환자는 54세 광부로 1991년 1월 낙반사고로 인해 가벼운 골반 골절상을 받았었고, 골절상은 완치되었으나 설사가 지속하고 체중이 심하게 감소하였다. 발병 약 9 개월만에 시행한 대변검사 및 조직 검사에서 분선충중(strongyloidiasis), 포자충중(isosporiasis) 및 인형 세포거대바이러스(heman cytomegalovirus)에 감염된 사실이 확인되었다. Albendazole로 치료하였으나 약 2개월 후 근로복지공사 장성병원에서 난치 상환 하에 퇴원하였으며, 퇴원 1주 후 사망하였다.
A 6-year-old intact male Doberman pinscher presented with a thin soft stool and urinary signs. On radiography, three masses were observed in the caudal abdomen. One mass was described as a large, round, mineralized mass, with an "eggshell" appearance. The second mass was located caudal to the mineralized mass, and the third mass was located between the two masses. The second and the third masses had a soft tissue density. Ultrasonography was performed to identify the features and the origins of the masses. The first mass contained a large amount of anechoic fluid and had a thin wall; it was determined to be a cyst. Strong acoustic shadowing artifact was created by the mineralized cystic wall. Caudal to the cyst, the second mass was shown to be the prostate. A hypoechoic stalk connected the prostate and the cyst, which represented the cyst as a paraprostatic cyst with mineralization. The third mass was the normal urinary bladder. The cyst was removed surgically and confirmed by histopathologic examination. This report described typical clinical findings, diagnostic imaging, and treatment of a paraprostatic cyst.
참굴큰입흡충(Gymnophalloides seoi)은 참굴이 매개하는 우리 나라 고유의 인체기생 장흡충의 하나로서, 전남 신안군 도서 지방에서 큰 유행지가 발견된 바 있다. 이 흡충은 췌장 질환과의 관계가 의심되어 의학적 관심의 대상이 되고 있다 이번 증례 보고에서는 당뇨병과 동반된 흥미있는 G. seoi감염 2례를 발견하여 보고하고자 한다. 환자들은 당뇨병 치료를 위해 내원하였으며 내원시 대변검사에서 참굴큰입흡충류(gymnophallid)의 충란이 검출되었다 프라지콴텔 및 하제를 투여한 후 충체를 수집한 결과 G. seoi 충체 772마리 및 10마리가 각각 검출되었다. 증례 I은 62세 남자로 G. seoi의 유행지 부근인 목포에 거주하고 있었고, 증례 II는 인천에 거주하고 있으면서 참굴을 날로 즐겨 먹었다고 하였다. 이 증례들을 볼 때 G. seoi감염이 당뇨병과 어떠한 상관관계를 가지고 있을 가능성을 시사하였다.
1. Objectives Theoretically Healthy Condition of four constitution is based on intact state of four fundamental factors - digestion, urination, perspiration, defecation. Each one of these four factors is specifically related to its respective constitution. We made SHI(Sasangin Health Index) 10 items from four factors, and examined clinical efficacy of the four factors as index of health status in four constitution. 2. Methods and Procedures We offered QSCCII, SHI and SF-36 questionnaires to adults who had a periodic medical examination. 233 adults were classified to one of four constitution. We compared means of 8 scale scores of SF-36, and computed the correlation coefficient between 8 scale scores of SF-36 and 10 item scores of SHI in four constitution. 3. Results In Soyangin, stool volume and frequency are weakly correlated with health status(P<0.05). In Soeumin, appetite is weakly correlated with health status(P<0.05). In Taeumin, there is no correlation between perspiration and health status. 4. Conclusions 2 factors - digestion, defecation - had clinical efficacy as index of health status.
A three-month-old female African buffalo born at Seoul Zoo showed signs of abdominal distension, bulging of the perineal skin, and small stool volume compared to feed intake. Upon physical examination, atresia ani with rectovaginal fistula was diagnosed. This case was subjected to surgery under inhalation anesthesia after injecting a sedative. Surgery was performed in two steps: anal reconstruction and closing the rectovaginal fistula. First, a circular skin incision was made at the end of the rectal pouch to create an anus, and then the skin of the anus and the mucous membrane of the rectum were brought into apposition by simple interrupted sutures. Second, the rectovaginal fistula was ligated on both vulval and anal side. Antibiotics were administered on every alternate day and the sutures were removed at ten days surgery under sedation. The rectovaginal communication was closed and the calf was able to urinate and defecate normally. The animal grew to become a normal adult without any complications. This is the first case report of atresia ani with rectovaginal fistula in an African buffalo, that was successfully treated by surgical intervention.

Coxsackievirus A16 (CA16), which primarily causes hand, foot, and mouth disease (HFMD), is associated with complications, such as encephalitis, acute flaccid paralysis, myocarditis, pericarditis, and shock. However, no case of pancreatitis associated with CA16 has been reported in children. We report a case of CA16-associated acute pancreatitis in a 3-year-old girl with HFMD. She was admitted because of poor oral intake and high fever for 1 day. Maculopapular rashes on both hands and feet and multiple vesicles on the soft palate were observed on physical examination. She was treated conservatively with intravenous fluids. On the fourth hospital day, she had severe abdominal pain and vomiting. The serum levels of amylase and lipase were remarkably elevated (amylase, 1,902 IU/L; reference range, 28-100 IU/L; lipase, >1,500 IU/L; reference range, 13-60 IU/L), and ultrasonography showed diffuse swelling of the pancreas with a small amount of ascites. The real-time reverse transcription polymerase chain reaction result from a stool sample was positive for CA16. CA16 can cause acute pancreatitis, and should be considered in the differential diagnosis of abdominal pain in children with HFMD.
We report here a case of strongyloidiasis in a 72-year-old diabetic patient (woman) accompanied by gastrointestinal stromal tumor receiving imatinib therapy, first diagnosed as hypereosinophilic syndrome and treated with steroids for uncontrolled eosinophilia. She suffered from lower back pain and intermittent abdominal discomfort with nausea and diagnosed with gastrointestinal stromal tumor. After post-operative imatinib treatment eosinophilia persisted, so that steroid therapy was started under an impression of hypereosinophilic syndrome. In spite of 6 months steroid therapy, eosinophilia persisted. Stool examination was performed to rule out intestinal helminth infections. Rhabditoid larvae of Strongyloides stercoralis were detected and the patient was diagnosed as strongyloidiasis. This diagnosis was confirmed again by PCR. The patient was treated with albendazole for 14 days and her abdominal pain and diarrhea improved. This case highlights the need for thorough investigation, including molecular approaches, to test for strongyloidiasis before and during steroid therapies.
Purpose: The purpose of this study was to estimate the effects of an infection control program on the Clonorchis sinensis (CS) infection ratio, CS-related knowledge and health behavior of people living near the Geum River. Method: A one-group pretest-posttest design was used with 102 participants. For 12 months the participants were provided with promotion announcements, health education, counseling, and medication. Outcome variables measured were the CS infection ratio by stool examination, CS-related knowledge and health behavior from self-report questionnaires. The pre intervention data were collected from January to February 2008 and the post intervention data during the same period in 2009. Collected data were analyzed using descriptive statistics, generalized estimating equation analysis, and paired t-test with SPSS for Windows version 15.0. Results: In the pre test 21 of the 102 participants showed infection with CS for the first time. In the post test 9 were newly infected with CS, and one was re-infected. The CS-related knowledge was significantly improved after the infection control program (p<0.05). The CS-related health behaviors did not improve. Conclusion: These results showed that an infection control program is effective in decreasing CS infection ratio and improving CS-related knowledge of people living near the river.
Fasciolopsiasis is rarely known as the parasitic disease in Nepal. Herein, we report a case of fasciolopsiasis in a 22-year-old man who was admitted in the hospital with abdominal pain, distension and loss of appetite for a month. He had previously diagnosed with acute viral hepatitis but, his abdominal pain was not resolving despite improvement in his liver function and general condition. During endoscopy an adult digenean worm was seen in the first part of the duodenum. After isolation, the worm was identified morphologically as Fasciolopsis buski. Microscogic examination of the patient's stool revealed eggs with a morphology consistent with F. buski. Eggs were yellow-brown, ellipsoidal, unembmbryonated, operculated, filled with yolk cells, with thin shell and ranging $118-130{\mu}m$ in length and $60-69{\mu}m$ in width. The abdominal pain of the patient was resolved after treatment with praziquantel. By the present study, it was confirmed for the first time that fasciolopsiasis is indigenously transmitted in Nepal. Accordingly, the epidemiological studies in humans and reservoir host animals should be performed intensively in near future.
This study aimed to examine the distribution of gastrointestinal parasitic infections in domestic pigs in the Republic of Korea. From May 2020 to October 2021, 364 pig fecal samples were collected from 75 farms in 7 Provinces and microscopically examined. A total of 170 (46.7%) pigs were infected with at least one of the following parasites: Balantioides coli, strongyles, Ascaris suum, Trichuris suis, and coccidia. By parasite species, B. coli, strongyles, A. suum, T. suis, and coccidia oocysts or eggs were detected in 144 (39.6%), 24 (6.6%), 14 (3.8%), 4 (1.1%), and 1 (0.3%) samples, respectively. One hundred fifty-four, 15, and 1 cases showed single, double, and triple infections, respectively. Of the swine fecal samples from 75 farms, 69 specimens (92.0%) were infected with 1 or more parasites. All surveyed farms across the country exhibited a positive rate of over 30%, among which the highest positive rate was 65.0% in Chungcheongnamdo, and Jeollabuk-do was followed by 61.9%. Winter showed a statistically lower prevalence than other seasons. This study showed that gastrointestinal parasites are prevalent in pigs in Korea, although the diversity of parasites is low.
