청소년들의 자살차원에 미치는 영향요인 (The Influence Factors of Adolescents' Suicide Dimension)

  • The purpose of this study is to find out the effects of the socio-demographic characteristics, self-recognition factors, and health behavior factors of Korean adolescents on suicide dimension, and grasp the effects of suicide thinking of adolescents on attempting suicide. This study used the eighth primitive data of the "Web-based Survey on Adolescents Health Behavior(2012)", and targeted 72,228 actual test respondent(93.8%). In conclusion, risk factors that greatly influence the suicide dimension of adolescents can be summarized into level of happiness, level of stress, depression, experience drinking, experience smoking, experience with drugs, suicide thinking, etc. and considering these risk factors, a preparation for preventing adolescents' suicide should be established, and along with a regular education, developing and expanding a treatment program for preventing recurrence afterwards of adolescents who have experience attempting suicide are necessary.

  • Gene-environment interactions are important in pathogenesis of suicide or suicidal behavior. Twin and adoption studies and family studies show that genetic factors play a critical role in suicide or suicidal behavior. Given the strong association between serotonergic neurotransmission and suicide, recent molecular genetic studies have focused on polymorphisms of serotonin genes, especially on serotonin transporter and tryptophan hydroxylase genes. Some studies have revealed a significant interaction between s allele of the serotonin transporter gene and the risk of suicide attempt associated with childhood trauma. In addition, the polymorphism of brain-derived neurotrophic factor gene also may influence the effect of childhood trauma in relation to the risk of attempting suicide. Future studies should explore genetic and environmental factors in suicide or suicidal behavior and examine for gene and environment interaction.

대학생의 자살관련행동 및 예방대책 (Suicide-Related Behaviors among College Students and suicide prevention)

  • 본 연구는 대학생의 자살행동(자살생각, 자살계획, 자살시도)의 실태를 파악하고 이에 영향을 미치는 요인을 규명하는 것으로 목적으로 시행된 서술적 조사연구이다. 분석을 위해 2012년 4월 2일부터 4월 21일까지 대전과 충청지역 대학생 421명을 대상으로 설문조사를 실시하였다. 이중 불성실한 응답자를 제외한 384부의 설문지를 분석하였으며, SPSS 통계 프로그램을 이용하여 분석하였다. 연구결과 대학생의 평생 자살생각률 32.3%, 자살계획률 4.7%, 자살시도율 3.0%이었으며 자살생각이 자살계획 및 자살시도를 예측할 수 있는 것으로 확인되었다. 자살생각, 자살계획, 자살시도에 공통적이며 주요한 위험요인으로 나타난 우울증에 대해 경도이상의 우울증을 경험하는 대학생이 42.2%였다. 이에 자살을 방지하기 위해서는 자살생각과 우울증의 관리가 요구된다. 자살방지를 위해서는 자살생각 및 우울증 있는 대상자의 선별검사의 제도화, 과거 자살시도자, 자살자 주위 사람에 대한 전문적 사후관리, gatekeeper 활용 및 자살위험행동에 대한 매뉴얼 개발, 사전관리를 위한 정신건강프로그램, 언론매체의 신중한 보도 등의 노력이 이루어져야 하겠다.

자살의 생물학적 기전 (Biological Mechanism of Suicide)

  • Suicide is a behavior that is intended to cause death by itself and requires medical treatment, resulting in suicidal attempt or completion. Suicide causes loss of life, damages the body, costs a lot of medical expenses, and causes families to fall into sorrow and suffering therefore this suicide is a huge loss to family and society. There have been attempts to reduce and prevent suicide by understanding the mechanism of suicide. The mechanism of suicide can be thought of as psychological mechanism and biological mechanism. In the past, if we considered the psychological and biological mechanisms separately, the development of neuroscience now connects and integrates these two. Psychological factors affect biological factors and biological temperaments also affect perception or thinking about the situation and increase psychological vulnerability. Distant factors in suicidal behavior-such as childhood adversity and family and genetic predisposition-increase the lifetime risk of suicide. They alter the response to stress and other processes through changes in gene expression and regulation of emotional and behavioral characteristics. Distant factors affect the biological system and consequently changes in these systems can increase the risk of suicide. In other words, the distal factor does not directly induce suicidal behavior but rather acts indirectly through developmental or mediating factors. These mediating factors are impulsive aggressive and anxious trait, and chronic use of substances. The mechanism of this disorder is the abnormality of the serotonin system and the abnormality of the lipid level. Proximal factors are associated with the onset of suicide events and include changes in the major neurotransmitter systems, inflammatory changes, and dysfunction of glial cells in the brain. A series of studies, including a variety of research methods and postmortem and in-vivo imaging studies, show the impairment of the serotonergic neurotransmitter system and hypothalamic-pituitary-adrenal axis stress response system for suicidal behavior. These disorders lead to suicidal behavior due to difficulty in cognitive control of mood, pessimism, reactive aggression, abnormality in problem solving abilities, excessive response to negative social signals, severe emotional distress, and cognitive dysregulation of suicidal ideation.

청소년의 자살예방을 위한 게이트키퍼 행동 영향요인 (The Influencing Factors on GateKeeper Behavior of Adolescents for Suicide Prevention)

  • 본 연구의 목적은 청소년 자살예방을 위한 게이트키퍼 행동 영향요인을 파악하는 것이다. 본 연구의 대상자는 D 광역시 소재 5개 고등학교에 재학 중인 남녀 학생이다. 수집된 자료의 분석은 SPSS 21.0 program을 이용하여 서술통계, t-test, ANOVA, Multiple regression을 수행하였다. 본 연구결과, 대상자의 게이트키퍼 행동은 성별(t=2.85, p=.005), 학교유형(t=-2.81, p=.006), 가족이나 친구의 자살경험 여부(t=-2.09, p=.038)에 따라 통계적으로 유의한 차이가 있었다. 또한 자살예방에 대한 인식(r=.23, p=.006) 및 지식(r=.42, p<.001)과 유의한 상관관계가 나타났다. 회귀분석결과, 자살예방에 대한 인식(${\beta}=.20$, p=.006)과 지식(${\beta}=.35$, p<.001)이 게이트키퍼 행동에 유의한 영향요인으로 나타났으며, 이들 변수는 게이트키퍼 행동에 대해 총 24.3%를 설명하였다. 따라서 본 연구결과를 토대로 청소년을 대상으로 자살예방에 대한 긍정적 인식과 지식 향상을 제공할 수 있는 중재 개발을 제언한다. 또한 추후 청소년의 자살예방을 위한 게이트키퍼 행동을 예측할 수 있는 다양한 변수를 확인할 수 있는 연구가 필요하다.

청소년의 자살사고(思考) 관련 요인 (The Associated Factors on Suicide Ideation among Adolescents in South Korea)

  • Purpose: Suicide was considered as a big public health concern in recent years because suicide mortality has been increased rapidly. This study was to investigate the prevalence and risk factors of suicidal ideation among adolescents in South Korea. Methods: The data of 2007 Youth Health Risk Behavior Web-based Survey collected by Korean Center for Disease Control were analyzed using logistic regression for this study. Results: The prevalence of suicidal ideation was 23.7% in South Korea. The risk factors of suicide ideation were depression (odds 8.43), perceived stress(odds 4.30), drug use (odds 2.47), sexual intercourse(odds 2.36), present smoking (odds 1.76), present alcohol drink (odds 2.39), feeling unhappiness (odds 2.89), living with none of parents (odds 1.71), gender(odds 1.72), health status (odds 1.66), and socioeconomic status (odds 2.0). Conclusion : Health risk behaviors and mental health status were related to suicide ideation. Health risk behavior such as smoking and drinking prevention programs and mental health promotion programs should be implemented to decrease suicide ideation among adolescents. Suicide, mental health and health risk behaviors should be prevented through comprehensive health promotion programs.

청소년의 자살시도에 영향을 주는 요인 (The Influencing Factors on Suicide Attempt among Adolescents in South Korea)

  • Purpose: Suicide has been considered a large public health concern in recent years because suicide mortality has been increasing rapidly. This study was done to investigate the prevalence and risk factors of a suicide attempt among adolescents in South Korea. Methods: The data of the 2006 Youth Health Risk Behavior web-based Survey collected by the Korean Center for Disease Control was analyzed using logistic regression for this study. Results: The prevalence of a suicide attempt was 5.2% in South Korea. The risk factors of a suicide attempt were suicidal ideation (odds 31.83), depression (odds 7.98), drug use (odds 4.67), currently smoking (odds 3.19), feeling unhappiness (odds 2.77), stress (odds 2.60), currently drinking alcohol (odds 2.39), sexual activity (odds 2.33), living with neither parent (odds 2.24), initial alcohol drinking by age 9 (odds 1.80), health status (odds 2.15), skipped breakfast (odds 1.75), disease (odds 1.65), and school records (odds 1.22). Conclusion: Suicide screening and prevention programs should be developed, considering these risk factors. Suicide prevention programs should be applied to the at risk population prior to a suicide attempt.

자살성향 측정척도들의 자살예측력에 대한 고찰 (A Review on Predictive Validity of Suicide Assessment Measures)

  • The increasing suicide rate is a serious problem in Korea. Because of increased awareness of suicidality as a problem and because completed suicide is the fourth leading cause of death, it is very important to assess the risk of suicide. The purpose of this review is to provide a systematic examination of predictive validity of measures of suicidal ideation and behavior. A number of instruments are described as useful for identifying individuals "at high risk" for suicidal behavior. However, the predictive validity for most suicide measures has not been established. The present review only includes suicide assessment instruments with published predictive validity. In addition to evaluating the suicide assessment with respect to predictive validity, the present review describes and summarizes the psychometric properties of each measure. In conclusion, because of the complexity of studying the risk of suicide and the paucity of well-designed studies, it is extremely difficult to compare and generalize these findings. In addition, only a few instruments, such as the Scale for Suicide Ideation, Suicide Intent Scale and the Beck Hopelessness Scale, have been found to be significant risk factors for completed suicide. Another problem in the field involves that there have been few suicide measures designed for elderly populations. Clearly, future research is needed to investigate the predictive validity of standardized measures for completed suicide, especially targeting elderly populations.

초등학생의 충동성과 자살행동의 관계에서 보호요인의 조절효과: 고학년을 중심으로 (The Moderating Effect of Protective Factors in the Relationship between Impulsivity and Suicide Behavior in Elementary School Students)

  • 본 연구의 목적은 초등학교 고학년을 대상으로 충동성과 자살행동(자살계획, 자살시도) 간의 관계에서 보호요인(개인요인, 가족요인, 또래요인, 학교요인)의 조절효과를 검증하는 것이다. 이를 위하여 서울지역 초등학생 5~6학년 573명을 대상으로 충동성, 자살행동, 보호요인 척도에 대한 설문을 실시하였다. 먼저 자살행동의 실태를 파악하기 위하여 빈도 분석을 하였으며, 변인들의 관계를 알아보기 위하여 상관분석을 실시하였다. 또한 충동성과 자살행동과의 관계에서 보호요인의 조절효과를 알아보기 위해 위계적 회귀분석을 실시하였다. 이를 통한 본 연구의 결과는 다음과 같다. 첫째, 연구대상의 29명(5.1%)은 자살계획을 세워 본 적이 있으며, 17명(3.0%)은 자살시도를 해 본 것으로 나타났다. 또한 자살행동을 보고한 학생은 41명(7.1%)으로 집계되었다. 둘째, 초등학생의 충동성과 보호요인(개인요인, 가정요인, 또래요인, 학교요인)은 모두 유의미한 부적 상관을 보였으며, 충동성과 자살행동은 유의미한 정적 상관이 있는 것으로 나타났다. 셋째, 초등학생의 충동성이 자살행동에 미치는 영향에 보호요인 모두 조절효과가 있는 것으로 나타났다. 즉 충동적인 초등학생이라도 보호요인이 높을수록 낮은 자살행동을 보인다는 것이다. 이렇듯 보호요인은 초등학생의 충동성에 따른 자살행동에 대한 완충제로 작용하기 때문에 이를 강화하기 위한 다차원적인 노력이 필요함에 대하여 논의하였다.

자살 행동과 연관된 생물학적, 유전적 예측인자 (Biological and Genetic Prediction Factors Associated with Suicidal Behavior)

  • Most suicides(about 90%) occur in the context of psychiatric disorders. Prediction of suicide risk in patients with mental illness is very important in preventing suicide attempts. However, current approaches to predict suicidality are based on clinical history and have low specificity and biological markers are not yet included. Many studies have explored the association between different biological parameters and suicidality. Studies of cerebro-spinal fluid(CSF) demonstrated that 5-HIAA and HVA levels were lower in patients with a history of suicide. Platelet serotonin transporter and the 5-HT2 serotonin receptor have also been studied in relation to violence and suicide. Depressive patients with greater suicidal tendency had significantly lower cholesterol concentrations but some researchers failed to find the correlation. DST non-supression is reported to predict suicidality in major depression. Several studies demonstrated a relationship between intron 7 polymorphism of tryptophan hydroxylase and suicidal behavior. Since suicide is not occurred in a single disease, the systematic and comprehensive study in large samples with various diagnoses is necessary to find the biological and genetic predictors of suicidal behavior.
