소아의 구강내에서 검출된 Streptococcus viridans에 대한 항균제 감수성 연구 (ANTIMICROBIAL SUSCEPTIBILITY TEST ON STREPTOCOCCUS VIRIDANS IN CHILDREN'S ORAL CAVITY)

  • A large number of streptococci that do not fit readily into any of the established classification schemes have been relegated to a large heterogeneous group called the Streptococcus viridans, which are members of the normal flora of the mucous membranes of the body, including the oral cavity, the nasopharynx, and genitourinary tract. This group includes S. mitis, S. oralis, S. sanguis, S. salivarius, S. milleri, etc. Surveying on the literature, it has been reported that infective endocarditis, meningitis, rhabdomyolysis, cholangitis, appendicitis caused by Streptococcus viridans, which were the most important pathogen in children with malignant hematologic disease. Various antibiotics has been chosen for treatment or prophylaxis for these infections, but were generally lower antimicrobial susceptibilities because of an abuse of antibiotics and advent of resistant group. Therefore, surveillant culture must be performed to evaluate personal antimicrobial susceptibilities of intraoral microbes for proper antimicrobial choice for dental procedures. This study examined sampling from subgingival plaque of 60 chidren's microbes. The cultured bacterial isolates, Streptococcus viridans were examined 10 antimicrobial drugs with the Kirby-Bauer agar disk diffusion method. The used drugs were Penicillin, Ampicillin, Oxacillin, Cephalothin, Imipenem, Gentamicin, Erythromycin, Vancomycin, Ciprofloxacin, Clindamycin. The results were as follows : 1. Sampling Streptococcus viridans were S. mitis(65%), S. oralis(22%), S. sanguis(5%), S. intermedius(3%), S. salivarius(2%), S acidominimus(2%), Unidentified streptococcus(2%). 2. The antimicrobial susceptibility of total Streptococcus viridans : Oxacillin< Erythromycin< Pencillin=Ciprofloxacin< Cephalothin< Ampicillin< Clindamycin< Gentamicin< Imipenem=Vancomycin. 3. The antimicrobial susceptibility of S. mitis : Oxacillin=Erythromycin< Ciprofloxacin< Cephalothin< Penicillin=Ampicillin< Gentamicin< Clidamycin< Imipenem=Vancomycin. 4. The antimicrobial susceptibility of S. oralis : Oxacillin< Erythromycin< Penicillin=Ciprofloxacin=Clindamycin< Cephalothin=Gentamicin< Ampicillin< Imipenem=Vancomycin. 5. There was no significant difference in the antimicrobial susceptibility among each Streptococcus viridans group.

  • The purpose of this study was to isolate and identify the bacteria in osteomyelitis lesion of 3 patients. Two lesions were due to the post-infection after extraction. The other was resulted from mal-fixation of both sides of mandibular angles. Pus samples were collected by needle aspiration from the lesion and examined by culture method. Bacterial culture was performed in three culture systems (anaerobic, $CO_2$, and aerobic incubator). Identification of the bacteria was performed by 16S rRNA gene cloning and nucleotide sequencing method. Our results showed that Streptococci species was predominantly isolated in both lesions of extraction socket. Only one species (Proteus vulagris) was detected in lesion of mandibular angle. This study was not sufficient to identify the causative bacteria in those osteomyelitis. However, our data may be offered the clue to solve the problem.

  • Streptococcus milleri Group Bacteria(SMG)는 인간의 정상 세균총이면서 농양 형성을 잘 하는 병원균으로도 작용하지만 그 병원성에 대해서는 잘 알려져 있지 않다. 따라서 SMG 감염시 경각심을 갖고, 적절한 치료에도 증상이 지속될 때에는 병변 이외의 다른 곳에 농양이 형성되어 있는지 반드시 살펴보아야 한다.

Streptococcus thermophilus KACC 91147 첨가우유 섭취가 혈당치에 미치는 영향 (Effect of Milk Containing Streptococcus thermophilus KACC 91147 on Blood Glucose Levels)

  • The lactase activities of nine species of lactic acid bacteria were compared using the chromogenic substrate, $o-nitrophenyl-{\beta}-D-galactopyranoside$. Streptococcus thermophilus KACC 91147 had the highest lactase activity among a total of thirty strains of Lactobacillus and S. thermophilus tested, including commercial strains. S. thermophilus KACC 91147 released $0.30{\pm}0.12\;mg/mL$ of galactose in treated milk A ($10^7\;CFU/mL$) and $6.49{\pm}0.38\;mg/mL$ in treated milk B ($10^9\;CFU/mL$ milk) over 2 hours. In milk tolerance tests, the blood glucose level (BGL) of 6 volunteers (2 males and 4 females) clinically diagnosed as lactose intolerant increased 3.0 mg/dl after drinking milk A, but a significant (p<0.05) additional increase of $11.2{\pm}4.18\;mg/dl$ was found after drinking milk B. This result suggests that the addition of S. thermophilus KACC 91147 cells into milk aids the digestion of lactose in milk and ameliorates the symptoms of lactose-intolerant individuals due to the activity of lactase from the lactic streptococci.

Inhibitory effects of Coptis chinensis extract on the growth and biofilm formation of Streptococcus mutans and Streptococcus sobrinus

  • Streptococcus mutans and Streptococcus sobrinus play important roles in dental caries. Coptis chinensis is a natural product with antimicrobial activity against enterobacteria; however, its effects on oral streptococci are still unknown. Therefore, the effects of C. chinensis on the growth and biofilm formation of the representative cariogenic bacteria S. mutans and S. sobrinus were investigated for the possible use of C. chinensis as an anticaries agent. The C. chinensis extract was diluted with sterile distilled water, and 0.1-2.5% of the extract was used in the experiment. The effects of the C. chinensis extract on the growth and glucan formation of S. mutans and S. sobrinus were measured by viable cell counting and spectrophotometry at 650 nm absorbance, respectively. Crystal violet staining was also carried out to confirm the C. chinensis extract's inhibitory effect on biofilm formation. The C. chinensis extract significantly inhibited the growth of S. mutans and S. sobrinus at concentrations of ≥ 0.3% as compared with the control group. The viable cell count of colonies decreased by 1.7-fold and 1.2-fold at 2.5% and 1.25%, respectively, compared with the control group. The biofilm formation of S. mutans and S. sobrinus was inhibited by > 20-fold at C. chinensis extract concentrations of ≥ 1.25% as compared with the control group. In summary, the C. chinensis extract inhibited the growth and biofilm and glucan formation of S. mutans and S. sobrinus. Therefore, C. chinensis might be a potential candidate for controlling dental caries.

Combined Antimicrobial Activity of Extracts from Quercus infectoria Galls and Scrophularia striata Aerial Parts for an Anticariogenic Herbal Mouthwash

  • Objectives: Dental caries is one of the most prevalent human diseases worldwide. The disease initiates with bacterial adherence to the tooth surface followed by the formation of dental plaques. Mutans streptococci and Candida albicans are principal oral microorganisms involved in the initiation and development of dental caries. Phytochemicals have been shown to possess promising antimicrobial properties against a wide range of microorganisms and can be used for the prevention and treatment of dental caries. Herein, we reviewed literature on plants that are traditionally used for their antimicrobial properties or possess promising anticariogenic activity. We selected aerial parts of Scrophularia striata (S. striata) and galls of Quercus infectoria (Q. infectoria) and investigated their antimicrobial activity against cariogenic microorganisms. Methods: Water soluble fractions were obtained from hydroalcoholic extracts of S. striata and Q. infectoria and their antimicrobial activity against Streptococcus mutans (S. mutans), Streptococcus sobrinus (S. sobrinus), and Candida albicans (C. albicans) was evaluated separately and in combination. The extracts were then used for preparing an herbal mouthwash whose stability and tannic acid content were evaluated over 60 days. Results: Q. infectoria gall extract possesses efficient antimicrobial activity that was synergistically enhanced in the presence of S. striata extract. Mouthwash prepared using these extracts showed desirable organoleptic characteristics, antimicrobial activity, and stability. Conclusion: Extracts of S. striata and Q. infectoria galls can be used together for preparing dental products with effective anticariogenic properties. Our study highlights the importance of extensive pharmacological investigations when using herbal products alone or in combination with other chemical substances.

Microbial isolates and antibiotic sensitivity in patients hospitalized with odontogenic infections at a tertiary center over 10 years

  • Objectives: This study investigated causative strains and their antibiotic sensitivity in patients who were hospitalized for maxillofacial odontogenic infections at a tertiary center in South Korea over the past 10 years with the aim of providing guidelines for the selection of appropriate empirical antibiotics. Materials and Methods: Patients with head and neck fascial space abscesses due to odontogenic infections who underwent incision and drainage surgery with pus culture tests between 2013 and 2022 at the Department of Oral and Maxillofacial Surgery, Dankook University Hospital were included. The bacterial isolates and antibiotic sensitivity of each strain were analyzed for 2013-2022, 2013-2017, and 2018-2022. The affected fascial spaces were classified into primary, secondary, and deep neck spaces. Results: In the 192 patients included in this study, 302 strains were detected. Viridans streptococcus had the highest frequency (51.7%), followed by Prevotella spp. (16.9%), Staphylococcus spp. (5.6%), and Klebsiella pneumoniae (4.6%). The identification rate of viridans streptococcus significantly increased from 41.8% in 2013-2017 to 60.9% in 2018-2022. Viridans streptococcus showed an antibiotic sensitivity of 80.5% to ampicillin; the sensitivity to penicillin antibiotics decreased over the study period. Antibiotic susceptibility was approximately 94% for third-generation cephalosporins. K. pneumoniae, which was identified at a high percentage in patients with deep neck space infection, showed increasing antibiotic resistance to most antibiotics over the study period. Conclusion: Viridans streptococcus was identified in head and neck fascial space abscesses with the highest frequency. Empirical antibiotics should be effective against this strain; penicillin antibiotics are considered inappropriate. For effective treatment of deep neck space abscesses, bacterial culture and antibiotic sensitivity tests performed as soon as possible are essential.

Comparative Evaluation of the Fluoride Releasing Ability and Microbial Attachment of Glass-Hybrid Restorative Material

  • This study aimed to compare the fluoride-releasing ability and degree of microbial attachment of a newly developed glass-hybrid restorative material (GH) with those of a high-viscosity glass ionomer (HvGIC), resin-modified glass ionomer (RMGI), and composite resin (CR). In addition, the correlation between fluoride-releasing ability and microbial attachment between materials was evaluated. Specimens were prepared in a disc shape and divided into 4 groups according to the materials (GH, HvGIC, RMGI, and CR). The fluoride release experiments were performed in each group (n = 15). The amount of fluoride released was measured on days 1, 3, 7, 14, 28, and 42 after storage. For the microbial attachment experiment, 12 specimens were produced per group using Mutans Streptococci (S.mutans ), a cariogenic microorganism. S. mutans was cultured on the specimens for 24 hours, and the number of bacteria was measured. GH had the highest cumulative fluoride release and showed a significant difference when compared with RMGI (p = 0.001) and CR (p < 0.0001). Microbial attachment was the lowest in GH; however, no significant difference was observed between the materials (p = 0.169). There was no significant correlation between fluoride release from materials and microbial attachment (p > 0.05). From this perspective, remineralization of low-mineralized areas could be expected due to the high fluoride release of GH, and the effect of delaying the progression of dental caries could be predicted from the low cariogenic microbial attachment. Therefore, GH might be a useful restorative material for treating immature permanent teeth with hypomineralized enamel. However, further studies are needed about the degree of remineralization of hypomineralized areas after restoration and the capacity to recharge fluoride.

소아 세균성 인두 편도염의 진단에 있어 신속 항원 검출 검사의 유용성과 비용 효과의 평가 (Efficacy and Cost Benefits of Rapid Antigen Detection Test for Group A Streptococci Obtained from Acute Pharyngitis in Children)

  • 목 적 : A군 사슬알균에 의한 인두염의 진단에 있어 미국의 감염 관련 학회에서는 도말 검사나 신속 검사 후 확진이 되면 항생제 사용하는 것을 추천한다. 국내에서는 아직 인두염시 항생제 사용에 대한 기준이 확립되지 않았다. 본 연구는 국내 인두염 환자에서 신속 검사와 인후 배양 검사를 시행하고 치료한 군과 검사 없이 경험적으로 치료한 군을 비교하여 신속 검사의 유용성과 이를 통한 항생제 사용의 타당성을 연구하기 위해 시행되었다. 방 법 : 경희의료원과 한일병원 외에 6개의 개인의원이 참여하였다. 총 369명의 환자들이 참여하였으며, 이들을 연구자 임의로 신속 검사와 인후 배양 검사 시행하고 치료를 한 군(신속 검사군)과 신속 검사나 인후 배양 검사 없이 연구자의 임상적 경험적으로 치료한 군(경험적 치료군)으로 나누어 연구를 시행하였다. 결 과 : 신속 검사군은 244명이었으며 경험적 치료군은 125명이었다. 신속 검사군 중 신속 검사 양성 환자는 71명, 신속 검사 음성 환자 173명이었으며, 인후 배양 검사 양성 환자는 49명, 인후 배양 검사 음성환자는 195명이었다. 인후 배양 검사를 기준으로 한 A군 사슬알균의 유병률은 20.1%였다. 신속 검사의 민감도는 89.8%, 특이도 86.1%, 양성 예측도 62.0%, 음성 예측도 97.1%였으며 위양성률 38.0%, 위음성률 13.9%였다. 신속 검사군과 경험적 치료군의 항생제 사용은 76%, 71%로 두 군 다 높게 나타났다. 이들을 적절하게 신속 검사 상 결과에 따라 항생제를 사용하였으면 항생제 사용을 37% 정도 줄일 수 있으며 신속 검사의 가격을 책정 가격의 63% 정도로 줄이면 신속 검사 시행 후 항생제 사용과 경험적 항생제 사용의 가격 차이는 없어진다. 결 론 : A군 사슬알균 인두염시 경험적 항생제 치료 보다 진단에 따른 치료가 필요하며 신속 검사의 가격을 책정가격의 63%로 줄이면 경험적 치료에 비해 증거에 입각한 진단을 내릴 수 있고 항생제 사용을 줄일 수 있다. 그러나 정확한 비용 효과의 평가를 위해 더 많은 연구가 필요하다.

신생아 세균성 뇌막염의 원인균에 따른 치료와 예후 (Treatment and Prognosis according to Causative Organisms in Neonatal Bacterial Meningitis)

  • 목 적 : 신생아 세균성 뇌막염은 임상 증상이 비특이적이며 여러가지 신경학적 후유증을 남길 수 있는 질환으로 본 저자들은 원인균 특히 그람 양성균과 음성균에 의한 뇌막염을 비 교 분석하여 치료 및 예후의 개선을 도모하고자 본 연구를 시행하였다. 방 법 : 1991년 1월부터 1996년 6월까지 중앙 길병원 신생아 중환자실과 소아과 병동에서 뇌척수액 검사상 균이 배양되었거나 latex agglutination test상 균이 밝혀진 24례를 대상으로 후향적으로 분석하였다. 결 과 : 1) 환아의 분포는 남아 17명, 여아 7명이었고, 그람 양성균에 의한 뇌막염 환아의 평균 출생 체중과 재태 기간은 $2.91{\pm}0.79kg$, $38.4{\pm}2.74$주, 그람 음성균군에서는 $3.30{\pm}0.90kg$, $37.7{\pm}3.33$ 주로 두 군간에 유의한 차이는 보이지 않았다. 2) 선행 요인으로는 미숙아, 태변 착색, 조기 양막 파수, 산모 당뇨병, 임신성 고혈압 등이 있었으며, 임상 증상 및 증후로는 발열, 경련, 수유량 감소, 대천문 팽대 등이 있었고, 증상 발현 시기는 조기 발현형이 11례(그람 양성균 4례, 그람 음성균 7례), 지연 발현형이 13례 (그람 양성균 7례, 그람 음성균 6례)였다. 3) 원인균은 그람 양성균이 11례로 coagulase-negative staphylococci와 group B streptococci가 각 3례, Staphyolococcus aureus와 Streptococcus viridans가 각 2례, enterococci가 1례가 있었으며, 그람 음성균은 13례로 Escherichia coli가 7례, Klebsiella pneumoniae가 3례 Pseudomonas aeruginosa, Acinetobactor, Emterobacter가 각 1례씩 있었다. 4) 뇌척수액 검사상 백혈구 수치는 그람 음성균군에서 통계적으로 유의하게 높았으나 단백 수치와 당 수치는 두 군간에 통계적으로 유의한 차이를 보이지 않았다. 5) 뇌초음파 검사상 그람 양성균군에서는 7례(63.6%)에서, 그람 음성균군에서는 10례 (76.9%)에서 비정상 소견을 보였으나 두 군간에 뇌초음파 검사상 비정상 소견의 빈도에 있어서는 통계적으로 유의한 차이를 보이지 않았다. 6) 항생제 감수성 검사에서 그람 양성균군에서는 penicillin계통, 1세대 cephalosporin, vancomycin에, 그람 음성균군에서는 amikacin과 3세대 cephalosporin에 대해 비교적 높은 감수성을 보였다. 7) 사망율(병원내 사망이나 hopeless discharge)은 20.8%(5례)였으며, 예후는 그람 양성균 군과 그람 음성균군에서 통계적으로 유의한 차이를 보이지 않았다. 결 론 : 신생아 세균성 뇌막염은 그람 양성균과 그람 음성균 간의 빈도와 예후의 차이는 보이지 않았고 예후가 불량하므로 세균성 뇌막염이 의심되는 환아에서는 적극적인 치료가 필요하리라 사료된다.
