The Ability of Muscle Functional MRI to Detect the Slight Effect of Exercise on Trunk Muscle Activity

  • Purpose: In this study, we provide a way to assess even a slight effect of exercise on trunk-muscle activity. Materials and Methods: Seven healthy male participants (mean age, 24.7 ± 3.2 years; height, 171.2 ± 9.8 cm; and weight, 63.8 ± 11.9 kg) performed 15 sets of an exercise with 20 repetitions of 90° hip and right-knee flexion while lying supine. The exercise intensity was measured using the 10-point Rating of Perceived Exertion Scale after the first and 15th sets of exercises. Although cross-sectional areas and functional T2 mapping using ultrafast imaging (fast-acquired muscle functional magnetic resonance imaging, fast-mfMRI) have been proposed for imaging to evaluate exercise-induced muscle activity in real time, no previous studies have reported on the evaluation of trunk-muscle activity using functional T2 mapping. As a method for assessing trunk-muscle activity, we compared functional T2 mapping using ultrafast imaging (fast-mfMRI) with cross-sectional areas. Results: Although the muscle cross-sectional areas were increased by the exercise, there was no significant difference at rest. On the other hand, for all sets, the changes in T2 were significant compared with those at rest (P < 0.01). These results demonstrate that T2, calculated from fast-mfMRI images can be used to detect even a small amount of muscle activity induced by acute exercise, which was impossible to do with cross-sectional areas. Conclusion: Fast-mfMRI, which can also display functional information with detailed forms, enabled non-invasive real-time imaging for identifying and evaluating the degree of deep trunk-muscle activity induced by exercise.

Detection of Malignant Primary Hepatic Neoplasms with Gadobenate Dimeglumine (Gd-BOPTA) Enhanced T1-Weighted Hepatocyte Phase MR Imaging: Results of Off-site Blinded Review in a Phase-II Multicenter Trial

  • Objective: To investigate the efficacy of gadobenate dimeglumine (GdBOPTA) enhanced MR imaging for the detection of liver lesions in patients with primary malignant hepatic neoplasms. Materials and Methods: Thirty-one patients with histologically proven primary malignancy of the liver were evaluated before and after administration of GdBOPTA at dose 0.05 or 0.10 mmol/kg. T1-weighted spin echo (T1W-SE) and gradient echo (T1W-GRE) images were evaluated for lesion number, location, size and confidence by three off-site independent reviewers and the findings were compared to reference standard imaging (intraoperative ultrasound, computed tomography during arterial portography or lipiodol computed tomography). Results were analyzed for significance using a two-sided McNemar's test. Results: More lesions were identified on Gd-BOPTA enhanced images than on unenhanced images and there was no significant difference in lesion detection between either concentration. The largest benefit was in detection of lesions under 1 cm in size (7 to 21, 9 to 15, 16 to 18 for reviewers A, B, C respectively). In 68% of the patients with more than one lesion, Gd-BOPTA increased the number of lesions detected. Conclusion: Liver MR imaging after Gd-BOPTA increases the detection of liver lesions in patients with primary malignant hepatic neoplasm.

  • Purpose: To evaluate the availability of magnetic resonance (MRI voiding cystography for the diagnosis of vesicoureteral reflux (VUR) and to compare the sensitivity of MR voiding cystography (MRVC) with that of radiographic voiding cystourethrography (VCUG) in the detection of VUR. Material and Methods : MRVC was performed upon 20 children referred for investigation of VUR. Either coronal T1-weighted spin-echo or spoiled gradient-echo images were obtained before and after transurethral administration of a mixture of normal saline and gadopentetate dimeglumine, and immediately after voiding. The findings of MRVC were compared with those of VCUG performed within 6 months of MRVC. Results 1 VUR was detected in 23 ureterorenal units f16 VUR's by both methods, five VUR's by VCUG, and two VUR's by MRVC). The sensitivity of VCUG and MRVC in detecting VUR was 91.3% (21/23) and 78.3% (18/23), respectively. MRVC detected renal scarring in 15 out of 17 kidneys with scintigraphically detected renal scarring. Conclusion : Although MRVC is slightly less sensitive than VCUG in the detection of VUR, it can be used for the diagnosis of VUR and renal scarring simultaneouslyl and thus will reduce the radiation hazard.

The V(IV) Species, Locaton and Adsorbate Interaction in VH-SAPO-11 studied by ESR and ESEM

  • Vanadium-incorporated aluminophophate molecular sieve VH-SAPO-11 has been studied by electron spin resonanace (ESR) and electron spin echo modulation (ESEM) spectroscopies to determine the vanadium locatin and interaction with various adsorbate molecules. As-synthsized VH-SAPO-11 contains only vanady1 species with distored octahral coordination. After calcinations in $O_2$ and exposure to moisture, only species A is observed with reduced intensities. Species A is suggested as a VO$(H_2O)_2^{2+$} complex coordinate to three framwork oxygen bonded to aluminum. When calcined, hydrate VH-SAPO-11 is dehydrated at elevated temperature, species A loses it water ligands and transforms to $VO^{2+}$ ions coordinated to three framework oxygens (species B). Species B reduces its intensities significantly after treatment with $O_2$at high temperature, thus suggesting oxidation of $v^{4+}$to $v^{5+}$. When dehydrated VH-SAPO-11 contacts with $D_2O$ at room temperature, the ESR signal of species A is observed. This species assumed as a $VO(O_f)_3(D_2O)_2$, by considering 3 framework oxygens. Adsorption of deuterated methanol on dehydrated VH-SAPO-11 results in another new vanadium species D, which is identified as a $VO(CD_{3}OH)$ complex. When deuterated ethanol is adsorbed on dehydrated VH-SAPO-11, another new vanadium species E identified as a $VO(C_{2}H_{5}OD)^{2+}$, is observed. When deuterated propanol is adsorbed on dehydrated VH-SAPO-11, a new vanadium species F identified as a $VO(C_{3}H_{7}OD)$, is observed. Possible coordination geometries of these various complexes are discussed.

  • The purpose of this study was to investigate the FA value which can produce the best T2-weighted images by measuring the signal intensity and noise according to the FA value change in the brain image and the abdominal image of the mouse using micro-MRI. Brain imaging and abdominal imaging of BALB / C mice weighing 20g were performed using 4.7T (Bruker BioSpin MRI GmbH) micro-MRI equipment, Turbo RARE-T2 (spin echo-T2) images were scanned at TR 3500 msec and TE 36 msec. The changes of the FA values were $60^{\circ}$, $80^{\circ}$, $100^{\circ}$, $120^{\circ}$, $140^{\circ}$, $160^{\circ}$ and $180^{\circ}$. We measured signal intensity according to FA values of ventricle and thalamus in brain imaging, The signal intensity of kidney and muscle around the kidney was measured in abdominal images. To obtain SNR and CNR, we measured the background signals of two different parts, not the tissue. In the brain (thalamus) image, the signal intensity of FA $100^{\circ}$ was 7,433 and SNR (6.49) was the highest. In the abdominal (kidney) image, the signal intensity was highest at 16,523 when FA was $120^{\circ}$, and the highest SNR was 8.54 when FA was $140^{\circ}$. The CNR value of the brain image was 1.38 at FA $60^{\circ}$ and gradually increased to 8.29 at FA $180^{\circ}$. The CNR value of the muscle adjacent to the kidney gradually increased from 2.36 when the FA value was $60^{\circ}$ and the highest value was 4,57 at the FA value $180^{\circ}$.

The Evaluation of Optimized Inversion-Recovery Fat-Suppression Techniques for T2-Weighted Abdominal MR Imaging : Preliminary report (복부의 T2강조 영상에서 지방소거기법의최적의 평가)

  • To test the real image quality of a spectral attenuated inversion-recovery (SPAIR) fat-suppression (FS) techniquein clinical abdominal MRI by comparison to turbo spin echo inversion-recovery (TSEIR) fat-suppression (FS) technique. 3.0T MRI studies of the abdomen were performed in 30 patients with liver lesions (hemangiomas n: 15; HCC n: 15). T2W sequences were acquired using SPAIR TSEIR. Measurements included retroperitoneal and mesenteric fat signal-to-noise (SNR) to evaluate FS; liver lesion contrast-to-noise (CNR) to evaluate bulk water signal recovery effects; and bowel wall delineation to evaluate susceptibility and physiological motion effects. SPAIR-TSEIR images produce significantly improved FS and liver lesion CNR. The mean SNR of the retroperitoneal and mesenteric fat for SPAIR were 20.5, 10.2 and TSEIR were 43.2, 24.1 (P<0.05). SPAIR-TSEIR images produced higher CNR for both hemangiomas CNR 164.88 vs 126.83 (P<0.05) and metastasis CNR 75.27 vs 53.19 (P<0.05). Bowel wall visualization was significantly improved using in both SPAIR-TSEIR (P< 0.05). The real image quality of SPAIR was better than over conventional TSEIR FS on clinical abdominal MRI scans.

  • A 3-year-old female, 5kg, Shih-tzu developed an acute onset of depression, disorientation, hypersalivation, nystagmus after falling down 2 meter height place. In plain skull radiography, there was fracture line in the frontal and parietal bones and next day magnetic resonance imaging examination was performed. Magnetic resonance imaging of the brain was performed with 3.0 Tesla unit. Under general anesthesia, the dog was placed in prone with its head positioned in a birdcage coil. Transverse, sagittal and coronal fast spin echo images of the brain were obtained with the following pulse sequences: T1 weighted images (TR = 560 ms and TE = 18.6 ms) and T2 weighted images (TR = 3500 ms and TE = 80 ms). Magnetic resonance imaging showed epidural hematoma in the left frontal area resulting in compression of the adjacent brain parenchyma. Left lateral ventricle was compressed secondarily and the longitudinal fissure shifted to the right, representing mass effect. The lesion was iso-to slightly hyperintense on T1 weighted image and iso-slightly hypointense signal on T2 weighted image. At necropsy, there was a skull fracture and epidural hematoma in the left frontal area. Magnetic resonance imaging of epidural hematoma is reviewed.

  • A new approach to silent MR imaging using a rotating DC gradient has been explored and experimentally studied. As is known, acoustic or sound noise has been one of the major problems in handling patients, mainly due to the fast gradient pulsings in interaction with the main magnetic field. The sound noise is also proportionally louder as the magnetic field strength becomes larger. In this article, we have described a new imaging technique using a mechanically rotating DC gradient coil as an approach toward silent MR imaging, i.e., a mechanically rotated DC gradient effectively replaces both the phase encoding as well as the readout gradient pulsings and data obtained in this manner provides a set of project ion data which later can be used or the projection reconstructionorwithsomeinterpolation techniques one can also perform conventional 2-D FFT (Fast Fourier Transform) image reconstruction. We found, with this new technique, that the sound noise intensity compared with the conventional imaging technique, such as spin echo sequence, is reduced down to -20.7 dB or about 117.5 times. The experimental pulse sequence and its principle are described and images obtained by the new silent MR imaging technique are reported.

Evaluation of Modified Turbo Spin Echo Technique Compared with Double Inversion Recovery Technique in Acquisition of Black Blood Brain Vessel Image

  • The main goal was to evaluate effectiveness of a modified TSE sequence compared with DIR (double inversion recovery) sequence in acquisition of fast flow brain vessel images using signal void effect. 32 healthy volunteers (10 men and 22 women; mean age of 31 years; ranging between 28-43 years) who underwent black blood DIR sequence (group A) and the modified TSE sequence (group B) were enrolled in our study. Signal to Noise Ratio (SNR) and Contrast to Noise Ratio (CNR) of the internal carotid arteries' lumen were compared in T1 and T2 weighted images for both group A and B. The images obtained from group B showed lower SNR values in internal carotid artery than the group A in both of the T1 and T2 weighted images (11.49% and 13.66% respectively). While the CNR values were higher in the group B than the group A in both of the T1 and T2 weighted images (8.69% and 7.55 % respectively). The qualitative score of all categories were not significantly different between the two groups. Furthermore approximately 49% of the total scan time was reduced from group B. Our study is to shorten the scanning time and minimize the inconveniences of the patients in acquisition of the black blood images of brain by using the signal void effect in the modified TSE technique while keeping the diagnostic value of the test.

  • 계면활성제는 수용액에서 미셀을 형성함에 있어서 자기확산이 분자 운동과 상변화에 대하여 자세한 정보를 제공하고 공업적으로 응용할 수 있는 기술이 점차 증가하고 있다. 계면활성제가 미셀을 형성함에 있어 자기확산 정도는 화학구조의 변화성과 상호 결합 및 회합현상에 매우 민감한 것은 사실이다. 특히 계면활성제 용액의 넓은 범위의 분자 시스템과 콜로이드 상태의 변화성은 다향한 물리, 화학적 성질에 기여됨이 많다. 더욱이 미셀 형성에서 자기확산 계수는 분자 치환에 직간접적으로 상호관계가 있어 NMR 분광학에서 스핀 이완속도의 해석과 분자의 재배열, 스핀 이완에 대한 모델선정 등에 많은 관심을 갖는다. 그중 미셀형성에 있어서 자기확산에 대한 측정 방법중 가장 많이 이용되고 있는 Fourier Transform Pulsed Gradient Spin Echo(FT-PGSE) 측정법은 계면활성제의 미셀형성에 대한 상변화성 및 물리, 화학적 성질을 다루는데 새로운 도구로 제공되고 있다. 이는 이 계통의 기술적 측정방법에 있어서 적절한 개선과 새로운 응용분야를 확장하는데 있어서 많은 가능성을 갖고있다. 그리하여 이들에 대한 역사적 배경과 기초적인 이론을 가지고 미셀 형성에 있어 자기확산에 대한 개념을 말하고 그에 대한 응용성을 계통적으로 설명하고자 한다.
