Although the number of anterior cruciate ligament reconstruction is increasing, complications after primary ACL reconstruction are more difficult to determine. Intraoperative and postoperative complications can lead to ultimate failure of a primary reconstructive procedure. Therefore, surgical success in ACL reconstruction requires detailed knowledge and technical advancements about ACL reconstruction. Preoperatively surgeon must pay attention to selection of grafts and methods of fixation, and intraoperatively, attention to the harvest of graft, passage of graft, intraarticuar placement of the graft, notchplasty, proper tensioning of the graft, and others. Postoperative complications must be detected early, including infection, abnormal healing responses, arthrofibrosis, graft rejection, and reflex sympathetic dystrophy. Careful patient selection, appropriate surgical timing, careful surgical technique, and supervised preoperative and postoperative rehabilitation can minimize postoperative complications.
Surgery remains the standard treatment for resectable thoracic esophageal cancer but that is justified only when acceptably low morbidity and mortality rates can be achieved even for advanced disease. The appropriate extent of resection to achieve the best outcome is the most controversial aspect of treating esopahgeal cancer and the optimal excent of lymphadenectomy also remains controversial. Until data from prospective randomized trials showing a clear benefit using a particular approach are available, surgeons should perform their better judgement in individualizing and selecting the most appropriate surgical approach to provide the best chance of cure or lasting palliation for this disease that challanges both patient and surgeon. Further studies are necessary to examine the utility and efficacy of each approach in treating thoracic esophageal cancer either alone or in combination with other modalities.
MMinimally invasive surgery (MIS) for early stage lung cancer has been an important treatment modality. However, the ergonomic discomfort and counterintuitive instruments hindered the application of video-assisted thoracic surgery (VATS) to more advanced procedures. To improve the compliance with MIS, robotic surgery was adopted. This advance aimed to alleviate the shortcomings of VATS by maximizing the comfort of the surgeon while providing instruments that enabled technically demanding operations and three-dimensional views with increased freedom for intrathoracic movement owing to EndoWrist$^{(R)}$. In this session, we introduced the clinical applications and its results of robot-assisted thoracic surgery in the field of lung cancer surgery. In conclusion, robot-assisted pulmonary resection with lymph node dissection for lung cancer is safe as well as feasible, and it results in a satisfying postoperative outcome. Robot-assisted surgery may provide a good alternative to conventional open or thoracoscopic surgery for lung cancer, provided that the cost effectiveness and long-term prognosis are confirmed.
Objectives The purpose of this article was to review the video-assisted mediastinoscopic surgery (VAM) as diagnostic or therapeutic tools. Summary The VAM was indicated in diagnosis of pleural diseases, dissection or sampling of mediastinal or paraesophageal lymph nodes, nodal staging in lung cancer, or various therapeutic approaches. The advantages of VAM were included that was less painful, cosmetic merit, no need additional drain, and no effect of cardiopulmonary function. An uncommon case of a patient with recurrent mandibular ameloblastoma involving various adjacent soft tissues is presented with plain film radiography, computed tomography and magnetic resonance imaging (MRI) and bone scan. The tumor involved mandible and eroded several bones including foramen ovale. This case shows that although an ameloblastoma primarily affect mandible, exact radiographic evaluation of tumor extent will assist the surgeon in the progress of rational approaches to the management of ameloblastoma.

The simultaneous surgical correction of bilateral cleft lip and nasal deformity has become a more common surgical technique that has greatly changed conventional strategies for secondary nasal correction. Mulliken has been known as one of the earliest proponents for the synchronous repair of bilateral cleft lip and nasal deformity, and he emphasized the responsibility of the treating surgeon to evaluate nasolabial growth by comparing anthropometric measurements with age-matched normal patients. Good outcomes from this surgical method have been reported in clinical cases worldwide. Herein, we describe the management of two cases of bilateral cleft repair, following the principles and methods established by Mulliken. We also provide a relevant review of the literature.
Surgical resection of lung cancer or other disease is recently required in patients with severely impaired lung function resulting from chronic obstructive pulmonary disease or disease extension. So prediction of pulmonary function after lung resection is very important in thoracic surgeon. We studied the accuracy of the prediction of postoperative pulmonary function using perfusion lung scan with 99m technetium macroaggregated albumin in 22 patients who received the pneumonectomy. The linear regression line derived from correlation between predicting[X and postoperative measured[Y values of FEV1 and FVC in patients are as follows: 1 Y[ml =0.713X + 381 in FEV1 [r=0.719 ,[P<0.01 2 Y[ml =0.645X + 556 in FVC [r=0.675 ,[P<0.01 In conclusion,the perfusion lung scan is noninvasive and very accurate for predicting postpneumonectomy pulmonary function.
