Tumor-like Presentation of Tubercular Brain Abscess: Case Report

  • A 17-year-old girl presented with complaints of headache and decreasing vision of one month's duration, without any history of fever, weight loss, or any evidence of an immuno-compromised state. Her neurological examination was normal, except for papilledema. Laboratory investigations were within normal limits, except for a slightly increased Erythrocyte Sedimentation Rate (ESR). Non-contrast computerized tomography of her head revealed complex mass in left frontal lobe with a concentric, slightly hyperdense, thickened wall, and moderate perilesional edema with mass effect. Differential diagnoses considered in this case were pilocytic astrocytoma, metastasis and abscess. Magnetic resonance imaging (MRI) obtained in 3.0 Tesla (3.0T) scanner revealed a lobulated outline cystic mass in the left frontal lobe with two concentric layers of T2 hypointense wall, with T2 hyperintensity between the concentric ring. Moderate perilesional edema and mass effect were seen. Post gadolinium study showed a markedly enhancing irregular wall with some enhancing nodular solid component. No restricted diffusion was seen in this mass in diffusion weighted imaging (DWI). Magnetic resonance spectroscopy (MRS) showed increased lactate and lipid peaks in the central part of this mass, although some areas at the wall and perilesional T2 hyperintensity showed an increased choline peak without significant decrease in N-acetylaspartate (NAA) level. Arterial spin labelling (ASL) and dynamic susceptibility contrast (DSC) enhanced perfusion study showed decrease in relative cerebral blood volume at this region. These features in MRI were suggestive of brain abscess. The patient underwent craniotomy with excision of a grayish nodular lesion. Abundant acid fast bacilli (AFB) in acid fast staining, and epithelioid cell granulomas, caseation necrosis and Langhans giant cells in histopathology, were conclusive of tubercular abscess. Tubercular brain abscess is a rare manifestation that simulates malignancy and cause diagnostic dilemma. MRI along with MRS and magnetic resonance perfusion studies, are powerful tools to differentiate lesions in such equivocal cases.

Detecting Peripheral Nerves in the Elbow using Three-Dimensional Diffusion-Weighted PSIF Sequences: a Feasibility Pilot Study

  • Purpose: To analyze the feasibility of three-dimensional (3D) diffusion-weighted (DW) PSIF (reversed FISP [fast imaging with steady-state free precession]) sequence in order to evaluate peripheral nerves in the elbow. Materials and Methods: Ten normal, asymptomatic volunteers were enrolled (6 men, 4 women, mean age 27.9 years). The following sequences of magnetic resonance images (MRI) of the elbow were obtained using a 3.0-T machine: 3D DW PSIF, 3D T2 SPACE (sampling perfection with application optimized contrasts using different flip angle evolution) with SPAIR (spectral adiabatic inversion recovery) and 2D T2 TSE (turbo spin echo) with modified Dixon (m-Dixon) sequence. Two observers used a 5-point grading system to analyze the image quality of the ulnar, median, and radial nerves. The signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of each nerve were measured. We compared 3D DW PSIF images with other sequences using the Wilcoxon-signed rank test and Friedman test. Inter-observer agreement was measured using intraclass correlation coefficient (ICC) analysis. Results: The mean 5-point scores of radial, median, and ulnar nerves in 3D DW PSIF (3.9/4.2/4.5, respectively) were higher than those in 3D T2 SPACE SPAIR (1.9/2.8/2.8) and 2D T2 TSE m-Dixon (1.7/2.8/2.9) sequences (P < 0.05). The mean SNR in 3D DW PSIF was lower than 3D T2 SPACE SPAIR, but there was no difference between 3D DW PSIF and 2D T2 TSE m-Dixon in all of the three nerves. The mean CNR in 3D DW PSIF was lower than 3D T2 SPACE SPAIR and 2D T2 TSE m-Dixon in the median and ulnar nerves, but no difference among the three sequences in the radial nerve. Conclusion: The three-dimensional DW PSIF sequence may be feasible to evaluate the peripheral nerves around the elbow in MR imaging. However, further optimization of the image quality (SNR, CNR) is required.

Fe-Al 전이금속 화합물의 자성과 결정구조의 상관관계에 대한 밀도범함수연구 (Density Functional Study on Correlation between Magnetism and Crystal Structure of Fe-Al Transition Metal Compounds)

  • Fe-Al 계의 전이금속 화합물은 구조적 안정성과 자성에 대한 이견이 많은 것으로 알려져 있다. 본 연구에서는 B2, $L1_2$, 및 $D0_3$ 구조를 갖는 Fe-Al 계 화합물의 자성과 원자구조의 상관관계를 논의하기 위해 전전자 총퍼텐셜선형보강평면파(all-electron fullpotential linearized augmented plane wave; FLAPW) 방법을 이용하여 일반기울기 근사(generalized gradient approximation; GGA) 하에서 계산하였다. 고려한 모든 구조에서 강자성이 비자성에 비해 안정한 것으로 계산되었다. Fe 원자의 계산된 스핀 자기모멘트는 B2, $L1_2$ 구조에서 각각 0.771 ${\mu}_B$, 2.373 ${\mu}_B$의 값을 얻었고, $D0_3$ 구조의 Fe(I) 및 Fe(II) 원자들은 각각 2.409 ${\mu}_B$, 1.911 ${\mu}_B$로 계산 되었다. $Fe_3Al$의 같은 조성을 갖는 $L1_2$$D0_3$ 구조간의 안정성을 조사하기 위하여 형성 엔탈피 계산을 통해 강자성 상태에서 16 meV/atom의 에너지 차이로 $D0_3$ 구조가 $L1_2$ 구조보다 더 안정함을 확인하였다. 이 결과는 실험연구와 잘 일치하며, 원자구조와 전자구조의 분석을 통해 구조적 안정성과 자성에 대해 이해하였다.

Drying seaweeds using hybrid hot water Goodle dryer (HHGD): comparison with freeze-dryer in chemical composition and antioxidant activity

  • Seaweeds are a potential source of minerals, essential amino acids, fatty acids, proteins, and various bioactive compounds such as antioxidants. The higher water content of seaweeds reduces the shelf life and this requires the appropriate drying method. The drying conditions play a major role in the conservation of nutrient composition in dried seaweeds. In recent years, the seaweed industry has used many different drying methods with advantages and limitations. Hybrid hot-water Goodle dryer (HHGD) which is a special dryer mixed with hot-water and a Korean traditional heating system (Goodlejang) might be a solution to avoid these limitations. The present study evaluated the effect of drying conditions in HHGD on nutrient composition and bioactivities of brown seaweeds. Moreover, freeze-dryer (FD) and HHGD were employed in this study to compare the dried outputs obtained from four brown seaweed species. The present study aims to evaluate the effect of the hybrid hot-water Goodle drying method (HHGDM) on the nutritional composition and antioxidant activity of dried seaweeds. AOAC standard methods were used to analyze the proximate composition of dried samples and their 70% ethanol extract. The intracellular and extracellular antioxidant activities were evaluated using Vero cells and electron spin resonance (ESR) spectrometer respectively. High performance liquid chromatography, apoptotic body formation, and in-vivo experiments were used for further confirmation of the quality of dried output. The proximate composition results obtained from drying in HHGD and FD did not exhibit any significant difference. Moreover, the seaweed extracts from the dried seaweeds by HHGD and FD dryings were also not different and both significantly down-regulated in-vivo and in-vitro oxidative stress. Furthermore, the high performance liquid chromatography results revealed that the two dryers did not make the major peaks different in the chromatograms. Freeze-drying method (FDM) provides elevated quality for dried output, but there are limitations such as high cost and low capacity. The results from a novel HHGD did not provide any significant difference with the results in FD and expressed a potential to avoid the limitations in FD. Overall, these findings solidified the applicability of HHGD over FD.

광전자분광법을 이용한 Co/Pd 다층박막의 전자구조연구 (Valence Band Photoemission Study of Co/Pd Multilayer)

  • 광전자분광법 (Photoelectron Spectroscopy : PES)을 이용하여, 차세대 광자기 기록매체로 유망한 Co/Pd 다층박막의 전자구조를 연구하였다. Co/Pd 다층박막의 Co 3d 전자 PES 스펙트럼에서는 페르미 에너지 ($E_{F}$) 근처에 폭이 좁은 피이크가 관찰되었고, 아울러 $E_{F}$로부터 약 2.5 eV 아래에 폭이 넓은 피이크도 관찰되었다. 그 중 $E_{F}$ 근처에 위치한 피이크의 폭은 bulk Co 3d 전자 PES 스펙트럼에서의 피이크폭에 비하여 훨씬 좁았는데, 이러한 차이는 Co 자기모멘트가 Co/Pd 다층박막에서 buik Co 에 비하여 증진되는 현상과 일치한다. 한편 $E_{F}$ 아래 2.5 eV에 의치한 피이크는 Pd의 valence band 구조와 유사함이 발견되었는데, 이는 Co 단층과 Pd 단층간에 상당한 상호작용 (hybridization)이 있음을 나타낸다고 볼 수 있다. Co/Pd 다층박막에 대하여 실험적으로 결정한 Co 3d 전자 Pes 스펙트럼을 국재스핀밀도함수이론을 이용하여 얻은 이론적 전자구조 계산결과와 비교하였다. 이상의 비교에 의하면 밴드이론계산에 의한 Co 3d 밴드폭은 실험과 잘 일치하였으나, PES 스펙트럼에서 관찰되어진 $E_{F}$ 근처의 폭이 좁은 피이크는 밴드이론이 잘 기술하지 못함이 발견되었다.

Added Value of Chemical Exchange-Dependent Saturation Transfer MRI for the Diagnosis of Dementia

  • Objective: Chemical exchange-dependent saturation transfer (CEST) MRI is sensitive for detecting solid-like proteins and may detect changes in the levels of mobile proteins and peptides in tissues. The objective of this study was to evaluate the characteristics of chemical exchange proton pools using the CEST MRI technique in patients with dementia. Materials and Methods: Our institutional review board approved this cross-sectional prospective study and informed consent was obtained from all participants. This study included 41 subjects (19 with dementia and 22 without dementia). Complete CEST data of the brain were obtained using a three-dimensional gradient and spin-echo sequence to map CEST indices, such as amide, amine, hydroxyl, and magnetization transfer ratio asymmetry (MTRasym) values, using six-pool Lorentzian fitting. Statistical analyses of CEST indices were performed to evaluate group comparisons, their correlations with gray matter volume (GMV) and Mini-Mental State Examination (MMSE) scores, and receiver operating characteristic (ROC) curves. Results: Amine signals (0.029 for non-dementia, 0.046 for dementia, p = 0.011 at hippocampus) and MTRasym values at 3 ppm (0.748 for non-dementia, 1.138 for dementia, p = 0.022 at hippocampus), and 3.5 ppm (0.463 for non-dementia, 0.875 for dementia, p = 0.029 at hippocampus) were significantly higher in the dementia group than in the non-dementia group. Most CEST indices were not significantly correlated with GMV; however, except amide, most indices were significantly correlated with the MMSE scores. The classification power of most CEST indices was lower than that of GMV but adding one of the CEST indices in GMV improved the classification between the subject groups. The largest improvement was seen in the MTRasym values at 2 ppm in the anterior cingulate (area under the ROC curve = 0.981), with a sensitivity of 100 and a specificity of 90.91. Conclusion: CEST MRI potentially allows noninvasive image alterations in the Alzheimer's disease brain without injecting isotopes for monitoring different disease states and may provide a new imaging biomarker in the future.

로터세일의 배열 형태가 양력 형성에 미치는 영향에 관한 수치해석적 연구 (Numerical Study on the Effect of the Arrangement Type of Rotor Sail on Lift Formation)

  • 최근 국제해사기구(IMO)를 비롯한 국제사회에서 선박의 대기오염 배출 규제를 강화하고 있으며, 배기가스 배출을 줄이기 위한 친환경 선박 개발이 활발하게 이루어지고 있다. 그중에서도 풍력 보조 선박추진 시스템 중 하나인 로터 세일(Rotor Sail, RS)이 다시금 주목받고 있다. RS는 선박 데크에 설치되는 원통형 실린더 장치로 마그누스 효과를 사용하여 유체 동역학적 양력을 생성하는 장치이다. 이는 차세대 친환경 보조 추진 기술 중 하나이며, RS 적용 선박을 개발한 Enercon 사(社)에서는 약 30% 이상의 연료 절감이 가능하다고 발표했다. 본 연구에서는 다중 RS를 선박에 설치할 경우 RS 간격 및 배열 형태와 같은 최적의 설치 조건을 선정하고자 하였으며, RS 배열에 따른 유동특성을 확인하기 위하여 AR(Aspect Ratio) = 5.1, SR(Spin Ratio) = 1.0 및 로터세일 지름과 엔드 플레이트 지름 비(De/D)= 2.0 로 고정하고 자유 유속 U = 5 m/s로 풍향은 +y 축 단방향에 대한 조건만 고려하였다. 배열 조건은 횡방향 거리는 +x 축 방향으로 3D ~ 15D까지 3D 간격으로 총 5가지 조건을, 종방향 거리는 +y 축 방향으로 5D ~ 25D까지 5D 간격으로 총 5가지 조건을 설정하였으며, 사각 형태(□)와 마름모 형태(◇) 배열에 따른 양력계수(CL), 항력계수(CD)와 공기역학적 효율(CL/CD)을 비교하였다. 결과적으로 종방향 간격에 따른 RS의 영향은 크게 차이가 없었으나, 횡방향 간격에 따른 RS 유동특성의 경우 두 RS가 바람 방향에 거의 일치할 때 RS의 상호작용 효과가 가장 크게 나타났다. 배열에 따른 RS 유동특성의 경우, 전방(0°) 방향에서 바람이 불 때 마름모 형태(◇) 배열이 RS 간의 후류 영향을 가장 덜 받는 것으로 나타났다.
