intermedia (old Bacteroides intermedius), the diameter of inhibition zone to Erythromycin($37.87mm{\pm}2.20$) was largest, those to Tetracycline($26.20mm{\pm}2.96$), Vancomycin($21.53mm{\pm}1.96$), Clindamycin($18.73mm{\pm}0.96$) was smaller than former orderly, and That to Ampicillin ($7.87mm{\pm}0.83$) was smallest. 3) In Actinomyces viscosus, the diameter of inhibition zone to Erythromycin($28.73mm{\pm}1.22$) was largest, those to Ampicillin($21.73mm{\pm}1.03$), Clindamycin($21.33mm{\pm}1.59$) was similarly next order, that to Vancomycin($19.00mm{\pm}1.96$) was smaller than Clindamycin, and that to Tetracycline($11.93mm{\pm}0.70$) was smallest. 4) In Fusobacterium nucleatum, the diameter of inhibition zone to Ampicillin($31.07mm{\pm}1.91$) was largest, that to Erythromycin($28.87mm{\pm}0.92$), Clindamycin($20.47mm{\pm}1.51$), Vancomycin ($16.73mm{\pm}0.96$), Tetracycline ($12.13mm{\pm}1.06$) are smaller than former orderly. 2. The results of root canal cements and pastes(Table 3) 1) The external diameter of tube is 4mm, so 4mm of the inhibition zone diameter means non-susceptable. Prevotella intermedia (old Bacteroides intermedius) was non-susceptable to Calcium hydroxide powder+D.D.W., Calcium hydroxide paste(pulpdent Tempcanal), and Actinomyces viscosus was non-susceptable to Zinc phosphate cement, Calcium hydroxide powder + D.D.W., Calcium hydroxide paste(pulpdent Tempcanal). 2) In Prevotella intermedia (old Bacteroides intermedius), the diameter of inhibition zone to Zinc oxide eugenol cement($13.67mm{\pm}3.30$) was largest, that to Vitapex($9.20mm{\pm}2.96$), Zinc phosphate cement($6.13mm{\pm}2.07$) was smaller than former. 3) In Actinomyces viscosus, the diameter of inhibition zone to Zinc oxide eugenol cement($17.40mm{\pm}5.20$) was largest and that to Vitapex($8.80mm{\pm}1.70$) was next order. 4) In Fusobacterium nucleatum, the diameter of inhibition zone to Vitapex($42.33mm{\pm}17.2$) was largest and those to Calcium hydroxide paste(Pulpdent Tempcanal)($14.47mm{\pm}3.72$) and Zinc oxide eugenol cement($8.93mm{\pm}2.71$), Zinc phosphate cement($8.20mm{\pm}2.27$), Calcium hydroxide powder+D.D.W.($5.53mm{\pm}2.10$)was next orderly. And then In Zinc oxide eugenol cement and Zinc phosphate cement group, two of fifteen samples showed no inhibition zone, in Calcium hydroxide powder + D.D.W. group, 8 of 15 samples showed no inhibition zone.

요로감염의 주요 원인균과 항생제 감수성의 변화에 관한 고찰 (Changes in Causative Organisms and Antimicrobial Susceptibility of the Urinary Tract Infection)

  • 요로감염은 병원 내 감염의 주요한 부분이며 최근 항생제의 남용 등으로 인해 요로감염에서의 효과적인 항생제의 선택이 제한받고 있다. 연구자들은 본 병원을 방문하여 시행한 요배양검사에서 의미있는 균이 동정된 외래 및 입원 환자의 요로감염의 주요 원인균과 항생제 감수성을 조사하여 요로감염의 치료에 도움을 주고자 하였다. 2010년 1월 1일부터 2015년 12월 31일까지 6년간 본 병원에서 시행한 요배양검사에서 의미 있는 원인균이 동정된 외래 및 입원환자를 대상으로 하였으며 균집락 단위(colony forming unit: CFU)가 $10^5/ml$이상인 환자를 대상으로 하였다. 배양된 동정균주에서 가장 흔한 원인균은 E.coli로 28.1%의 비율이었다. E.coli의 경우 남성에서는 2010년부터 2015년까지 빈도의 차이는 없었으나 여성의 경우 2014년 이후 그 빈도가 점차 감소하였다. 항생제 감수성 결과 가장 흔한 균주인 E.coli의 감수성 검사에서는 ampicillin 72.2%, TMP/SMX (trimethoprim/sulfamethoxazole) 44.9%, ciprofloxacin 41.3%의 내성율을 보였으며, cefazolin 12.4%, 2,3,4세대 cephalosporin에는 5% 이하의 낮은 내성율을 보였다. 요로감염의 원인균은 시간이 지날수록 다양해지고 빈도 또한 변화를 보인다. 따라서 TMP/SMX와 ciprofloxacin 등 기존 항생제에 대한 내성도 또한 비교적 높은 것으로 나타나 향후 요로감염에 대한 경험적인 항생제의 사용에 대해 더욱 신중한 선택을 해야 할 것으로 생각한다.

백두옹 추출물의 치주 병인균에 대한 항균효과 (The Antimicrobial Effect of Pulsatilla Koreana Extracts to Oral Micro-Organism)

  • Gingivitis and periodontitis are infectious diseases in that microorganisms are the primary extrinsic cause of the diseases. the occurrence of gingivitis has been associated clearly with the presence of microorganisms at the disease site, and the histologic nature of the tissue involved is indicative of an inflammatory response induced by microorganisms. additional evidence for the microbial etiology of periodontal disease is that numerous antimicrobial agents are effective in reducing plaque accumulation and periodontal diseases. the purpose of this article is to analyze the antimicrobial effects of Pulsatilla koreana. Well-dried Pulsatilla koreana purchased from herbs distributor was ground and extracted into methanol(MeOH), ethylacetate(EtoAc), chlorform($CHCl_3$) and Butyl alcohol(BuOH). we have then applied each solution to the bacteria samples(Bacteroides forsythus, Streptococcus mutans, Streptococcus sanguis, Porphylomonas gingivalis, Actinobacillus actinomycetemcomitans, Eikenella corrodens, Prevotella intermedia, Actinomyces viscosus, Prevotella nigrescens , Rothia dentocariosa, Fusobacterium nucleatum, Pseudomonas aeruginosa, Staphylococcus aureus) collected from several organizations. To conduct susceptibility test(Kirby-Bauer method), plate contained each periodontopathic bacteria is spread extracted into methanol(MeOH), ethylacetate(EtoAc), chlorform($CHCl_3$) and Butyl alcohol(BuOH) and to measure the minimum inhibition concentration(MIC) of the bacteria against the solutions to ultimately determine antimicrobial effects of the solutions, insert bacteria sample into $20{\mu\ell}/{m\ell}$, $10{\mu\ell}/{m\ell}$, $5{\mu\ell}/{m\ell}$, $2.5{\mu\ell}/{m\ell}$ of each solution and control group(not contained solution) 1. Solution extracted into methanol did not show clear zone against all bacteria samples. Only P.nigrescens, S. mutans and S. sanguis in solution extracted into ethylacetate, S. mutans and S. anguis in solutions extracted into chlorform and Butyl alcohol showed clear zone against all bacteria samples. Solution extracted into Butyl alcohol showed clear zone against 13 types of bacteria, excluding P. gingivalis. 2. In Solution extracted into methanol, the bacteria samples grew in the highest concentrated plate, showing minimal variation from control group. 3. In Solution extracted into Butyl alcohol, S. aureus, P. intermedia, E. corrodens, A. actinomycetemcomitans, B. forsythus, P. gingivalis et al. showed decreased growth in the highest concentrated plate. P. auruginosa, R. dentocariosa, A. viscosus, P. nigrescens, S. mutans et al. showed decreased growth at MIC $20{\mu\ell}/{m\ell}$ and S. sanguis showed decreased growth at MIC $10{\mu\ell}/{m\ell}$. 4. By analyzing the MIC level through considering the results from Kirby-Bauer method, Solution extracted into methanol did not reveal any antimicrobial effects and Solution extracted into Butyl alcohol showed the highest antimicrobial effects In conclusion, it can be used the extracts of Pulsatilla koreana as wide spectrum antimicrobial agent.

양식 붉바리 장관에서 분리된 미생물의 프로바이오틱 잠재력 평가 (Evaluation of the Probiotic Potential of Microorganisms Isolated from the Intestinal Tract of Cultured Epinephelus akaara)

  • 본 연구에서는 제주도 양식장에서 사육되고 있는 붉바리 장내에서 균주를 분리·동정하여, 산업적으로 활용할 수 있는 프로바이오틱스의 잠재적 가능성을 평가하였다. 붉바리 장내에서 순수 분리된 129균주를 대상으로 1차적으로 자체적인 간이 항균활성 실험을 통해 clear zone이 10 mm 이상 되는 12균주를 선별하여 내담즙성, pH 농도별 내산성, 항산화 활성, 항생제 내성, API kti을 이용한 생리생화학적 특성 실험에 이용하였다. 각 특성 실험을 통해 probiotics으로 잠재적 가능성을 가진 균으로 선발되어진 미생물은 G1, G3, G15, G21, B1, B2, B3, B5 총 8종이다. 8종 중 B균주 그룹(B1, B2, B3, B5)은 G균주 그룹(G1, G3, G15, G21)보다 유의하게 높은 활성을 나타내었다. 선발되어진 미생물의 16S rRNA 염기서열 분석을 통한 계통 분석을 한 결과를 바탕으로 B1 균주는 L. paracasei B1, B2는 L. lactis B2, B3 균주는 Lb. plantarum B3, B5 균주는 L. lactis subsp. hordniae B5, G1은 B. licheniformis G1, G3는 B. velezensis G3, G15는 B. frigoritolerans G15, G21 균주는 B. pumilus G21로 명명하였다.

Current Diagnostic Methods for Periprosthetic Joint Infection

  • Total joint arthroplasty is a successful joint replacement treatment that improves joint function and overall quality of life and provides pain relief. However, the prevalence of periprosthetic joint infection (PJI) has become prevalent with the rise in the incidence of arthroplasty surgery. PJI occurs rarely following arthroplasty however presents with serious complications, including high morbidity. The identification of causative microorganisms is essential for the treatment of PJI. Managing PJI requires complex treatment strategies, including long-term antibacterial treatment, and significant medical costs can be incurred. The American Academy of Orthopedic Surgeons, the Centers for Disease Control and Prevention, and Surgical Care Improvement Project guidelines recommend that prophylactic antibiotics such as first-generation cephalosporins be infused completely 1 hour before surgical incision. However, these preventative antibiotics are very limited, therefore risk factors must be identified to diagnosis and treat patients effectively. Moreover, determining antimicrobial susceptibility during artificial joint surgery and choosing the most appropriate treatment strategy following an accurate diagnosis of microbial infections are essential. In the present review, we describe the management, including the etiology, diagnosis, and classification of PJI, and approaches to its diagnosis using the available novel molecular diagnostic methods.

Probiotic Potential of Pediococcus acidilactici and Enterococcus faecium Isolated from Indigenous Yogurt and Raw Goat Milk

  • Probiotics are live microorganisms that, when administered in adequate amounts, confer health benefits to the host. This study was conducted for the isolation of potential lactic acid bacteria (LAB) with probiotic properties from goat milk and yogurt. Several tests were conducted in vitro using the standard procedures for evaluating the inhibitory spectra of LAB against pathogenic bacteria; tolerance to NaCl, bile salt, and phenol; hemolytic, milk coagulation, and bile salt hydrolase activities; gastrointestinal transit tolerance; adhesion properties; and antibiotic susceptibility. Among 40 LAB strains screened according to culture characteristics, five isolates exhibited antagonistic properties. Three were identified as Pediococcus acidilactici, and two were identified as Enterococcus faecium, exploiting 16S rRNA gene sequencing. All the isolates succeeded in the gastrointestinal transit tolerance assay and successively colonized mucosal epithelial cells. Based on the results of these in vitro assays, both P. acidilactici and E. faecium can be considered as potential probiotic candidates.

단삼 추출물의 세포독성과 항균효과 (Cytotoxicity and Antimicrobial Effects of Extracts from Salvia miltiorrhiza)

  • This study was carried out to evaluate cytotoxic effects of Salvia miltiorrhiza extracts on NIH 3T3 fibroblasts and KB cell lines. Disruptions in cell organelles were determined by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay. The comparison of $IC_{50}$ of values of Salvia miltiorrhiza extracts in KB cell lines showed that their susceptibility to these extracts decreased in the following order: hexane extract > chloroform extract > methanol extract> dichloromethane extract > ethyl acetate extract>ethanol extract by the MTT method. The dried roots of Salvia miltiorrhiza was extracted several solvents, and then antimicrobial activity was investigated. The minimal inhibitory concentrations (MIC's) of the extract against microorganisms were also examined. Amtimicrobial activity of ketoconazol as reference was compared to those of extracts of hexane, chloroform, dichloromethane, ethyl acetate, ethanol and methanol. The antimicrobial activity of all extracts from the sample had growth inhibition activity against gram-negative bacteria, gram-positive bacteria and fungi. These results suggest that the hexane and chloroform soluble extracts of Salvia miltiorrhiza may be a valuable choice for the studies on the tumor cell lines and growth inhibition activity.

Antimicrobial Effect of Medical Adhesive Composed of Aldehyded Dextran and ${\varepsilon}$-Poly(L-Lysine)

  • Infection of surgical wounds is a severe problem. Conventional tissue reattachment methods have limits of incomplete sealing and high susceptibility to infection. Medical adhesives have several advantages over traditional tissue reattachment techniques, but still have drawbacks, such as the probability of infection, low adhesive strength, and high cytotoxicity. Recently, a new medical adhesive (new-adhesive) with high adhesive strength and low cytotoxicity, composed of aldehyded dextran and ${\varepsilon}$-poly(L-lysine), was developed. The antimicrobial activity of the new-adhesive was assayed using agar media and porcine skin. In the agar diffusion method, inoculated microorganisms that contacted the new-adhesive were inactivated, but this was not dependent on the amount of new-adhesive. Similar to the agar media results, the topical antimicrobial effect of new-adhesive was confirmed using a porcine skin antimicrobial assay, and the effect was not due to physical blocking based on comparison with the group whose wounds were wrapped.
