Suggestions for the Study of Acupoint Indications in the Era of Artificial Intelligence (인공지능시대의 경혈 주치 연구를 위한 제언)

  • Artificial intelligence technology sheds light on new ways of innovating acupuncture research. As acupoint selection is specific to target diseases, each acupoint is generally believed to have a specific indication. However, the specificity of acupoint selection may be not always same with the specificity of acupoint indication. In this review, we propose that the specificity of acupoint indication can be inferred from clinical data using reverse inference. Using forward inference, the prescribed acupoints for each disease can be quantified for the specificity of acupoint selection. Using reverse inference, targeted diseases for each acupoint can be quantified for the specificity of acupoint indication. It is noteworthy that the selection of an acupoint for a particular disease does not imply the acupoint has specific indications for that disease. Electronic medical record includes various symptoms and chosen acupoint combinations. Data mining approach can be useful to reveal the complex relationships between diseases and acupoints from clinical data. Combining the clinical information and the bodily sensation map, the spatial patterns of acupoint indication can be further estimated. Interoperable medical data should be collected for medical knowledge discovery and clinical decision support system. In the era of artificial intelligence, machine learning can reveal the associations between diseases and prescribed acupoints from large scale clinical data warehouse.

  • Objectives : This study aims to show the rationale of point-selection with regard to acupuncture and moxibustion as described in the Hand section(the Oehyeong chapter) of Dong Ui Bo Gam. Methods : First, the cause of each disease in the Hand section(the Oehyeong chapter) of Dong Ui Bo Gam was summarized, based on which the rationale of acupoint-selection with regard to the cause of disease, physiology of the Oriental medicine, properties of each acupoints, flow of meridian pathways and specific acupoints, etc. Results and Conclusions : The most of the acupoints referred to in the Hand section(the Oehyeong chapter) of Dong Ui Bo Gam, belong to the area surrounding the tender point. Still, when a point around the tender point is taken, it does not simply mean it works because of its proximity to the spot of a specific symptom. Rather, such acupoint-selection should be explained by the cause of disease, physiology of the Oriental Medicine and properties of each acupoint along with aspects such as the flow of meridian pathways and properties of specific acupoint.

The Effect of Smoking on the Bioelectrical Capacitance Measured at Specific Acupoints of Lung Meridian: A Cross-Over Study (흡연이 수태음폐경 특정혈의 체표생체전하에 미치는 영향: 교차대조 연구)

  • Objective : The objective of this study is to investigate the effects of smoking on the skin bioelectrical capacitance at specific acupoints of lung meridian. Methods : Bioelectrical capacitance was measured on bilateral six source points(bilateral LU10, LU9, LU7, LU6, LU5, LU1), and the changes with time and between left and right side were analyzed. Results : The skin bioelectrical capacitance at specific acupoints of lung meridian was significantly increased after smoking. And it recovered as time passed. The change of the skin bioelectrical capacitances at specific acupoints of lung meridian with time were similar between left and right. Conclusion : Smoking increases the bioelectrical capacitance at specific acupoints of lung meridian. There is no difference between the effects of smoking on the bioelectrical capacitance at left and right specific acupoints of lung meridian.

Methods : First, We summarized the cause of each disease in the Rhinological section(in the Oehyeong chapter) of the Dong Ui BO Gam. Then, We explained the rationale of acupoint-selection referring to the cause of disease, physiology of the Oriental medicine, other uses of each acupoints in the Dong Ui BO Gam, character of each acupoints, flow of meridian pathways and specific acupoints etc. Results and Conclusions : Total 15 acupoints were used in the Rhinological section(in the Oehyeong chapter) of the Dong Ui BO Gam. Most of acupoints were specific acupoints. But, some rationale of acupoint-selection were explained by the cause of disease, physiology of the Oriental medicine, other uses of each acupoints in the Dong Ui BO Gam, flow of meridian pathways etc.

The Effect of Electroacupuncture at the $H_7$ (Shinmun) on the EEG (신문$(H_7)$ 전침 자극이 EEG의 변화에 미치는 영향)

  • Objectives : The aim of this study was to examine the effects of electroacupuncture(EA) at the $H_7$ (Shinmun) on normal humans using power spectral analysis. Methods : EEG power spectrum exhibit site-specific and state-related differences in specific frequency bands. In this study, power spectrum was used as a measure of complexity. 32 channel EEG study was carried out in 9 subjects (9 males; age=25,8 years). Results : In alpha band, the power values at Fp2 channels(p<0.05) during the $H_7-acupoint$ treatment significantly were decreased. In beta band, the power values at Fp1, Fp2 channels(p<0.05) during the $H_7-acupoint$ treatment significantly were decreased. In delta and theta band, the power values at the $H_7-acupoint$ treatment significantly was increased than the before-acupuncture treatment. Conclusions : This results suggest that electroacupuncture at the $H_7$ is significantly in beta band of EEG.

  • Objectives : The aim of this study is to introduce pain measurement tools that are considered suitable for clinical practice and research for Korean Medicine Doctors. Methods : We analysed some widely used and also useful pain measurement tools in terms of their methods and dimensions. Results : Diagrams, scales and questions are usually used to measure pain intensity, temporal pattern, treatment including exacerbating and/or relieving factors, pain location, pain interference, pain quality, pain affect, pain duration, pain beliefs and pain history. Specific pain measurements are also available for specific conditions such as Western Ontario and McMaster Universities Osteoarthritis Index, Oswestry Disability Index and Neck Disability Index. Conclusions : Faces Pain Rating Scale, numeric rating scale, visual analogue scale, McGill Pain Questionnaire and Brief Pain Inventory and commonly used pain measurements. Specific measurements should be considered depending on research topics.

The Effect of Electroacupuncture at the PC6(Naegwan) and TE5 (Oegwan) on the EEG (내관, 외관 전침 자극이 뇌파변화에 미치는 영향)

  • Objectives : The aim of this study was to examine the effects of electroacupuncture(EA) at the PC6(Naegwan) and the TE5 (Oegwan) on nounal humans using power spectral analysis. Methods : EEG power spectrum exhibit site-specific and state-related differences in specific frequency bands. In this study, power spectrum was used as a measure of complexity. 30 channel EEG study was carried out in 30 subjects(30 males ; age=23.7 years). Results : In ${\alpha}$(alpha) band, the power values at F7 channels(p<0.05) during the PC6-acupoint treatment were significantly were decreased. In ${\beta}$(beta) band, the power values at Fp1, Fz, TT1, T5, P3, P4, Po1, P02, O1, Oz, O2 channels(p<0.05) during the non-acupoint treatment and at Fp1, F4, F8 channels(p<0.05) during the TE5-acupoint treatment significantly were increased. In ${\theta}$(theta) band, the power values at Fp1 channels(p<0.05) during the non-acupoint treatment and at Oz channels(p<0.05) the TE5-acupoint treatment significantly were increased. but, the power values at F7 channels(p<0.05) during the non-acupoint treatment were significantly were decreased. In ${\delta}$(delta) band, the power values at TCP1, TCP2, CP1, T5 channels(p<0.05) during PC6-acupoint treatment were increased and the power values at F7, TT2 channels(p<0.05) during non-acupoint treatment were increased. but, the power values at the TE5-acupoint treatment significantly was decreased than the before-acupuncture treatment.

Differential effect of electroacupuncture analgesia depending on the point selection in a rat model of CFA-induced arthritis (CFA로 유도한 관절염에서 선혈(選穴)에 따른 전침(電鍼)의 진통효과 차이)

  • Objective : In the present study, the effect of electroacupuncture (EA) applied to several acupoints and non-acupoint in CFA-induced knee arthritis was examined. Methods : A common source of persistent pain in humans is arthritis. Arthritis was induced by injection of CFA $125\;{\mu}l$ into knee joint cavity under enflurane anesthesia. The rat subsequently showed swelling of the ankle and a reduced stepping force of the affected limb for the next several days. The reduced stepping force of the limb was presumably due to a painful knee. EA was applied to either of $LR_2,\;LI_4$, or non-acupoint on the contralateral forelimb for 30 min under gaseous anesthesia. After the termination of EA, behavioral tests measuring stepping force were periodically conducted during the next 4 h. Results : EA applied to $LR_2$ point produced a significant improvement of stepping force of the affected foot lasting for at least 2 h. However, neigher $LI_4$ point nor non-point produced any significant increase of weight bearing force. The improvement of stepping pressure was interpreted as an analgesic effect. The analgesic effect was specific to the acupuncture point since the analgesic effect on CFA-induced knee arthritic pain model could not be mimicked by EA applied to a point, $LI_4$ or non-acupoint. In addition, both NO production and iNOS protein expression increased by arthritis were suppressed by EA applied to $LR_2$ point. Conclusion : These data suggest that EA produces a potent analgesic effect in the rat model of CFA-induced knee arthritis. This analgesic effect is produced by applying EA to an acupoint at opposite side from the painful area in a stimulus point-specific way.
