투구 손상의 보존적 치료 (Conservative treatment of throwing injury)

  • 투구 동작은 선수 견관절의 연부 조직이 심한 스트레스를 주며 이와 같은 투구 동작을 성공적으로 시행하기 위해서는 안정된 와상완 관절 위에 좋은 운동 범위를 가지는 것이 필수적이다. 투수의 견관절 손상은 급성 외상에 의하여 발생하는 경우도 있으나 흔히 반복적인 투구에 의한 과사용 손상의 결과로 발생한다. 견관절 주위의 근육이나 연부 조직의 균형이 깨진다면 이는 와상완 관절에 생역학을 변화시켜 이차적으로 손상을 유발할 수 있다 투수가 건강한 견관절을 유지하기 위해 선행되어야 하는 것은 정기적인 견관절 강화 프로그램이지 만 근육의 손상이나 약화로 잘 맞은 균형상태가 깨진다면 견관절의 불안정성과 손상이 발생된다. 치료를 시행하는 의사는 먼저 투구 동작과 각각의 근육 연부 조직의 기능을 이해해 야 하고 이를 바탕으로 투수의 견관절 손상에 대한 이학적 검사와 진단을 시행해야 한다 진단이 시행된 후에는 보존적 치료와 수술적 치료의 장단점을 잘 이해하고 이에 대한 치료 방법을 선택해야 좋은 치료 결과를 기대할 수 있다.

경도의 슬관절 십자 인대 손상환자에 대하여 시행한 열 위축술 (Radiofrequency Shrinkage Method for Minor Degree of Cruciate Ligament Injury of knee joint)

  • 목적: 재건술의적응이되지않는전,후방십자인대손상환자에서radiofrequency 방식을이용한열위축술을시행하여술기및단기추시결과를보고하고자한다. 대상및방법 : 재건술의적응이되지않는전방십자인대9예, 후방십자인대5예를대상으로하였으며평균추시기간은6개월이었다. 결과 : 일상생활에서느꼈던불안정성은감소됐으며파행이나통증등이현저하게호전되었다. 다만후방십자인대에서시행한경우시간이지남에따라다시슬관절이완이증가하는소견을관찰할수있었다. 결론 : Radiofrequency를이용한열위축술은많은활동을요하는젊은사람의슬관절십자인대손상에서, 특히경도의부분파열및수술의적응이되지않는이완환자에서증상을보일경우시도해볼수있는술식중의하나로추천할만하다. 그러나본결과는단기추시이므로장기추시와대조군연구가요할것으로사료된다.

Clinical characteristics of pediatric nasal bone fractures by age group

  • Background: The clinical characteristics of pediatric nasal fractures can vary depending on the child's age, social activities, and environment. Therefore, this study aimed to analyze these characteristics in different age groups. Methods: We retrospectively reviewed of a series of patients aged under 12 years who received treatment between 2013 and 2021. The initial study design involved dividing the patients into four age groups, corresponding to different developmental ages, but there were no cases in infants aged 0 to 1 year. Therefore, the patients were divided into three groups: group I, between 2 and 5; group II, between 6 and 9; and group III, between 10 and 12 years of age. The following parameters were evaluated: sex, age, etiology, fracture type and severity, and the incidence of septal injuries. Results: In total, 98 patients were included in this study. In group III, the ratio of boys to girls was 3.88:1, exceeding the overall ratio of 1.97:1. The most common cause varied with age: slipping down in group I, bumping accidents in group II, and sports accidents in group III. Concomitant septal injuries were present in 4.17% of patients in group I, 5.71% of patients in group II, and 28.21% of patients in group III. Conclusion: Increasing age was accompanied by a greater tendency for male predominance and a higher prevalence of sports-related causes and septal injuries. Violence was infrequent but started to become a contributing factor during school age. These varying environmental factors across age groups can offer valuable insights into the epidemiology and clinical characteristics of pediatric nasal bone fractures.

Clinical characteristics of adolescent nasal bone fractures

  • Background: There have been many reports of nasal bone fractures in adults, but there are few clinical reports of them in adolescents, although adolescence is the main growth period of the nasal bone. In addition, previous studies have tended to classify and describe child and adolescent nasal fractures in the same category. The aim of this study was to identify the clinical aspects of nasal fractures in adolescents, and to evaluate the characteristics of nasal fractures in the growth period. Methods: Our institution's database was reviewed to extract data on adolescent patients between the age of 13 to 18 who had isolated nasal bone fractures from March 2012 to February 2020. The study excluded patients with other facial fractures, previous facial fractures, or congenital deformities. Results: This study included 243 boys and 26 girls. Interpersonal violence and sports-related accidents accounted for 85 and 79 cases, respectively. There were 128 frontal impact injuries and 132 lateral impact injuries, and 97 patients had accompanying septal fractures. An accompanying septal fracture was present in 36.06% of all patients, but in 51.20% of those who underwent surgery. Conclusion: The prevalence of adolescent nasal fractures was significantly higher in boys, to a greater extent than observed for pediatric or adult fractures. The main causes of fractures were interpersonal violence and sports-related accidents. An explanation for these results is that adolescent boys are more prone to have violent accidents or companionship with active sports than other age or sex groups, leading to substantially more fractures.

검도 운동에서 발생된 스포츠 손상의 분석 (Analysis of Sports Injuries in Kendo)

  • 목적: 일반인의 검도 운동 중 발생하는 스포츠 손상에 대하여 조사하고, 손상 기전, 치료 및 임상 결과에 관하여 분석함으로써 검도 손상 환자의 치료에 도움이 되고자 하였다. 대상 및 방법: 대한 검도회에 등록되어 있는 검도 도장의 관원 중 6개월 이상 수련한 고등학생 이상 관원에서 검도를 시작한 후 통증이나 불편감을 느꼈던 총 98명 (평균 연령 : 30.2세)을 대상으로 부위별, 증상별로 구분하였다. 이에 대한 치료를 시행한 경우에는 치료 장소와 방법에 대한 설문 조사와 면담을 실시하였다. 결과: 증상의 출현은 운동 후 2주에서 6개월 이내가 많았으며 32.2%에서 공격을 시도하다가 손상받았다. 운동 기간이 짧은 경우에는 족부 손상으로 인한 족부 통증이 가장 많았고, 이 중 족저근막염이 가장 많았다. 병원 치료는 대부분 조기에 이루어 졌으나, 59명(60.2%)에 불과하였다. 그 중 정형외과를 방문한 경우는 36%, 한의원을 포함한 기타 진료과에서 치료를 받은 경우가 38%였다. 이환부는 족부와 족관절이 가장 많았으며(38.8%), 완관절(13.1%), 견관절(11.9%), 슬관절(11.9%) 순이었다. 치료후 이전 수준으로의 운동 복귀는 50%에서 가능하였다. 결론: 검도 운동에서 발생하는 손상은 족부와 족관절이 가장 많았으며, 완관절, 견관절, 슬관절 순이었다. 적절한 치료를 받은 경우는 조사 대상자의 60.2%에 불과하였고, 이 중 정형외과를 내원한 경우는 36%였다.

Prevalence of Neck Pain among Athletes: A Systematic Review

  • Many studies have investigated the prevalence of neck pain (NP) and its risk factors in the general population. However, the prevalence of NP among athletes has not yet been systematically investigated. We aimed to systematically review the prevalence of NP in athletes. A comprehensive search was conducted in September 2015 using PubMed, Ovid SP Medline, ISI, and Google Scholar. We included studies in English that reported the prevalence of NP in an athletic population during a defined period of time. We assessed the risk of bias in studies due to various definitions of NP, response rates, and reliability of the study instruments. Two reviewers independently assessed the studies' quality and performed data extraction. Of 1,675 titles identified, eight articles were assessed for risk of bias, and six with low or moderate risk were included. NP was shown to be prevalent in athletes, with a 1-week prevalence ranging from 8% to 45%, a 1-year prevalence ranging from 38% to 73%, and a lifetime prevalence of about 48%. The prevalence of NP in athletes is high. More studies regarding the prevalence and risk factors of NP may be useful for planning educational programs and developing appropriate rehabilitation protocols and preventive guidelines. Researchers are encouraged to perform epidemiologic studies in athletes with a low risk of bias.

Immediate Effects of Joint Mobilization Techniques on Clinical Measures in Individuals with CAI

  • Objective: Epidemiological research shows that 47 to 73% of athletes suffer from recurrent ankle sprains. Joint mobilization techniques (JMT) implemented in correcting may be beneficial in the management of ankle injuries. The purpose of this study is to examine the immediate JM on ankle complex as clinical measures in individuals with chronic ankle instability (CAI) through intervention. Method: Thirteen subjects with CAI (8 males and 5 females) participated in this study. Each subject tried total four alignments (Navicular drop test: NDT, Standing rearfoot angle: SRA, Tibia torsion: TT, and dorsiflexion range of motion: DFROM). The participants were performed the 10 meter shuttle run after JMT for post-task. Finally, it was tried to compare between pre-post tasks after shuttle run. Results: SRA and DFROM after intervention showed significant differences. SRA (p=.026), and DFROM (p=.034). Conclusion: We concluded that the JMT has resulted in improvement in SRA, DFROM. Increased DFROM and varus shapes of foot would be closed kinetic chain, indicating that reduce the risk factors of ankle sprain. Future study needs to be conducted in order to measure the effects of prolonged intervention of JMT.

제천스포츠센터의 수직 화재확산 해석을 위한 화재시뮬레이션 분석에 관한 연구 (A Study on the Fire Simulation for Vertical Fire Diffusion Analysis of Jecheon Sports Center)

  • In December 2017, a fire at the Jecheon Sports Center caused 29 deaths and 40 injuries, and about 2 billion won in property damage. It is a facility used by unspecified people as a publicly used establishment with a piloti structure. The damage was expanded due to the ignition of combustible exterior materials, Lack of fire protection in horizontal and vertical penetrations. Although legislation has improved since the fire, it is not retroactively applied, increasing the risk of fire in existing buildings. Accordingly, it is necessary to examine the case of the Jecheon Sports Center fire and draw out the problems. In this study, the fire simulation results and the Jecheon Sports Center fire are comparatively analyzed and used as basic data for fire reduction measures in publicly used establishment. As a result of the fire simulation drive, in the case of the second floor, the room temperature exceeded the human life safety standard of 60 ℃ after about 700 seconds had passed. In the case of three floor, it is predicted that the indoor temperature will exceed about 350 ℃. and temporary complexation will occur in the indoor combustibles.

Effect of Prior Muscle Contraction or Passive Stretching on Eccentric-Induced Muscle Damage

  • Purpose: This tutorial review investigated the effect of prior fatigue and passive stretches on eccentric contraction-induced muscle injuries, as well as the underlying mechanisms of eccentric contraction-related injuries. Methods: Contraction-induced muscle damage is the most common disabling problem in sports and routines. The mechanisms underlying the pathology and prevention of muscle damage lessened by prior fatigue or stretches are critical in assessing musculoskeletal injuries. Even though there are treatments to reduce eccentric contraction-induced muscle injuries, fatigue negatively influences them. Therefore, we reviewed previous studies on eccentric contraction-induced muscle injuries with prior treatments using the MEDLINE and PubMed databases. Results: Prior passive stretching had a preventative and therapeutic effect, but prior lengthening contractions did not. On the other hand, prior isometric contractions involving relatively small forces may not provide a sufficient stimulus to induce protection. As a result, high force isometric contractions may be necessary. The studies supported the positive effects of prior fatigue, concluding that it was a factor in determining the amount of damage caused by eccentric exercise. This was due to a reduction in force and increased temperature. Studies that did not support the positive effects of prior fatigue concluded that a shift in optimal length to a longer length and reduced energy absorption during lengthening are evidence that fatigue is not related to muscle injuries induced by lengthening. Conclusion: The variability of the experiment models, conditions, muscles, and treatment methods make it necessary to interpret the conditions of previous studies carefully and draw conclusions without making direct comparisons. Thus, additional studies should be carefully conducted to investigate the positive effect of fatigue on lengthening.

Vitamin D in athletes: focus on physical performance and musculoskeletal injuries

  • [Purpose] The aim of this review was to discuss the effects of vitamin D on physical performance and musculoskeletal injuries in athletes and provide information on the field applications of vitamin D. [Methods] A systematic review was conducted to identify studies on vitamin D in athletes that assessed serum vitamin D levels, vitamin D and physical performance, vitamin D and musculoskeletal injuries, and practical guidelines for supplementation of vitamin D. [Results] Several studies reported that a high proportion of athletes had vitamin D insufficiency or deficiency. Low serum levels of vitamin D in athletes were more pronounced in winter than in other seasons, and indoor athletes had lower serum vitamin D levels than outdoor athletes. Low vitamin D levels have been demonstrated to have negative effects on muscle strength, power, and endurance; increase stress fractures and other musculoskeletal injuries; and affect acute muscle injuries and inflammation following high-intensity exercises. Therefore, periodic assessment and monitoring of vitamin D levels are necessary in athletes; the recommended serum level of 25(OH)D is > 32 ng/mL and the preferred level is > 40 ng/mL (-1). In those with low levels of vitamin D, exposure to sunlight and an improved diet or supplements may be helpful. Particularly, 2000-6000 IU of supplemental vitamin D3 can be consumed daily. [Conclusion] Vitamin D is a potential nutritional factor that can significantly affect physical performance and musculoskeletal injuries in athletes. The importance and role of vitamin D in athletes should be emphasized, and the current levels of vitamin D should be assessed. Therefore, it is essential to periodically evaluate and monitor serum vitamin D levels in athletes.
