Solitary Subungual Myxoid Neurofibroma of the Thumb: A Case Report (엄지손톱 아래 발생한 단일성 점액성 신경섬유종)

  • Purpose: Subungual tumors are a common cause of nail plate deformity, and may be caused by fibrokeratoma, Koene's tumor and glomus tumors. Neurofibromas, either as part of neurofibromatosis or as a solitary tumor are exceptionally rare in the digits. Methods: A 44-year-old man presented with painless onychodystrophy and nail plate elevation of the right thumb due to a small subungual mass that had started growing 3 years ago. Sensory evaluation of the distal phalanx was normal, and no discoloration nor infection signs were seen. The nail plate was extracted under local anesthesia, and the mass was delicately removed without injury to the nail bed. The nail matrix was repaired with primary closure. Results: Histopathology shows a well circumscribed, cellular tumor with myxoid stroma. Tumor cells were S-100 protein positive, and the patient was diagnosed with myxoid neurofibroma. There has been no sign of recurrence to date, 14 months after the operation. Conclusion: Presentation of cutaneous neurofibromas in the digits is an uncommon finding. They may occur as a manifestation of neurofibromatosis or as a solitary tumor. Subungual neurofibromas are exceptionally rare. To our knowledge, there are only ten reports of solitary subungual neurofibroma unrelated to neurofibromatosis to date. We report a rare case of solitary subungual myxoid neurofibroma of the thumb, that was treated through total excision, with preservation of the nail matrix.

The Social Meanings of Typicality(Prison, [Solitary] Confinement, and Conduit[Passage]) in Peter Halley's Works (피터 핼리 작품에 나타난 전형성 (감옥, [독]방, 도관[통로])의 사회적 의미)

  • This study first examined the social meanings of typicality found in the prison, (solitary) confinement, and conduit(passage) in Peter Halley's works, in which both the prison and (solitary) confinement represent a suppressed structural framework. The former has freedom and escape excluded from it, and the latter allows for mutual regulation and connection as a space of positive mediation. Conduits are interpreted to be flexible and have the potential of creating something new through connection and communication with the outside world. The study then compared and analyzed the meanings of typicality in Halley's works and the concept of segments proposed by Gilles Deleuze and Félix Guattari as social justice in that both of them were in the same context. The findings lead to a conclusion that Halley's prison, (solitary) confinement, and conduit(passage) can be connected to a solid, flexible, and escape segment, respectively, by Deleuze and Guattari.

Numerical Simulation on Reduced Runup Height of Solitary Wave by Fixed Submerged and Floating Rectangular Obstacles (고정된 사각형 수중 및 부유식 구조물에 의한 고립파의 처오름높이 저감 수치모의)

  • The wave runup height is one of the most important parameters for affecting the design of coastal structures such as dikes, revetments, and breakwaters. In this study, SWASH (Zijlema et al., 2011), a non-hydrostatic pressure numerical model, was used to analyze the effect of reducing The wave runup height of solitary waves by submerged and floating rectangular obstacles. It was confirmed that the SWASH model reproduces the propagation, breaking, and runup of solitary waves quite well. In addition, it was confirmed that the wave deformation of the solitary wave by submerged and floating rectangular obstacles was well reproduced. Finally, we conducted an examination of the effect of reducing the runup height of submerged and floating rectangular obstacles. Reduced runup heights are calculated and the characteristics of runup height reduction according to the dimensions of the obstacle were analyzed. The energy attenuation effect of the floating obstacle is greater than the submerged obstacle, and it is shown to be more effective in reducing the runup height.

A Big Data Analysis to Prevent Elderly Solitary Deaths by High-risk Area Clusterization (노인 고독사 방지를 위한 빅데이터 기반 고독사 고위험 지역 탐지 연구)

  • This study proposes a big data-based analytical method to detect high-risk areas for solitary deaths among the elderly in Seoul. The study categorizes and analyzes the risk factors of solitary deaths into demographic, health, economic, and socio-environmental factors. Using data collected from the Seoul Open Data Plaza and Public Data Portal, variables were generated and scatter plots were created using K-means clustering, followed by visual implementation through map creation. The analysis identified Jungnang-gu, Gangbuk-gu, Nowon-gu, Eunpyeong-gu, Gangseo-gu, and Gwanak-gu as the highest-risk areas. This study addresses the limitations of previous survey-based research through big data analysis. The findings are expected to enhance the efficiency of solitary death prevention programs and serve as a basis for informed decision-making in budget allocation across districts.

Estimating the Likelihood of Malignancy in Solitary Pulmonary Nodules by Bayesian Approach (Bayes식 접근법에 의한 고립성 폐결절의 악성도 예측)

  • Background : The causes of solitary pulmonary nodule are many, but the main concern is whether the nodule is benign or malignant. Because a solitary pulmonary nodule is the initial manifestation of the majority of lung cancer, accurate clinical and radiologic interpretation is important. Bayes' theorem is a simple method of combining clinical and radiologic findings to estimate the probability that a nodule in an individual patients is malignant. We estimated the probability of malignancy of solitary pulmonary nodules with a specific combination of features by Bayesian approach. Method : One hundred and eighty patients with solitary pulmonary nodules were identified from multi-center analysis. The hospital records of these patients were reviewed and patient age, smoking history, original radiologic findings, and diagnosis of the solitary pulmonary nodules were recorded. The diagnosis of solitary pulmonary nodule was established pathologically in all patients. We used to Bayes' theorem to devise a simple scheme for estimating the likelihood that a solitary pulmonary nodule is malignant based on radiological and clinical characteristics. Results : In patients characteristics, the probability of malignancy increases with advancing age, peaking in patients older than 66 year of age(LR : 3.64), and higher in patients with smoking history more than 46 pack years(LR : 8.38). In radiological features, the likelihood ratios were increased with increasing size of the nodule and nodule with lobulated or spiculated margin. Conclusion : In conclusion, the likelihood ratios of malignancy may improve the accuracy of the probability of malignancy, and can be a guide of management of solitary pulmonary nodule.

Solitary Pulmonary Nodule (고립성 폐결절)

  • The experience with operative treatment for peripheral situated solitary circumscribed lesions of the lung at the Department of Thorac. & Cardiovasc. Surg., Korea University Hospital during 8 years from March 1974, through April, 1982 was reviewed. Our criteria for Solitary pulmonary nodule were 1. Round or Ovoid shape 2. Surrounded by normal lung Parenchyme 3. Well circumscribed peripheral location 4. No other visible pulmonary diseases on chest X-ray except minimal atelectasis or pneumonitis 5. Largest diameter less than 8 cm Of the 55 patients reviewed, there were 69% of malignancy and 31% of benign pulmonary diseases. In malignancy 38 patients, there were 18 patients with squamous cell carcinoma, 8 patients with undifferentiated large cell carcinoma, 2 patients with undifferentiated small cell carcinoma, 10 patients with adenocarcinoma and patient with metastatic carcinoma. In benign pulmonary nodule 17 patients, here were 5 patients with tuberculoma, 5 patients with aspergilloma, 2 patients with A-V fistula, 1 patient with pulmonary blastoma, 1 patient with paragonimiasis, and 1 patient with lung abscess. Overall male to female occurrence ratio was 39:16, and most prevalent age incidence was 7th decades. Most frequent size distribution was 4-6 cm in diameter. All of benign diseases were cured by resection and 66% of malignancy performed operation and has 75% resectability.

  • The solitary osteochondroma is a hamartoma of the skeleton which derives from an aberrant subperiosteal germ of the fertile cartilage. Osteochondromas, which are uncommon in the hand can occur at the distal end of the proximal and middle phalanx away from the epiphyseal plate region. We experienced a rare case of solitary osteochondroma arising from distal end of fifth proximal phalanx of hand, and limiting the active motion of proximal interphalangeal joint. The patient was treated by marginal excision and tumor showed characteristic microscopic findings of osteochondroma.

  • Solitary fibrous tumor(SFT) is a spindle cell neoplasm that usually arises in the pleura. Its involvement of the meninges is rare but increasingly being reported. Our case was a 33-year-old man who presented with clonic seizure around the right mouth angle and sensory disturbance at the right hand. Radiological studies showed a large extra-axial, dural-based mass in the left fronto-parietal region, typically isointense with gray matter and heterogeneous strong enhancement on T1-weighted images. Histologically, the tumor was composed of spindle-shaped cells growing in fascicles within the collagenous matrix. Immunohistochemical staining demonstrated diffuse CD34 positivity. Awareness of this neoplasm is necessary to accurate diagnosis.

  • Benign papillary tumors commonly arise in the upper respiratory tract but are rare in the lower respiratory tract, especially in the bronchi. Solitary benign papillary tumors arising at the bronchi,covered with stratified squamous epithelium and associated with little or no inflammatory cell response,seem to be rarest of all types of benign bronchial tumors. Only six cases of such tumors had been reported in the medical literature over the past 74 years until 1968. The following is a case report of solitary benign papilloma arising at the opening of the left main bronchus which was treated with bronchoscopic removal at Chest Surgery Depariment, N.M.C. on Sept.

  • Primary intrapulmonary thymoma (PIT) is a very rare lesion of uncertain pathogenesis. PIT should be considered when the histopathological appearance of a lung tumor shows features that are uncommon but similar to those of a thymoma. In this case report, we discuss the case of a 5 9-year-old female with a solitary pulmonary nodule that was confirmed to be PIT on the basis of pathological tests. Treatment with complete resection showed good results.
