The Effects of Robot-Assisted Rehabilitation on the Gait Ability of Stroke Patients with Hemiplegia: A Mixed Methods Research Study (보행로봇 재활치료가 편마비 뇌졸중 환자의 보행능력에 미치는 효과: 혼합연구설계)

  • Purpose: This study used a mixed methods research design in an attempt to verify the effects of robot-assisted rehabilitation on the gait ability of stroke patients with hemiplegia, and thereby further understand the benefits and challenges of stroke patients' experiences relying on robot-assisted rehabilitation. Methods: An exploratory sequential mixed methods study design was used in order to combine both quantitative and qualitative data. For the quantitative data collection, a total of 30 stroke patients with hemiplegia were recruited from one rehabilitation hospital. Qualitative data were collected through individual interviews using semi-structured questionnaires for a group of 15 patients who were currently undergoing robot-assisted rehabilitation. The data were analyzed through qualitative content analysis. Results: As a result of the quantitative analysis, there were significant differences between the two groups in terms of daily living activity patterns, total number of steps, and average walking speed. As a result of the qualitative analysis, the four main themes derived consisted of, 'curiosity about the usage of robot-assisted rehabilitation,' 'pleasure experienced while using the robots,' 'insufficient information about robots,' and 'a lack of education about robot-assisted rehabilitation.' Conclusions: Robot-assisted rehabilitation had a significant effect on the walking ability of stroke patients with hemiplegia. Additionally, stroke patients with hemiplegia experienced difficulty during the course of their robot-assisted rehabilitation, due to a lack of sufficient information on correct usage techniques. These quantitative and qualitative findings could provide the basic foundation for the development of an educational program on robot-assisted rehabilitation.

Effect of Mask Filter on Respiratory Function in Chronic Stroke Patients (마스크 필터가 만성 뇌졸중 환자의 호흡기능에 미치는 영향)

  • Purpose : This study investigated the effects of wearing a mask and different mask filters on the respiratory function of stroke patients. Methods : A total of 15 stroke patients were selected according to the inclusion and exclusion criteria. The respiratory functions were compared between participants with and without masks and among respiratory functions with three different mask filters. The order of using masks was non-wearing masks, Dental masks, KF80 masks, and KF94 masks; the difference in respiratory volumes among these conditions were measured. For accuracy of the measurement, sufficient education on the respiratory measurement method was provided to the researcher, and the heart rate of the participants was estimated to confirm their stability before the measurements. To ensure accuracy, the subjects were educated on the researchers' respiratory measurement methods. Each measurement was followed by 10 min breathing stability before replacing the next mask. Results : The results of this study showed that the difference in respiratory functions, including forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), and maximal voluntary ventilation (MVV), in stroke patients was statistically significant among different masks (p<.05). Afterwards, the values of FVC, FEV1, and MVV in stroke patients wearing masks were significantly lower than those of the non-masked control group (p<.05). The difference in respiratory functions with different mask filters showed no statistical significance (p<.05). Conclusion : This study showed that participants wearing any of the masks presented a lower respiratory function than that of those without using masks; additionally, no difference in respiratory functions was observed with differences in mask filters. Therefore, wearing a mask for a prolonged period is confirmed to affect breathing in stroke patients with weak respiratory function.

  • We developed a part of storke sensing cylinder using magnetic sensor and estimated is performance. In this paper, for the performance estimation of stroke sensing cylinder. We consist of hydrallic system using solenoid valve with ON/OFF motion. In order to the control of solenoid valve for the position control of cylinder rod, PWM (Pulse Width Modulation) method which modulates time pulse width in proportion to error was used. A performance of cylinder rod with magnetic scales was evaluated by its hydraulic system.

  • Nowadays, the axial stiffness of crankshaft of long-stroke diesel engine is low compared to that of the old types of engine by increasing stroke/bore ratio and major critical speed might occur within engine operation speed. An axial damper needs to be installed in order to reduce the axial vibration of the crankshaft in the event of reduced or stopped axial damper function are discussed.

Mechanically Modulated Nonlinear Digital Microactuators for Purified Digital Stroke and Nano-Precision Actuation (기계적 비선형 변조기를 이용한 디지털 구동의 안정화와 나노 구동정도 구현을 위한 디지털 마이크로액추에이터)

  • This paper presents a nonlinearly modulated digital actuator (NMDA) for producing nano-precision digital stroke. The NMDA, composed of a digital microactuator and a nonlinear micromechanical modulator, purifies the stroke of the digital actuator in order to generate the high-precision displacement output required for nano-positioning devices. The function and concept of the nonlinear micromechanical modulator are equivalent to those of the nonlinear electrical limiters. The linear and nonlinear modulators, having an identical input and output strokes of 15.2${\mu}{\textrm}{m}$ and 5.4${\mu}{\textrm}{m}$, are designed, fabricated and tested, respectively. The linear and nonlinear modulators are linked to identical digital actuators in order to compare the characteristics of the linearly modulated microactuator (LMDA) and NMDA. In addition, an identical linear modulator is attached to the output ports of LMDA and NMDA. The NMDA shows the repeatability of 12.3$\pm$2.9nm, superior to that of 27.8$\pm$2.9nm achieved by LMDA. When the identical linear modulator is connected to LMDA and NMDA, the final modulated output from NMDA shows the repeatability of 10.3$\pm$7.2nm, superior to that of 15.7$\pm$7.7nm from LMDA. We experimentally verify the displacement purifying capability of the nonlinear mechanical modulator, applicable to nano-precision positioning devices and systems.

A Study of Stroke Patients' Leisure Activities and Quality of Life

  • This study was conducted in order to examine relationship between stroke patients' degree of satisfaction with leisure and quality of life according to their leisure activity types and whether they conducted leisure activities. A direct survey was performed from April 8 to May 3, 2013 on 92 inpatients and outpatients who received occupational therapy at hospitals located in Jeonju and Gunsan. A frequency analysis was carried out in order to look at the general characteristics of the subjects, their degree of participation in leisure activities, and their degree of satisfaction with leisure activities. A Pearson's coefficient was used to examine relationship between their degree of satisfaction and quality of life according to participation in leisure activities. Their degree of satisfaction according to participation in leisure activities was significantly higher when they took part in leisure activities such as handicraft activities, sports activities, or travel or tourism activities than when they did not perform any leisure activities. Their quality of life according to participation in leisure activities was higher when they conducted sports activities, outing activities, or tourism activities than when they did not. There was significant correlation between their degree of satisfaction with leisure and quality of life according to leisure activities.

Observational Study of Glossodynia (설통에 대한 관찰연구)

  • Objectives : The purpose of this study was to report the clinical characteristics of glossodynia patients and effects of traditional Korean treatment on glossodynia and to gather data in order to carry out future study. Methods : We surveyed 40 glossodynia patients visiting the Department of Korean Internal Medicine of Stroke and Neurological Disorders Center, Kyung Hee University Hospital at Gangdong and analyzed their glossodynia questionnaires, examination results, and progression of symptoms. Results : In Glossodynia patients the average age was 62.4 years old. Glossodynia was more prevalent in females than males (5.67:1). Red tongue color, white fur, and fissured tongue were observed most often. The average glossodynia duration was 36.1 months and the average glossodynia degree was VAS 5.6. The most commonly appearing symptom was burning sense, and dryness of tongue. In Korean medicine diagnosis, Yin, deficiency, Tae-eum was most commonly diagnosed. In herbal medicine, Gamichungsim-tang was the most frequently prescribed. In laboratory tests, zinc deficiency was the most common result. In heart rate variability (HRV), average SDNN was 34.03 and average LF/HF ratio was 2.11. Of 40 patients, 23 completed treatment and their pain was relieved from average VAS 5.52 to VAS 3.63. Pre-and post-treatment results showed a significant difference. The average duration of treatment was 34.91 days. Conclusions : The results of this study suggest that Korean medical approach and treatment for glossodynia could be effective. Further study should be conducted for more valuable information.

  • Purpose : This article reviewed the advances in the understanding of the effect of motor rehabilitation and brain plasticity on functional recovery after CNS damage. Methods : This is literature study with Pubmed, Medline and Science journal. Results : The inability of CNS neurons to regenerate is largely associated with nonneuronal aspects of the CNS environment. Especially, this neuronal growth inhibition is mediated by myelin associated glycoprotein, olygodendrocyte-myelin glycoprotein, and NOGO. Enriched environment, motor learning, forced limb use have been utilized in scientific studies to promote functional reorganization and brain plasticity. Especially, enriched environment and motor enrichment may prime the brain to respond more adaptively to injury, in part by expressed neurotrophic factors. Conclusions : These reviews suggest that activity-induced neural plasticity occur in damaged brain areas in order to functional reorganization, where it could contribute to motor recovery, and represent a target for stroke rehabilitation.

  • In the past two stroke low speed diesel engine were widely used for marine propulsion. these engine have many merit for example, higher thermal efficiency and durability. however, shaft vibration problem was continuously rise up due to large power of two stroke low speed diesel engine. specially, the initial stage engine revolution counter & stress has acculated. For that reason, sometimes occurred crankshaft crack accident. In this study, regarding the initial stage engine, trying to analyze what is cause the crack phenomenon using by latest torsional vibration calculation program & measurement equipment.

Calculation of Electric Potential Rise of Horizontal Grounding Electrode Caused by Lightning Stroke Currents (뇌격전류에 의한 수평접지극의 전위상승 계산)

  • An electric potential rise of the grounding electrode developed by the lightning stroke currents should be calculated for the purpose of effective protection of electrical and electronic equipment. In this paper, the electromagnetic model was applied to calculate the harmonic impedance of grounding electrode. Also the empirical equation related to the permittivity and resistivity of soil was used. The lightning current waveforms, which are expressed by the Heidler's equation, were used in order to calculate accurately transient electric potential rises. The transient voltage was obtained by using the simulated harmonic impedance and the lightning current in frequency domain. Finally, the transient voltages of horizontal grounding electrode(10m) under lightning stroke currents were calculated by IFFT(Inverse Fast Fourier Transform).
