Factors Associated With Stillbirth Among Pregnant Women in Nepal

  • Objectives: Stillbirth is a common adverse pregnancy outcome that represents a distressing and traumatic event for women and their partners. The aim of this study was to identify factors associated with stillbirth in ever-pregnant women in Nepal. Methods: This study utilized the individual women's dataset from the Nepal Demographic and Health Survey, conducted in 2016. The dependent variable of interest was whether women had at least 1 stillbirth during their lifetime. The associations between independent variables and the dependent variable of the study were analyzed using a multiple logistic regression model. Results: Among 8918 ever-pregnant women aged 15-49 years, 488 had experienced at least 1 stillbirth during their lifetime, representing 5.5% of the total. After adjusting each factor for the confounding effects of other factors, maternal age, maternal education, place of residence, and sub-region remained significantly associated with having experienced stillbirth. Conclusions: Stillbirth continues to be a major problem among women, especially those with higher maternal age, those who are illiterate, and residents of certain geographical regions. To minimize stillbirth in Nepal, plans and policies should be focused on women with low education levels and residents of rural areas, especially in the western mountain and far-western hill regions.

양수과소증으로 인한 사산 후 산모 치험 1례 (A Case Report of Patient Treated after Stillbirth Caused by Oligohydramnios)

  • Purpose: The purpose of this study is to report the effect of Traditional Korean Medicine therapy for patient in the state of post stillbirth by oligohydramnios Methods: The patient in this case was 33-year-old female who was pregnant by artificial insemination in 2010. Intrauterine growth restriction was discovered at 17 weeks after gestation. And oligohydramnios was found at 20 weeks after gestation. Amnioinfusion was done three times, amniotic fluid infection occurred in the process of amnioinfusion and she had a stillbirth. After stillbirth she came to our clinic with symptoms of abdominal tenderness, distention, mammalgia. The patient was treated by traditional herbal medicine therapy such as Saenghwa-tang(生化湯), Gungguijohyeol-eum(芎歸調血飮), Silso-san(失笑散) and Sasang constitution medicine(荊防地黃湯, 荊防敗毒散, 荊防瀉白散). The progresses of symptoms were evaluated by visual analogue scale. Results: After Traditional Korean Medicine therapy, the patient's symptoms caused by stillbirth was improved. Conclusion: This case study shows that the postpartum treatment in Traditional Korean Medicine therapy and Sasang constitution medicine is effective for the patient in the state of post stillbirth.

가습기살균제 CMIT/MIT의 기도 점적투여를 통한 임신마우스의 사산에 대한 영향 (Intra-tracheal Administration of the Disinfectant Chloromethylisothiazolinone/methylisothiazolinone (CMIT/MIT) in a Pregnant Mouse Model for Evaluating Causal Association with Stillbirth)

  • Objectives: Recently, a report was published that the humidifier disinfectant CMIT/MIT did not cause developmental toxicity and was not detected in systemic circulation as a result of an inhalation toxicity test. Therefore, this study was carried out to investigate any associations between CMIT/MIT exposure and developmental toxicity using the in vivo apical toxicity test method. Methods: Groups of pregnant ICR mice were instilled in the trachea with chloromethylisothiazolinone/methylisothiazolinone (CMIT/MIT) using a visual instillobot over a period of seven days from days 11 to 17 days post-coitum. For the in vivo apical toxicity test method, an $LD_{50}$-based dose-range finding model was applied to decide the dose range for inducing developmental toxicity. Results: Among the groups of 0, 0.1, 0.5, 1.0, and 1.5 mg ai/kg/day CMIT/MIT, the exposure groups of 0.5 mg and 1.0 ai/kg/day CMIT/MIT were estimated to reflect the thresholds for the stillbirth and death of pregnant mice, respectively. The groups of 0.5, 1.0, and 1.5 mg ai/kg/day CMIT/MIT induced stillbirth rates of 2.57, 10, and 53.8%, respectively. Another exposure group of 0.75 mg ai/kg/day CMIT/MIT did not induce any deaths of pregnant mice and resulted in a stillbirth rate of 8% in only one of six pregnant mice. Conclusions: CMIT/MIT can induce stillbirth in pregnant mice. It was also concluded that CMIT/MIT moves through the pulmonary circulation system and then continues on through systemic circulation and the placenta. There is a possibility of stillbirth and other health causalities in humans beyond the lungs caused by CMIT/MIT exposure.

전북 동부지역 한우에서의 유사산 유발 질병 항체가 조사 (Seroprevalence of abortion and stillbirth inducing disease in Hanwoo, in Jeonbuk eastern area)

  • Reproductive losses in a herd can be huge. Fetal reabsorption or undefined infertility often remain undetected. Routine herds monitoring for exposure, controlling the introduction of potential agent carriers, appropriate biosafety procedures, and vaccination where possible are together the best security against abortion and stillbirth inducing disease. For biosecurity of local farms, we performed antibody titers of abortion and stillbirth related diseases such as bovine viral diarrhea virus (BVDV), infectious bovine rhinotracheitis virus (IBRV), Neospora caninum, Toxoplasma gondii and Campylobacter fetus subsp venerealis. The blood samples were collected from 500 female Hanwoo over 1 year old of 100 farms in Jeonbuk eastern area. Champhylobater serological test was evaluated by the standard tube agglutination test (STAT) and other pathogen's antibodies were detected by indirect-enzyme linked immunoassay (I-ELISA). The seroprevalence of abortion and stillbirth inducing disease were BVDV 72.4%, IBRV 13.0%, N. caninum 1.2%, T. gondii 10.4% and C. venerealis 0.6%, irrespectively.

  • Objectives Arrhythmia can occur frequently when state of pregnancy or after carriage, but it is rare that the symptom continues. We have a case of arrhythmia, which occurred after stillbirth and has lasted for about 4 months, treated by Korean medicine treatment, so we report a case on arrhythmia occurred after stillbirth.Methods Patient, who suffered from arrhythmia after stillbirth, was enrolled in this study. She received Korean medicine therapies such as herbal medicine, acupuncture and negative cupping.Results Discomfort and frequency of arrhythmia was reduced by Korean medicine therapiesConclusions The present study suggests that Korean medicine therapies have a effect on arrhythmia caused by stillbirth.

Association of Length of Pregnancy with Other Reproductive Traits in Dairy Cattle

  • The experiment involved observations of 2,514 Holstein-Friesian cows to determine the effects of environmental factors (cow's age, calving season, weight and sex of calves, housing system) and genetic factors on gestation length in dairy cattle and the correlation between gestation length and other reproductive traits (calving ease, stillbirth rates and placental expulsion). Genetic parameters were estimated based on the sires of calved cows (indirect effect) and the sires of live-born calves (direct effect). The following factors were found to contribute to prolonged gestation: increasing cow's age, male fetuses and growing fetus weight. Optimal gestation length was determined in the range of 275-277 days based on calving ease and stillbirth rates. The heritability of gestation length was estimated at 0.201-0.210 by the direct effect and 0.055-0.073 by the indirect effect. The resulting genetic correlations suggest that the efforts to optimize (prolong) gestation length could exert an adverse influence on the breeding value of bulls by increasing perinatal mortality and calving difficulty. The standard errors of the investigated parameters were relatively high, suggesting that any attempts to modify gestation length for the purpose of improving calving ease and reducing stillbirth rates should be introduced with great caution.

모돈 한약재 급여에 따른 자돈 사산 및 폐사 감소효과 연구 (A study of reduction effects of stillbirth and mortality rates in pigs by medicinal herbs supplementation in sow)

  • This study was based on a survey of 100 sows which were raising in Jeonnam province for 11 months, from February to December 2008 in order to evaluate the medicinal herbs supplementation in sows on the effects of mortality in suckling pigs, and milk quality of sows. The medicinal herbs were supplemented in the feed of sows from 1 weeks before the expected date of delivery to 10 days after the delivery. As comparing stillbirth rate, test group was reduced 9.05%, and control group, 17.37%. It was certain that the medicinal herbs was very effective for reducing stillbirth rate. In comparison of mortality rate of suckling pig, test group was decreased by 1.52% as compared with control group. In analysis of milk components of sows, milk fat was 6.04% in test group and 5.22% in control group. Milk protein contents of test group and control group were 18.06% and 18.36%, respectively, showing no significant differences between two groups. Milk sugar contents showed also no difference between two groups. By milk somatic cell counts, test group ($582{\times}10^3$cells/ml) was found decreased number of milk somatic cell as compared with control group ($688{\times}10^3$cells/ml). These results imply that the medicinal herbs supplementation in sow might be effective for preventing stillbirth and mortality in piglets and mastitis in sow.

A case report of congenitally abnormal rabbit-headed stillbirth Najdi lamb

  • The congenital head anomalies are most often used to describe defects in the eyes, mouth, nose, skull, and or brain. The faulty embryogenesis most likely found to be associated with abnormal genetic or epigenetic causes during pregnancy. Eventually it leads to congenital anomalies. Rabbit-headed Lamb (RH) is a disorder in sheep breeding that is characterized by some deformities in the head and the face. A dead -day old- crossbred white Najdi lamb with a deformed face and head was reported to be born in the current case. The external and physical examination revealed a deformed skull and facial region with a unilateral eye, fused mouth, pig-like nose, and patent skull with the brain coming out from left eye orbit. Additionally, the lamb was very skinny with unusual long extremities. This is the first report of this syndrome that describes such deformities in a stillbirth Najdi breed lamb.

  • Studies were conducted on the survey of HI antibody abortion and stillbirth from J.E. virus in swine for 1968-1969 in Korea HI antibody against J.E. virus showed postive in June or July and reached 80~100 percent of postive in August or September and J.E. virus strains were isolated from the brain material of the aborted piglets.

경북지역 돼지의 바이러스성 유사산 원인조사 (Etiological Study of Porcine Viral Abortions and Stillbirths in Gyeongbuk Province)

  • A total of 170 litters (575 samples) of aborted and stillbirth fetuses submitted to the Gyeongsangbuk-Do Veterinary Service Laboratory (GVSL) between January 2006 and December 2010 from pig farms in Gyeongbuk province were studied to identify porcine abortion- and stillbirth-associated viruses such as Porcine parvovirus (PPV), Encephalomyocarditis Virus (EMCV), Japanese Encephalitis Virus (JEV), Porcine Reproductive and Respiratory Syndrome Virus (PRRSV), and Aujeszky's Disease Virus (ADV). Virus was not detected by PCR in 36 litters, but viral antibody was detected by HI and ELISA in 93 litters. The majority of etiological viruses were PPV (67 litters, 39.4%), EMCV (50 litters, 29.4%), PRRSV (15 litters, 8.8%), and JEV (11 litters, 6.5%); ADV was not detected by either PCR or ELISA. Single infection occurred in 52 litters (30.6%), co-infection occurred in 41 litters (24.1%), and unknown cases with no detection of any of the five viruses occurred in 77 litters (45.3%).
