소아중풍(小兒中風) 환자(患者)에 대한 치험(治驗) 1례(例) (One Case of Stroke in Childhood)

  • Objectives : The purpose of this study is case report of stroke in childhood. The childhood cerebrovascular diseases are interesting than those seen in adulthood, because the etiology and prognosis of the disease in children are quite different from those of adults. Cerebrovascular diseases in children is more common than once recognized. However, relatively little attention has been paid to the epidemiology of childhood stroke. Methods : We experienced one case of ischemic stroke in childhood whose symptoms are similar to that of adults. There is no evidence of cerebrovascular malformation at MRI, MRA film. The cause of stroke is undetermined and just we can see the little infarction lesion on Lt. pons. We treated the patient with herb medicine, acupuncture-Tx, negative-Tx, moxa-Tx and physical treatment. Results : After the treatment of oriental medicine, the patient recovered from the stroke symptoms. Conclusion : we thought that the more follow-up is needed. So we repoert one case of cerebral infarction in children that recovered from stroke symptoms after the treatment of oriental medicine.

  • Objectives The purpose of the case study is to report the effects of stroke in childhood especially for Intracranial Hemorrhage sequela treated by oriental medical treatment. Methods The patient was treated by herbal medicine, acupuncture, moxa, and physical treatments. Results By using of oriental medical treatment, we could see significant improvement for Stroke symptoms. Conclusions This case study shows that oriental medical treatment can be an effective treatment for Stroke in Childhood. However, the further study with longer, and large clinical trials are needed for accuracy.

  • Cerebrovascular disease in children is more common than once recognized and the etiology and prognosis of the disease in children are quite different from those of adults. In Korea, the most common cause of stroke is moyamoya disease in ischemic stroke, arteriovenous malformation in hemorrhagic stroke. We experienced two cases of ischemic stroke in childhood whose symptoms are similar to that of adults. They had cerebrovascular malformation(narrowing of ICA, MCA, basilar artery). we treated them with herb medicine, acupuncture treatment, laser acupuncture treatment and physical treatment. After treatment, they recovered from the stroke symptoms, but the primary cause of stroke was not eliminated, so it is thought that more follow up is needed. We report that we had good effects of oriental medical treatment on two case of cerebral infarction in children.

  • Cerebral infarction without cerebravascular malformation in childhood is very uncommon. One case of cerebral infarction without cerebravascular malformation in childhood, with symptoms similar to those of an adult, was encountered. This female patient had no cerebravascular malformation and was just ten years-old. She was treated with herb medicine, acupunture treatment and physical treatment. After treatment she recovered from stroke symptoms but the primary cause of stroke was not revealed. Follow-up research is needed to determine the cause of stroke. Results suggest that oriental medical treatment had beneficial effects in treatment of cerebral infarction in this child.

  • Hypertension is a major risk factor for myocardial infarction, stroke, and cardiovascular mortality in adults; its treatment reduces the risk of cardiovascular events. In recent times, attention is being paid to monitoring of blood pressure from childhood to adulthood. Childhood hypertension is associated with hypertension in later life, and early intervention is important. In the Korean socioeconomic background, a rapid increase is observed in the number of obesity cases and the rate of increase in the incidence of obesity is more in childhood. The strong association of high blood pressure with obesity and the marked increase in the incidence of childhood obesity indicate that both hypertension and prehypertension are becoming significant health issues. In this comprehensive review, we acquaint the clinician with the available literature on childhood hypertension to provide recommendations for the diagnosis, evaluation, and treatment of hypertension based on the available evidence and consensus in Korean clinical conditions.

모야모야병 환아(患兒) 1예(例)에 대한 증례(症例) (A Case of Moyamoya Disease)

  • Moyamoya is a chronic cerebrovascular disease chracterized by progressive stenosis or occlusion of the terminal parts of both internal carotid arteries with telangiectatic vascular network of collateral circuration at the base of the brain and leptomeningeal arteries. The etiology and pathophysiology of this disease are still unknown. The clinical course in those whose first symptoms occur in childhood is different from those in whom symptoms develop in adult life. The term moyamoya disease should be resserved for those cases in which the chracteristic angiographic pattern is idiopathic; moayamoya syndrome is used when the underlying condition is known. we have experienced a case of moyamoya syndrome in a 5-year-2-month-old boy who presented right-sided hemiparesis. A cerebral angiogram revealed occlusion of abnomal collateral network. Moyamoya disease is applicable to stroke of an infant from oriental medicine point of view, and The symptoms is similar to adult stroke, we have treated adult stroke patint with herb medicine and acupuncutre and physical treatment. The acute stage of stroke is applied to the external treatment(標治), and The recovery stage is applied to the basic treament(本治).

  • 고혈압은 소아 청소년시기에 발생하는 흔한 만성질환 중 하나이다. 고혈압을 치료하지 않으면 여러가지 소아의 장기에 악영향을 미칠 수 있다. 여기서는 고혈압이 중추신경계에 미치는 영향에 대해 알아 보았다. 혈압의 변화에 따라 대뇌가 받는 영향은 다른 장기에 비해 덜 하다. 그것은 대뇌 자동 조절 기전때문이다. 즉 혈압의 변화에 따라 뇌혈관의 저항이 변하기 때문에 뇌혈류량이 일정하게 유지 된다. 그러나 이런 자동 조절 기전의 항상성이 파괴되면 급성으로 또는 만성으로 급성 고혈압 뇌병증, 허혈성, 출혈성 뇌경색, 학습 장애와 인지 기능 장애 등이 발생한다. 이에 고혈압이 대뇌에 미치는 영향에 대해 숙지하고 빠른 시일 내에 적절한 치료를 하면 이런 합병증을 예방할 수 있다.

  • 좌심실 비대는 심부전, 허혈성 심질환, 부정맥과 같은 심혈관계 질환의 이환율 및 사망률을 높이는 강력한 위험 인자이며 고혈압으로 초래되는 가장 초기 단계의 심장 변화 이기도 하다. 소아청소년 고혈압 환자들은 분명한 심혈관계 합병증 보다는 무증상의 좌심실 비대만 볼 수 있는 경우가 대부분이나 치료하지 않는 경우 성인에서의 심혈관 합병증으로 이행될 위험이 크다. 표적장기 손상의 가장 대표적인 증거이기도 한 좌심실 비대는 소아청소년에서 고혈압 전 단계 상태에서도 나타나므로 고혈압의 진단 당시와 추후의 추적 진료 중 정기적으로 심초음파를 통한 좌심실 질량의 측정이 이루어 져야 한다. 좌심실 비대가 보인다면 적극적인 약물 치료가 또한 이루어져야 한다.

모야모야병 환자의 뼈목동맥관 직경과 임상표현과의 관계 (Relation of Bony Carotid Canal Diameter and Clinical Manifestations in Patients with Moyamoya Disease)

  • Background: Moyamoya disease is characterized by a progressive stenosis or occlusion of the intracranial internal carotid artery and/or the proximal portion of the anterior cerebral artery and middle cerebral artery. Whether the onset time was childhood or adulthood, the bony carotid canal diameter might be different, but reflects the size of internal carotid artery passing through the bony carotid canal. In this study, we aimed to identify the relationship between bony carotid canal diameter and clinical manifestation. Methods: 146 consecutive patients diagnosed with moyamoya disease by brain imaging studies were included. We measured the diameter of a transverse portion of bony carotid canal on bone window of a brain computed tomography(CT) image. Patients were divided into two groups, ischemic or hemorrhagic stroke according to clinical manifestation. As a result, 115 patients were included. The Suzuki stage was used as criteria for disease progression. Results: Bony carotid canal diameter was $3.6{\pm}0.5$ (right) and $3.6{\pm}0.4$ (left) in the hemorrhagic stroke group, and $3.7{\pm}0.4$ (right) and $3.6{\pm}0.4$ (left) in the ischemic stroke group. The bony carotid canal diameter of the moyamoya vessels (3.6 mm) was smaller than the diameter of non-moyamoya vessels (3.8 mm), significantly (p = 0.042). However, there was no difference in the collateral patterns and clinical manifestation in a comparison of both groups. Conclusions: In our study, there was no significant difference of clinical manifestations and collateral patterns depend on the bony carotid canal diameter in patients with moyamoya disease. These findings suggest that the clinical presentations of moyamoya disease are not related to the onset time of the disease.

멜라스 증후군의 개요 (Overview of Mitochondrial Encephalomyopathy with Lactic Acidosis and Stroke-like episodes (MELAS) syndrome)

  • MELAS 증후군은 다양한 임상 증상을 나타내는 복잡하고 다면적인 미토콘드리아 질환으로, 반복적인 뇌졸중 유사 에피소드, 발작, 당뇨병, 심근병증 등을 포함한다. 이러한 증상들은 뇌, 심장, 근육과 같은 대사적으로 활발한 조직에 미토콘드리아 기능 장애가 미치는 심각한 영향을 반영한다. 현재의 치료는 이러한 증상을 완화하고 미토콘드리아 기능을 개선하는 데 중점을 두고 있으며, 증상 치료, 고용량 비타민 요법 및 고용량 타우린 보충과 같은 혁신적인 접근 방식을 포함한다. 유전자 치료 및 미토콘드리아 표적 약물 분야의 새로운 치료법은 근본적인 유전자 돌연변이를 해결하고 미토콘드리아 건강을 향상시킬 수 있는 유망한 새로운 길을 제공한다. MELAS에 대한 이해가 계속 깊어짐에 따라, 유전자 검사 및 치료적 개입의 발전은 환자의 결과를 크게 개선할 가능성을 갖고 있다. MELAS 치료의 미래는 낙관적이며, 진행 중인 연구는 더 효과적이고 표적화된 치료법을 위한 길을 열어 이 질환의 부담을 줄이고 영향을 받는 개인들의 삶의 질을 향상시키는 것을 목표로 하고 있다.
