Technical Aspects of Endobronchial Ultrasound-Guided Transbronchial Needle Aspiration

  • Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is becoming a standard method for invasive mediastinal staging and for the diagnosis of paratracheal and peribronchial lesions. It is essential to understand the technical aspects of EBUS-TBNA to ensure safe and efficient procedures. In this review, we discuss the practical aspects to be considered during EBUS-TBNA, including anesthesia, manipulation of equipment, understanding mediastinal ultrasound images, target selection, number of aspirations needed per target, sample handling, and complications.

  • A new methodology is presented to diagnose faults in equipment plasma. This is accomplished by using neural networks as a pattern recognizer of radio frequency(rf) impedance match data. Using a realtime match monitor system, the match data were collected. The monitor system consisted mainly of a multifunction board and a signal flow diagram coded by Visual Designer. Plasma anomaly was effectively represented by electrical match positions. Twenty sets of fault-symptom patterns were experimentally simulated with experimental variations in process factors, which include rf source power, pressure, Ar and O$_2$ flow rates.   • Based on the Kepco's standard supply terms, the customer should pay for installation the regulating device or protective device which are granted by Kepco in case the customer hinder other customer's electricity usage or it may cause the interruption because of harmonics or waveform distortion. It is necessary to consider the phase angle of voltage, current for harmonics so we can set up the measure of harmonic damages.

  • This study is about modeling pronunciation dictionary necessary for PLU(phoneme like unit) level word recognition. The recognition of nonnative speakers' pronunciation enables an automatic diagnosis and an error detection which are the core of English pronunciation tutoring system. The above system needs two pronunciation dictionaries. One is for representing standard English pronunciation. The other is for representing Korean speakers' English Pronunciation. Both dictionaries are integrated to generate pronunciation networks for variants.

The influence of different scan resolutions on the detection of proximal caries lesions

  • Purpose: This study was conducted to evaluate the effect of different spatial resolutions of a photostimulable phosphor plate (PSP) radiography system on the detection of proximal caries lesions. Materials and Methods: Forty-five extracted human permanent teeth were radiographed using a PSP system (VistaScan Perio Plus) and scanned at the 4 resolutions (10 lp/mm, 20 lp/mm, 25 lp/mm, and 40 lp/mm) available in the system. Three independent examiners scored the images for the presence and absence of proximal caries lesions using a 5-point scale. The presence or absence of caries was confirmed by histological sections of the examined teeth (defined as the gold standard). Intra- and inter-observer reproducibility was calculated by the weighted kappa test. One-way analysis of variance with the post hoc Tukey test was used to compare the area under the receiver operating characteristic curve for the classifications made with each resolution. Results: For the detection of enamel lesions, the spatial resolution of 10 lp/mm was significantly superior to the other resolutions. However, the spatial resolution did not affect the detection of caries lesions in dentin (P>0.05). Conclusion: Spatial resolution may influence the accuracy of the detection of incipient caries lesions in radiographs with PSP plates. Images with low spatial resolution seem to be more appropriate for this purpose.

Comparative Assessment of the Diagnostic Value of Transbronchial Lung Biopsy and Bronchoalveolar Lavage Fluid Cytology in Lung Cancer

  • Background: This study was designed to determine the accuracy of bronchoalveolar lavage fluid cytology (BAL) using histopathologic examination of transbronchial biopsy specimens as the gold standard in diagnosis of lung carcinoma at our center. Materials and Methods: A retrospective study was conducted to investigate a total of 388 patients who were suspected of having lung cancer and had undergone fiberoptic bronchoscopy in Shahid Sadoughi hospital from 2006 to 2011. Lung masses were proven to be malignant by histology. Results: Transbronchial lung biopsy (TBLB) identified malignancy in 183 of the 388 cases, including 48 cases (26.2%) with adenocarcinoma, 4(2.1%) with bronchioloalveolar carcinoma, 47(25.6%)with squamous cell carcinoma, 34(18.5%) with well-diffentiated neuroendocrine carcinoma, 35(19.1%) with small cell carcinoma, 14 (7.6%) with non-small cell carcinoma, and 1 (0.54%) with large cell carcinoma. A total of 205 cases were correctly classified as negative. BAL was also performed in 388 patients; 86/103 cases were consistent with the final diagnosis of lung cancer and 188/285 cases were correctly classified as negative. The sensitivity of BAL was 46.9%(CI:41.9%, 51.8%)) and its specificity was 91.6%(CI:88.8%, 94.3%). BAL had a positive predictive value (PPV) of 83.4%(CI:79.7%, 87.1%) and a negative predictive value (NPV) of 65.8%(CI:61%, 70.5%). The overall accuracy of BAL was 70.5% and the exact concordance was 39%. Conclusions: Our findings suggest that BAL cytology is not sensitive but is a specific test for diagnosis of lung carcinoma. If transbronchial lung biopsy is combined with bronchoalveolar lavage, the positive diagnostic rate will be further elevated.

  • This study was conducted to review data utilization plans and suggest solutions based on the need for tools that can objectively diagnose problems in rural areas. The study extracted items through prior research and was carried out as a process of reviewing data capable of constructing spatial data. The results performed in the above manner are as follows. First, prior research and expert surveys were conducted to select diagnostic items to derive details of a total of three items that should be considered for rural space plans such as regional revitalization and regeneration using regional characteristics such as environment, society, and culture. Second, as a result of listing data available for regional diagnosis, 6 types of environment, 15 types of society, and 18 types of culture were selected out of a total of 529. Finally, it was suggested to establish a national standard spatial unit by grasping the limitations of public data such as omission of spatial data and accuracy. This study is significant in that it presented implications for data utilization as well as selection of items for rural diagnosis and reviewed data utilization based on rural specialized districts of "The Act on Support for Restructuring and Regeneration of Rural Spaces" to be implemented in 2024. This is considered to be valuable as a study for sustainable rural diagnosis if processes such as spatial data construction and weight setting are carried out in the future.

Establishment for Improving Productivity of Cattle by Fecal Steroid and Milk Urea Nitrogen analysis - II. Application of Early Pregnancy Diagnosis and Monitoring Postpartum Ovarian Activity

  • Progesterone levels in the blood plasma or skim milk of cows are considered to be very useful indicator fur the detection of estrus cycle and early pregnancy diagnosis. During 13 to 14 days after estrus the level of progesterone in plasma or skim milk were not different between the inseminated arid non-inseminated cows. In the pregnant cows the peak level of progesterone reached on 14th day after artificial insemination (AI), but in the absence of conceptus the level declines after the 14th day slowly, and then very rapidly towards the basal level after the 17th day. This low level persists about 4 days, including those of estrus and ovulation a highly characteristic pattern which differs so markedly from that in the pregnant cows. Progesterone levels in blood plasma or skim milk can provided a reliable diagnosis of early pregnancy and monitoring ovarian activity in cows. The mean $\pm$ standard deviation of milk urea nitrogen(MUN) and protein concentration in the cows at 9 herds were 17.7$\pm$2.35 mg/dL and 3.2$\pm$0.17%, respectively. The days of nonpregnant after parturition was shorter in the cows in which the lower level of MUN than higher level of MUN concentration.

Does the metal artifact reduction algorithm activation mode influence the magnitude of artifacts in CBCT images?

  • Purpose: This study was conducted to assess the effectiveness of a metal artifact reduction (MAR) algorithm activated at different times during cone-beam computed tomography (CBCT) acquisition on the magnitude of artifacts generated by a zirconium implant. Materials and Methods: Volumes were obtained with and without a zirconium implant in a human mandible, using the OP300 Maxio unit. Three modes were tested: without MAR, with MAR activated after acquisition, and with MAR activated before acquisition. Artifacts were assessed in terms of the standard deviation (SD) of gray values and the contrast-to-noise ratio (CNR) in 6 regions of interest with different distances (10 to 35 mm, from the nearest to the farthest) and angulations(70° to 135°) from the implant region. Results: In the acquisitions without MAR, the regions closer to the implant(10 and 15mm) had a higher SD and lower CNR than the farther regions. When MAR was activated (before or after), SD values did not differ among the regions (P>0.05). The region closest to the implant presented a significantly lower CNR in the acquisitions without MAR than when MAR was activated after the acquisition; however, activating MAR before the acquisition did not yield significant differences from either of the other conditions. Conclusion: Both modes of MAR activation were effective in decreasing the magnitude of CBCT artifacts, especially when the effects of the artifacts were more noticeable.

Implementation of point-of-care platforms for rapid detection of porcine circovirus type 2

  • Background: Porcine circovirus type 2 (PCV2) infection is ubiquitous around the world. Diagnosis of the porcine circovirus-associated disease requires clinic-pathological elements together with the quantification of viral loads. Furthermore, given pig farms in regions lacking access to sufficient laboratory equipment, developing diagnostic devices with high accuracy, accessibility, and affordability is a necessity. Objectives: This study aims to investigate two newly developed diagnostic tools that may satisfy these criteria. Methods: We collected 250 specimens, including 170 PCV2-positive and 80 PCV2-negative samples. The standard diagnosis and cycle threshold (Ct) values were determined by quantitative polymerase chain reaction (qPCR). Then, two point-of-care (POC) diagnostic platforms, convective polymerase chain reaction (cPCR, qualitative assay: positive or negative results are shown) and EZtargex (quantitative assay: Ct values are shown), were examined and analyzed. Results: The sensitivity and specificity of cPCR were 88.23% and 100%, respectively; the sensitivity and specificity of EZtargex were 87.65% and 100%, respectively. These assays also showed excellent concordance compared with the qPCR assay (κ = 0.828 for cPCR and κ = 0.820 for EZtargex). The statistical analysis showed a great diagnostic power of the EZtargex assay to discriminate between samples with different levels of positivity. Conclusions: The two point-of-care diagnostic platforms are accurate, rapid, convenient and require little training for PCV2 diagnosis. These POC platforms can discriminate viral loads to predict the clinical status of the animals. The current study provided evidence that these diagnostics were applicable with high sensitivity and specificity in the diagnosis of PCV2 infection in the field.
