척추 신경섬유종 진단을 받은 요통 환자에 대한 복합적 한방처치 및 영상학적 경과 관찰 증례 보고 1례 (A Patient with a Diagnosis of Spinal Neurofibroma and Complaining of Lower Back Pain: A Case Report, Including Two Follow-ups after Discharge)

  • Objective: This study was performed to report the changes in symptoms and radiological consequences of a patient diagnosed with spinal neurofibroma and complaining of lower back pain. Methods: The patient was diagnosed with a spinal neurofibroma after MRI examination and was treated with traditional Korean medicine, including traditional Korean medication and acupuncture. We measured the change in symptom severity using a numerical rating scale (NRS) and the Oswestry Disability Index (ODI). We also used the EQ-5D (EuroQoL-5D) scale to assess the patient's quality of life. In addition, we examined the change in the size of the cyst by MRI twice after discharge: at about 3 months after discharge and at about 8 months after discharge. Results: After about 6 weeks of treatment, most pathological symptoms had decreased. The patient showed a decline in NRS and ODI scores and showed an increase in quality of life. The two MRI examinations after the discharge revealed a decrease in the size of the cyst. Conclusions: Traditional Korean medicine can be a solution for patients with spinal neurofibroma causing neurological pain.

종격동에 발생한 다발성 신경섬유종증 -3례 보고- (Neurofibromatosis (Von Recklinghausen's Diseased) in Mediastinum -3 case-)

  • We report three cases of posterior mediastinal tumor in Von Recklinghausen's disease on 12-yearsy old boy, 8-years-old girl and 6-years-old boy. Especially the last case was combinded with "Dumbbell" neurilemmoma of cervical spine. In the first case, preoperative CT scan showed a homogeneous large mass In the posterior mediastinum. But the mass did not show intraspinal invasion on the chest MRI scan. In the last case, cervical spine CT with thecal enhancement shows dumbbell shaped neurofibroma, compressed spinal cord and thecal sac In three cases we removed all the masses successfully and the patients were discharged safely without significant problem.t problem.

A Dumbbell-Shaped Meningioma Mimicking a Schwannoma in the Thoracic Spine

  • A 50-year-old man presented bilateral hypesthesia on and below the T6 dermatome and paresthesia. Magnetic resonance imaging (MRI) showed an intraspinal extradural tumor, which located from the 6th thoracic vertebral body to the upper margin of the 7th vertebral body, continuing dumbbell-like through the intervertebral foramen into the right middle thorax suggesting a neurogenic tumor (neurofibroma or neurilemmoma). With the patient in a prone position, we exposed and excised the tumor via a one stage posterior approach through a hemi-laminictomy of T6. Histologic examination showed a grade 1 meningothelial meningioma, according to the World Health Organization classification. Initially, we assumed the mass was a schwannoma because of its location and dumbbell shape. However, the tumor was actually a meningioma. Postoperatively, hypesthesia resolved completely and motor power of the leg gradually full recovered. A postoperative MRI revealed no evidence of residual tumor.
