물리치료를 받는 뇌졸중 환자의 발병 관계요인에 관한 조사연구 (A Study on Risk Factor Found in the Patients with Stroke in Physical Therapy)

  • The purpose of this study is to investigate risk factor found in the patients with stroke in physical therapy. A questionnaire survey was carried out for 130 the patients, who had comparatively alert mentality, hospitalized in kyung hee oriental medical hospital with stroke from January, 1st to May 31st, 1996. Major findings of this stuty were as follows. 1. The stroke patients 67(51.5%)were males and 63(48.5%) femals. The age groups of 50 to 69 years accounted for 67.6% of the total stroke patients are the highest. The job groups of commerce and business of patients 58.5%. The living groups of major citys of the revealed the highest incidence(40.8%). 2. There were 40 patients (30.8 %) who had hemorrhage related condions as the direct cause of their stroke, and 90 patients (69.2 %) who had cerebral infarction with no hemorrhaging as the cause. of the disease that the patients suffered from before their stroke, blood pressure related problems were most numberous 76 patients(58.5%) had such problems. 3. A month before their attack of stroke, were asked what thay thought could become the causes of their strokes. The most frequently mentioned answers were fatigue, drinking too much, and smoking, as chosen by 45 patients(34.6 %). 4. In regarding the relation between stress and stroke, (37.7%), of the surveyed belonged to the most numerous group had of people who said they were not under stress. Yet this group had a large number of parents. And 22.3 % of the surveyed named conflicts among parents, siblings, and relatives. 5. Questions were asked to find out the relationship between complications of stroke and smoking or drinking. It was surprising to see a high rate of attack among the 55.4 % majority who answered that they did not smoke nor drink. 6. To sum up the affect of food on attack of stroke, 42.5% of the people who had a sprcey and salty diet had a higher rate of attack then the group of people with leisurely nature. 7. Looking at the family history, 40.4% of the patients said their parents also suffered the disease. This percentage is the highest. And 40.0% answered that there is no one who suffered in their family history. Judging from the percentages and family.

중풍 발생 위험인자에 대한 환자-대조군 연구 (The Case-Control Study on the Risk Factors of Stroke in Korean Adults)

  • Background and Purpose : The purpose of this case-control study was to show the relationship between risk factors(past medical history, BMI, WHR, smoking, drinking), warning signs(dyscinesia, sightless, dysarthria, sensory disorder, numbness, blephalospasm, facial spasm, tension) and the incidence of stroke in korean adults. Methods : 455 stroke patients were enrolled as the case group and 180 non-stroke patients as control group from Oct. 2005 to Feb. 2006. Patients were hospitalized within 2 weeks after the onset of stroke. Obesity were defined as $BMI{\geq}25kg/m2$, $WHR{\geq}0.9$ in male and $WHR{\geq}0.8$ in female. Risk factors and warning signs were obtained from personal interview. The analysis of the data was done by chi-square test. Fisher's exact test and test-sample t-test. Results : The percentage of current smokers(or current drinkers) of case group is higher significantly than that of control group. The past medical history of risk factors were found to be transient ischemic attack(p=0.0698), facial palsy(p=0.4061), hypertension(p<0.0001), hyperlipidemia(p=0.1484), DM(p<0.0001), ischemic heart disease(p=0.0093), migraine(p=0.0014) and hypochondria(p=0.2370). $WHR{\geq}0.9$ in male had a 6.696 (3.711-12.082) odds ratio, $WHR{\geq}0.8$ in female had a 1.567 (0.659-3.726) odds ratio. $BMI{\geq}25kg/m2$ had a 2.017(1.263-3.222). The dyscinesia and sensory disorder of warning signs were found to be statistical difference between case and control group. Conclusions : According to the above results, it was found that smoking, drinking, BMI, WHR, hypertension, DM, ischemic heart disease, migraine affected to the incidence of stroke.

마리화나 흡연으로 인해 발생한 젊은 연령 뇌경색 1례 (A Case of Ischemic Stroke in Young Patient Associated with Marijuana Use)

  • A 21-year-old female patient complaining of hemiparesis was diagnosed with right middle cerebral artery infarction. No risk factor was found, despite an extensive young-age stroke work-up, except her history of marijuana use. The patient had smoked marijuana for treating depression for more than five years. Magnetic resonance angiography showed multifocal intra- and extracranial stenoses, suggesting cannabinoid-induced vasculopathy. Since the use of illicit drugs has increased nationwide, physicians should consider it as a possible cause of a stroke due to an unknown etiology.

  • Transient Ischemic Attack(TIA) is an important risk factor of stroke but often underestimated by clinicians. In this case, we administered oriental-western cooperative therapy to a 72 years old female TIA patient with severe MCA stenosis. After treatment, all symptoms were disappeared and has not reappeared until 1 year follow-up.

  • Background: Nowadays, increasing women's socio-economic activities increased the grandmothers raising grandchildren. Because of the contemporary situation, the health problems of the care giving grandparents is becoming an issue. Purpose: The purpose of this study was to examine correlationship between the incidence of stroke and raising grandchildren. Methode: This article contrasts the ratio of recent experience of raising grandchildren of 195 normal and 323 stroke patients, especially women. Result: There were 43.08% of women raising their grandchildren in the normal group while 13.31% of women of stroke patients group were raising their grandchildren before the onset of stroke. Conclusion: The finding suggests the positive effect of raising grandchildren on reducing the risk for stroke.

Risk Factors for Complications Following Resection of Gastric Cancer

  • Purpose: Operative morbidity and mortality from gastric cancer have decreased in recent years, but many studies have demonstrated that its prevalence is still high. Therefore, we investigated the risk factors for morbidity and mortality considering the type of complication in patients with gastric cancer. Materials and Methods: A total of 259 gastrectomies between 2004 and 2008 were retrospectively reviewed. Results: Overall morbidity and mortality rates were 26.6% and 1.9%, respectively. A major risk factor for morbidity was combined resection (especially more than two organs) (P=0.005). The risk factors for major complications in which a re-operation or intervention were required were type of gastrectomy, upper location of lesion, combined resection, and respiratory comorbidity (P=0.042, P=0.002, P=0.031). Mortality was associated with preexisting neurologic disease such as cerebral stroke (P=0.016). In the analysis of differen complication's risk factors, a wound complication was not associated with any risk factor, but combined resection was associated with bleeding (P=0.007). Combined resection was an independent risk factor for a major complication, surgical complication, and anastomotic leakage (P=0.01, P=0.003, P=0.011, respectively). Palliative resection was an independent risk factor for major complications and a previous surgery for malignant disease was significantly related to anastomosis site leakage (P=0.033, P=0.007, respectively). Conclusions: The risk factors for gastrectomy complications of gastric cancer were combined resection, palliative resection, and a previous surgery for a malignant disease. To decrease post-gastrectomy complications, we should make an effort to minimize the range of combined resection, if a palliative gastrectomy is needed for advanced gastric cancer.

재원일별 진료비 발생양상과 재원일수의 결정요인 (The Determinant Factors and Medical Charges Pattern by Length of Stay in Hospital)

  • Stroke is a high-risk disease. The future of the medical environment is that the proportion of elderly population is increasing, the average life expectancy is being increased, while the fatal rate of stroke will be low. These situation will due to the financial burden on medical insurance. The most important factor that affects on the medical costs of stroke patients is the length of stay. In this study the mean length of hospital for stroke stay was 21.81days(37.97days for intracerebral hemorrhage, 18.89 days for cerebral infarction). The payment per case of stroke was 6.86 million won(12.6 million won for intracerebral hemorrhage, 5.72 million won for cerebral infarction). The payment per case of intracerebral hemorrhage was 2.2 times more than that of cerebral infarction. The payment in the day of hospitalization was the highest and until the second day medical costs was high. After the third day medical costs tended to decline, after that seemed to show an almost constant level. The length of hospital stay was found to be the most important determinant of inpatient charges for stroke. Accordingly rational management of the length of stay will be beneficial to health care consumers, providers, states.

뇌졸중 후 우울에 영향을 미치는 요인 (Predicting Factors of Post-Stroke Depression)

  • Predicting factors of post-stroke depression were investigated. The subjects were 254 stroke patients who had been discharged and visited regularly the outpatient clinic for follow up care. The influencing factors were classified into five categories : demographic, disease-related, current state, social support, and illness intrusiveness. Sex, age, job, and educational level were defined as the demographic factor. The disease-related factors included stroke type, illness duration, attack frequency, and NIH score. ADL, cognitive function, and social activity were considered as the current state factors. The social support was measured as the perceived amount of social support. Illness intrusiveness means the perceived illness induced life style disruptions. Demographic, disease-related, current state, and social support were hypothesized to directly and indirectly affect post-stroke depression through illness intrusiveness. The illness intrusiveness was hypothesized to directly affect post-stroke depression. The hierarchial multiple regression was used to identify significant factors. The result showed that this model explained 43.3% of variance of post-stroke depression. And the prevalence of post-stroke depression was 38.8%. Among the demographic factors, job was identified as a main contributor to indirectly increase the post-stroke depression. Among the disease-related factors, stroke type, attack frequency, and NIH score were found to indirectly affect the post-stroke depression. Among the current state factors, ADL and social activity indirectly affected the post-stroke depression through illness intrusiveness. Social support and illness intrusiveness were identified to directly affect the post-stroke depression. This study has proved the factors likely to be implicated in the development of post-stroke depression. Based upon these results, it is recommended that the nurses who take care of post-stroke patients consider the risk factors such as social support, illness intrusiveness et al. Also programs which decrease the illness intrusiveness and increase the social support to reduce post-stoke depression recommended to be developed.

기후(氣候) 및 간지(干支)와 중풍(中風) 發病日(발병일)에 관(關)한 상관성(相關性) 연구(硏究) (A Study of the Correlation between Stroke Incidence by Climate of Day and Stems and Branches)

  • Objectives : This study investigated the correlation between the incidence by the climate of the day and the Stems and Branches(干支) of stroke patients. Methods : From January in 2000 to March in 2008, we studied 370 stroke patients hospitalized at Dongguk University Bundang Hospital. Each participant was investigated for the day's average temperature, the day's average air humidity, the day's average wind speed, and the day's Stems and Branches (干支) in oriental medical theory. Results: Concerning the day's average temperature, the group at a temperature under 18 $^{\circ}C$ had the highest risk. For the day's average air humidity, the group at over 60 % humidity showed a higher ratio of stroke. In respect of the day's average wind speed, the group at speeds under 2.26 $^m/s^2$ (the last 5 years average wind speed in Gyeonggi-do) had a higher risk. In the aspect of Stems and Branches (干支), stroke occurred more in Yin day(陰日) than in Yang day(陽日). In the view of the Five Circuits (五運), the Wood (木) was most common followed by Fire(火). Using the relation between Stems and Branches (干支), the mutuality cooperate pattern was more common than the mutuality control group and same characteristic group. Using the Theory of Five Circuits (五運) and Six Qi (六氣), the Fire group (火) showed the highest risk. The Wood group (木) came next. Conclusions : We could suggest that stroke attack might have some significant relationship with climate, stems, and branches in oriental medical theory.

중풍(中風)의 요일별(曜日別) 발생(發生)과 위험요인(危險要因)과의 상관성(相關性)에 관(關)한 연구(硏究) (A Study of the Correlation between Stroke Incidence by Day of the Week and Risk Factors)

  • Objective : This study was aimed to investigated the correlation between the stroke incidence by day of the week and risk factors in acute stroke patients. Methods : From October in 2005 to October in 2007, 673 acute stroke patients wereincluded. Patients were hospitalized within 14 days after the onset of stroke in DongGuk University International Hospital, Kyungwon University In-cheon Oriental Medical Hospital or the Department of Cardiovascular and Neurologic Diseases (Stroke Center), Kyung Hee University Oriental hospital. We investigated general characteristics, stroke types, age group, educational backgrounds, medical history, lifestyle (the impact of stress, exercise, smoking and drinking) and Sasang constitution according to the stroke incidence by day of the week. Results : After examining each participants with day of the week, the order of days by incidence was Monday, Wednesday, Sunday, Saturday, Tuesday, Thursday and Friday respectively. Monday and Tuesday showed the highest ratio. As the aspect of demographic data of subjects, the group under 55 years and from 55 years to 70 years showed the highest ratio on Monday. In the group between 25 years to 55 years, the ratio, by the classification of actual productive age, was statistically higheron Monday. In addition, the ratio showed statistically higherby educational background, from elementary school to high school and over high school. In regard tothe medical aspect, incidence of cerebral hemorrhage showed higher ratio on Sunday, and incidence of brain infarction was higheron Monday. However, there was no difference of the past history between the two groups. With regard tothe aspect of lifestyle of subjects, the group with huge stress before the incidence of stroke showed a higher ratio of stroke incidence on Monday. Participants who didn't exercise regularly and participants who didn't smoke or drink showed higher ratio of stroke incidence on Monday. However, this was not important statistically. In regard to the aspect of Sasang constitution, Soeumin showed the highest ratio of stroke incidence on Wednesday, Taeuminon Monday, and Soyangin on Saturday and Monday. Conclusion : According to these results, several cardiovascular risk factors affect stroke incidences on Monday. Further studies will be needed to help understand the correlation between stroke incidence by day of the week and risk factors in acute stroke patients.
