Comparison of Objective and Subjective Diagnostic Tests for Assessing Oral Dryness in Healthy Participants

  • Purpose: Xerostomia is subjective feeling of dry mouth. It is complicated and multifactorial, which burdens clinicians in diagnosis and treatment of the problem. The goal of this study was to discuss the clinical importance of salivary flow rate, pH and subjective symptoms for evaluating oral dryness among young healthy male subjects. Methods: Thirty male participants were recruited in this study (mean age±standard deviation of 25.70±1.84). All participants completed 'Xerostomia Inventory' to measure subjective oral dryness scores. Unstimulated saliva and stimulated saliva were collected from each participant twice a day at 12:00 pm and 5:00 pm, using spitting method. Salivary flow rates and pH were measured immediately after collection. Relationship between objective and subjective measurements were analyzed. Results: There were excellent intra-examiner reliability for salivary flow rate and pH and good internal consistency for Xerostomia Inventory. Objective measurements and subjective symptoms did not exhibit positive association. Salivary flow rate in unstimulated and stimulated condition showed positive association and also for salivary pH. Stimulated salivary flow rate also presented positive correlation with stimulated salivary pH. Conclusions: Comprehensive assessment of objective measurements and subjective symptoms may be complimentary for assessing oral dryness, which would assist in implementing early interventions to improve patient's quality of life.

노인 구강건조증에 대한 음허 진단의 유용성 및 주관적 구강건조감의 영향요인 : 전향적 단면 조사 연구 (Availability of Diagnosis of Yin-deficiency in Elderly People with Xerostomia and Factors Influencing Subjective Oral Dryness: A Prospective Cross-sectional Study)

  • Objectives: The aims of this study were to investigate the availability of diagnosis of Yin-deficiency in the elderly with xerostomia and factors influencing subjective oral dryness. Methods: We surveyed 50 patients recruited by the clinical trial, 'Efficacy of Yukmijihwang-tang on Xerostomia in the Elderly: A Randomized, Double-blind, Placebo-controlled, Two-center Trial'. The subjects were assessed on their subjective oral dryness using the Dry Mouth Symptom Questionnaire (DMSQ). Their salivary functions were measured by Unstimulated Salivary Flow Rate (USFR) measurements. In addition, the subjects were evaluated on their Qi-stagnation and Yin-deficiency conditions using the Qi-stagnation questionnaire and Yin-deficiency questionnaire. Results: There were statistically significant correlations between three variables (USFR, DMSQ score and Qi-stagnation score) and Yin-deficiency score. In the multiple regression analysis, the regression model was statistically significant (F = 10.273, p < .001). The factor most strongly influencing the subjective oral dryness was USFR (${\beta}$ = -0.386). Yin-deficiency had the next strongest impact on the subjective oral dryness (${\beta}$ = 0.371). Qi-stagnation affected the subjective oral dryness weakly (${\beta}$ = 0.075). In the simple regression analysis, Yin-deficiency had a statistically significant effect on each of six subscales of DMSQ (p < .01). Among the six subscales, DMSQ-1 ('Oral dryness at night or on awakening') was the most strongly influenced by Yin-deficiency. Conclusions: The results of this study show that the diagnosis of Yin-deficiency in the elderly with xerostomia was available and Yin-deficiency was an important factor influencing the subjective oral dryness. Therefore, the consideration of Yin-deficiency is significant for diagnosis and treatment in the elderly with xerostomia.

구강건조증 증상 평가를 위한 설문지의 신뢰도에 관한 연구 (Reliability of a Questionnaire for Evaluation of Dry Mouth Symptoms)

  • 구강건조증은 입안이 마르는 느낌의 주관적 증상을 의미하는 것으로 타액분비의 감소에 따른 불충분한 구강점막의 습윤도에 의해 발생할 수 있다. 그러나 구강건조증의 정도는 실제 측정된 객관적인 타액분비율과 상관관계를 보이지 않는 경우도 있어 그 진단과 치료에 어려움을 야기한다. 본 연구에서는 이러한 구강건조증의 진단과 평가를 위해 구강건조증 환자의 문제 해결을 위한 설문지를 개발하고 설문지의 문항들이 갖는 신뢰도를 분석하여 향후 구강건조증의 연구 및 진단 도구로서의 활용가능성을 알아보고자 하였다. 이를 위하여 구강건조감의 정도를 Visual Analogue Scale(VAS) 척도로 답하는 6 가지 문항과 구강건조감에 따른 행동을 파악하기 위한 4 가지 문항을 개발하고 문항들이 갖는 신뢰도를 평가하기 위하여 구강건조증의 증상을 호소하지 않는 건강한 성인 남녀 88 명(남자 44 명, 평균 $25.6{\pm}3.1$세, 여자 44 명, 평균 $24.3{\pm}2.1$세)을 대상으로 2 회에 걸쳐 질문에 답하도록 하여 다음과 같은 결과를 얻었다. 1. 구강건조감의 정도를 묻는 6 가지 문항의 급내상관계수(intraclass correlation coefficient; ICC) 값은 낮 시간의 구강건조감의 정도(Dry-day)가 0.767, 입안에 침이 적다고 느끼는 정도(Am-sal)가 0.850, 구강건조감으로 인한 일상 생활의 불편감 정도(Eff-life)가 0.791로 매우 높은 신뢰도를 나타내었고, 밤 또는 아침 기상시의 구강건조감의 정도(Dry-PM)가 0.563, 식사시의 구강건조감의 정도(Dry-eat)가 0.674, 음식물을 삼키기 힘든 정도(Dif-swal)가 0.641로 높은 급내상관계수 값을 나타내었다. 2. 6 개의 문항들의 내적 일치도를 평가하는 크론바흐의 알파(Cronbach's alpha) 값은 0.928로 매우 높게 나타났다. 3. 구강건조감에 따른 행동을 파악하기 위한 4 가지 문항들의 코헨의 카파(Cohen's kappa) 값은 잠자리 옆에 물을 준비하게 되는 빈도($H_2O$-bed)는 0.850으로 매우 높은 신뢰도를 나타내었고, 입이 말라 잠을 깨는 빈도(Night-awake)가 0.506, 마른 음식 섭취시에 물을 마시게 되는 빈도(Sip-liq)가 0.419, 껌이나 사탕을 먹게 되는 빈도(Gum-candy)가 0.407로 높은 신뢰도를 나타내었다. 이러한 결과로부터 VAS 척도로 답변하는 주관적 구강건조감의 정도를 묻는 6 가지 질문과 주어진 보기를 선택하여 답변하는 구강건조증에 따른 행동을 묻는 4 가지 질문으로 구성된 설문은 중등도 이상의 신뢰도를 가지고 있고 VAS 척도로 답변하는 6 가지 질문은 매우 높은 내적 일치도를 가지고 있음을 알 수 있었다.

노인의 주관적 구강건강과 구강건조증이 우울과 삶의 질에 미치는 영향 -우울의 매개효과를 중심으로- (The Subjective Oral Health and Oral Dryness of the Elderly Effects on Depression and Quality of Life -Focused on Mediating Effects of Depression-)

  • 본 연구는 노인의 구강건조증이 우울을 매개로 삶의 질에 미치는 직간접적 영향을 알아보고자 65세 이상의 노인 154명을 대상으로 1:1 설문조사를 시행하였다. 그 결과 노인의 주관적 구강건강과 구강건조증, 우울, 삶의 질은 상관성이 있었다. 주관적 구강건강과 구강건조증이 삶의 질에 대한 우울의 매개효과에서, 주관적 구강건강은 삶의 질(β=.23, p<.01), 우울(β=-.24, p<.01)에 유의한 영향을 미쳤고, 우울이 통제될 때 주관적 구강건강의 영향력(β=.08, p>.05)은 유의하지 않아 완전 매개하였다. 구강건조증은 삶의 질(β=-.24, p<.01)과 우울(β=.37, p<.001)에 유의한 영향을 미치는 것으로 나타났고 우울이 통제되었을 때의 구강건조증의 영향력(β=.00, p>.05)은 유의하지 않아 완전매개 하였다. 이상의 연구결과를 바탕으로 노인의 삶의 질을 높이기 위해서는 구강건강과 구강기능을 향상 시킬 수 있는 포괄적 중재 프로그램을 개발, 적용할 필요가 있으며 추후 적용 효과에 대한 연구가 필요할 것으로 생각된다.

일부 사회복지시설 종사자의 구강건강상태에 따른 구강건강관련 삶의 질 (Oral health-related quality of life in social welfare workers according to oral health status)

  • Objectives: The purpose of the study was to investigate the influencing factors of oral health-related quality of life in social workers. Methods: A self-reported questionnaire was completed by 240 social workers in Gwangju by convenience sampling method. The questionnaire consisted of general characteristics of the subjects(age, monthly salary, smoking, alcohol consumption) and systemic health condition(systemic diseases, medication, oral health status, and stress). The factors associated with oral health-related quality of life included skin dryness, eye dryness, lip dryness, and nasal dryness. The subjective dry mouth consisted of 6 questions measured by visual analogue scale(VAS). Cronbach's alpha was 0.868 in the study. The data were analyzed for t-test, one-way ANOVA and multiple regression analysis using SPSS 18.0 program. Results: The oral health-related quality of life in social workers varied by age, oral health status, stress status, and halitosis. The oral health-related quality of life had positive correlations with the subjective perception of dry mouth and whole body dry symptoms. The health status(${\beta}=-0.410$) had the influence on the oral health-related quality of life, nasal dryness(${\beta}=0.230$), age(${\beta}=0.189$), and halitosis (${\beta}=0.162) in order. Conclusions: The oral health-related quality of life was closely related to the health status and nasal dryness. It is necessary to develop the quality of life improvement programs including oral health prevention and care program in the social workers.

노인의 인지기능과 구강건강상태가 저작능력에 미치는 영향 (Effect of cognitive function and oral health status on mastication ability in elderly individuals)

  • Objectives: This study was conducted to characterize the impact of cognitive function and oral health status on mastication in senior citizens, ${\geq}65$ years of age, using senior centers in the city of Wonju, South Korea. Methods: A cross-sectional study consisting of a simple oral examination and survey questionnaires was performed in 154 individuals. General characteristics, subjective masticatory function, objective masticatory function, cognitive function, and oral health status were collected as variables. Correlation and multiple linear regression analyses were conducted. A p-value of <0.05 was considered to be statistically significant. Results: The subjective masticatory function was scored using the 5-point Likert scale. When subjective masticatory function was analyzed in groups according to cognitive function, the mean subjective masticatory function scores were 4.31, 4.09, and 3.29 in the normal group (cognitive score of ${\geq}16$), suspected dementia group (cognitive score of 1215), and mild dementia group (cognitive score of ${\leq}11$), respectively. Thus, subjective masticatory function decreased along with decreasing cognitive function. When cognitive function, subjective masticatory function, and objective masticatory function were compared with indicators of oral health status (number of functional teeth, oral dryness), subjective masticatory function exhibited a significant positive correlation with objective masticatory function (r=0.635, p<0.01), cognitive function (r=0.292, p<0.01), and total number of functional teeth, including prosthetic appliances (dentures) (r=0.305, p<0.01). According to the regression analysis, age, sex, number of functional teeth, and cognitive function affected subjective masticatory function. Conclusions: The results of this study revealed that age, sex, number of functional teeth, and cognitive function affected subjective masticatory function, whereas oral dryness did not. Therefore, dental professionals must consider subjective masticatory function when providing oral care in senior patients with low cognitive function.

구강 자가관리 프로그램이 노인의 구강건조증과 구강건강 관련 삶의 질에 미치는 효과 (Effects of an Oral Self-care Program on the Elderly's Xerostomia and Oral Health-related Quality of Life)

  • Purpose: The purpose of this study is to investigate effects of an oral self-care program on oral dryness and oral health related quality of life in the elderly. Methods: The current research design isa nonequivalent control group protest-posttest design with two groups of elders from two different senior centers in D city. Tools for the research include paper of salivary absorption (WF41-1850, Whatman, Germany), 6 items for subjective oral dryness and OHIP-14 for oral health related to quality of life. The data were analyzed to test three hypotheses by using the SPSS 22.0 program. Results: The three hypotheses were significantly accepted. The subjective oral dryness score was lower in the experimental group than in the control group (t=-2.45, p=.022). Salivary absorption was higher in the experimental group than in the control group (t=3.83, p<.001).Oral health related quality of life was better in the experimental group than in the control group (t=-2.11, p=.044). Conclusion: The oral self-care program has appeared to be effective on oral dryness and an oral health related quality of life in the elderly. Therefore, it can be suggested that the oral self-care program is applicable as an intervention program motivating the elderly to maintain oral health.

구강안면운동장애에 대한 필로카핀의 적용 (The Effects of Pilocarpine in Patients with Orofacial Movement Disorder)

  • 하악 및 구강주위근육의 조절되지 않는 움직임으로 정의 되는 구강안면운동장애(orofacial movement disorder, OMD)는 노인층에서 많이 발생하며 그 원인이 아직 정확히 밝혀지지 않고 치료방법 또한 다양하다. 임상적 특징을 분석하고 치료방법을 연구한 자료는 많이 있으나 타액분비와 OMD의 상관관계에 대한 구체적인 연구는 아직 없다. 본 연구에서는 부산대학교치과병원 구강내과에 내원한 OMD 환자 중 구강건조감을 호소한 4명의 환자를 증례로 하여 필로카핀(pilocarpine)을 경구로 투여한 후 OMD 증상의 변화를 관찰하였다. 연구결과 타액분비량의 저하를 보여 구강건조증으로 진단된 증례와 정상타액분비량을 보이는 증례 모두에서 필로카핀의 경구투여 후 OMD증상의 개선을 보였다. 따라서 향후 OMD와 구강건조증의 상관관계에 대한 추가적인 연구가 필요할 것으로 사료된다.

노인의 구강건강상태와 전신건강의 상태 (The relationship of oral state and health condition among elderly people)

  • The purpose of this study was to examine the relationship of oral state and health condition among elderly people. The subjects in this study were 220 senior citizens who were randomly selected from among dwellers in Eunpyeong-gu and Jongno-gu, Seoul. A survey was conducted by one-on-one interview to find out their oral state and health condition. The collected data were analyzed with SPSS 10.0 program, and variance analysis, correlation analysis and regression procedure were employed. The findings of this study were as follows: First, Regarding subjective oral health state, 52.3 percent of the elderly people, more than the half, considered their mouth to be in good health. The number of their mean remaining permanent teeth was 14.85, which was one of the indexes to indicate their objective oral state. Second, Concerning their abnormal oral health experience, 75 percent didn't feel that their masticatory force was good enough, and 55.9 percent felt pain in their teeth, 70.0 percent suffered from mouth dryness. Those fact showed that a lot of the elderly people felt there was something wrong with their oral cavity, and the unhealthy oral state made it difficult for them to eat food, as their masticatory force weakened and they couldn't get a good taste for food. Third, Those who viewed their oral cavity as healthy thought that they were in good shape, and there was a significant correlational relationship between their subjective oral and overall physical health states. Their own perception of oral state made a difference to their subjective health status, and the subjective oral health was significantly related to ADL as well, which implied that their ADL was different according to the way they looked at their own oral health status.

농촌 노인의 스트레스와 주관적 구강건조감의 관련성: 예비조사 (Relationship between stress and subjective oral dryness in the elderly in a rural region: a pilot study)

  • Objectives: Stress is a psychological reaction to stimuli such as anxiety or threat felt by a person either physically or mentally when placed in a difficult situation. Although a relationship between stress and dry mouth has been reported, it remains understudied. The purpose of this study was to investigate the association between stress and subjective dry mouth among the elderly living in a rural region. Methods: A total of 214 people aged ${\geq}60years$ living in a rural area were recruited with an informed consent for a cross-sectional analysis. Information about stress and subjective dry mouth was obtained by an interview survey with a constructed questionnaire. Data on other potential confounding factors (including oral factors) such as socio-demographic data, health-related behaviors, the number of remaining teeth, and subjective chewing ability were also gathered at the same time. Logistic regression models were used to analyze the relationship of stress and dry mouth using SPSS. Results: The Crude odds ratio (OR) of stress awareness affecting subjective dry mouth was 2.59 (95% confidence interval: 1.43-4.68). After adjusting for sex, education, income, smoking, and alcohol intake, the adjusted OR was 2.52 (95% confidence interval: 1.30-4.87) which was statistically significant. Conclusions: Elderly people who were stressed had an approximately 2-fold increase in experiencing subjective dry mouth when compared to their stress-free counterparts.
